RESUMEN

Following the accident at the Fukushima Dai-ichi Nuclear Power Station, radiocaesium concentrations were specifically elevated in rockfish species compared to other fish species. To clarify the likely reasons, a caesium metabolic rate in the Japanese rockfish Sebastes cheni was derived by an aquarium experiment of live fish collected from the area off Fukushima. Stable caesium and 137Cs concentration in prey organisms, stomach contents and muscle of rockfish were measured and the bioavailable fraction in prey organisms was evaluated. Using derived transfer parameters, 137Cs radioactivity levels in S. cheni and prey organisms were simulated by a model, and verified by the measured radioactivity concentrations of biota in coastal waters south of the Fukushima Dai-ichi Nuclear Power Station. As a result, slow caesium metabolism in S. cheni was confirmed with the biological half-life (Tb1/2) of 190 d. The determining factor for the initial 137Cs radioactivity levels in S. cheni, was the maximum radioactivity levels in surrounding seawater which was constrained by the sedentary nature of rockfish. Controlling factors of depuration rate of 137Cs levels in S. cheni were slow caesium metabolism, enhanced 137Cs radioactivity level of prey organisms, and survival of older contaminated individuals due to a long life-span. During the study period 2017-2021, 137Cs radioactivity concentrations in seawater decreased close to the level measured before 2010, whereas those in prey organisms and rockfish in southern Fukushima waters were still above the levels that existed before 2010. An additional source for enhancing 137Cs radioactivity in rockfish and biota of the food chain was indicated by the greater 137Cs/133Cs atom ratios in rockfish compared to those in the surrounding seawater, however it was considered to be radiologically insignificant in relation to seafood safety limits.

Parotid tumors present a wide range of histological features, from benign to malignant. Periostin, an extracellular matrix protein specifically expressed in the periosteum and periodontal ligament, is isolated from osteoblast cell lines. It regulates fibrosis and collagen deposition and plays an important role in myocardial repair after myocardial infarction. It is also known to be involved in otorhinolaryngological-diseases. This study included 36 patients [38 specimens; 16 men and 20 women, mean age 59.2 (range 26-82) years] who underwent parotid tumor resection at the Division of Otorhinolaryngology, Tohoku Medical and Pharmaceutical University, between April 2017 and March 2022 and were clinically and pathologically diagnosed as having benign parotid tumors. Formalin-fixed, paraffin-embedded sections from the surgical specimens were autoclaved and immunostained with anti-periostin antibodies to evaluate the expression and distribution of periostin. Histologically, the tumors were diagnosed as pleomorphic adenomas in 15 cases (15 specimens), Warthin's tumors in 13 cases (15 specimens), basal cell adenomas in 2 cases (2 specimens), oncocytomas in 4 cases (4 specimens), and myoepitheliomas in 2 cases (2 specimens). An increased expression of periostin was found in 32 of 38 samples (84.2%) in the stroma of benign parotid tumors. Four distinct patterns of periostin expression were observed in benign parotid gland tumors: negative, superficial, infiltrative, and diffuse. Statistically significant differences were found between periostin expression patterns and histological classification of the tumors. Our results suggest that periostin may be involved in the pathogenesis of benign parotid tumors and could serve as a new biomarker for these tumors.

BACKGROUND: Long-term voice-abuse or sudden vocal fold microvascular disruption can lead to injury and subsequent repair/remodeling of the vocal fold mucosa. Periostin is known to be involved in airway remodeling and in various otolaryngological diseases. In ischemic heart disease, increased CD31 expression has been observed around cardiomyocytes during remodeling, and endothelial proliferation has been reported to occur at these sites. OBJECTIVES: We investigated the expression and the roles of CD31, CD34, and periostin in the formation of vocal fold polyps. MATERIALS AND METHODS: Fifty-seven samples of vocal fold polyps were investigate histopathologically and immunohistochemically. RESULT: Expression of CD31 and CD34 was detected in 41 (71.9%) and 53 (93.0%) samples, respectively, obtained from patients with vocal fold polyp. Expression of periostin was detected in 41 (71.9%) samples obtained from patients with vocal polyps. The vocal polyp samples could be classified into three histological subtypes. Three patterns of CD31 and CD34 expression were observed in the vocal polyp. Four patterns of periostin expression were observed in vocal polyps. An association was observed between the CD31 expression pattern and the histological subtype of vocal fold polyps. CONCLUSION AND SIGNIFICANCE: In vocal fold polyps, evaluation of vascular endothelial markers may be useful for staging.

