RESUMEN

Three new minor steroidal glycosides were isolated from the whole plants of Hoya parasitica (Wall. ex Hornem.) Wight. Their structures were further elucidated as 3ß,4α-dihydroxy-5ß-spirost-(25)27-en-1ß-yl O-α-L-arabinopyranoside (1), 3ß-hydroxy-5ß-spirost-25(27)-en-1ß-yl O-α-L-rhamnopyranosyl-(1→2)-ß-D-xylopyranoside (2), and (23S,24S,25S)-3ß,4α,23,24-tetrahydroxy-5ß-spirostan-1ß-yl O-α-L-rhamnopyranosyl-(1→2)-ß-D-fucopyranoside (3) through interpretation of the spectroscopic data (one-dimensional and two-dimensional) and mass spectrometry (HR-ESI-MS). To the best of our knowledge, this is the first report of the isolation of spirostane-type steroidal saponins from the Hoya genus.

Introduction: Telepharmacy, the application of information and communication technologies in healthcare services, has been adopted in many countries to provide patients with pharmaceutical care. However, it has yet to be widely used in Vietnam. This study was conducted to assess the current status of use and the factors associated with the willingness to use telepharmacy of pharmacists in Vietnam. Methods: A descriptive cross-sectional study was conducted from February to July 2021; 414 pharmacists were recruited to fill in an online survey. Results: Overall, 86.7% of participants have used telepharmacy application and 87.2% of them were willing to apply telepharmacy in pharmacy practice. According to our multivariate analysis, the level of readiness was associated with positive attitude (odds ratio [OR] = 4.67; 95% confidence interval [CI]: 2.26-9.66), and a good behavior (OR = 11.34; 95% CI: 3.84-33.45). Discussion: Developing a telepharmacy system with appropriate features is essential to meet the requirements of pharmacy practice amid the spread of the COVID-19 pandemic.

BACKGROUND: Asymptomatic transmission was found to be the Achilles' heel of the symptom-based screening strategy, necessitating the implementation of mass testing to efficiently contain the transmission of COVID-19 pandemic. However, the global shortage of molecular reagents and the low throughput of available realtime PCR facilities were major limiting factors. METHODS: A novel semi-nested and heptaplex (7-plex) RT-PCR assay with melting analysis for detection of SARS-CoV-2 RNA has been established for either individual testing or 96-sample pooled testing. The complex melting spectrum collected from the heptaplex RT-PCR amplicons was interpreted with the support of an artificial intelligence algorithm for the detection of SARS-CoV-2 RNA. The analytical and clinical performance of the semi-nested RT-PCR assay was evaluated using RNAs synthesized in-vitro and those isolated from nasopharyngeal samples. RESULTS: The LOD of the assay for individual testing was estimated to be 7.2 copies/reaction. Clinical performance evaluation indicated a sensitivity of 100% (95% CI: 97.83-100) and a specificity of 99.87% (95% CI: 99.55-99.98). More importantly, the assay supports a breakthrough sample pooling method, which makes possible parallel screening of up to 96 samples in one real-time PCR well without loss of sensitivity. As a result, up to 8,820 individual pre-amplified samples could be screened for SARS-CoV-2 within each 96-well plate of realtime PCR using the pooled testing procedure. CONCLUSION: The novel semi-nested RT-PCR assay provides a solution for highly multiplex (7-plex) detection of SARS-CoV-2 and enables 96-sample pooled detection for increase of testing capacity. .

