RESUMEN

BACKGROUND: Dysthyroidism (DT) is a common toxicity of immune checkpoint inhibitors (ICIs) and prior work suggests that dysthyroidism (DT) might be associated with ICI efficacy. PATIENTS AND METHODS: ConSoRe, a new generation data mining solution, was used in this retrospective study, to extract data from electronic patient records of adult cancer patients treated with ICI at Institut Paoli-Calmettes (Marseille, France). Every DT was verified and only ICI-induced DT was retained. Survival analyses were performed by Kaplan-Meier method (log-rank test) and Cox model. To account for immortal time bias, a conditional landmark analysis was performed (2 months and 6 months), together with a time-varying Cox model. RESULTS: Data extraction identified 1385 patients treated with ICI between 2011 and 2021. DT was associated with improved overall survival (OS) (HR 0.46, (95% CI 0.33 to 0.65), p<0.001), with a median OS of 35.3 months in DT group vs 15.4 months in non-DT group (NDT). Survival impact of DT was consistent using a 6-month landmark analysis with a median OS of 36.7 months (95% CI 29.4 to not reported) in the DT group vs 25.5 months (95% CI 22.8 to 27.8) in the NDT group. In multivariate analysis, DT was independently associated with improved OS (HR 0.49, 95% CI 0.35 to 0.69, p=0.001). After adjustment in time-varying Cox model, this association remained significant (adjusted HR 0.64, 95% CI 0.45 to 0.90, p=0.010). Moreover, patients with DT and additional immune-related adverse event had increased OS compared with patients with isolated DT, with median OS of 38.8 months vs 21.4 months, respectively. CONCLUSION: Data mining identified a large number of patients with ICI-induced DT, which was associated with improved OS accounting for immortal time bias.

PURPOSE: Many institutions throughout the world have launched precision medicine initiatives in oncology, and a large amount of clinical and genomic data is being produced. Although there have been attempts at data sharing with the community, initiatives are still limited. In this context, a French task force composed of Integrated Cancer Research Sites (SIRICs), comprehensive cancer centers from the Unicancer network (one of Europe's largest cancer research organization), and university hospitals launched an initiative to improve and accelerate retrospective and prospective clinical and genomic data sharing in oncology. MATERIALS AND METHODS: For 5 years, the OSIRIS group has worked on structuring data and identifying technical solutions for collecting and sharing them. The group used a multidisciplinary approach that included weekly scientific and technical meetings over several months to foster a national consensus on a minimal data set. RESULTS: The resulting OSIRIS set and event-based data model, which is able to capture the disease course, was built with 67 clinical and 65 omics items. The group made it compatible with the HL7 Fast Healthcare Interoperability Resources (FHIR) format to maximize interoperability. The OSIRIS set was reviewed, approved by a National Plan Strategic Committee, and freely released to the community. A proof-of-concept study was carried out to put the OSIRIS set and Common Data Model into practice using a cohort of 300 patients. CONCLUSION: Using a national and bottom-up approach, the OSIRIS group has defined a model including a minimal set of clinical and genomic data that can be used to accelerate data sharing produced in oncology. The model relies on clear and formally defined terminologies and, as such, may also benefit the larger international community.

Se realizó un estudio experimental con el objetivo de determinar la efectividad de la terapia floral en el tratamiento de la depresión leve y moderada, en adultos mayores con cáncer, en el Hospital Provincial Docente de Oncología María Curie, de la provincia de Camagüey, Cuba; el período de estudio comprendió desde enero de 2011 hasta enero de 2012. El universo contempló los pacientes neoplásicos de la tercera edad con síntomas de depresión, remitidos a la consulta de Medicina Tradicional y Natural de la institución y la muestra quedó integrada por 100 pacientes que cumplían los criterios de inclusión y exclusión, los cuales se dividieron en dos grupos de 50 cada uno: el A con tratamiento floral y el B con tratamiento convencional. Se evaluó la persistencia de los síntomas depresivos a los tres y a los seis meses después de aplicado el tratamiento. La terapia floral fue efectiva en el 94 por ciento de los casos. Se evidenció la existencia de situaciones intensas antes de ser diagnosticados con cáncer, la más relevante fue la separación familiar, en el 70 por ciento de la muestra. Los pacientes del grupo A evolucionaron mejor que los del B. En los primeros, se redujeron notablemente los síntomas depresivos a los tres meses, situación más notable a los seis meses de tratamiento, avalando como efectiva la terapia floral empleada. No se reportaron efectos adversos (AU)


