RESUMEN

The purpose of this study was to elucidate the state of preparation of the dental healthcare system for a large-scale disaster in terms of health-related crisis management by public health centers (PHCs). Questionnaires were sent by e-mail to 549 PHCs. The response rate was 51.4%. The results showed that the percentage of preparedness for a large-scale disaster was only 26.2% and the dental healthcare system has not been constructed nationwide, especially among PHCs located in areas with smaller populations. The main reasons given for why such a dental healthcare system has not been effectively established were related to lack of concern about such a crisis and a sense that the situation is not critical. Nevertheless, PHCs have made progress in assigning dentists and dental hygienists to the dental healthcare system in preparation for a large-scale disaster.

The aim of this study was to investigate the relationship between dental clinic credibility and patient satisfaction with regular dental check-ups, to compare the level of satisfaction of patients who had regular check-ups with those who did not, and to identify factors associated with regular dental checkups. Thirty-nine private dental clinics in 17 prefectures throughout Japan participated in this study. A total of 9024 questionnaires were distributed to the patients at these clinics, and patients returned the questionnaires using a pre-paid envelope (response rate 56.8%). The questionnaires consisted of items related to patient demographics and 11 items concerning the level of patient satisfaction with the dental clinic. Using multiple logistic regression analysis, the strongest correlation was found between dental clinic credibility in regular checkups and the technical competence of the dentist. The responses to those items concerning making an appointment, and the cleanliness and neatness of the waiting room were different between patients who had regular check-ups and those who did not. To investigate the factors that encourage regular dental check-ups, correlation of factors with regular check up experience were analyzed. Multivariate-adjusted odds ratios indicated a strong association between regular dental check ups and both the technical competence of the dental hygienist and the cost of treatment.

The purpose of this survey was to investigate the relationship between demographic characteristics and willingness of patients to pay for regular dental check-ups in Japan. Questionnaires were distributed at private dental offices and collected via postage-paid return envelopes addressed to the center of the study groups. Questions focused on demographics and willingness to pay for regular check-ups. Five thousand one hundred thirty-two questionnaires were collected (response rate 56.8%). The 3 groups most likely to have regular dental check-ups were found to be the under 20s, 50 to 59 year olds and civil servants. Of these groups, civil servants were found to be the most likely of all to have regular check-ups. More females than males were represented in the sample. More than 60% of the patients responded that they would be willing to pay for regular check-ups if the cost were less than 2,000 yen (about 20 dollars). However, no statistically significant differences were observed in relation to household income. The results suggested that participation in regular dental check-ups might be related to gender and age, but not to household income.

This survey classified the issues involved in promoting the regular check-up system in dental clinics. The survey was conducted in two parts: interviews with 39 dental clinics and a questionnaire survey of 5,129 patients (response rate: 56.8%). The results are summarized as follows: 1. Among the dental clinics that participated in this survey, the proportion of regular patients, the number of dental hygienists working at clinics, and the number of patients per day were higher than those of the national average. 2. The demographic patterns of regularly attending patients were "female, aged under 14 years and students or public servants". 3. Satisfaction with the technical competence of hygienists has the strongest correlation with regularity of visiting. The degree of correlation was about twice that of the correlation with technical competence of the dentist. 4. Patients' priorities for improving dental services to enhance satisfaction were treatment charges, appointment system, and attitude of staff and doctors toward welcoming the patients.

The purpose of this study was to investigate the effects of oral care using simple tools and methods on the cleanliness of the oral cavity in the elderly. Enrolled were 84 elderly subjects with a mean (+/-S.D) age of 85.1+/-7.0 years in a nursing home. They were given tongue and oral mucosa cleaning (the oral care) after lunch every day or every other day for two consecutive weeks by the authors. The effect of the oral care was studied in terms of Candida scores in tongue coating, concentration of volatile sulfur compounds (VSC) which are the main causative substance of bad breath, and change in tongue coating scores. The above parameters were measured five times; just before the oral care program, weekly during, and at the end of the oral care program. The groups of patients, who were given the oral care, especially the group of patients cared with sponge brushes every day, showed a significant reduction in Candida scores but not in VSC concentration and tongue coating scores. The present method of oral care proved effective in cleaning the tongue and oral mucosa, and the Candida scores appeared to be a reliable indicator for evaluation. It is suggested that this way of oral care is simple, easy and useful not only for the elderly at a nursing home but for the house-bound elder people who will rapidly increase in the near future in Japan.
