Introduction: Maintaining healthy weight is a challenge for all children, and particularly for children with IDD compared to nondisabled children and for Latino children compared to non-Latino White children. Parenting practices related to food intake and physical activity have been found to be important in maintaining children's weight. In this study, we describe the prevalence of overweight and obesity status among Latino children with IDD and their maternal caregivers and determine the relationship between food and physical activity parenting practices and childhood obesity among Latino children with IDD. Methods: We interviewed 94 Latino parent/child dyads and collected information about parenting practices, home environment, and parent and child height and weight using standardized measures. Parent body mass index (BMI) and child BMI percentile were calculated from height and weight. Results: The combined overweight/obesity status for children in our sample was high (60.3%) compared to national rates among nondisabled Latino children (56%) and non-Latino White children with autism (37%). Contrary to research on nondisabled children, we found that greater parental use of controlling dietary strategies was associated with lower BMI percentile in Latino children with IDD. These findings may be indicative of the fact that children with IDD tend to have unique dietary behaviors that warrant more disability and culturally sensitive strategies. Discussion: Our findings suggest that overweight and obesity is especially prevalent for Latino children with IDD and that more research is needed on family factors that promote health in Latino families of children with IDD.

Parent-focused interventions have been designed to provide training and support to caregivers who are essential in achieving positive outcomes for children with intellectual and developmental disabilities (IDD). In 2020, significant crises, including the COVID-19 pandemic and continued racial tensions, profoundly impacted the livelihood of children with IDD and their families. Many ongoing efforts to address disparities among this population were halted temporarily and required further adaptations. Researchers adapted interventions and support to address the disparities impacting children with IDD and their families with limited guidance. We provide a descriptive case analysis of four parent-focused interventions that responded to the global crises to continue serving children with IDD and their families. The four distinct programs were based on applied behavior analysis and naturalistic, developmental-behavioral paradigms that were culturally adapted for families of young children with IDD from diverse cultural and socioeconomic backgrounds. We present the qualitative reports on the challenges and benefits that arose with adapting the four parent-focused interventions for telehealth implementation. We focused specifically on adaptations made in recruitment and retention, instrumentation and measurement, research staff training, and intervention delivery. We synthesize our experience with challenges and solutions in adapting parent-focused interventions for racially/ethnically and socioeconomically diverse children with IDD and their families. We conclude with recommendations for researchers and practitioners on methods for adapting parent-focused interventions to address the significant health disparities that impact racially, ethnically, and socioeconomically diverse children with IDD and their families.

Abstract Life support withdrawal can be a challenging decision, but it should be considered as an option when death is inevitable or recovery to an acceptable quality of life is not possible. The process is beset by obstacles that must be overcome to finally offer patients comfort and a peaceful death. In this article, we offer a series of tools that seek to solve the challenges of palliative extubation, as well as a protocol that could facilitate the decision to withdraw life support, making palliative extubation an alternative to consider instead of artificially prolonging life at the expense of unacceptable human and economic costs.


Resumen La interrupción de la asistencia vital puede ser una decisión complicada, aun cuando se debe considerar como una opción cuando la muerte es inevitable o la recuperación a una calidad de vida aceptable no es posible. A lo largo del proceso se encuentran obstáculos que se deben sortear para finalmente ofrecer a los pacientes una muerte tranquila y confortable. En este artículo ofrecemos una serie de herramientas que buscan solucionar los desafíos de la extubación paliativa y presentamos una guía de extubación que podría facilitar la decisión de retiro del soporte vital, haciendo de la extubación paliativa una alternativa por considerar en lugar de prolongar la vida de manera artificial a expensas de un costo humano y económico inaceptable.

The COVID-19 pandemic has impacted the entire world in unprecedented ways. However, populations that have had a history of marginalization have experienced a more profound impact. One such group is Latinx families of children with intellectual and developmental disabilities (IDD) in the Unites States. In this study, we used a mixed methods approach to explore the impact of the pandemic on the mental health and well-being of Latinx caregivers of children with IDD. Specifically, we (1) identified which social determinants of health are correlated with maternal caregivers perceived general health, mental health, and well-being; (2) explored the impact of the pandemic on families' overall eating and physical activity routines; and (3) identified emergent themes from caregivers' experiences during the pandemic. Thirty-seven Latinx caregivers participated in three interviews in which several validated instruments were administered. The results indicated that perceived social support, annual family income, food security, and receipt of financial benefits were correlated with fewer depressive symptoms. Annual family income was also significantly correlated with perceived general health. Most caregivers reported that the pandemic had placed a strain on their economic situation; increased their isolation; and disrupted their child's therapeutic supports, online education, eating routines, and engagement in physical activity. Meanwhile, some caregivers reported positive changes as a result of the pandemic. Implications for future research and practice are discussed.

