BACKGROUND: Sexual violence remains a persistent and devastating issue in eastern Democratic Republic of Congo (DRC). AIM: To elucidate the sociodemographic, sexual, and obstetrical characteristics associated with the experiences of victims of sexual violence (VSV) among women in the region. MATERIALS AND METHODS: A cross-sectional study was conducted involving 625 women from eastern DRC. Participants provided self-reported data, collected through interviews conducted by trained female interviewers in secure environments. Associations between VSV and various sociodemographic and reproductive health factors were examined. RESULTS: Of the respondents, 26.1% reported experiences of sexual violence. VSV were predominantly younger, with 56.44% aged between 15 and 24 years. Single women comprised 57.67% of VSV, and 37.42% identified as farmers. There were 33.13% of VSV who were illiterate, and 81.60% belonged to the low socio-economic stratum. Early physiological and reproductive milestones characterised VSV: 52.15% experienced menarche at or before 13 years, 34.97% initiated sexual intercourse before age 15, and 18.70% reported their first pregnancy before age 15. Higher nulliparity was observed in VSV (29.45%) compared to non-VSV (9.31%). A lower prevalence of HIV infection was found among VSV (11.04%) relative to non-VSV (25.76%). CONCLUSION: Sexual violence in the eastern DRC exhibits multifactorial associations. Younger women, those in certain occupations, and those with specific reproductive histories appear more vulnerable. The findings underscore the urgency for targeted interventions, enhanced access to education, and improved reproductive health services. Addressing these pressing issues should remain a primary focus in both societal and public health spheres.

OBJECTIVE: To describe the care pathway of patients with pelvic organ prolapse in a high-volume resource-limited setting and characterize patients undergoing surgery. METHODS: The patient care pathway at a large referral hospital in eastern Democratic Republic of Congo was determined through interviews with key personnel. Patients with apical prolapse (with or without anterior/posterior prolapse) who underwent surgery between January and December 2018 were included. Demographics and outcomes were characterized. Data were presented as means (standard deviation [SD]), medians (interquartile range), or number (percentages). RESULTS: A holistic care model was described. During the study period, 772 patients underwent prolapse repairs, 235 met inclusion criteria. Mean age was 55 (±14) years, and 75% (176/235) were postmenopausal. Median parity was 7 (5-9). A majority (56%, 131/233) had body mass index <18.5 (calculated as weight in kilograms divided by the square of height in meters). Most were farmers (77%, 182/235) and had no formal education (76%, 178/235). Postmenopausal patients underwent hysterectomy, whereas premenopausal patients were treated with uterine-preserving techniques. Most repairs were performed vaginally (96%, 225/235), and 40% (94/234) had concurrent multicompartment repairs. Most common complications were hemorrhage (4%, 9/235, intraoperative) and urinary tract infection (5%, 11/235, postoperative). CONCLUSION: High-volume surgical services for treating prolapse can be integrated into existing healthcare delivery models. Our demographic of patients differs from studies in high-income countries. The degree to which these studies can be generalized to patients in settings similar to ours represents an opportunity for further research.

BACKGROUND: Malnutrition is a public health problem, as wasting affects 7.5% of children worldwide. The harmful effects of severe acute malnutrition (SAM) can last a lifetime, but how SAM in childhood affects later breastfeeding ability is not clear. In the present study, we assessed the human milk output and body composition among mothers with a history of childhood SAM. METHODS: This retrospective cohort study was carried out in Miti-Murhesa Health Zone (Democratic Republic of Congo) from January 15 to March 17, 2020. We selected lactating mothers with breastfed infants aged 2-12 months. Two categories of mothers were included: those who had been treated for SAM during their childhood (years 1988-2003; n = 39) and a community control with no history of SAM (n = 40). The weight, height, and mid-upper arm circumference were measured and body mass index (BMI) calculated as weight/height2. Body composition and human milk output were assessed using standard deuterium dilution methods. Student t and chi2 tests ware applied to compare two groups. RESULTS: The mean age ± standard deviation of the mothers was 24.4 ± 5.1 and 26.0 ± 6.1 years for the SAM and control groups, respectively (p = 0.186). The age of their infants was 5.4 ± 2.3 months in both groups (p = 0.962). In the SAM and control groups, the mean maternal BMI was 23.8 ± 2.3 and 23.6 ± 3.7 kg/m2 (p = 0.849), mean Fat Mass 27.1% ± 5.0 and 27.1% ± 5.8% (p = 0.708), and the mean Fat Free mass 72.9% ± 5.0 and 72.9% ± 5.8% (p = 0.998), respectively. Human milk output was 833.7 ± 152.1 g/d in SAM group and 827.4 ± 171.4 g/d in the control group (p = 0.864). CONCLUSIONS: We found no significant difference in human milk output and body composition in mothers treated for SAM during childhood compared to community controls.