Periostin is known to be involved in airway remodeling and also in various otolaryngological diseases. D-ß-aspartic acid is the major isomer of D-aspartic acid found in elderly tissue. In this study we investigated the expression and the role of D-ß-aspartic acid and periostin in the formation of vocal fold polyps. The expression patterns of D-ß-aspartic acid and periostin in 36 surgical specimens of vocal fold polyps from 36 patients were investigated immunohistochemically. In the epithelium of vocal polyps, D-ß-aspartic acid was expressed in all cases. Expression of D-ß-aspartic acid was detected in 25 samples obtained from patients with vocal fold polyps stroma. Expression of periostin was detected in 28 samples obtained from patients with vocal fold polyps. Two patterns of D-ß-aspartic acid expression were observed in vocal fold polyps stroma: positive type and negative type. The following four patterns of periostin expression were observed in vocal fold polyps: negative type, superficial type, infiltrative type, and diffuse type. An association was observed between D-ß-aspartic acid expression patterns and periostin expression patterns. From these findings we speculate that periostin and D-ß-aspartic acid participate in certain pathological changes in vocal fold polyps, such as extracellular matrix accumulation, local fibrosis, and the formation and development of vocal fold polyps.

The scientific interest in radiocesium and plutonium found in the oceans and seas has increased enormously in the past years as a consequence of the Fukushima Dai-ichi Nuclear Power Plant (FDNPP) accident and is expected to be ongoing due to many unresolved questions. Hence, continuous development of new and verification of old analytical methods should be at the top of the list of the community, working on the topic. In this study, we processed and analyzed several seawater samples, collected in different time frames (2011-2015) from the North Pacific Ocean offshore Fukushima, to determine their radiocesium activities, 134Cs/137Cs activity ratios and 240Pu/239Pu isotopic ratios using the sequential scavenging method, gamma spectrometry and accelerator mass spectrometry (AMS). The observed radiocesium levels in seawater (0.07-0.042 Bq L-1) clearly indicated that the investigated region remained impacted by releases from the damaged power plant even after four years after the accident. Regarding plutonium, its successful separation from large volume seawater samples was confirmed by detection of 240Pu by AMS. However, several problems emerged during the analyzes, which we tried to address with the use of additional methods (e.g., measurements of uranium by ICPMS). The efficiencies of the applied methods and other issues are also discussed.

Periostin is known to be involved in airway remodeling and also in various otolaryngological diseases. The aim of this article was to investigate the expression and the role of periostin in the formation of vocal fold polyps. The expression patterns of periostin in 59 surgical specimens of vocal fold polyps from 54 patients were investigated immunohistochemically. Normal vocal fold mucosa specimens from 5 patients who had undergone total laryngectomy were used as the control group. Retrospective study with planned data collection was conducted at Tohoku Medical and Pharmaceutical University. Expression of periostin was detected in 43 (72.9%) samples and four patterns of periostin expression were observed in vocal fold polyps: negative type, superficial type, infiltrative type, and diffuse type. An association was observed between periostin expression patterns and the histological subtypes of vocal fold polyps. The infiltrative pattern of periostin expression was significantly dominant in vascular-hyaline types. Expression of transforming growth factor-ß (TGF-ß) was also detected in the vocal fold polyps. Our results confirmed that periostin might be involved in certain pathological changes in vocal fold polyps, such as extracellular matrix accumulation, local fibrosis, and formation and development of vocal fold polyps.

