RESUMEN

Parental burnout (PB) is characterised by parental exhaustion resulting from exposure to chronic parental stress. Due to the social and economic changes resulting from the COVID-19 pandemic that impacted families, there is a significant scientific interest in identifying factors related to PB within the pandemic context. This study aimed to analyse the relationships between parental stress (parental concerns, parental satisfaction, lack of control, and fears and anxieties), family functioning, psychological morbidity (anxiety and depression), and burnout in parents. The sample consisted of 253 parents, legal guardians or primary caregivers of children aged between 0 and 6 years. Results revealed that the child's age, psychological morbidity (depression), and parental stress were significant predictors of PB. Anxiety and family functioning played a moderating role in the relationship between parental satisfaction and PB. According to the results, intervention programs should focus on anxiety symptoms and family functioning to mitigate the effects of parental stress on PB.

Diabetic foot ulcers (DFU) are one of the most frequent and debilitating complications of diabetes. DFU wound healing is a highly complex process, resulting in significant medical, economic and social challenges. Therefore, early identification of patients with a high-risk profile would be important to adequate treatment and more successful health outcomes. This study explores risk assessment profiles for DFU healing and healing prognosis, using machine learning predictive approaches and decision tree algorithms. Patients were evaluated at baseline (T0; N = 158) and 2 months later (T1; N = 108) on sociodemographic, clinical, biochemical and psychological variables. The performance evaluation of the models comprised F1-score, accuracy, precision and recall. Only profiles with F1-score >0.7 were selected for analysis. According to the two profiles generated for DFU healing, the most important predictive factors were illness representations on T1 IPQ-B (IPQ-B ≤ 9.5 and < 10.5) and the DFU duration (≤ 13 weeks). The two predictive models for DFU healing prognosis suggest that biochemical factors are the best predictors of a favorable healing prognosis, namely IL-6, microRNA-146a-5p and PECAM-1 at T0 and angiopoietin-2 at T1. Illness perception at T0 (IPQ-B ≤ 39.5) also emerged as a relevant predictor for healing prognosis. The results emphasize the importance of DFU duration, illness perception and biochemical markers as predictors of  healing in chronic DFUs. Future research is needed to confirm and test the obtained predictive models.

Objective: Diabetic foot ulcers (DFUs) are devastating complications of diabetes, responsible for a high number of amputations worldwide. Due to its impact on chronic inflammation, psychological distress may negatively impact the healing process. Thus, this study evaluated the influence of psychological distress on physiological indicators of healing prognosis and the potential of stress-reducing therapies for DFU healing. Approach: Patients with chronic DFU were recruited and assessed at enrollment and 2 months later. According to psychological scores at enrollment, participants were allocated into groups without (group 1) or with (group 3) psychological distress. Participants who reported clinical distress were then randomly allocated into a control (no stress-reducing intervention-group 4) or experimental (with stress-reducing interventions-group 5) group. Subsequently, indicators of healing prognosis were measured. Results: Groups 1 and 3 presented no differences in the Perfusion, Extent, Depth, Infection and Sensation score, glycated hemoglobin, or inflammatory and angiogenic markers. However, the immune cell ratio was increased by more than twofold in group 3, compared with group 1. Importantly, the expression of circulating microRNAs was significantly increased in group 3 (miR-21-5p, miR-155-5p, miR-146a-5p, and miR-221-3p [p < 0.05]), compared with group 1. Two months later, group 5 displayed a significant improvement in the Perceived Stress Scale and Hospital Anxiety and Depression Scale scores (p < 0.01), and the immune cell ratio was decreased by more than 2.5-fold. Innovation: This study helped to identify which variables and psychological interventions are more successful in promoting DFU healing. Conclusion: Psychological distress influenced clinical and physiological parameters, leading to compromised DFU healing and consequently underlining the potential of adjuvant stress-reducing approaches.

AIMS: This study aimed to identify demographic, clinical, and psychological contributors to DFU healing and favorable healing process. METHODS: Patients with a chronic DFU were evaluated at baseline (T0; n = 153), two months later (T1; n = 108), and six months later (T2; n = 71). Patients were evaluated on health literacy, perceived stress, anxiety, depression, and illness perceptions. Cox proportional hazard models were built to analyze the predictors of DFU healing and favorable healing process (wound area reduction), including the assessment of time to achieve those outcomes. RESULTS: More than half of patients had their DFU healed (56.1%) or showed a favorable healing process (83.6%). Median time for healing was 112 days, while for favorable process was 30 days. Illness perceptions were the only predictor of wound healing. Being female , with adequate health literacy, and a first DFU predicted a favorable healing process. CONCLUSIONS: This is the first study showing that beliefs about DFU are significant predictors of DFU healing, and that health literacy is a significant predictor of a favorable healing process. Brief, comprehensive interventions should be implemented, at the treatment initial stage, in order to change misperceptions and to promote DFU literacy and better health outcomes.

