OBJETIVO Comparar los resultados clínicos, funcionales e imagenológicos de dos técnicas quirúrgicas para el manejo de fracturas conminutas de patela: con y sin eversión patelar. MÉTODOS En una serie de casos retrospectivos de fracturas conminutas de patela tratadas en el mismo centro entre 2014 y 2017, con un seguimiento ≥ 3 meses, se hizo una comparación entre el grupo con eversión y el grupo sin eversión. Los criterios de exclusión fueron patelectomía parcial o total, reinserción tendínea, o rehabilitación incompleta. Las variables analizadas fueron edad, sexo, tabaquismo, diabetes mellitus, energía del accidente, tipo de fractura, variables quirúrgicas (banda de tensión, tornillos, alambres, nudos, cerclaje circular), rango de movimiento (RDM) articular postoperatorio, presencia de osteosíntesis sintomática, puntaje de escalas funcionales (de Tegner-Lysholm y de Kujala) al alta definitiva, complicaciones (rigidez articular, infección, trombosis venosa profunda), y variables imagenológicos con tomografías computarizadas pre- y postoperatorias (brecha, desnivel articular > 2 mm, elementos de fijación intraarticular). RESULTADOS En total, 20 de 22 pacientes, 13 con eversión y 7 sin eversión, cumplieron con los criterios de selección. El seguimiento fue de 3 a 12 meses, y no hubo diferencias estadísticamente significativas respecto a las variables demográficas entre ambos grupos, lo cual los hace comparables. Destacaron el tiempo desde el ingreso al alta, con 7 meses para los pacientes con eversión y 5 meses para los sin eversión (p = 0.032), la proporción de pacientes con desnivel articular > 2 mm, con 7.7% para los con eversión y 14.3% para los sin eversión (p = 0.016), y una tendencia a resultados superiores en escalas funcionales para el grupo con eversión. CONCLUSIÓN El tratamiento de fracturas conminutas de patela con eversión parece ser una alternativa viable dados sus resultados imagenológicos y funcionales superiores a los de la técnica habitual.


PURPOSE To compare the clinical, functional and imaging outcomes of two surgical techniques for the treatment of comminuted patellar fractures: with and without eversion. METHODS In a retrospective series of cases of comminuted patellar fractures treated at a single center between 2014 and 2017, with a follow-up 3 months , we performed a comparison between the eversion group and the non-eversion group. The exclusion criteria were partial or total patellectomy, tendon reinsertion, or incomplete rehabilitation. The variables analyzed were age, gender, smoking, diabetes mellitus, the energy of the accident, the fracture type, surgical variables (tension band, screws, wires, knots, circular cerclage), postoperative joint range of motion (ROM), presence of symptomatic osteosynthesis, the scores on the functional scales (of Tegner-Lysholm and of Kujala) at the final discharge, complications (joint stiffness, infection, deep vein thrombosis), and pre- and postoperative computed tomography imaging variables (gap, step-off > 2mm, intra-articular fixation elements). RESULTS In total, 20 out of 22 patients, 13 undegoing eversion and 7 not undergoing eversion, met the selection criteria.. The follow-up ranged from 3 to 12 months, and there were no statistically significant differences regarding the demographic variables between both groups, which makes them comparable. The most remarkable results were the time from admission to final discharge, of 7 months for the patients in the eversion group, and of 5 months for those in the non-eversion group (p » 0.032), the proportion of patients with a step-off > 2 mm, with 7.7% for the eversion group and 14.3% for the non-eversion group (p » 0.016), and a tendency towards higher scores in the functional scales for the eversion group. CONCLUSION The treatment of comminuted patellar fractures with eversion seems to be a viable alternative, given its superior imaging and functional results compared to those of the usual technique.