BACKGROUND: Breast cancer (BC) is the most common cancer for women all over the world. Great interests have been paid to discover accurate and noninvasive methods for breast cancer diagnosis and prognosis. Although the diagnostic and prognostic value of microRNA-200 (miRNA- 200, miR-200) family has been revealed in many studies, the results were inconsistent. Thus, this meta-analysis aims to assess the overall value of miRNA-200 family in breast cancer diagnosis and prognosis. METHOD: Relevant studies were searched from the following databases: PubMed, PMC, EMBASE, and ScienceDirect using key words: ("miRNA-200 family" or "miR-141" or "miR-200a" or "miR-200b" or "miR-200c" or "miR-429") and ("HER2" or "Luminal A" or "Luminal B" or "TNBC") and ("breast cancers" or "breast carcinoma" or "breast malignancy" or "breast tumor"). The sensitivity, specificity, AUC were then calculated to estimate the diagnostic accuracy of the miR-200 family. As for the prognostic value of the miR-200 family, the pooled hazard ratio (HR) was assessed. Heterogeneity among individual studies was also examined by subgroup analyses. RESULT: A total of 24 articles were included in the meta-analysis. The diagnostic value of miR-200s in BC was presented by the pooled sensitivity was 0.86 (95% CI: 0.83-0.88); the pooled specificity was 0.82 (95% CI: 0.72-0.89); the pooled AUC was 0.931 (95% CI: 0.919-0.942). Besides, expression of miR-200s in metastatic breast cancer has sensitivity, specificity and AUC of 0.70 (95%CI: 0.56-0.81), 0.72 (95%CI: 0.61-0.81), and 0.814 (95%CI: 0.741-0.903), respectively. The meta-analysis then revealed that high expression of miR-200 family corresponded to poor OS (HR: 1.63, 95% CI: 1.03-2.52), poor DFS (HR: 1.55, 95% CI: 0.95-2.56) in BC patients while downregulation of miRNA-200s corresponded to poor OS (HR= 0.84, 95%CI: 0.46-1.63) in TNBC patients and poor OS (HR=0.49; 95%CI: 0.27-0.88) in luminal BC patient. CONCLUSION: The MiR-200 family has high diagnostic accuracy and can be used as an important biomarker to prognosticate breast cancer.

Objective: To assess the acceptance of coronavirus disease (COVID-19) vaccine among healthcare workers at two general hospitals in Vietnam when it is available. Methods: A cross-sectional study was conducted using a convenience sampling from January to February 2021 among 410 healthcare workers at two general hospitals in Vietnam via a self-administered questionnaire. A multivariable regression analysis was performed to determine predictors of vaccine acceptance including the demographic factors, COVID-19 knowledge, and vaccine beliefs based on the domains of Health Belief Model. Results: Among 410 healthcare workers, 76.10% showed vaccination willingness. Predictors of acceptance were determined that the group reporting as 'vaccine acceptance' was more likely to be positive towards the perceived susceptibility and severity of COVID-19 (OR 2.45; 95% CI 1.48-4.06, P<0.05), perceived benefits of vaccination, and cues to action (OR 4.36; 95% CI 2.35-8.09, and OR 5.49; 95% CI 2.84-10.61, respectively, all P<0.001), but less likely to have the perceived barriers to vaccination (OR 0.19; 95% CI 0.09-0.38; P<0.001) compared with the no acceptance group. Besides, people who had a good knowledge regarding the severity of illness were 3.37 times more likely to have identified as vaccine acceptance (OR 3.37; 95% CI 1.04-10.86, P<0.05). The demographic factors were also associated with willingness to receive the vaccine, with participants who were staff and received COVID-19 information from relatives were less likely to accept the vaccine over those who were doctors and not receiving information from relatives (OR 0.36; 95% CI 0.13-0.96, and OR 0.37; 95% CI 0.17-0.78, respectively, all P<0.05). Conclusions: A rate of willingness to get vaccinated against COVID-19 was relatively high with discrepancies between occupation, receiving information from relatives, knowledge toward the severity of illness, and the elements of Health Belief Model. The findings will provide information for the management authorities to develop relevant interventions to promote COVID-19 vaccination uptake.

RESUMEN

The pandemic COVID-19 caused by the zoonotic virus SARS-CoV-2 has devastated countries worldwide, infecting more than 4.5 million people and leading to more than 300,000 deaths. Whole genome sequencing (WGS) is an effective tool to monitor emerging strains and provide information for intervention, thus help to inform outbreak control decisions. Here, we reported the first effort to sequence and de novo assemble the whole genome of SARS-CoV-2 using PacBios SMRT sequencing technology in Vietnam. We also presented the annotation results and a brief analysis of the variants found in our SARS-CoV-2 strain, which was isolated from a Vietnamese patient. The sequencing was successfully completed and de novo assembled in less than 30 hours, resulting in one contig with no gap and a length of 29,766 bp. All detected variants as compared to the NCBI reference were highly accurate as confirmed by Sanger sequencing. The results have shown the potential of long read sequencing to provide high quality WGS data to support public health responses, and advance understanding of this and future pandemics.