INTRODUCTION: Eosinophilic chronic rhinosinusitis (ECRS) is a refractory chronic disease defined by recurrent nasal polyps with severe eosinophilic infiltration. This is mainly due to enhanced type 2-dominant immune responses, but the underlying mechanism is still not fully understood. OBJECTIVE AND METHODS: In the present study, we aimed to determine the characteristics of dendritic cells (DCs) and cytokine profiles of T cells in the peripheral blood of individuals with ECRS and age- and sex-matched healthy controls (HC). RESULTS AND CONCLUSION: The ratios of myeloid (m)DC1s to DCs and PD-L1+ mDC1s to mDC1s were higher in ECRS patients than in HC. The proportions of plasmacytoid (p)DCs in DCs, and human leukocyte antigen-DR+ pDCs and ILT3+ pDCs in pDCs were lower in ECRS patients than in HC. In a characterization of T cells, IL-4+CD4+, IFN-γ+CD4+, IL-4+IFN-γ+CD4+, IL-4+Foxp3+CD4+, IFN-γ+Foxp3+CD4+, IFN-γ+IL-4-Foxp3-CD4+, IL-4+CD8+, IL-4+IFN-γ+CD8+, and IL-4+Foxp3+CD8+ T-cell populations were significantly higher in ECRS patients than in HC. These results suggest that the enhanced immune regulation of mDC1, diminished capacity of pDCs, and increased proportion of the T-cell phenotypes in peripheral blood might be factors in ECRS pathogenesis.

HIV-associated salivary gland disease refers to the pathology in head and neck lesions such as ranula, salivary gland swelling, xerostomia, and benign lymphoepithelial cysts in the parotid gland. Here, we present a unique case of the ranula patient with HIV infection treated with OK-423 sclerotherapy. Case report: The patient was a 42-year-old Japanese male with a few months history of oral floor swelling. Computed tomography (CT) showed a low-density area limited within the right floor of the mouth. Magnetic resonance imaging (MRI) revealed a distinct T2-high intensity area localized on the same location. The puncture fluid was bloody mucus, and the cytology was no malignancy. We diagnosed a simple ranula. He was, however, found to be HIV-antibody positive at the examination before treatment by chance. He was referred to the department of infectious diseases and definitively diagnosed HIV infection by western blot. We chose OK-432 sclerotherapy because of its minimally invasive and the risk of HIV infecting medical staff. Two times OK-432 injection made the lesion disappear. Conclusion: The case indicated that OK-432 sclerotherapy could be effective for ranula related to HIV.

A 79-year-old Japanese woman presented with a rare case of metastatic renal cell carcinoma to the left sphenoid sinus with left nasal bleeding. She had previously had right radical nephrectomy for renal cell carcinoma at the age of 64 years and brain and spinal cord infarction at 74 years. Endoscopic examination revealed no mass in the nasal cavity. CT and MRI revealed a tumor in the left sphenoid sinus. The size of the tumor increased gradually from 12 to 15 years after the radical nephrectomy. Complete resection with endoscopic surgery was performed without preoperative embolization. The tumor cells had clear cytoplasm and were arranged in a trabecular pattern lined by a layer of endothelial cells. These findings were identical to the pathological findings of the surgical specimen of the renal cell carcinoma from 15 years previous. A pathological diagnosis of metastatic renal cell carcinoma of clear cell type (grade 1) was made. PET-CT demonstrated no metastasis. The patient's condition was successfully managed with excision of the tumor, and she remains well with no evidence of recurrence and metastasis 36 months after treatment. Metastatic renal cell carcinoma to the sphenoid sinus is rare, but it might be considered in the differential diagnosis of masses in the paranasal sinus even long after initial treatment of renal cancer.