INTRODUCCIÓN: La luxación expuesta de rodilla es compleja, de incidencia baja con grandes secuelas funcionales. Existen escasos reportes de series en la literatura relativos a su manejo y resultados. OBJETIVO: El objetivo de este trabajo fue describir los resultados obtenidos de todos los pacientes con luxación expuesta de rodilla tratados durante las últimas dos décadas en nuestro hospital bajo un mismo estándar de tratamiento. MÉTODO: Estudio descriptivo retrospectivo en una serie quirúrgica de 11 pacientes con luxación expuesta de rodilla, tratados entre 1994 y 2015. Todos fueron estudiados y manejados según esquema estandarizado: Angiografía/ angioTC, aseo quirúrgico, fijador externo y reparación neurovascular en casos necesarios. Revisión de registros clínicos e imagenológicos consignando datos demográficos, lesiones concomitantes, número y tipo de cirugías, y complicaciones asociadas. Seguimiento promedio fue de 10,7 años con evaluación mediante encuesta funcionales SF-12/IKDC durante el mes de marzo de 2015. RESULTADOS: Diez pacientes eran hombres, 1 mujeres. Edad promedio al accidente 38,6 años. Mecanismo lesional de alta energía; como referencia la clasificación de Schenck, 1 lesión III-M, 4 tipo IV y los 6 restantes una tipo V. 4 lesiones vasculares (36.4%) y 7 lesiones neurológicas (63.6%). Tratamiento definitivo consistió en 4 reconstrucciones ligamentarias, 2 prótesis, 3 artrodesis y 2 amputaciones supracondíleas. Evaluaciones funcionales dieron como resultados un puntaje promedio de 37 y 48,5 para SF-12 físico y mental respectivamente, y de 44,1 para IKDC. CONCLUSIÓN: La luxación expuesta de rodilla es una lesión infrecuente, muy compleja, asociada a accidentes de alta energía, con complicaciones severas, lo que determina resultados funcionales relativamente malos. La estandarización permite sistematizar las distintas etapas de atención, racionalizar los recursos disponibles evitando la improvisación en momentos críticos, lo que podría incidir en la obtención de resultados.


INTRODUCTION: Open knee dislocation is a complex lesion of low incidence and large functional sequelae. There are few series reports in the literature regarding its management and outcomes. OBJECTIVE: Describe the results obtained from all patients with open knee dislocation treated during the last two decades in our hospital under the same treatment standard. METHOD: Retrospective descriptive study in a surgical series of 11 patients with open knee dislocation, treated between 1994 and 2015. All were studied and managed according to a standardized protocol: Angiography/angioCT, surgical debridement, external fixation and neurovascular repair in necessary cases. Review of clinical and imaging records, recording demographic data, concomitant injuries, number and type of surgeries and associated complications. Average follow-up was 10.7 years, with an evaluation through functional surveys SF-12/IKDC during March 2015. RESULTS: 10 patients were men, 1 woman. Average age at accident 38.6 years. High-energy trauma mechanism of injury; Schenk classification as reference, 1 lesion type III-M, 4 type IV and the remaining 6 type V. 4 vascular lesions (41.7%) and 7 neurological lesions (63.6%). Definitive treatment consisted in 4 ligamentous reconstructions, 2 prostheses, 3 arthrodesis and 3 supracondylar amputations. Functional evaluations showed an average score of 37 and 48.5 for SF-12 physical and mental respectively, and 44.1 for IKDC. CONCLUSION: Open knee dislocation is a rare, very complex, associated to high-energy trauma, with severe complications, which determines relatively poor functional outcomes. Standardization allows to systematize the different stages of attention and rationalize available resources avoiding improvisation at critical moments, which could affect the results.