This paper describes the route, from simulations toward experiments, for optimizing the magnetoelectric (ME) geometries for vortex magnetic field sensors. The research is performed on the base of the Metglas/Piezoelectric (PZT) laminates in both open and closed magnetic circuit (OMC and CMC) geometries with different widths (W), lengths (L), and diameters (D). Among these geometries, the CMC laminates demonstrate advantages not only in their magnetic flux distribution, but also in their sensitivity and in their independence of the position of the vortex center. In addition, the ME voltage signal is found to be enhanced by increasing the magnetostrictive volume fraction. Optimal issues are incorporated to realize a CMC-based ME double sandwich current sensor in the ring shape with D × W = 6 mm × 1.5 mm and four layers of Metglas. At the resonant frequency of 174.4 kHz, this sensor exhibits the record sensitivity of 5.426 V/A as compared to variety of devices such as the CMC ME sensor family, fluxgate, magnetoresistive, and Hall-effect-based devices. It opens a potential to commercialize a new generation of ME-based current and (or) vortex magnetic sensors.

Vietnam is facing a problem of over-crowding in public hospitals. Long waiting time is a major dissatisfaction for patients. Reducing waiting time benefits not only patients but also hospitals in decreasing overall workload. To identify factors contributing to long waiting time in outpatient clinics, a cross-sectional study was conducted at a national hospital in Vietnam. The time for each process of 7,931 patients who visited the outpatient clinics within Thong Nhat Hospital from 5 to 9 September 2016 was collected from the computerized clinical database. Characteristics of patients (age, sex, address, day of visit, registration time, visited department and test) and waiting times were studied at the outcome measures. Multivariate analyses using the linear regression model was carried out to evaluate the contribution of these factors to the total waiting time. Among 7,931 patients, 52.3% were women, 46.3% were 60-80 years, and 64.8% registered between 5:30 and 9:30. The mean total waiting time was 104.1 minutes. The analysis on total waiting time among 4,564 patients who visited one department without any test showed that department group and registration time were significantly associated with a total waiting time. In 1,259 patients who visited one department with one test, early registration and undergoing blood tests were significantly associated with a longer total waiting time. These results showed that old age, visiting internal medicine departments, early registration time, and undergoing blood tests were factors contributing to a longer total waiting time in the outpatient clinics within Thong Nhat Hospital.

Antibiotic residues in aquatic products in Vietnam were investigated. A total of 511 fish and shrimp samples were collected from markets in Ho Chi Minh City (HCMC), Thai Binh (TB), and Nha Trang (NT) from July 2013 to October 2015. The samples were extracted with 2% formic acid in acetonitrile and washed with dispersive C18 sorbent. Thirty-two antibiotics were analyzed by LC-MS/MS. Of the 362 samples from HCMC, antibiotic residues were found in 53 samples. Enrofloxacin was commonly detected, at a rate of 10.8%. In contrast, samples from TB and NT were less contaminated: only 1 of 118 analyzed samples showed residues in TB and only 1 of 31 showed residues in NT. These differences were attributed to the local manufacturing/distribution systems. To understand the current status of antibiotic use and prevent adverse effects that may be caused by their overuse, continual monitoring is required.

Background: Identifying the factors which affect the invasion and metastasis process takes a vital role in the treatment of cancer. This is the first study on the relationship between the expression of matrix metalloproteinase (MMP \u2013 9), invasion and metastasis of nasopharyngeal carcinoma (NPC) in Vietnam. Objectives: To evaluate the significance of matrix metalloproteinase \u2013 9 in predicting the metastasis potential of NPC. Subject and Method: The expression of MMP \u2013 9 was studied by immunohistochemical analysis in 33 nasopharyngeal carcinoma sections and the associations of MMP-9 with lymph nodes metastasis and clinical features. Results: Positive immunostaining for MMP-9 was observed in 94.94% of the NPCs, 39.4% of the cases being extensively positive (+++). The expression of MMP-9 correlated significantly with cervical lymph nodes metastasis (p<0.05). 5/16 cases (31.25% of the cases) without cervical lymph nodes metastasis expressed MMP-9 with 2+ or 3+ level, non-significant of MMP-9 expression with T classification and clinical stage. Conclusions: The expression of MMP-9 often goes along with lymph nodes metastasis in nasopharyngeal carcinoma.