This article summarizes and discusses mass balance calculations of the activities of Fukushima-derived 137Cs released to the atmosphere and ocean prior to 2018 as well as the 137Cs inventories on land and in the ocean, biota, and sediment. We propose that the consensus value of the total amount of 137Cs released to the atmosphere was 15-21 PBq; atmospheric deposition of 137Cs on land was 3-6 PBq; atmospheric deposition of 137Cs on the North Pacific was 12-15 PBq; and direct discharge of 137Cs to the ocean was 3-6 PBq. We also evaluated the movement of 137Cs from one domain to another for several years after the accident. We calculated that the amount of 137Cs transported by rivers might be 40 TBq. The annual deposition of 137Cs due to resuspension at Okuma during the period 2014-2018 was 4-10 TBq year-1. The 137Cs discharged to the ocean was 0.73-1.0 TBq year-1 in 2016-2018. The integrated amount of FNPP1-derived 137Cs that entered the Sea of Japan from the Pacific Ocean from 2011 until 2017 was 270 ± 20 TBq, 6.4% of the estimated amount of FNPP1-derived 137Cs in Subtropical Mode Water in the North Pacific. The integrated amount of FNPP1-derived 137Cs that returned to the North Pacific Ocean through the Tsugaru Strait from the Sea of Japan was 110 ± 10 TBq. Decontamination efforts removed 134 TBq of 137Cs from surface soil prior to February 2019, an amount that corresponded to 4% of the137Cs deposited on land in Japan.

Radiocesium was released to the North Pacific coastal waters by the accident at the Fukushima Dai-ichi Nuclear Power Plant (1FNPP) of the Tokyo Electric Power Company (TEPCO) in March 2011. Since the radiocesium in the sediment off Fukushima was suggested as a possible source for the transfer of this radionuclide through the benthic food chain, we conducted numerical simulations of 137Cs in sediments off the Fukushima coast by using a model which incorporates dynamic transfer processes between seawater and the labile and refractory fractions in sediment particles. This model reproduced the measured temporal changes of 137Cs concentration in seabed surface sediment off Fukusima coasts, by normalizing the radiocsium transfer between seawater and sediment according to the particle diameter sizes. We found that the 137Cs level in sediment decreased by desorption during the first several months after the accident, followed by a reduction in the labile fraction until the end of 2012. The apparent decrease of the total radiocesium level in surface sediment was estimated to occur at rates of approximately 0.2 y-1 within a 20 km distance from the 1FNPP. The comparison of 137Cs level decreases in the demersal fish and the simulated temporal labile fraction in fine sediment demonstrated that the consideration of radiocesium transfer via sediment is important for determining the 137Cs depuration mechanism in some demersal fish.

A series of accidents at the Fukushima Dai-ichi Nuclear Power Plant (1F NPP) following the Great East Japan Earthquake and tsunami of 11 March 2011 resulted in the release of radioactive materials to the ocean. We used the Regional Ocean Model System (ROMS) to simulate the 137Cs activity in the oceanic area off Fukushima, with the sources of radioactivity being direct release, atmospheric deposition, river discharge, and inflow across the domain boundary. The direct release rate of 137Cs after the accident until the end of 2016 was estimated by comparing simulated results with measured 137Cs activities adjacent to the 1F NPP. River discharge rates of 137Cs were estimated by multiplying simulated river flow rates by the dissolved 137Cs activities, which were estimated by an empirical function. Inflow of 137Cs across the domain boundary was set according to the results of a North Pacific Ocean model. Because the spatiotemporal variability of 137Cs activity was large, the simulated results were compared with the annual averaged observed 137Cs activity distribution. Normalized annual averaged 137Cs activity distributions in the regional ocean were similar for each year from 2013 to 2016. This result suggests that the annual averaged distribution is predictable. Simulated 137Cs activity attributable to direct release was in good agreement with measurement data from the coastal zone adjacent to the 1F NPP. Comparison of the simulated results with measured activity in the offshore area indicated that the simulation slightly underestimated the activity attributable to inflow across the domain boundary. This result suggests that recirculation of subducted 137Cs to the surface layer was underestimated by the North Pacific model. During the study period, the effect of river discharge on oceanic 137Cs activity was small compared to the effect of directly released 137Cs.