El enfrentamiento y el manejo de las fracturas de platillos tibiales de alta energía representan un desafío para todo cirujano. El compromiso de partes blandas y la conminución articular secundaria a la energía involucrada determinan una mayor tasa de complicaciones y morbilidad. OBJETIVOS: Evaluar la tasa de complicaciones en fracturas de platillos tibiales de alta energía. MATERIALES Y MÉTODO: Estudio clínico imagenológico retrospectivo de fracturas de platillo tibial Schatzker V y VI con mecanismo de alta energía, tratadas con fijador externo y osteosíntesis diferida, entre 2007-2013. Análisis estadístico usando STATA 12.0. RESULTADOS: Cincuenta y dos fracturas se presentaron durante el periodo de estudio, 39 de ellas fueron seleccionadas considerando los criterios de inclusión. Promedio edad 42,4 años, seguimiento 41,8 meses. Complicaciones: neuropraxia 5%, infección Schatzker V (20%) 16,2%, VI (80%) 12,1% (p=0,759). Sin asociación entre infección y edad (p=0,6056), mecanismo involucrado (p=0,131), tiempo de prefijador externo (p=0,0556), ni tiempo en el uso de fijador externo (p=0,53). DISCUSIÓN: Las fracturas de platillos tibiales secundarias a traumatismos de alta energía se asocian a un importante compromiso de partes blandas. La reducción y osteosíntesis inmediata determina altas tasas de complicaciones; utilizar fijador externo transitorio las disminuiría. El abordaje anterior en línea media ha mostrado elevadas complicaciones, mientras que el uso de doble abordaje regularía esta situación. CONCLUSIÓN: Al afrontar el tratamiento de las fracturas de platillos tibiales de alta energía debemos considerar el uso de fijador externo transitorio para un adecuado control de la fractura y de las partes blandas para la cirugía definitiva.


The approach and management of high energy tibial plateau fractures is a challenge for every surgeon. Soft tissue injuries and joint comminution due to the energy involved are associated with a higher rate of complications and morbidity. OBJECTIVES: To evaluate the rate of complications in high energy tibial plateau fractures. MATERIALS AND METHODS: A retrospective imaging-clinical study of Schatzker type V and IV high-energy tibial plateau fractures, treated with an external fixation and delayed osteosynthesis, was conducted between 2007 and 2013. Statistical analysis was performed using STATA 12.0. RESULTS: A total of 52 fractures were treated during the study period, of which 39 met the criteria for selection. The mean age of the patients was 42.4 years, with a follow-up time of 41.8 months. The complications include, neuropraxia, 5%, Schatzker V infection (20%), 16.2%, and Vl (80%), 12.1% (P=.759). There was no relationship between infection and age (P=.6056), mechanism involved (P=.131), time between accidents and the installation of an external fixator (P=.0556), or the time used for external fixation (P=.53). DISCUSSION: High-energy tibial plateau fractures are associated with significant soft tissue injuries. Immediate reduction and osteosynthesis are determining factors for high rates of complications, while using transient external fixation should decrease them. A midline anterior approach has shown increased complications, while a double approach could improve this situation. CONCLUSION: The approach to high energy tibial plateau fractures must consider the use of a temporary external fixation for an adequate control of the fracture and the soft tissue for a definitive surgical treatment.

Femoral head fractures associated to hip dislocations are uncommon lesions. Most reports present only short-term results. Objectives: To show functional outcome after a mid term follow up of patients with Pipkin II femoral head fractures treated operatively. Materials and Methods: A retrospective and descriptive evaluation of a 4 consecutive case series (four men, mean age of 36.5 years) treated operatively with an 89-month average follow up (range form 72-108 month). Controlled femoral head dislocation was performed in all patients and two 2.4 mm screws were used for fixation. At final follow up, the Harris Hip Score (HHS) and complications were assessed. Results: No immediate or late complications were reported. The mean HHS was of 95 points (range from 89 to 100 points).No signs of postraumatic arthritis or femoral head osteonecrosis were seen at the last radiographic assessment. Conclusion: in this series of patients with Pipkin II femoral head fractures mid-term functional results are good and no postraumatic arthritis or femoral head osteonecrosis were observed at final follow-up.


Las fracturas de cabeza femoral, asociada a luxación de cadera, corresponden a lesiones infrecuentes. La mayoría de los estudios reportan resultados a corto plazo. Objetivo: Describir el resultado funcional de pacientes con fractura Pipkin II operados con seguimiento a mediano plazo. Material y Métodos: Evaluación retrospectiva y descriptiva de una serie consecutiva de 4 pacientes (cuatro hombres, mediana de edad 36,5 años) operados con un seguimiento de mediana de 89 meses (72-108 meses). En todos los casos se realizó luxación controlada de la cabeza femoral y se utilizaron 2 tornillos de 2,4 mm. Se evaluaron complicaciones y Harris Hip Score (HHS) al último control clínico. Resultados: No se presentaron complicaciones tempranas ni tardías. El HHS promedio fue de 95 puntos (89-100 puntos). La última evaluación radiográfica no demostró signos de artrosis postraumática ni signos de necrosis avascular de cabeza femoral. Conclusión: En este grupo de pacientes con fractura de Pipkin tipo II los resultados funcionales a mediano plazo son buenos, no registrándose en el seguimiento artrosis de cadera postraumática ni necrosis avascular de la cabeza femoral.