A timely diagnosis of autism spectrum disorder (ASD) is important for children as that may help inform the best services and supports needed for optimal outcomes. However, disparities in the identification of ASD have been consistently documented for racial/ethnic minority and immigrant populations. The majority of studies of immigrant populations have focused on the qualitative experience of ASD, however, greater knowledge about the predictors of the age of initial ASD diagnosis is needed. This study examined the child, maternal, and family-level factors that predicted the age of initial ASD diagnosis in a diverse clinical sample through a retrospective medical record review. Medical records of clinical evaluations conducted between 2004 and 2014 were reviewed for children with ASD born to immigrant and non-immigrant mothers. Regression analyses found that for both groups of children with ASD, using verbal language to communicate and having another diagnosis (attention-deficit/hyperactivity disorder or another condition) predicted an older age of initial ASD diagnosis. For children with ASD born to immigrant mothers, residing in a multilingual household was associated with a younger age of diagnosis. These results highlight the complexities of an ASD diagnosis among diverse children and families, particularly among immigrant communities. LAY SUMMARY: This research looked at predictors of when children received their first autism spectrum disorder diagnosis across children born to immigrant mothers and children born to U.S.-born mothers. Living in a multilingual home was related to a younger age of diagnosis for children of immigrant mothers; being verbal and having another diagnosis was related to an older age of diagnosis for both groups of children. This highlights the need to improve diagnostic services for diverse children.

During the transition to adulthood, effective and culturally relevant supports are critical for families of youth with autism spectrum disorder (ASD). There is a dearth of documented program development and research on supports for Spanish-speaking Latino families during this life stage. The present work describes the cultural adaptation process of an evidence-based transition program for Latino families of youth with ASD. A model of the actions necessary to meaningfully conduct a cultural adaptation in this context is described. After implementing the culturally adapted program titled Juntos en la Transición with five Spanish-speaking families, parents reported high social validity of the program through surveys and interviews. The cultural adaptation process followed in this work is important for the further development of programs that address the transition needs of Latino youth with ASD and their families. Our impressions may also be useful to those who aim to develop culturally sensitive and ecologically valid multifamily group intervention programs for families from cultural and linguistic minority groups.


Durante la transición a la adultez, las adaptaciones eficaces y culturalmente relevantes son fundamentales para las familias de jóvenes con trastorno del espectro autista (TEA). Hay una escasez de investigaciones y desarrollo de programas documentados sobre las adaptaciones para las familias latinas de habla hispana durante esta etapa de la vida. El presente trabajo describe el proceso de adaptación cultural de un programa de transición de base fáctica para familias latinas de jóvenes con TEA. Se describe un modelo de las medidas necesarias para llevar a cabo significativamente una adaptación cultural en este contexto. Después de implementar el programa adaptado culturalmente y titulado Juntos en la Transición con cinco familias de habla hispana, los padres informaron una alta validez social del programa mediante encuestas y entrevistas. El proceso de adaptación cultural seguido en este trabajo es importante para el futuro desarrollo de programas que aborden las necesidades de transición de los jóvenes latinos con TEA y sus familias. Nuestras impresiones también pueden ser útiles para aquellos que desean desarrollar programas de intervención en grupos multifamiliares ecológicamente válidos y culturalmente adaptados para familias de grupos culturales y lingüísticos minoritarios.

In this study, monolingual (English) and bilingual (English/Spanish, English/Urdu) five- and six-year-old children completed a grammaticality judgment test in order to assess their awareness of the grammaticality of two types of syntactic constructions in English: word order and gender representation. All children were better at detecting grammatically correct and incorrect word order constructions than gender constructions, regardless of language group. In fact, bilingualism per se did not impact the results as much as receptive vocabulary range. For example, children with the highest receptive vocabulary scores were more accurate in detecting incorrect word order constructions (i.e., word order violations, semantic anomalies) and incorrect gender agreement than children in the lower receptive vocabulary ranges. However, no differences were found between the ranges for ambiguous gender constructions. These results highlight the importance of receptive vocabulary ability on syntactic awareness performance, regardless of language group.