The first-line physicians' practice in Kisangani city in Democratic Republic of Congo: Towards a typology. BACKGROUND: In the Democratic Republic of the Congo (DRC), for a number of years, there has been a spontaneous and growing phenomenon of physicians operating at the front line of the health system, while this role is traditionally devolved to nurse-practitioners. This phenomenon does not align with the current health policy. AIM: The aim of this paper is to develop and discuss the main types of frontline physicians in the city of Kisangani. SETTING: We conducted a descriptive cross-sectional study in two urban districts in the city of Kisangani. METHODS: The study population consisted of all first-line health facilities that employed at least one physician. The construction of a typology of first-line physicians consisted of three stages: identification and definition of relevant dimensions of analysis; grouping cases based on empirical data; and analysis of significant relationships and establishment of the typology itself. RESULTS: An involvement of physicians in healthcare delivery at the first line was observed in 60% of all first line facilities in the two urban districts. Two main types of first-line physicians were identified: firstly, and by large the most prevalent one (96% of cases), the 'hospital-like physician', and secondly, the much less frequent type of the 'supervision physician'. CONCLUSION: The involvement of physicians in first line healthcare is today a growing phenomenon in the DRC, especially in urban areas. The most dominant expression of this phenomenon is a transposition of the hospital-based physician model to the first line healthcare services, which thereby jeopardizing the specificity of first line healthcare.

Introduction : La maladie à CIVD 19 a été déclarée par l'Organisation Mondiale de la Santé (OMS) 'urgence de santé publique de portée internationale' le 30 janvier 2020 [1] et 'pandémie' le 11 mars 2020 Son impact bnégatif est visible dans tous les secteurs de la société humaine jusqu'à remettre en cause la résilience des systèmes sanitaires mondiaux. L'Afrique a enregistré son premier cas de Covid-19 en Egypte et la RDC dans la ville province de Kinshasa au mois de Mars 2020. Cette synthèse vise à décrire la fréquence, de traitement et la proportion de décès en RDC. Méthodologie : Il s'agissait d'une étude transversale basée sur une revue documentaire ayant couvert la période du Mars 2020 au 31 Mars 2021. Les données ont été collectées à la division provinciale de la sante, du sitrep de la République Démocratique du Congo et le site l'OMS. Résultats : En RDC, la guérison et le décès des cas de COVID 19 ont été observées respectivement dans 90% et 2,6 % des cas. Conclusion: En RDC, la guérison et le décès des cas de COVID 19 ont été observées respectivement dans 90% et 2,6 % des cas. Conclusion : La RDC reste le pays de l'Afrique centrale ayant enregistré plus de décès lié à la Covid 19. Nous pensons qu'il est important de réévaluer notre stratégie de riposte contre la Covid - 19. La couverture des mesures promotionnelle et préventive est faiblement observée dans notre pays.