Objetivos: Estudiar el comportamiento poblacional de las larvas de Aedes aegypti (L.) para explicar sus fluctuaciones a través de tres indicadores entomológicos (IE) y estimar los casos de dengue en la ciudad de Yurimaguas, Loreto, Perú. Materiales y métodos: Se recogieron los datos de los censos larvales al 100 por ciento en el periodo de estudio a través de tres IEs [índice aédico (IA), índice de recipientes (IR) e índice de Breteau (IB)], así como los casos autóctonos de dengue mensuales de la Oficina de Epidemiología del Hospital de Apoyo de Yurimaguas. Se usó lacorrelación de Spearman para ver la relación entre los tres IE y los casos de dengue, finalmente se calcularonecuaciones de regresión para estimar a partir del los IE los casos de dengue entre el año 2000 al 2004. Resultados: El promedio mensual del IA, IR e IB fue de 4,03±1,85, 1,31±1,07 y 4,88±2,31, respectivamente. Se encontró que los tres índices entomológicos presentaron diferencias entre las dieciocho evaluaciones realizadas del 2000 al 2004. En los tres IE se ha notado una tendencia a descender en los meses de abril-2000 a julio-2003, y un ligero incremento de diciembre-2003 a diciembre-2004. El promedio de casos de dengue mensuales durante todo el estudio fue 8,11±6,66. IA, IB e IR estuvieron todos altamente correlacionados lineal y positivamente durante el 2000 al 2004. Los tres IE y loscasos de dengue total (CDT), se encuentran correlacionados lineal y positivamente. Se establecieron cuatro ecuaciones capaces de estimar los casos de dengue, sólo con el IA. Conclusiones: Teniendo como base a los datos obtenidos en cinco años de evaluación, se proponen cuatro modelos de regresión válidos para estimar CDT mensuales a partir del IA. La ecuación: log (CDT+1) = 0,50 + 0,44 (logIA+1) que la más apropiada para la estimación de los CDT mensuales.


Objectives: To study the population behavior of Aedes aegypti larvae in order to explain its fluctuations using three entomological indexes and estimate dengue fever cases in Yurimaguas, Loreto, Peru. Materials and methods:Data from 100% larval counts for the three-year study period was collected using three entomological indexes: Aedes index, Recipient index and Breteau index, as well as monthly native cases of dengue fever from the Epidemiology Office in Yurimaguas Hospital. Spearman’s correlation test was used for assessing the relationship between the three entomological instruments and dengue fever cases, and ultimately regression equations were used for estimating the number of dengue fever cases occurring between 2000 and 2004 from the entomological indicators. Results: Monthly average values for Aedes index, Recipient index and Breteau index were 4,03 ± 1,85; 1.31 ± 1,07; and 4,88 ± 2,31, respectively. The three entomological indexes showed differences between the eighteen assessments performed from 2000 to 2004. There was a trend for a reduction in the aforementioned indexes from April 2000 to July 2003, and a mild increase from December 2003 to December 2004. Average monthly number of dengue fever cases throughout the study was 8,11 ± 6,66. All the indexes were linearly and positively highly correlated between 2000 and 2004. Four equations capable of estimating the number of dengue fever cases were constructed, only using the Aedes index. Conclusions: Using the data obtained after five years’ assessment, four valid regression models are proposed for estimating the total monthly number of dengue fever cases using the Aedes index as baseline. The equation: log (total of dengue fever cases) + 1) = 0,50 + 0,44 (log Aedes index + 1) was the most appropriate for estimating the monthly number of dengue fever cases.