Takeda G protein-coupled receptor 5 (TGR5) agonists induce systemic release of glucagon-like peptides (GLPs) from intestinal L cells, a potentially therapeutic action against metabolic diseases such as nonalcoholic steatohepatitis (NASH), nonalcoholic fatty liver disease (NAFLD), and Type 2 diabetes. Historically, TGR5 agonist use has been hindered by side effects, including inhibition of gallbladder emptying. Here, we characterize RDX8940, a novel, orally administered TGR5 agonist designed to have minimal systemic effects and investigate its activity in mice fed a Western diet, a model of NAFLD and mild insulin resistance. Agonist activity, binding selectivity, toxicity, solubility, and permeability of RDX8940 were characterized in standard in vitro models. RDX8940 pharmacokinetics and effects on GLP secretion, insulin sensitivity, and liver steatosis were assessed in C57BL/6 mice fed normal or Western diet chow and given single or repeated doses of RDX8940 or vehicle, with or without dipeptidyl peptidase-4 (DPP4) inhibitors. Gallbladder effects were assessed in CD-1 mice fed normal chow and given RDX8940 or a systemic TGR5 agonist or vehicle. Our results showed that RDX8940 is minimally systemic, potent, and selective, and induces incretin (GLP-1, GLP-2, and peptide YY) secretion. RDX8940-induced increases in plasma active GLP-1 (aGLP-1) levels were enhanced by repeated dosing and by coadministration of DPP4 inhibitors. RDX8940 increased hepatic exposure to aGLP-1 without requiring coadministration of a DPP4 inhibitor. In mice fed a Western diet, RDX8940 improved liver steatosis and insulin sensitivity. Unlike systemic TGR5 agonists, RDX8940 did not inhibit gallbladder emptying. These results indicate that RDX8940 may have therapeutic potential in patients with NAFLD/NASH. NEW & NOTEWORTHY Takeda G protein-coupled receptor 5 (TGR5) agonists have potential as a treatment for nonalcoholic steatohepatitis and nonalcoholic fatty liver disease (NAFLD) but have until now been associated with undesirable side effects associated with systemic TGR5 agonism, including blockade of gallbladder emptying. We demonstrate that RDX8940, a potent, selective, minimally systemic oral TGR5 agonist, improves liver steatosis and insulin sensitivity in a mouse model of NAFLD and does not inhibit gallbladder emptying in mice.

Resumen Introducción: Se espera que pacientes con lupus pediátrico próximos a la transición asuman responsabilidades de su cuidado, pero muchos no están preparados, lo cual empobrece su pronóstico. Según el modelo autorregulatorio, las percepciones de la enfermedad determinan las respuestas emocionales, guían las conductas de afrontamiento, su evaluación, retroalimentación y estrategias de asimilación. Objetivo: Describir las percepciones sobre el lupus en adolescentes próximos a la transición. Materiales y métodos: Desde un enfoque hermenéutico y utilizando técnicas de la teoría fundada, se realizaron 11 entrevistas semiestructuradas entre junio de 2013 y septiembre de 2014 a 9 adolescentes con diagnóstico de lupus. Resultados: Se obtuvieron 1.800 códigos. Emergieron como categorías preliminares: «Intentando explicar el origen¼, donde se interpreta la causa de la enfermedad en términos de inmunosupresión y autoinmunidad, asociación con cáncer, culpa e influencia de factores emocionales, además el proceso diagnóstico. «Lo que se pierde¼, enmarcado en cambios, trato diferencial y límites. Finalmente, los «aspectos positivos¼: enfermedad como modulador de la conducta, adquisición de cualidades, aprendizaje sobre el funcionamiento corporal y ganancia secundaria. Conclusión: Las percepciones de la enfermedad en estos adolescentes, expresan en la experiencia de cambio las implicaciones del diagnóstico, las cuales impactan múltiples aspectos de sus vidas y traen consigo incertidumbre y necesidad de ajustes que les llevan a la búsqueda de explicaciones. Se requiere mayor conciencia sobre estas percepciones, debido a que, junto con otros factores, determinarán las estrategias que los adolescentes desarrollen para garantizar su autocuidado y adaptación a situaciones derivadas del vivir con la enfermedad.