Immunoglobulin G4-related disease (IgG4-RD) is a recently recognized disease, characterized by high serum IgG4 concentrations and IgG4-producing plasma cell expansion with fibrotic or sclerotic changes in affected organs. Recent work has focused on the relationship between IgG4-RD and malignancies, but there is no report of malignancies associated with IgG4-RD in head and neck regions. The aim of this study was to analyze the clinicopathological characteristics of malignancies in patients with IgG4-RD in head and neck regions. We retrospectively analyzed 26 patients with IgG4-RD (12 men and 14 women aged 60.6 ± 11.6 years). The mean follow-up period was 26.6 months (from 12 to 96 months). These patients were divided into single-lesion group (n = 12) with IgG4-RD only in head and neck regions and multiple-lesion group (n = 14) with IgG4-RD in other regions. There was no significant difference in serum IgG4 concentrations between the single-lesion group (459.4 ± 336.4 mg/dL) and the multiple-lesion group (908.0 ± 739.2 mg/dL) (P = 0.07), whereas the IgG4/IgG ratio was significantly lower in the single-lesion group (22.8 ± 11.0%; n = 11) compared with the multiple-lesion group (31.7 ± 15.0%; n = 11, P = 0.02). Among the 26 patients, two patients (7.7%), both in the multiple-lesion group, developed life-threatening malignancies (salivary duct carcinoma in the submandibular gland and lymphoma in the orbital tissue). All physicians need to keep in mind the possible coexistence of malignancies in patients with IgG4-RD with high IgG4/IgG ratio and multiple lesions at the time of diagnosis.

OBJECTIVES: The aim of this article was to investigate the effectiveness and underlying mechanisms of OK-432 therapy in patients with auricular hematomas. STUDY DESIGN AND SETTING: Case series with planned data collection. SUBJECTS AND METHODS: We tried this therapy in 47 patients with auricular hematoma between April 2008 and August 2018. We aspirated as much of the fluid content of each lesion as possible with a 21-gage needle. We injected OK-432 solution into the lesion with the same needle that we used for aspiration. We performed this treatment at an outpatient basis without hospitalization. RESULTS: Disappearance and marked reduction of the lesion were observed in all patients who had this therapy, and local scarring and deformity of the auricle did not occur in any patients. As adverse effects, local pain at the injection site and fever (37-38.5°C) were observed in some cases of the patients who had this therapy. The concentrations of various cytokines in each aspirate before and after OK-432 therapy were investigated. The production of tumor necrosis factor-α, interleukin-6, interleukin-8, interferon gamma, vascular endothelial growth factor, and periostin was significantly elevated in the aspirate fluid after OK-432 therapy. CONCLUSION: OK-432 therapy is simple, easy, safe, effective, and can be used as a substitute for surgery in the treatment of auricular hematoma.

The cesium depuration mechanisms were studied in Japanese rockfish Sebastes cheni off Fukushima, in which the radiocesium level remains higher than in other teleost. Samples were collected approximately 5 km south from the nuclear power plant during 2014-2016, and the 137Cs concentrations in fish, stomach content and prey species were measured. The stable cesium content in fish was also analyzed and compared with fish age which was determined by annual ring analysis in otoliths. The 137Cs concentrations in the dominant prey species, mysids and brown shrimp, were several Bq kg-w.w.-1; indicating that transfer via the food chain was substantial compared to that from seawater during the study period. The 137Cs concentrations in S. cheni decreased from 2014 to 2016 due to the metabolic excretion and the rate of decrease in its diet. Biokinetic model analyses confirmed the slower turnover of stable cesium in S. cheni, represented as a biological half-life (Tb1/2) of 140-215 d, and was associated with stable Cs levels in food of 5-7 ng g-w.w.-1. The 137Cs levels in S. cheni were also simulated, which showed that the 137Cs depuration in fish exposed to the initial contaminated plume in 2011 resulted from slower metabolic excretion, while the 137Cs levels in fish born after 2012 could be regarded as equilibrated with the environmental levels of 137Cs. Furthermore, the simulation results suggest that 137Cs depuration in S. cheni population was also caused by the alternation of generation, which can be substantial by the addition of new year class population hatched after 2012 that were not contaminated by the initial contaminated plume from the 2011 accident.