Objetivos: Describir las características y relaciones de tres indicadores entomológicos (IE) de Aedes aegypti encuanto a su distribución espacial, efecto estacional y tipo de recipiente más común en la ciudad de Yurimaguas, Perú, durante los años 2000 al 2004. Materiales y métodos: Se recogieron los datos de los censos larvales al 100 por ciento en el periodo de estudio a través de tres IE [índice aédico (IA), índice de recipientes (IR) e índice de Breteau (IB)]. Se dividió la ciudad en once zonas, los depósitos se clasificaron en diez categorías y las estaciones en periodo lluvioso (noviembre-abril) y seco (mayo-octubre). Para establecer diferencias entre los censos según meses, años, tipo de recipientes, zonas y estacionalidad se usó la prueba t de student y ANDEVA; y la correlación de Spearman para ver la relación entre los IE. Resultados: Se censaron 10 5421 viviendas y se inspeccionaron 538 802 depósitos. Los tres IE presentaron diferencias entre las once zonas, se notaron valores altos de recipientes positivos para las cincocategorías siguientes: inservibles, llantas, tanque bajo, florero-macetero y barril-cilindro-sansón. Los inservibles correspondieron a 37,37 por ciento de los recipientes positivos; 161 (4,2 por ciento) viviendas presentaron > 3 recurrencias en las inspecciones entre el 2000 al 2004. Los mayores valores de IE se presentaron durante el periodo lluvioso y los tresIE estuvieron relacionados entre sí. Conclusiones: La distribución espacial, el tipo de recipiente más común y el periodo lluvioso son factores que influyen en la dinámica poblacional de los IEs de Ae. aegypti en Yurimaguas, Perú.


Objectives: To define characteristics and associations of three entomological indexes for Aedes aegypti with respect to their spatial distribution, season effect, and most commonly used water recipients in Yurimaguas, Peru, between 2000 and 2004. Materials and Methods: Data from 100% larval censuses for the study period using three entomological indexes: Aedes index, recipient index, and Breteau index. The city was divided in eleven areas, containers were classified in ten categories, and seasons were classified as rainy season (November to April) and dry season (May to October). In order to establish differences between censuses according to months, years, types of containers, zones, and seasonality, the Student’s T test and an ANOVA were used; and Spearman’s correlation was used to assess the relationship between entomological indexes. Results: 10 542 households were surveyed, and 538 802 containers were inspected. The three entomological indexes showed differences between the eleven areas studied. Highly positive values were found for the following five categories: junk, tires, short tanks, vases-flowerpot holders, and barrels-cylinders. Junk accounted for 37,37% of positive containers; 161 (4,2% households had >3 recurrences in inspections between 2000 and 2004. Highest values in entomological indexes were found during the rainy season, and the three entomological indexes were related between them. Conclusions: Spatial distribution, most commonly used containers and rainy season are factors influencing population dynamics of entomological indexes for A. aegypti in Yurimaguas, Peru.

To determine clinical and epidemiological features of scorpion stings in two departments of Colombia, a descriptive study was performed in the hospitals of 10 towns from Antioquia (2 256 071 inhabitants) and five from Tolima (630 424 inhabitants). One hundred and twenty-nine cases were admitted during one year, 51 in Antioquia, 78 in Tolima and 41 were children less than 15 years old. Most stings (70.5%) occurred inside the house; 27.9% were on the hands and 26.4% on the feet. The scorpion species involved were Tityus pachyurus (51), Centruroides gracilis (31), T. fuehrmanni (29), T. asthenes (7) and Chactas spp. (1). In 10 cases the scorpion involved was not identified. Systemic envenoming signs (e.g. vomiting, tachypnea) were significantly more frequent in children than in adults (P < 0.05). Four children had hypertension, but none developed pulmonary oedema. One 3-year-old girl, stung by T. asthenes, had acute oedematous pancreatitis. Ninety-eight patients had mild envenoming. Moderate (27 patients) and severe (four patients) envenoming was significantly more frequent in children than in adults (P = 0.003; relative risk = 2.97). A pepsin-digested anti-Centruroides spp. antivenom was administered to 19 of 31 patients presenting systemic envenoming signs. No adverse reactions to antivenom were observed.