Background: Tophi is an important symptom of chronic gout. Objective: To assess clinical characteristics of topbi and its relationships with other manifestations of chronic gout. Subjects and method: A study was conducted in 65 patients diagnosed chronic gout based on Bennett and Wood criteria 1968, with tophi, treated at Department of Rheumatology, Bach Mai hospital, from December 2003 to June 2004. This was a retrospective, prospective, cross-sectional study. Results and conclusions: All patients were males and the average age was 57.8 \xb1 11.71 years. 72.3% of patients aged from 30 to 59 years. The average time of occurrence of tophi in chronic gout was 7.73 years. The mean number of tophi was 24.8 \xb1 9.4 (ranged from 1 to 70). Erosion of tophi occurred 25%. 92.9% of patients had distribution in inferiors limbs and 80.4% had superior limbs. There were relationships between number of tophi and renal lesions, distribution of tophi and number of painful joints.

Background: Multiple myeloma is a debilitating malignancy that is part of a spectrum of diseases ranging from monoclonal gammopathy of unknown significance to plasma cell leukemia. Objective: To evaluate the effectiveness in treatment of multiple myeloma with VAD. Subjects and method: A study included 26 patients with multiple myeloma diagnosed according to Tubiana - 1993 criteria, who treated by VAD at Bach Mai Hospital, from January 1998 to December 2006. This was a retrospective combined to longitudinal follow-up examination study. Results: Response rate to treatment was 75%, complete response rate was 41.7%. Remission rate was 91.67%, complete remission rate was 66.7%. The parameters such as hemoglobin, speed of blood precipition in the first hour, globulin, and creatinine changed statistically significant after treatment. The average survival time of the patients was 30.13 \xb1 4.8 months. Survival rate of 1 year after diagnosis was 80%, at 3 years after diagnosis was 26.67% and more than 5 years after diagnosis was 6.67%. Conclusion: VAD had relatively good results in treatment of multiple myeloma.

RESUMEN

Background: Sodium bicarbonate is highly alkaline and may be helpful in relieving symptoms of gout. Sodium bicarbonate helps to adjust your body's pH level, slightly creating an alkaline-forming state that lessens the likelihood of gout attacks. We can prepare sodium bicarbonate in several ways to treat gout symptoms. Objective: To examine the efficacy of sodium bicarbonate for treatment of gout. Subjects and method: A study was conducted in 54 patients with gout at Department of Rheumatology of Bach Mai hospital, from September 2006 to March 2007. Patients were divided into two groups, group 1 included 29 patients treated with intravenous sodium bicarbonate and group 2 included 25 patients treated with oral sodium bicarbonate. This was a descriptive, cross-sectional study. Results: No significant changes showed between serum uric acid level, 24-hour urine uric acid level at before and after treatment in 2 groups. Gout patients with renal failure had increased level of 24-hour urine uric acid after treatment (p < 0.05). There were significantly increased level of urine volume, urine pH and decreased level of urine specific gravity after treatment. The effect of sodium bicarbonate was not different between intravenous usage and oral usage. Conclusion: Sodium bicarbonate could change urine environment, limit crystallization of urine uric acid of gout patients.