Abstract Introduction: Patients with paediatric lupus nearing transition to adult care are expected to take responsibility for their care. Nevertheless, many are not prepared for this, and thus have a poorer prognosis. Using the self-regulation model, the perception of a condition determines the emotional responses and guides coping efforts, appraisal, feedback, and assimilation strategies. Objective: To describe how adolescents nearing transition perceive lupus. Materials and methods: Eleven semi-structured interviews were conducted using a hermeneutic approach with techniques from grounded theory. Interviews took place between July 2013 and September 2014. The participants were nine adolescents with diagnosed lupus. Results: A total of 1,800 codes were obtained that emerged as the following preliminary categories: "Attempting to explain the origin", where the cause of the disease is interpreted as immunosuppression, autoimmunity, association with cancer, guilt and influence of emotional factors, along with the diagnosis process. "What is lost", which includes changes, being treated differently, and having limitations. The last category was "positive aspects" deals with illness as a behavioural moderator, acquiring qualities, learning about bodily functioning and secondary gain. Conclusion: Adolescents perceive their condition based on the implications of the changes experienced in their lives as a result of the diagnosis. Lupus affects several aspects of their lives and brings uncertainty and a need to adjust, leading them to look for explanations. More awareness of these perceptions is required because the latter, along with other factors, determine the strategies that adolescents develop to ensure their self-care and adaptation to any situations arising from living with the condition.

Self-conscious emotions, such as shame, guilt and pride, facilitate our social interactions by motivating us to adhere to social norms and external standards. In this study, we examined proneness to shame, guilt, hubristic pride and authentic pride in adults with Autism Spectrum Disorder traits (ASD-T) and in neurotypical (NT) adults. Relations between proneness to self-conscious emotions and theory of mind (ToM), fear of negative evaluation, and social functioning were also assessed. Adults with ASD-T showed greater proneness to shame, and less proneness to guilt and pride than NT adults. Both ToM and fear of negative evaluation predicted proneness to self-conscious emotions in ASD-T. These findings are discussed in terms of understanding complex emotion processing in adults with ASD-T.

Latino children in the US are systematically underdiagnosed with Autism Spectrum Disorder (ASD); therefore, it is important that recent changes to the diagnostic process do not exacerbate this pattern of under-identification. Previous research has found that the Autism Diagnostic Interview-Revised (ADI-R) algorithm, based on the Diagnostic and Statistical Manual of Mental Disorder, Fourth Edition, Text Revision (DSM-IV-TR), has limitations with Latino children of Spanish speaking parents. We evaluated whether an ADI-R algorithm based on the new DSM-5 classification for ASD would be more sensitive in identifying Latino children of Spanish speaking parents who have a clinical diagnosis of ASD. Findings suggest that the DSM-5 algorithm shows better sensitivity than the DSM-IV-TR algorithm for Latino children.

The Autism Diagnostic Interview-Revised (ADI-R) has been validated as a tool to aid in the diagnosis of Autism; however, given the growing diversity in the United States, the ADI-R must be validated for different languages and cultures. This study evaluates the validity of the ADI-R in a US-based Latino, Spanish-speaking population of 50 children and adolescents with ASD and developmental disability. Sensitivity and specificity of the ADI-R as a diagnostic tool were moderate, but lower than previously reported values. Validity of the social reciprocity and restrictive and repetitive behaviors domains was high, but low in the communication domain. Findings suggest that language discordance between caregiver and child may influence reporting of communication symptoms and contribute to lower sensitivity and specificity.

In three experiments, script-based stories were used to provide thematic emotion. Consistent with past research expressing emotion thematically, emotion aided recall of both central and peripheral information. However, emotion had to be directly associated with the central or peripheral information, and emotion only facilitated memory for peripheral information when peripheral information alone was associated with the emotion. Emotion did not aid overall recall of stories. Positive and negative emotions were generally better recalled when they were incongruent with the information they were associated with, such as a positive emotion associated with an interruptive action. These results provide evidence that thematically expressed, low arousal emotion can have facilitative effects on memory. Implications for the role of emotion on memory are discussed.

