RESUMEN

BACKGROUND: Cachexia is one of the side effects of cancer diseases that can be reduced weight, and lower overall survival. Weight loss has been associated with adverse outcomes in both cancer patients and patients with benign diseases. There is no definitive treatment for fully reverse cachexia. studies showed higher levels of inflammatory markers in patient with cachectic cancer. Therefore, this study aimed to investigate the dose-response effects of omega-3 as an anti-inflammatory supplement on body weight in patients with cancer cachexia. METHODS: Online databases including PubMed, Scopus, and Web of Science were systematically searched by relevant keywords up to January 2022. Random effect analysis was applied to perform meta-analysis. Subgroup analyses were performed to find heterogeneity sources. Quality assessment was conducted using Revised Cochrane Collaboration's tool II. Trim and fill analysis were also carried out in case of the presence of publication bias. The certainty in the evaluations was assessed by the GRADE approach. RESULTS: Omega-3 supplementation resulted in a significant increase of body weight in patients with cancer cachexia when the age of study participants was ≥67 years and the baseline weight of them was ≤60 kg (WMD = 0.99; 95 % CI: 0.06, 1.92 and WMD = 1.22; 95 % CI: 0.14, 2.30, respectively). Also, there was a non-significant linear relationship between the dosage of omega-3 supplementation and body weight in patients with cancer cachexia. CONCLUSION: Omega-3 supplementation may be a promising agent to increase body weight in patients with cancer cachexia. Also, a non-significant linear relationship between the dosage of omega-3 supplementation and body weight was found in these patients.

Purpose: Three radiation therapy techniques for breast are common, namely three-dimensional conformal radiotherapy (3D-CRT), Field-in-Field (FIF), and Intensity-Modulated Radiotherapy (IMRT). The purpose of this study was to determine and compare dosimetric parameters of three different treatment planning planning types; 3D-CRT, FIF, and IMRT in target and normal tissues after breast-conserving surgery. Methods: One hundred patients with left or right breast cancer cooperated in this study. They were divided into three categories (small, medium, and large size) based on breast volume. Three treatment planning techniques were carried out by planner for each patient in Prowess® 5.2 Treatment Planning System. The dosimetric parameters were obtained from dose-volume histograms using the CERR software (MATLAB Company, Washington, USA), which runs as an add-on in MATLAB software. Results: 3D-CRT technique with the highest value of Dmax creates more hot spots than the other techniques in the tumor region (P = 0.013). IMRT and FIF showed the best uniformity compared to 3D-CRT in all groups with respect to the values of the parameters D98 and D2. IMRT provided the best coverage in the tumor compared to other methods (P < 0.001). 3D-CRT technique yielded a high volume receiving ≥107% of the prescription dose (P < 0.001). Among the three methods, the FIF method results in a lower dose to the lung for treatment based on the V5 and V20 parameters (P < 0.001). Homogeneity index for IMRT was better than FIF, as well as, conformity index (CI) for IMRT and FIF was better than 3D-CRT. Conclusion: IMRT and FIF plans offered excellent target coverage and uniformity, whereas FIF had better protection of healthy tissues. Thus FIF method is an efficient method to improve the quality of treatment for breast cancer patients.

It has been indicated that calorie restriction (CR) leads to several neuroprotective effects against physiological aging and different neurodegenerative disorders. Unfortunately, the definite therapeutic strategy is not introduced for Multiple sclerosis (MS) as an autoimmune disease of central nervous system (CNS) and researchers are striving to find the best treatment procedures and then optimize them. More recently, several preclinical studies have reported beneficial effects of CR on MS. It was stated that CR can decline demyelination, improve remyelination and decrease neuroinflammation in animal model of MS, as well as reduce body weight and enhance emotional wellbeing in MS patients. In this context we designed this review to examine studies exploring the effects of CR on MS disease based on the clinical and animal models to highlight involved mechanistic implications and future prospective.

OBJECTIVE: To evaluate the dose calculation accuracy in the Prowess Panther treatment planning system (TPS) using the collapsed cone convolution (CCC) algorithm. METHODS: The BEAMnrc Monte Carlo (MC) package was used to predict the dose distribution of photon beams produced by the Oncor® linear accelerator (linac). The MC model of an 18 MV photon beam was verified by measurement using a p-type diode dosimeter. Percent depth dose (PDD) and dose profiles were used for comparison based on three field sizes: 5×5, 10×10, and 20×20cm2. The accuracy of the CCC dosimetry was also evaluated using a plan composed of a simple parallel-opposed field (11×16cm2) in a lung phantom comprised of four tissue simulating media namely, lung, soft tissue, bone and spinal cord. The CCC dose calculation accuracy was evaluated by MC simulation and measurements according to the dose difference and 3D gamma analysis. Gamma analysis was carried out through comparison of the Monte Carlo simulation and the TPS calculated dose. RESULTS: Compared to the dosimetric results measured by the Farmer chamber, the CCC algorithm underestimated dose in the planning target volume (PTV), right lung and lung-tissue interface regions by about -0.11%, -1.6 %, and -2.9%, respectively. Moreover, the CCC algorithm underestimated the dose at the PTV, right lung and lung-tissue interface regions in the order of -0.34%, -0.4% and -3.5%, respectively, when compared to the MC simulation. Gamma analysis results showed that the passing rates within the PTV and heterogeneous region were above 59% and 76%. For the right lung and spinal cord, the passing rates were above 80% for all gamma criteria. CONCLUSIONS: This study demonstrates that the CCC algorithm has potential to calculate dose with sufficient accuracy for 3D conformal radiotherapy within the thorax where a significant amount of tissue heterogeneity exists.