BACKGROUND: The present study aimed to assess the perceptions of patients with chronic diabetic foot ulcers (DFUs) and their family caregivers regarding the impact of two stress reduction interventions on DFU and psychological wellbeing. The intervention included progressive muscle relaxation and hypnosis sessions. METHODS: This study used a qualitative exploratory design and included individual interviews with eight patients with chronic DFUs and six family caregivers, using a semi-structured interview guide. Transcript analysis employed thematic content analysis. RESULTS: Four key themes common to patients and their caregivers were found: 1) perspectives regarding the intervention; 2) intervention effectiveness; 3) perceived importance of psychology in the DFU treatment; and 4) emotional consequences associated with DFUs. Although themes were common to both intervention groups, sub-themes from the last two themes differed for patients that received muscle relaxation versus those who received hypnosis. CONCLUSION: Patients and caregivers reported perceived benefits from both interventions, regarding DFU healing and emotional wellbeing. Patients who received hypnosis and their caregivers also reported lasting effects. Participants suggested that psychological interventions such as stress reduction interventions could be included in the DFU standard treatment as an adjuvant to the clinical protocol for DFU treatment, preferably offered early on, when patients begin treatment at the diabetic foot consultation.

For most individuals, changes occurring during the aging process may cause dissatisfaction and concerns regarding body image, with implications on quality of life (QoL). The aim of this study was to assess the contribution of sociodemographic and psychological variables and the type of anti-aging products/treatments on men's QoL, as well as to evaluate the moderating role of aging perceptions in the relationship between psychological morbidity and QoL. This study used a cross-sectional design and included a sample of 111 male participants who used anti-aging cosmetic products/aesthetic treatments. Participants were assessed on body image, self-esteem, psychological morbidity, aging perceptions, traits of perfectionism, and QoL. Results suggested that psychological morbidity and perceptions of the aging process as chronic negatively contributed to QoL, and emotional representations of aging moderated the relationship between psychological morbidity and QoL. According to the findings, psychological intervention programs to improve QoL should focus on men with higher levels of psychological morbidity and more negative emotional responses to the aging process.

OBJECTIVE: To analyze the associations among and contributions of sociodemographic, clinical, and psychological variables to quality of life (QoL) related to diabetic foot ulcers (DFUs; DFUQoL). METHODS: The sample comprised 70 patients with DFUs who answered the following questionnaires: the Hospital Anxiety and Depression Scale, Illness Perception Questionnaire-Brief, Diabetic Foot Ulcer-Short Form, Medical Term Recognition test, and a sociodemographic and clinical questionnaire. Pearson and biserial correlation coefficients, hierarchical regressions, and moderation analyses were conducted to study the relationships among variables. RESULTS: The results showed that a large DFU, threatening representations about DFU, and more psychological distress were associated with lower DFUQoL. Also, more psychological distress, more threatening DFU representations, and taking antibiotics contributed to worse DFUQoL. Clinically significant levels of psychological distress moderated the relationship between the DFU extent and negative emotions. CONCLUSIONS: This study showed that QoL and its domains were associated with psychological distress and that QoL was more influenced by DFU representations than by a larger-sized wound, emphasizing the importance of psychological interventions in the treatment of patients with DFUs.

Diabetic foot ulcer (DFU) is the leading cause of lower-limb amputations, with a significant impact on patients, families, and society. Since DFU medical treatments represent a major socioeconomic burden, cost-effective interventions are needed. This trial aims to assess the effectiveness of a muscle relaxation intervention compared to a hypnosis intervention versus active and passive control groups on DFU healing, physiological indicators of healing prognosis, and quality of life (QoL) in clinically distressed patients with a chronic DFU. A multicenter, randomized controlled trial with three assessment moments (baseline, two months post-intervention, and four months follow-up) will be conducted. Approximately 170 patients will be randomized and allocated to either treatment or control groups. Primary outcomes will be DFU healing, physiological indicators of healing prognosis, and QoL. Secondary outcomes will include perceived stress, psychological morbidity, and DFU representations. The efficacy of sessions on DFU healing will be qualitatively assessed in 12 patients allocated to the treatment and active control groups, as well as their family caregivers. This study will provide evidence regarding the effectiveness of two psychological interventions for the DFU healing process and the QoL of patients, with direct clinical relevance regarding DFU treatment and recurrence.