INTRODUCTION: Sub-Saharan Africa has low Caesarean (CS) levels, despite a global increase in CS use. In conflict settings, the pattern of CS use is unclear because of scanty data. We aimed to examine the opportunity of using routine facility data to describe the CS use in conflict settings. METHODS: We conducted a facility-based cross-sectional study in 8 health zones (HZ) of South Kivu province in eastern DR Congo. We reviewed patient hospital records, maternity registers and operative protocol books, from January to December 2018. Data on direct conflict fatalities were obtained from the Uppsala Conflict Data Program. Based on conflict intensity and chronicity (expressed as a 6-year cumulative conflict death rate), HZ were classified as unstable (higher conflict death rate), intermediate and stable (lower conflict death rate). To describe the Caesarean section practice, we used the Robson classification system. Based on parity, history of previous CS, onset of labour, foetal lie and presentation, number of neonates and gestational age, the Robson classification categorises deliveries into 10 mutually exclusive groups. We performed a descriptive analysis of the relative contribution of each Robson group to the overall CS rate in the conflict stratum. RESULTS: Among the 29,600 deliveries reported by health facilities, 5,520 (18.6%) were by CS; 5,325 (96.5%) records were reviewed, of which 2,883 (54.1%) could be classified. The overall estimated population CS rate was 6.9%. The proportion of health facility deliveries that occurred in secondary hospitals was much smaller in unstable health zones (22.4%) than in intermediate (40.25) or stable health zones (43.0%). Robson groups 5 (previous CS, single cephalic, ≥ 37 weeks), 1 (nulliparous, single cephalic, ≥ 37 weeks, spontaneous labour) and 3 (multiparous, no previous CS, single cephalic, ≥ 37 weeks, spontaneous labour) were the leading contributors to the overall CS rate; and represented 75% of all CS deliveries. In unstable zones, previous CS (27.1%) and abnormal position of the fetus (breech, transverse lie, 3.3%) were much less frequent than in unstable and intermediate (44.3% and 6.0% respectively) and stable (46.7%and 6.2% respectively). Premature delivery and multiple pregnancy were more prominent Robson groups in unstable zones. CONCLUSION: In South Kivu province, conflict exposure is linked with an uneven estimated CS rate at HZ level with at high-risks women in conflict affected settings likely to have lower access to CS compared to low-risk mothers in stable health zones.

Trois Zones d'Apprentissage et de Recherche (ZAR) sont développées par le programme RIPSEC en République Démocratique duCongo (RDC). Cette stratégie consiste à utiliser un district sanitaire comme substrat pour former des managers des districts de santé et mener des recherches sur les systèmes de santé. L'objet de cet article est d'explorer les principes, les effets et les conditions de succès de cette stratégie à travers une évaluation qualitative des expériences ZAR antérieures en Afrique Subsaharienne. Elle concerne quatre expériences, deux menées au Niger, une en RDC et une au Congo-Brazzaville. Les données proviennent des témoignages de responsables du développement des expériences, présentés dans un séminaire en 2014 et approfondis par des entretiens en 2015, et des publications traitant des expériences. Les faits communs aux quatre expériences sont la diffusion nationale et souvent internationale des bonnes pratiques et des modèles organisationnels, leur développement inscrit dans le long-terme et l'implication des managers des ZAR dans tous les volets de leur transformation. Le centrage de la recherche sur les problèmes de la ZAR et la promotion de la réflexivité dans les décisions managériales sont importants pour cette transformation. L'utilisation des résultats par le programme RIPSEC, les logiques imbriquées dans le concept ZAR et l'apport potentiel de l'implication d'écoles de santé publiques nationales dans leur développement sont ensuite discutés. La validité de l'étude est limitée mais renforcer le leadership des managers des districts sanitaires est un défi fréquent pour les systèmes de santé aux ressources limitées. Cet article peut aider à développer une stratégie pour le renforcer.