OBJECTIVE: Interleukin-1ß (IL-1ß) has recently been implicated as a major cytokine that is involved in the pancreatic islet inflammation of type 2 diabetes mellitus. This inflammation impairs insulin secretion by inducing beta-cell apoptosis. Recent evidence has suggested that in obesity-induced inflammation, IL-1ß plays a key role in causing insulin resistance in peripheral tissues. DESIGN AND METHODS: To further investigate the pathophysiological role of IL-1ß in causing insulin resistance, the inhibitory effects of IL-1ß on several insulin-dependent metabolic processes in vitro has been neutralized by XOMA 052. The role IL-1ß plays in insulin resistance in adipose tissue was assessed using differentiated 3T3-L1 adipocytes and several parameters involved in insulin signaling and lipid metabolism were examined. RESULTS AND CONCLUSION: IL-1ß inhibited insulin-induced activation of Akt phosphorylation, glucose transport, and fatty acid uptake. IL-1ß also blocked insulin-mediated downregulation of suppressor of cytokine signaling-3 expression. Co-preincubation of IL-1ß with XOMA 052 neutralized nearly all of these inhibitory effects in 3T3-L1 adipocytes. These studies provide evidence, therefore, that IL-1ß is a key proinflammatory cytokine that is involved in inducing insulin resistance. These studies also suggest that the monoclonal antibody XOMA 052 may be a possible therapeutic to effectively neutralize cytokine-mediated insulin resistance in adipose tissue.

a adaptación de lentes esclerales en el manejo del queratocono puede ser una buena alternativa. Objetivo: comparar la adaptación de lentes rígidos gas permeables esclerales en pacientes con queratocono, calculados matemáticamente con base en datos del visante oct (optical tomography coherence) y el método tradicional (ensayo y error). Materiales y métodos: se realizó investigación observacional descriptiva. A ocho pacientes (16 ojos) con diagnóstico de queratocono se les adaptaron lentes de contacto rígidos esclerales, usando dos métodos de cálculo: el tradicional, de ensayo y error, y calculando los parámetros de los lentes mediante un modelo matemático, a partir de los datos de la topografía y la tomografía de alta resolución con visante oct. Con cada método se evaluó la agudeza visual, el número de pruebas necesarias para la adaptación final, el confort y la superficie ocular. Resultados: la agudeza visual dio igual en los métodos y la valoración de la superficie ocular presentó diferencias estadística (p < 0,05 según la prueba de rangos de Wilcoxon) y clínicamente significativas, siendo mejor el método del modelo matemático; igualmente, con el método de modelo matemático se logró mejor confort y menor número de pruebas que con el tradicional. Conclusiones: la utilización de la tecnología actual puede contribuir a mejorar las adaptaciones en lentes esclerales y optimizar tiempo y recursos.


itting scleral lens in management of keratoconus can be a good alternative. Objectives: To compare the adaptation of rigid gas permeable scleral lens in patients with keratoconus, mathematically calculated based on data from visante oct (optical tomography coherence) and the traditional method (trial and error). Materials and methods: Descriptive observational research was carried out. Rigid scleral contact lenses were adapted to eight patients (16 eyes) diagnosed with keratoconus, using two calculation methods: the traditional trial and error and calculating the parameters of the lens using a mathematical model, based on the topography data and the high-resolution ct with visante oct. With each method, the visual acuity was evaluated, as well as the number of tests needed for the final adaptation, the comfort and the ocular surface. Results: Visual acuity was the same in both methods and the assessment of the ocular surface presented statistical differences (p < 0.05 according to the Wilcoxon rank test) and clinically significant, where the method of mathematical modeling was better; likewise, the method of mathematical modeling achieved better comfort and a lower number of tests than the traditional. Conclusions: Using current technology can contribute to improve scleral lenses adaptation and to optimize time and resources.

