RESUMEN

BACKGROUND: Takotsubo cardiomyopathy (TCM) is a temporary and reversible systolic abnormality of the left ventricular apical area resembling a myocardial infarction. Cardiac rupture due to TCM is a rare but fatal complication. Without cardiac surgery, 94% of patients with left ventricular free wall rupture (LVFWR) due to TCM die. Furthermore, successful surgical cases are rare. We report herein the successful treatment of multiple LVFWRs due to TCM using a sutureless repair. CASE PRESENTATION: An 80-year-old man quarreled with his daughter and had a sudden onset of chest pain. He was transferred to our hospital in shock. Electrocardiography showed ST elevation and contrast-enhanced computed tomography revealed a bloody pericardial effusion. Emergent coronary angiography showed no significant stenosis. Cardiac arrest ensued because of cardiac tamponade. Emergent surgery was undertaken and three oozing lacerations on the lateral and inferior walls were noted. A sutureless repair was performed using TachoSil® patches. We also applied Surgicel Nu-Knit® absorbable hemostat with Hydrofit® where TachoSil® failed to completely adhere because of hematoma formation and achieved complete hemostasis. We diagnosed the ruptures due to TCM according to the Mayo criteria. The patient was discharged on postoperative day 71. CONCLUSIONS: A sutureless repair using TachoSil® patches and Surgicel® with Hydrofit® is a minimally invasive and effective method for the treatment of multiple LVFWRs due to TCM.

The development of a fistula between the aorta and the right atrium is a relatively rare but well-documented complication after cardiac surgery and proximal aortic dissection, and has a high mortality rate if it is not diagnosed adequately and surgically treated without delay. We report a rare case of extracardiac aorta-right atrial fistula. An 86-year-old woman underwent aortic valve replacement via median sternotomy. Two weeks after surgery, the upper median skin incision reopened, which exposed the sternum and revealed purulent discharge inside the wound. Wound and blood cultures were positive for methicillin-resistant Staphylococcus aureus. The wound was treated, and healed in approximately 2 weeks. Six weeks after surgery, the patient suddenly presented with dyspnea because of heart failure.Extracardiac aorto-right atrial fistula was confirmed by computed tomography. During surgery, we found an extracardiac fistula formed in the hematoma between the sites where the aortic vent suture was tied and the caval cannula was removed. The infection seemed to have contributed to the development of the fistula and may have persisted in the ascending aorta or artificial valve, which may have led to cerebral hemorrhage resulting in death 4 months later.

PURPOSE: We investigated whether mitral annuloplasty (MAP) should be performed for mild ischemic mitral regurgitation (IMR). METHODS: We selected 57 patients with preoperatively mild IMR. Twenty-eight patients who previously had moderate MR or more, underwent MAP (group 1) while 29 patients with persistent mild MR, did not (group 2). We reviewed MR changes and outcomes of these patients. We also investigated other IMR patients with preoperatively moderate or more MR as reference data (group 3). RESULTS: In group 1, MR was none or trace in 25 patients immediately after operation, however, eleven out of these patients (44%) showed postoperative MR up-grade. The trends of MR changes in group 1 were similar to those of patients in group 3. In group 2, MR was graded mild in 79% of patients in mid-term postoperative stage although 28% of patients were up-graded or down-graded during postoperative follow-up. CONCLUSION: MAP is not necessary for patients with persistently mild IMR. Patients with preoperatively mild IMR with episodes of MR exacerbation had better be treated similarly as those with moderate or more IMR and undergo MAP.

RESUMEN

Coronary artery aneurysm (CAA) is an uncommon disease with an incidence of 1%-5% in evaluated patients. Atherosclerosis is its most common etiology in adults, occurring in 50% of cases. Also, CAA is associated with predisposing factors such as Kawasaki disease, stent-angioplasty, and notably, coronary artery fistula. Most CAAs are asymptomatic but occasionally lead to life-threatening conditions, such as rupture, infection, and myocardial ischemia. Thus caution is urged for cardiac surgeons. We here report on seven surgical cases of CAA with coronary artery fistula in our institute, focusing on their surgical treatment.

BACKGROUND: A Tei index is known to reflect overall cardiac performance including systolic and diastolic function in a variety of heart disease. We investigated the relationship between preoperative Tei index and postoperative left ventricular (LV) mass regression and survival after aortic valve replacement (AVR) for aortic valve stenosis (AS). METHODS: One hundred fifty-four patients with AS were classified into a group with abnormal (Abn) LV function (n=47, 0.45 < or = Tei index) and a group with normal (Nor) LV function (n=107, Tei index < 0.45). The pre- and postoperative echocardiographic variables including LV dimension, LV wall thickness, and LV mass regression as well as 6-year survival were compared between the two groups. RESULTS: There was a significant difference in both absolute and relative LV mass index (LVMI) regression (P=0.004 and 0.0007). Multiple linear regression analysis revealed that the preoperative LVMI, Tei index, and follow-up period were independent predictors of LVMI regression after AVR. Thirteen patients died (valve-related death in 5). Although the overall survival rate in the Nor-LV group (92.8%) was significantly better than that in the Abn-LV group (71.6%), there was no significant difference in survival free from valve-related death. CONCLUSIONS: Preoperative Tei index can be one of the significant predictors of LVMI regression and overall survival after AVR.

BACKGROUND: The impact of aortic valve replacement (AVR) with prosthesis-patient mismatch (PPM) on intermediate-term outcome and left ventricular mass (LVM) regression in patients with aortic stenosis (AS) was investigated. METHODS: One hundred fifty patients with AS (87 pure stenosis and 63 combined stenosis and regurgitation) were classified into a PPM group (n = 34, indexed effective orifice area (EOAI) >0.65 cm(2)/m(2) and < or =0.85 cm(2)/m(2); moderate PPM) and a non-PPM group (n = 116, EOAI > 0.85). Mean age, mean and peak aortic pressure gradient (PG) were not different between the groups (PPM, 99.7 +/- 37.2 and 54.9 +/- 23.2 mmHg; non-PPM, 95.9 +/- 29.2 and 54.4 +/- 16.0 mmHg). The absolute and relative regression in indexed left ventricular mass (LVMI) was estimated by preoperative and postoperative echocardiography (n = 98). RESULTS: Twelve patients died (valve-related death in 7) during 5 years of follow-up. Comparing the PPM and non-PPM groups, overall survival (78.7% vs. 87.8%) and survival free from valve-related death (96.8% vs. 92.1%) were not significantly different. New York Heart Association (NYHA) functional class improved in all patients and there were no patients in class III or IV. The postoperative mean PG was 14.6 +/- 6.1 mmHg in the PPM group and 9.4 +/- 3.8 mmHg in the non-PPM group (p = 0.0005), with an inverse correlation (r = -0.48, p < 0.0001) between EOAI and the postoperative mean PG. However, there was no significant difference in the absolute and relative LVMI regression between the two groups. Multiple linear regression analysis was performed and higher preoperative LVMI and mean aortic PG were independent predictors of greater LVMI regression after AVR. CONCLUSIONS: Moderate PPM does not appear to alter LVMI regression, NYHA class, or intermediate-term outcome in AS patients undergoing AVR with mechanical prostheses. In multivariate analysis, preoperative LVMI and mean aortic PG were important independent predictors of LVMI regression.