Background: In Vietnam the juxta-articular soft tissue diseasesare very popular, but they have not been studied yet. Objectives: (1) Describe clinical feature of some juxta-articular soft tissue diseases (tendinopathy) (except periarthritis of the shoulder). (2) Determine the risk factors of these diseases. Subjects and method:118 patients with juxta-articular soft tissue diseases. Diagnosed following the standard criteria and treated in Rheumatology Department Bach Mai hospital (Hanoi - Vietnam) in the period from December 2005 to August 2006 There was a prospective descriptive method of study.Results:(1) Clinical feature of some juxta-articular soft tissue diseases: average age: 50.2 \xb1 10.4 years, female/male ration: 4:1, the most frequent clinical forms: Tendinosis of the Elbow (Tennis Elbow) (25.4%), de Quervain\u2019s tenosynovitis (22.9%), trigger finger (17.8%), Pes Anserine Bursitis (23.7%). Clinical symtoms: pain intensity: average VAS score: 5,8 \xb1 1,3 mild and moderate limitation of movement, inflammatory signs were not evident: Mild swelling (55%), redness, increase of local temperature (4-5%), pain character: Tendinosis of the Elbow and Pes Anserine Bursitis: permanent mild pain (73.6% and 68%), de Quervain\u2019s tenosynovitis and trigger finger: pain with irrdiation along the tendon (100%). (2)The risk factors ofthese diseases: manual and repetitive work, care of children (74%), middle age (73%), female (80,5%), osteoarthritis (21.2%), diabetes associated with trigger finger (19%).Conclusion: (1) The juxta-articular soft tissue diseases were seen at middle age, with feminine preponderance, moderate pain, not evident inflammation. Clinical forms: the prevalence of each group odd diseases (Tendinosis of the Elbow, de Quervain\u2019s tenosynovitis, trigger finger, Pes Anserine Bursitis) are quite eqiuvalent. (2) The risk factors of these diseases: manual and repetitive work, care of children, middle age, female, osteoarthritis, diabetes.

Background: Gougerot-Sj\xf6gren syndrome is an autoimmune disorder with two remarkable symptoms such as dry eyes and dry mouth. Objective: To study the symptoms of dry mouth of Gougerot-Sj\xf6gren syndrome after rheumatoid arthritis; To evaluate clinical and subclinical manifestations of dry mouth. Subjects and method: A prospective, descriptive, cross-sectional study included 160 patients with rheumatoid arthritis, who treated at Department of Rheumatology of Bach Mai hospital, from 1998 to 2003. The patients were divided into 2 groups: 60 patients with Gougerot-Sj\xf6gren syndrome and 100 patients without Gougerot-Sj\xf6gren syndrome. Results: The average age of patients with Gougerot-Sj\xf6gren syndrome was 54.55 \xb1 10.91 years. The mean time of having dry mouth was 8.65 \xb1 8.39 months. Clinical manifestations of dry mouth were sensation of dry mouth (90%), lost of saliva (80%), drink a lot of water while eating (58.3%), enlargement of parotid glands (15%). Degrees of dry mouth were mild and moderate (36.7%), severe (53.3%). 93.3% of patients decreased total salivary flow.82% of patients had 3 and 4 anatomopathologic stages according to Chilsom classification. Conclusion: Incidence of symptoms of dry mouth was higher significantly in the rheumatoid arthritis patients with Gougerot-Sj\xf6gren syndrome than those without Gougerot-Sj\xf6gren syndrome.

RESUMEN

Background: Tophi gout refers to the condition, where consistently high level of uric acid in the body causes the deposition of uric acid or monosodium urate crystals on the joints. Objective: To describe clinical and subclinical features of tophi gout. Subjects and method: A study conducted in 51 patients with tophi gout (according to Bennett-Wood criteria - 1968) in Department of Rheumatology, Bach Mai hospital, from February 2005 to February 2006. This was a cross-sectional prospective study. Results: The average time of tophi appearance lasted 5.3 \xb1 5.7 years. The average number of tophi was 4.5 \xb1 3.9. The most common positions of tophi were tarso - metatarsal - phalangeal joints and elbow joints (43-47%). 100% had gouty arthritis (in which 66.6% > 4 joints), 50-55% involved tarso - metatarsal - phalangeal joints (43.1% had deforming joints). On X-ray, 62.7% cases has injuries, para-articular erosions and overhanging margins (62.5%), joints space narrowing (71.9%), proliferation osseous change (43.8%). Renal stones examined by ultrasound were 41%, renal failure 24.6%. Blood analysis showed hyperuricemia (88.2%), hypertriglyceridemia (54.9%), hyperglycaemia (13.6%). Conclusions: Common features tophi gout were injuries in tarso - metatarsal - phalangeal joints; gouty polyarthritis over 4 joints. X-ray images showed para-articular erosions, joints space narrowing, proliferation osseous changes.