OBJECTIVE: Atherosclerosis is a condition that is increasingly contributing to worldwide mortality through complications such as stroke and myocardial infarction. IL-1ß plays multiple direct, local roles in the formation and stability of the atheroma by eliciting the production of additional cytokines and proteolytic enzymes from macrophages, endothelial cells (EC) and smooth muscle cells (SMC). We therefore tested whether an anti-IL-1ß antibody, XOMA 052, might inhibit the secretion of pro-atherogenic cytokines from macrophages in vitro and affect a positive outcome in the Apolipoprotein E-deficient mouse (ApoE(-/-)) model of atherosclerosis in vivo. METHODS AND RESULTS: In an in vitro co-culture model, XOMA 052 inhibited macrophage-induced secretion of key atherogenic cytokines from EC and SMC, including IL-6, IL-8, MCP-1 and TNFα. The release of degradative enzymes, such as the matrix metalloproteinases MMP-3 and MMP-9, was also decreased by XOMA 052. In addition, XOMA 052 inhibited the secretion of IL-7 from EC and IL-4 from SMC, cytokines not previously reported to be driven by IL-1ß in this context. In vivo, XMA052 MG1K, a chimeric murine version of XOMA 052, inhibited the formation of atherosclerotic lesions in the ApoE(-/-) model at all three doses tested. This effect was comparable to that reported for complete genetic ablation of IL-1ß or IL-1R1 on an ApoE(-/-) background and was associated with decreases in plasma non-HDL/HDL cholesterol ratio and plaque lipid content and macrophage infiltration. CONCLUSIONS: These results demonstrate for the first time that an antibody targeting IL-1ß can inhibit the progression of atherosclerosis in vivo, highlighting the importance of this key cytokine in cardiovascular disease.

Many therapeutic antibodies act as antagonists to competitively block cellular signaling pathways. We describe here an approach for the therapeutic use of monoclonal antibodies based on context-dependent attenuation to reduce pathologically high activity while allowing homeostatic signaling in biologically important pathways. Such attenuation is achieved by modulating the kinetics of a ligand binding to its various receptors and regulatory proteins rather than by complete blockade of signaling pathways. The anti-interleukin-1beta (IL-1beta) antibody XOMA 052 is a potent inhibitor of IL-1beta activity that reduces the affinity of IL-1beta for its signaling receptor and co-receptor but not for its decoy and soluble inhibitory receptors. This mechanism shifts the effective dose response of the cytokine so that the potency of IL-1beta bound by XOMA 052 is 20-100-fold lower than that of IL-1beta in the absence of antibody in a variety of in vitro cell-based assays. We propose that by decreasing potency of IL-1beta while allowing binding to its clearance and inhibitory receptors, XOMA 052 treatment will attenuate IL-1beta activity in concert with endogenous regulatory mechanisms. Furthermore, the ability to bind the decoy receptor may reduce the potential for accumulation of antibody.target complexes. Regulatory antibodies like XOMA 052, which selectively modulate signaling pathways, may represent a new mechanistic class of therapeutic antibodies.

The world's languages draw on a common set of event components for their verb systems. Yet, these components are differentially distributed across languages. At what age do children begin to use language-specific patterns to narrow possible verb meanings? English-, Japanese-, and Spanish-speaking adults, toddlers, and preschoolers were shown videos of an animated star performing a novel manner along a novel path paired with a language-appropriate nonsense verb. They were then asked to extend that verb to either the same manner or the same path as in training. Across languages, toddlers (2- and 2.5-year-olds) revealed a significant preference for interpreting the verb as a path verb. In preschool (3- and 5-year-olds) and adulthood, the participants displayed language-specific patterns of verb construal. These findings illuminate the way in which verb construal comes to reflect the properties of the input language.

El queratocono se incluye entre las alteraciones corneales degenerativas en las que se produce deformación y adelgazamiento progresivos. Se conoce desde hace tiempo, pero durante los últimos años se han propuesto novedosas técnicas diagnósticas y tratamientos corneales que han llevado a que esta patología sea más estudiada. Del queratocono sabemos en qué consiste la alteración y cuál es su línea evolutiva y algunos tratamientos. Sin embargo, desconocemos la causa, los mecanismos exactos de la enfermedad, cómo prevenirla y como curarla; por tanto, debemos seguir investigando para mejorar la calidad de vida y abrir esperanzas a los que padecen esta dolencia.


Keratoconus is included among the degenerative corneal changes that occur in a progressive thinning and distortion. It has been known for a long time, but in recent years, novel corneal diagnostic techniques and treatments have been proposed that have led to further studies on this condition. About keratoconus we know what alterations occur, its evolution, and some treatments. Although the cause and the exact mechanisms, how to prevent and cure it, are unknown, so we must continue research to improve quality of life of people affected by this anomaly.