Radiocesium (134Cs and 137Cs) released from the Fukushima Dai-ichi Nuclear Power Plant (1FNPP) accident contaminated the fish inhabiting the port of 1FNPP. Radiocesium concentrations in some fishes, especially rockfish, have still remained at elevated levels, while concentrations in olive flounder have decreased in 2015 to the level which is close to the Japanese regulatory limit for seafood products (0.1 kBq kg-wet-1). In this study a dynamic food chain transfer model was applied to reconstruct radiocesium levels in olive flounder residing around the port area. As a result, the observed 137Cs concentrations in olive flounder collected from the port could be explained by the simulated values in the fish, using the seawater level records at the port entrance. The reconstructed maximum 134+137Cs concentration in olive flounder inhabiting the port area was 72 kB kg-wet-1 in July 2011 and the ecological half-life (EHL) was estimated as being 180 days during the period of 2014-2015. Short term simulation which assumed that the coastal water fish swam into the port during 1 month, demonstrated that the radiocesium level in the olive flounder may become equivalent to the depurated level in the fish which were initially contaminated. This result indicated that the increase of radiocesium levels in wandering fish is unlikely to change total radiocesium concentrations in the olive flounder. In this sense, the radiocesium levels in the olive flounder of the port area can be interpreted as being convergent in 2015, regardless of the differences in their contamination histories. On the other hand, the higher 137Cs concentrations in fat greenling, compared to the olive flounder, can be attributed to a history of exposure to the contaminated seawater and food at the inner area of the port, such as the shallow draft quay and seawall area. As a result of the reconstructed initial higher radiocesium concentration, constrained by exposure history at the inner area of the port, the depurated radiocesium concentration in fat greenling is still likely to be greater than the regulatory level in the port area in 2015.

To clarify the relationship between zooplankton biomass and the environmental kinetics of the natural radionuclide 210Po during a one-year period (October 1995 to November 1996) in northwestern Mediterranean coastal waters, a modelling analysis was applied. Using 210Po concentrations in seawater and zooplankton, the 210Po uptake rate constant from food for zooplankton was evaluated using a biokinetics calculation involving the uptake and the excretion rate constants between seawater and zooplankton. Using the transfer constants obtained, the 210Po concentrations in zooplankton were reconstructed and validated by observed concentrations. The simulation results were in good agreement with the measured 210Po concentrations in zooplankton. Assuming that 210Po fecal excretion represents the majority of the excretion of 210Po from zooplankton, the fecal matter associated 210Po vertical flux was calculated, and compared with the observed vertical fluxes of 210Po measured in sediment traps. The modelling evaluation showed that fecal pellet vertical transport could not fully explain the observed sinking fluxes of particulate organic matter at 150 m depth, suggesting that other sinking biodetrital aggregates are also important components of the plankton-derived vertical flux of 210Po. The relationship between 210Po concentration in seawater and that in rain and dry fallout and their potential effect on 210Po concentrations in zooplankton at this location were also examined. A similar, but diphased trend between 210Po in zooplankton and 210Po in rain and dry fallout deposition rate was demonstrated. 210Po concentrations in the dissolved phase of seawater tended to diminish as mean daily rainfall increased suggesting that rain inputs serve as a 210Po dilution mechanism in seawater at this location.

Radiocesium ((134)Cs and (137)Cs) released into the Fukushima coastal environment was transferred to marine biota inhabiting the Pacific Ocean coastal waters of eastern Japan. Though the levels in most of the edible marine species decreased overtime, radiocesium concentrations in some fishes were still remained higher than the Japanese regulatory limit for seafood products. In this study, a dynamic food chain transfer model was applied to reconstruct (137)Cs levels in olive flounder by adopting the radiocesium concentrations in small demersal fish which constitute an important fraction of the diet of the olive flounder particularly inhabiting area near Fukushima. In addition, (137)Cs levels in slime flounder were also simulated using reported radiocesium concentrations in some prey organisms. The simulated results from Onahama on the southern border of the Fukushima coastline, and at Choshi the southernmost point where the contaminated water mass was transported by the Oyashio current, were assessed in order to identify what can be explained from present information, and what remains to be clarified three years after the Fukushima Dai-ichi nuclear power plant (1FNPP) accident. As a result, the observed (137)Cs concentrations in planktivorous fish and their predator fish could be explained by the theoretically-derived simulated levels. On the other hand, the slow (137)Cs depuration in slime flounder can be attributed to uptake from unknown sources for which the uptake fluxes were of a similar magnitude as the excretion fluxes. Since the reported (137)Cs concentrations in benthic invertebrates off Onahama were higher than the simulated values, radiocesium transfer from these benthic detritivorous invertebrates to slime flounder via ingestion was suggested as a cause for the observed slow depuration of (137)Cs in demersal fish off southern Fukushima. Furthermore, the slower depuration in the demersal fish likely required an additional source of (137)Cs, i.e. contaminated detritus or sediment which was entrained with the prey during the active sediment feeding of this fish species.

