RESUMEN
The results of the treatment of 250 patients with a carbuncle of various location were analyzed retrospectively. A differential approach to choice of surgical method with consideration of the degree, phase and localization of inflammation is preferable. Early and radical surgery, antimicrobic drugs, infusion therapy make base of up-to-date and adequate treatment. In the phase of infiltration deep incision often stops the disease or promotes a decrease of the necrotic zone. In suppurations linear incisions were made only on the face for cosmetic reasons. In other areas a few large connected T-, [symbol: see text]-, [symbol: see text]- or cross-formed incisions were preferable. In 30 cases I-formed incision with two rectangular flaps was used.

Results of clinical trial of the new pyrimidine derivative--ximedone are presented. The drug was administered per os 0.5 g x 4 times a day (0.5 h before meals). Total course dose achieved 60 g. Ximedone implementation for complex treatment of burns, trophic ulcer of lower limbs, different inflammations demonstrated positive results. Prophylaxy administration of the drug in abdominal surgery at acute appendix resulted with lower incidence of postoperative complications. High efficacy and good tolerability of the drug allows to recommend it for wide medical use.

It was established in experiments on albino unbred mice and during treatment of patients with osteomyelitis that 30 mg/kg of ximidon suppresses the formation of micronuclear polychromatophilic erythrocytes found in the bone marrow of mice and peripheral blood of patients with chronic osteomyelitis. The interrelationship of the results obtained with the modulating effect of ximidon on the mitochondrial, thiol, and adenylate cyclase-dependent mechanisms of cell regulation is discussed.

The authors present data based on the analysis of diagnosis and treatment of 11 patients with sternal osteomyelitis which in 9 patients was of traumatic origin and in 2 was hematogenous. Early diagnosis is difficult. Clinical manifestations and roentgenography of the sternum are essential for diagnosis. In combined treatment of osteomyelitis preference is given to radical removal of osteomyelitic masses within healthy tissues with tamponade of the residual osteal cavity by local tissues.

During 30 years of practical work the authors observed 6 patients with anaerobic mephitic gangrene after amputation of the extremity for atherosclerotic (in 3 patients), diabetic (in 2 patients) and vast wounds of the foot. Among the most substantial noninfective causes of the development of anaerobic mephitic gangrene in the postoperative period there were continuous preoperative ischemia of the extremity against the background of a decreased level of haemoglobin, total protein, lymphocytes. An early operation and intensive complex specific and nonspecific therapy resulted in recovery of 3 patients.

Single injection of hydrocortisone to guinea-pigs provoked a remarkable transitory T-lymphocytopenia with transition of T- lymphocytes into the bone marrow during the early stages. On the contrary, the content of B-lymphocytes in the peripheral blood gradually rose and exceeded the normal level twice as much by the end of the first day. Histological analysis did not reveal any signs of massive destruction of lymphocytes. It is suggested that the redistribution mechanism lies at the basis of changes in the lymphocyte population ratio induced by corticosteroid administration.

From the observations on 18 patients and the experiments on 77 dogs with experimental pancreatitis neuro-humoral interrelations and hydrocortisone effect upon the respiratory function have been studied. Both in the clinic and in the experiment the authors have found the intermediate metabolism disturbed: adrenaline-like substances blood level rises, while the cholinergic complex blood level drops dramatically. The conclusion on a vital importance of a consecutive neuro-humoral correction with the use of cholinomimetics and corticosteroids in pancreatitis is drawn.