INTRODUCTION: Since the early days, liver transplantation (OLT) has conquered several barriers worldwide to become a proven therapy. We assessed the evolution of our adult liver transplant program. METHODS: We studied all adult patients who underwent OLT since the inception from November 1993 through May 2003. Donor data, recipient pretransplantation evaluation, surgical technique, results, and costs were examined over our evolution, stratifying 3 groups over time, based on the number of adult OLT per year. RESULTS: Between November 1993 and May 2003, 70 OLT were performed in 64 patients older than 15 years of age. Preoperative Child score, preoperative creatinine level, donor and recipient age, and proportion of emergencies were similar in the 3 groups. Over time, the predominant surgical technique was the piggyback technique (97% of OLT) with a decrease in the use of bypass from 63% to 5% during the last time period. Over the 10 years of our program's existence, warm ischemia time has been reduced to less than 1 hour, whereas cold ischemia time has remained constant at around 5 hours. Biliary and vascular complications decreased over time to around 10%. The mean length of hospital stay (LOS) decreased to 12 days (excluding emergencies). Since inception, our 1-year patient survival rate average is 91%; however, in just the last 3 years of our program (2000 through 2003), the 1-year patient survival rate is 97%. CONCLUSIONS: In summary, our surgical technique has evolved toward piggyback use without veno-venous bypass with a significant decrease in warm ischemia times. As expected, our results have improved over time and our LOS and costs have decreased. Finally, our current results are similar to the best ones reported in the medical literature today.

RESUMEN

We report a 58 years old male that developed a bone marrow aplasia associated to the use of ticlopidine, prescribed after coronary artery stenting. The patient developed a pneumonia as a complication. He was admitted to the Intermediate Treatment Unit, receiving wide spectrum antimicrobial therapy and a granulocyte colony stimulating factor (Neupogen(r)) with favourable response. Ticlodipine is an effective anti-platelet agent, but has serious hematological and other side effects. Its prescription requires a close follow up and search for complications

We report a 52 year old man with a pancreatic pseudocyst, that was admitted with severe abdominal pain, severe vomiting, fever and malaise. The clinical picture was considered secondary to a pseudocyst infection and the patient was operated, draining the infected cyst performing a necrosis surgery and pancreatojejunostomy. Forty eight hours after the operation, an ostomy bleeding was detected. A upper mesenteric artery angiography showed two pseudoaneurysms in the gastroduodenal artery, that were embolized. Bleeding stopped initially, but seven days later, it reappeared. The patient was subjected to an emergency pancreatoduodenectomy. Postoperative evolution was uneventful and the patient was discharged two weeks later. Spontaneous bleeding of pseudoaneurysms secondary to chronic pancreatitis is a complication with a 15 to 40% mortality that must be bore in mind.

We have found conclusive evidence for inducible nitric oxide synthase (iNOS) activity in rainbow trout (Oncorhynchus mykiss) tissue by means of biochemical, immunohistochemical, and immunoblotting analyses. This Ca(2+)-independent enzyme uses L-arginine to produce nitric oxide and L-citrulline. It was significantly inhibited by the L-arginine analogs N(omega)-monomethyl-L-arginine and N(G)-nitro-L-arginine methyl ester. Kinetic analyses showed typical Michaelian behavior with no evidence of cooperative effects. The specific activities of the liver and head kidney enzymes were 27 and 106 pmoles. min(-1). mg protein(-1), respectively, with similar values for K(m) (11 microM), all of which correspond well with the values for other previously characterized iNOS. Western blot analyses revealed a single band of M(R) = 130 kDa tested with an iNOS antiserum. At the ultrastructural level, cells with NADPH-diaphorase activity and iNOS immunoreactivity were identified as being heterophilic granulocytes in head kidney tissue and neutrophils and macrophages in hepatic tissue. The presence of an iNOS isoform in these fish tissues implies that these cells are capable of generating nitric oxide, thus pointing to the potential role of this enzyme in fish defense mechanisms.