Radiocesium ((134)Cs and (137)Cs) originating from the Fukushima Dai-ichi Nuclear Power Plant (1FNPP) has contaminated coastal waters and been subsequently transferred to the marine biota along the Pacific coastal region of eastern Japan. To clarify the mechanism of radiocesium biokinetics in olive flounder, a commercially valuable and piscivorous predator, the biokinetics of (137)Cs was simulated using a dynamic biological compartment model and then validated with the measured concentrations in available monitoring data. The (137)Cs concentrations in seawater of the Pacific coastal sites of eastern Japan, from Kesen-numa (170 km north from the 1FNPP) to Choshi (190 km south from the 1FNPP), were reconstructed by fitting the simulated levels to the observed concentrations. Simulated values were verified by measured radiocesium levels in sedentary organism such as macro-algae and mussels inhabiting each study site which had accumulated radiocesium in their ambient environment from the beginning of the accident. Using reconstructed (137)Cs concentrations in seawater, the (137)Cs levels in olive flounder and its main planktivorous prey fish, e.g. anchovy, sand lance, whitebait, etc., were simulated and compared with observed concentrations to clarify the biokinetics of radiocesium in these organisms. This assessment showed that the determining factor for the maximum radiocesium concentrations in fish in the plankton food chain is likely to be the initial radiocesium concentration which they were exposed to during the contamination stage. Furthermore, the simulated (137)Cs concentrations in gut contents of olive flounder were verified by measured (137)Cs concentrations in the stomach contents of this fish collected within 30 km from the 1FNPP. These results indicated that the decrease of (137)Cs levels in their prey organisms was the primary determining factor of radiocesium depuration, and the resultant ecological half-lives were 140-160 d in the olive flounder, by the simulation.

On March 11, 2011, Japan's northeast Pacific coast was hit by a gigantic earthquake and subsequent tsunami. Soma City in Fukushima Prefecture is situated approximately 44 km north of Fukushima Daiichi Nuclear Power Plant. Soma General Hospital is the only hospital in Soma City that provides full-time otolaryngological medical care. We investigated the changes in new patients from one year before to three years after the disaster. We investigated 18,167 new patients treated at our department during the four years from April 1, 2010 to March 31, 2014. Of the new patients, we categorized the diagnoses into Meniere's disease, acute low-tone sensorineural hearing loss, vertigo, sudden deafness, tinnitus, and facial palsy as neuro-otologic symptoms. We also investigated the changes in the numbers of patients whom we examined at that time concerning other otolaryngological disorders, including epistaxis, infectious diseases of the laryngopharynx, and allergic rhinitis. The total number of new patients did not change remarkably on a year-to-year basis. Conversely, cases of vertigo, Meniere's disease, and acute low-tone sensorineural hearing loss increased in number immediately after the disaster, reaching a plateau in the second year and slightly decreasing in the third year. Specifically, 4.8% of patients suffering from these neuro-otologic diseases had complications from depression and other mental diseases. With regard to new patients in our department, there was no apparent increase in the number of patients suffering from diseases other than neuro-otologic diseases, including epistaxis, and allergic rhinitis. Patients suffering from vertigo and/or dizziness increased during the first few years after the disaster. These results are attributed to the continuing stress and tension of the inhabitants. This investigation of those living in the disaster area highlights the need for long-term support.