Cardiotocography (CTG) has recently come into use in Lubumbashi but no thorough study has yet been conducted to identify its impact on perinatal morbi-mortality. This study aims to determine the frequency of fetal heart rate abnormalities (FHR)in order to identify the associated factors and to propose a suitable management. We conducted a cross-sectional, descriptive study of 411 women in labour over a period of 19 months (March 2015-December 2016). In patients with pathologic FHR abnormalities, sensitivity and positive predictive value of cardiotocography in the screening test for acute fetal distress were 82.95% and 45.35% respectively. FHR abnormalities were found in two women in labour out of five. Decelerations were the most frequent FHR abnormalities observed (50.8%) with a remarkable predominance of late decelerations (22.1% of all abnormalities). The factors associated with pathological FHR abnormalities were prolonged labor (OR = 14.64, CI = 3.91-54.81), chorioamnionitis (OR = 14.56, CI = 3.83-55.34), chronic maternal anemia (OR = 4.99, CI = 1.48-16.85), primiparity (OR = 2.69, CI = 1.49-4.85), prematurity (OR = 2.90, CI = 1.51-5.54) and prolonged pregnancy (OR = 3.22, CI = 1.38-7.52). Intrauterine growth retardation and arterial hypertension were mainly associated with flat lines and late decelerations (OR = 7.79, CI = 2.50-24.30 and OR=2.74, CI = 1.31-5.72). CTG is a screening tool for the identification of acute fetal distress but with high false-positive rate (55%); it should be associated with other second-line screening tests for acute fetal distress in order to reduce this rate. Factors associated with pathologic FHR abnormalities often cause acute fetal distress thus requiring a rigorous analysis of CTG traces.

Introduction: l'objectif de cette étude était de déterminer la fréquence et d'évaluer le pronostic maternel et périnatal lors de l'accouchement chez les adolescentes dans la ville de Lubumbashi.Méthodes: c'était une étude cas-témoin des accouchées d'une grossesse monofoetale de Décembre 2013 à Mai 2014 dans 10 maternités de référence à Lubumbashi (RD Congo). Les adolescentes (< 20 ans) ont été comparées aux femmes âgées de 20-34 ans. Les paramètres sociodémographiques maternels, la morbi-mortalité maternelle et périnatale ont été analysées. Les statistiques usuelles et la régression logistique ont été utilisées pour analyser les résultats. Le seuil de signification a été fixé à une valeur de p<0,05.Résultats: la fréquence d'accouchement chez les adolescentes était de 7,7%. Nous avons observé que la césarienne (ORa=1,9 (1,1-3,1)), l'épisiotomie (ORa=4,2 (2,9-5,9)), la délivrance pathologique (ORa= 2,7 (1,1-6,5)), l'éclampsie (ORa= 4,4 (1,3-14,5)) et le faible poids de naissance (ORa=2,0 (1,3-3,0)) ont été significativement plus élevés chez les adolescentes que chez les adultes.Conclusion: l'accouchement chez les adolescentes, comparativement à celui de femmes âgées de 20-34 ans, reste associé à un mauvais pronostic. D'où l'organisation des séances de sensibilisation pour une meilleure fréquentation des services consultations prénatales, une optimisation du dépistage, de la surveillance et de la prévention des pathologies de la grossesse chez les adolescentes s'avère importante et urgente

INTRODUCTION: Fetal macrosomia is usually defined when the estimated fetal weight is greater than or equal to 4000 grams. The aim of this study was to determine the incidence of macrosomia, to identify its etiological factors and to evaluate maternal and perinatal prognosis. METHODS: This is a case-control study conducted in maternity units of 10 general referral hospitals in the city of Lubumbashi in Democratic Republic of Congo between 1 December 2013 and 31 March 2014. The post-partum women were divided into two groups according to their fetal birth weight: group I (≥4000 grams or more) and groups II (from 2500 to 3500 grams). Maternal characteristics, obstetrical environment as well as maternal and perinatal prognosis were studied and compared in the two groups. Data were analyzed using Epi Info 7.1 software. Differences were considered significant if p <0,05. RESULTS: A total of 668 mothers-newborns pairs were included in the study, 167 macrosoma and 501 controls. The incidence of fetal macrosomia was 5,7%. Compared to mothers from the control group, we found that mothers of macrosomic infants were older, multiparous, multigravidae, obese, diabetic and had previously delivered a macrosomic fetus. The rates of cesarean delivery and pathological delivery were significantly higher in mothers of macrosomic infants than in those of the control group. Male sex was significantly more prevalent among macrosomic infants than among the control group. Shoulder dystocia was recorded only in the macrosomic group. CONCLUSION: The prevalence of delivery of a macrosomic infant in Lubumbashi is 5,7%. Macrosomia is often the cause of maternal and perinatal complications. Reduction of maternal and perinatal complications passes through a better understanding of risk factors and an early detection.