In Antioquia and Chocó, traditional healers attend 60% of snakebites. With the aim to produce an inventory of the plants used by the healers to treat snakebites and to document the methods of preparation, administration, the dosage, number of patients treated throughout their years of practice with treatment results, 20 healers with experience in Bothrops, Porthidium and Bothriechis envenomations were interviewed between August, 1996 and November, 1998. They belong to nine black and three indigenous rural communities located near the towns of Bojayá, Vigía del Fuerte, Unguía (Atrato river valley), Nuquí and Bahía Solano (Pacific coast). Based on field interviews, 101 species of plants were identified as used to treat snakebites. The part used of each plant varies according to the species. Sixty plants are used in the form of drinks prepared by infusion, decoction or maceration; 78 as external baths on the affected extremity; 11 for steam application and 39 for poultices; the latter is used mainly when the bite is complicated by local necrosis. In mild and moderate envenomations, they generally use a mixture of three plants, while in severe cases they mix from five to 12, a handful of each one. Treatment is generally performed for 1-3 days, when the patient reacts positively. They reported to have treated 454 patients during their years of experience, 20 of them (4.4%) died. With the guidance of the healers, 77 species of plants were collected and photographed. These plants belong to 41 families, of which Piperaceae (13 species), Araceae (six species), Asteraceae (five species) and Gesneriaceae (three species) have the highest number of species.

The presence of nitric-oxide synthase (NOS) in peroxisomes from leaves of pea plants (Pisum sativum L.) was studied. Plant organelles were purified by differential and sucrose density gradient centrifugation. In purified intact peroxisomes a Ca(2+)-dependent NOS activity of 5.61 nmol of L-[(3)H]citrulline mg(-1) protein min(-1) was measured while no activity was detected in mitochondria. The peroxisomal NOS activity was clearly inhibited (60-90%) by different well characterized inhibitors of mammalian NO synthases. The immunoblot analysis of peroxisomes with a polyclonal antibody against the C terminus region of murine iNOS revealed an immunoreactive protein of 130 kDa. Electron microscopy immunogold-labeling confirmed the subcellular localization of NOS in the matrix of peroxisomes as well as in chloroplasts. The presence of NOS in peroxisomes suggests that these oxidative organelles are a cellular source of nitric oxide (NO) and implies new roles for peroxisomes in the cellular signal transduction mechanisms.

The prevalence and the mechanisms of hepatic fibrosis in chronic alcoholic pancreatitis remain uncertain. The aim of this study was to investigate the fibrogenic activity of the liver in patients with chronic pancreatitis and its relation with either the alcohol or cholestasis. Liver biopsies were obtained from 16 patients with chronic pancreatitis at the time of surgery and from 10 organ donors. Samples were processed for histologic examination to assess the presence and extent of fibrosis, inflammatory reactions, and cholestasis- and alcohol-related lesions. In other samples, the collagen content was measured by morphometry, and prolylhydroxylase activity was determined. Liver-function tests, ultrasonography, and endoscopic retrograde cholangiopancreatography were performed before surgery in all the patients. Of patients with chronic pancreatitis, 75% had significantly greater hepatic fibrosis and prolylhydroxylase activity than the control group. Moreover, prolylhydroxylase activity in patients with chronic pancreatitis was higher in those with cholestasis or partial obstruction of the common bile duct than in those without cholestasis or partial obstruction of the common bile duct. Both the fibrogenic activity and the collagen content in the livers of patients with chronic alcoholic pancreatitis are significantly increased, even in those without histologic lesions, and these alterations may be secondary to a partial occlusion of the common bile duct.