An experimental study was carried out with the objective to determine the effectiveness of floral therapy for the treatment of mild to moderate depression in the elderly suffering from cancer in Marie Curie Oncology Teaching Hospital in Camagüey, Cuba, from January, 2011 to January 2012. The universe was made up of the neoplastic elderly patients with depression symptoms referred to the Traditional and Natural Medicine Office of the institution, and the sample was formed by 100 patients fulfilling the inclusion and exclusion criteria and who were divided into two groups of 50 patients each: A with floral treatment and B with conventional treatment. The persistence of the symptoms was evaluated at three and six months after the application of the treatment. The floral therapy was effective in 94 percent of the cases. The existence of intense situations before the cancer diagnosis was evident; the most outstanding one was the family separation, in 70percent of the sample. The patients from group A had a better evolution than the patients from group B. Also, the symptoms of depression were remarkably reduced in the first group at three months of treatment, being even more notable at six months, corroborating the effectiveness of the floral therapy. No adverse reactions were reported (AU)

RESUMEN

The purpose of this study was to determine the effects of an osteoporosis educational intervention on knowledge, health beliefs, and self-efficacy in college-age Puerto Rican women. The Health Belief Model and Purnell Model formed the theoretical framework for the study. Three hypotheses were tested with a convenience, randomized sample of 51 experimental and 54 control subjects, ages 18-25. The findings support the hypotheses that educational intervention improved osteoporosis knowledge, health beliefs, but had no effect on self-efficacy. Results and directions for future research are discussed.

This study used propositions from the social learning theory to explore the effects of the combined influences of child maltreatment, childhood witness to parental violence, sibling violence, and gender on dating violence perpetration using a modified version of the Conflict Tactics Scale 2 (CTS2). A weighted scoring method was utilized to determine how severity of violence in the home impacts dating violence perpetration. Bivariate correlations and linear regression models indicate significant associations between child maltreatment, sibling violence perpetration, childhood witness to parental violence, gender, and subsequent dating violence perpetration. Multiple regression analyses indicate that for men, history of severe violence victimization (i.e., child maltreatment and childhood witness to parental violence) and severe perpetration (sibling violence) significantly predict dating violence perpetration.

This study provided an experimental test of a drug abuse treatment enrollment and retention intervention in a sample of 103 Black mothers of substance-exposed infants. Significantly more women assigned to the Engaging Moms Program enrolled into drug abuse treatment than did women assigned to the control condition (88% vs. 46%). Sixty-seven percent of participants in the Engaging Moms Program received at least 4 weeks of drug abuse treatment compared with 38% of the control women. However, there were no differences between the groups 90 days following treatment entry. Logistic regressions revealed that readiness for treatment predicted both short-term and long-term treatment retention. The Engaging Moms Program has considerable promise in facilitating treatment entry and short-term retention, but it did not influence long-term retention.

This study examined the role of family functioning and social support in protecting HIV-positive African American women from the adverse psychological consequences associated with deterioration in their CD4 cell count. Participants were 38 African American HIV-positive women who had recently given birth. Results demonstrated that changes in CD4 cell counts were inversely predictive of psychological distress and were moderated by family functioning and social support satisfaction. Women with good family functioning were less affected by changes in their CD4 cell counts, and women with poor family functioning were more emotionally responsive to changes in CD4 cell count. Unexpectedly, women from families where conflicts tended to be clearly laid out and discussed were also more responsive to both changes in CD4 cell counts. Interventions are recommended that increase a client's social support satisfaction, foster an adaptive level of connectedness to family, and enhance the family's range of conflict resolution styles.