Background: Wrist joint damage is early, common symptom and in diagnostic criteria of rheumatoid arthritis. Objective: to describe characteristics of MRI of the wrist in rheumatoid arthritis. Subjects and methods: This was a describe study. The study included 41 patients who were diagnosed rheumatoid arthritis according to the criteria of American College of Rheumatology- 1987 from January to October - 2006. Results: The average disease duration was 16.4 \xb114.71months. According to Rheumatoid Arthritis Magnetic Resonance Imaging Score (RAMRIS) 2002, damages on MRI included: 100% of patients had synovitis (5.9 \xb1 2.20), 100% of patients had bone erosions (15.3 \xb1 11.06) and 90% of patients had bone edema (7.3 \xb1 5.86), total score 28.6 \xb1 16.58. The correlative coefficient between synovitis and bone edema: r=0.43, P<0.01, between synovitis and bone erosions:r=0.61, P<0.01, between bone erosions and bone edema: r=0.55, P<0.01. MRI revealed bone erosions in 100% of patients having rheumatoid arthritis less six months after symptom onset compared with 0% of patients on radiography. In patients with over 2 years of disease duration, this rate on MRI was 100% of patients compared with 87.5% of patients on radiography. RESUMEN

Background: Rheumatoid arthritis (RA) is a chronic autoimmune disease that causes inflammation and deformity of the joints. Objectives: The study have two purposes: (1) Identify the sensitivity of anti - CCP2 antibodies in rheumatoid arthritis. (2) Identify the correlation between anti - CCP2 antibodies and clinical manifestations, laboratory features of rheumatoid arthritis. Subjects and method: The study was carried out on 70 patients with rheumatoid arthritis from March to July 2006 in rheumatology department at Bach Mai hospital. All they were diagnosed according to the criteria of American College of Rheumatology 1987. Results: Results of study showed that: (1) The sensitivity of anti - CCP2 antibodies is 67.1% (2) The sensitivity of anti - CCP2 antibodies in patients with rheumatoid arthritis is 67.1 % and is as high as the sensitivity of rheumatoid factor. In early stage of disease (the duration of disease under 12 months) the sensitivity of anti - CCP2 antibodies is higher than rheumatoid factor (70% versus 57.5%). There are no significant differences between the group having anti - CCP2 antibodies and the group who did not have anti - CCP2 anti- bodies in any stages of disease about Ritchie index, DAS - 28 index, X - rays, erythrocyte sedimentation rate in the first hour, CRP ratio. Conclusions: The sensitivity of anti - CCP2 antibodies in patients with rheumatoid arthritis is 67.1 %. There is the relation between anti - CCP2 antibodies and the severity of the inflammation and the severity of X - rays.

RESUMEN

Background: Sciatic neuralgia syndrome is a quite common condition in clinical of internal medicine diseases. The syndrome is not life threatening but reduce the ability to work and activities of the patients. The disease is more common in men than women and most of working age.\r\n', u'Objectives: To define the clinical characteristics of sciatic neuralgia syndrome. \r\n', u'Subjects and methods: The cross sectional study was carried out on 48 sciatic neuralgia patients aged from 16 to 70, treated at Bach Mai hospital. The research indices were clinical and traditional medicine characteristics of sciatic neuralgia syndrome.\r\n', u'Results: Sciatic neuralgia was common in the 20-39 year old (87.5%) age group. The male to female ratio was 1.7 (p<0.01). The sciatic neuralgia syndrome rose after excessive exercise (50%). In almost hernia cases of vertebral disc on spinal column was detected by MRI (83.3%). The impaired nerve root was L5 (64.6%). Acupuncture sites were on gallbladder meridian (50.0%), bladder meridian (20.8%) and coordinated (29.2%)\r\n', u'Conclusion: The sciatic neuralgia syndrome causing hernia on spinal column was common at 20-59 year old age. The male number was higher than female. Almost sciatic neuralgia patients had noticed pain on the gallbladder meridian (p<0.05).\r\n', u'\r\n', u'

RESUMEN

The prevalence of Chlamydial infection in pregnant women was 8,1%. Pregnant women under 25 years old were at high risk for Chlamydial infection. Chlamydial infection was not associated with gestational age. There is no difference about the prevalence of Chlamydial infection between the rural and urban women. Oral amoxicillin had a efficacy of 82,2% after 1 stage and 86,4% after 2 stages of treatment. The result suggested that amoxicillin is a safe, well tolerated and effective for treatment of Chlamydia in pregnant women.