The mechanism of fibrogenesis in the pancreas is not well known. We analyzed the role of prolylhydroxylase and collagenase activities in the development of fibrosis in chronic alcoholic pancreatitis (CAP). Nineteen patients with CAP and 11 controls (organ donors) with normal pancreatic histology were included in the study. Pancreatic tissue was obtained from all subjects to measure (a) area of fibrosis (histomorphometric method); (b) prolylhydroxylase activity (PHase), which reflects the intracellular synthesis of collagen (Hutton's method); and (c) collagenase activity, which reflects the degradation of collagen (collagenase assay system, 3H). The percentage of the fibrosis area in relation to the total area of pancreatic tissue was significantly higher in CAP than in the control group (70.6+/-20.2% vs. 4.6+/-1.8%; p<0.001). Mean pancreatic PHase activity was also significantly higher in CAP than in the control group (775+/-258 cpm/mg protein/h vs. 405+/-151 cpm/mg protein/h; p<0.001). The collagenase activity was significantly lower in CAP than in the control group (8.7+/-3.5 cpm/cpm added/mg protein vs. 18.0+/-3.9 cpm/cpm added/mg protein; p<0.001). A significant correlation was observed between percentage fibrosis evaluated histomorphometrically and PHase activity in all patients (r = 0.72; p<0.001), and between PHase and collagenase activities in controls (r = 0.70; p = 0.024), but not in CAP. Pancreatic tissue in CAP has an increased fibrogenic activity and an impaired collagen-degradation capacity. These findings might explain the excessive development of fibrosis in CAP.

RESUMEN

An outbreak of 25 cases of Andes virus-associated hantavirus pulmonary syndrome (HPS) was recognized in southern Chile from July 1997 through January 1998. In addition to the HPS patients, three persons with mild hantaviral disease and one person with asymptomatic acute infection were identified. Epidemiologic studies suggested person-to-person transmission in two of three family clusters. Ecologic studies showed very high densities of several species of sigmodontine rodents in the area.

Neuronal and inducible nitric oxide synthase (nNOS and iNOS) and nitrotyrosine immunoreactivities were localized and semiquantitatively assessed in the cerebral cortex of aged rats by means of light microscopic immunocytochemistry and Western blotting, using a new series of specific polyclonal antibodies. In the aged rats the strongly nNOS-immunoreactive multipolar neurons found in layers II-VI of the cortex of young rats were seen in similar numbers, but showed varicose, vacuolated, and fragmented processes, with an irregular outline and loss of spines. A large number of more weakly nNOS-positive neurons, characterized by a ring of immunoreactive cytoplasm, and not seen in young rats, were observed in layers II-VI of aged rat cortex. While no iNOS-immunopositive neurons were found in the cortex of young rats, a large number of such neurons appeared throughout the aged rat cortex. Nitrotyrosine-positive cells outnumbered total NOS-positive neurons in the cortex of young rats, but this relation was inverted in the aged rats, although these showed a slight increase in the number and staining intensity of nitrotyrosine-positive cells. Western blots of brain extracts showed a several-fold increase in both nNOS- and iNOS-immunoreactive bands in the aged rat, but a less marked increase in nitrotyrosine-containing proteins. The results suggest that while nNOS and iNOS expression is substantially increased in the aged rat cortex, this is not necessarily accompanied by a proportionate increase in nitric oxide synthesis. The mechanisms underlying the increased expression of nNOS and iNOS, and the functional implications of this increase, require elucidation.

BACKGROUND: The prevalence of chronic pancreatitis in Chile is not known and there is no local information about the surgical treatment of this disease. AIM: To review retrospectively the results of surgical treatment of chronic pancreatitis. MATERIAL AND METHODS: The charts of 17 patients (12 male), aged 7 to 65 years old, with chronic pancreatitis that were operated in three different Chilean regions, were reviewed. RESULTS: Seven patients had previous endoscopic therapeutic procedures (papillotomy in 4 and stent placing in 3). Seven patients had been subjected to previous biliary surgical procedures. Indications for surgery were severe pain in 14 patients, the suspicion of a pancreatic carcinoma in 4 patients, an infected pseudocyst in one and massive bleeding of multiple pseudo-aneurysms in a pseudocyst in one patient. Twelve patients were subjected to decompressions and 5 to pancreatic resections. There was no operative mortality and one transient pancreatic fistula. After an average follow up of 22 months, pain improved in 94% of cases, pancreatic cancer was diagnosed in one patient and 79% of subjects gained weight. One patient became insulin dependent, one increased his insulin requirements and one had transient steatorrhea, since she could not afford pancreatic enzyme replacement therapy. CONCLUSIONS: The multidisciplinary approach of patients with chronic pancreatitis, with selective use of surgery, may greatly improve their quality of life.