BACKGROUND: Ageing is accompanied by physical changes, both at the physiological and appearance levels. The way people perceive these changes have important consequences on general health and quality of life (QoL). This study analysed the relationship between sociodemographic variables, psychological variables, use of antiageing cosmetic products/aesthetic treatments, and QoL. METHODS: This cross-sectional study included a sample of 271 women, aged between 25 and 70 years, users of antiageing cosmetic products and/or aesthetic services. Participants were assessed on psychological morbidity (Hospital Anxiety and Depression Scale), appearance schemes (Appearance Schemas Inventory - Revised), self-esteem (Rosenberg Self-Esteem Scale), perfectionism (Frost Multidimensional Perfectionism Scale), self-perceptions of ageing (Brief Aging Perceptions Questionnaire), and QoL (SF-12v2). RESULTS: Being older and having a higher household income was associated with better QoL. The use of facial firming products, hair colouring products, and sunscreen was also associated with better QoL. Psychological morbidity and perfectionism contributed negatively to QoL, while household income contributed positively. Ageing perceptions moderated the relationship between self-esteem and QoL. CONCLUSION: According to the findings, intervention programs to reduce psychological morbidity, increase self-esteem, promote more adaptive patterns of perfectionism, and recognize the role of age perception are needed to improve women's QoL. The perceived influence of the usage of cosmetic products to prevent/minimize aging signs should be further explored.


CONTEXTE: Le vieillissement s'accompagne de changements physiques, tant au niveau physiologique qu'au niveau de l'apparence. La façon dont les gens perçoivent ces changements a des conséquences importantes sur la santé générale et la qualité de vie (QdV). Cette étude a analysé la relation entre les variables sociodémographiques, les variables psychologiques, l'utilisation de produits cosmétiques anti-âge/soins esthétiques et la QdV. MÉTHODES: Cette étude transversale a inclus un échantillon de 271 femmes, âgées de 25 à 70 ans, utilisatrices de produits cosmétiques anti-âge et/ou de soins esthétiques. Les participants ont été évalués sur la morbidité psychologique (Hospit Anxiety and Depression Scale), les schémas d'apparence (Appearance Schemas Inventory - Revised), l'estime de soi (Roenberg Self-Esteem Scale), le perfectionnisme (Frost Multidimensional Perfectionism Scale), l'auto-perception du vieillissement (Brief Questionnaire sur les perceptions du vieillissement) et QdV (SF-12v2). RÉSULTATS: Être plus âgé et avoir un revenu du ménage plus élevé était associé à une meilleure qualité de vie. L'utilisation de produits raffermissants pour le visage, de produits de coloration capillaire et de soins solaires était également associée à une meilleure qualité de vie. La morbidité psychologique et le perfectionnisme ont contribué négativement à la qualité de vie, tandis que le revenu du ménage a contribué positivement. Les perceptions du vieillissement ont modéré la relation entre l'estime de soi et la qualité de vie. CONCLUSION: Selon les résultats, des programmes d'intervention visant à réduire la morbidité psychologique, à augmenter l'estime de soi, à promouvoir des schémas de perfectionnisme plus adaptatifs et à reconnaître le rôle de la perception de l'âge sont nécessaires pour améliorer la qualité de vie des femmes. L'influence de l'utilisation de produits cosmétiques pour prévenir/minimiser les signes du vieillissement doit être davantage perçue.

BACKGROUND: Multiple myeloma (MM) affects mainly the older population and is the second most prevalent haematologic cancer. MM patients' unmet needs are diverse, arise at different stages, and are associated with greater psychological distress. This study examined the psychometric properties of the Short-Form Survivor Unmet Needs Survey (SF-SUNS) in Portuguese MM patients. METHODS: This cross-sectional study included 213 MM patients. The internal consistency of the scale was analyzed with Cronbach's α. Principal component analysis and confirmatory factor analysis were used to assess construct validity. Convergent validity was examined by using correlations with quality of life, satisfaction with social support, and psychological morbidity. Multiple regression analyses were conducted to explore the contribution of sociodemographic and clinical variables to unmet needs. RESULTS: The Portuguese SF-SUNS's factor structure follows the original's structure, although it includes fewer items. For each domain, Cronbach's α was ≤0.70, the minimum acceptable criterion. For construct validity, only unmet relationship and emotional needs had significant correlations (r ≥ 0.40)-specifically negative associations with quality of life and social support and positive associations with psychological morbidity. Regarding patient variables and SF-SUNS results, only cancer stage contributed significantly to unmet information needs, with patients at stage I reporting more needs than patients at stage III. CONCLUSIONS: The SF-SUNS represents a valid and reliable tool to assess unmet needs among Portuguese MM patients. It may be useful in designing and monitoring interventions to improve well-being in cancer survivors.

Objectives: This study aimed to assess the relationship between sociodemographic, clinical, and psychological variables with quality of life (QoL) and the moderating role of caregivers' age and caregiving duration in caregivers of patients with Multiple Myeloma.Method: The sample included 118 caregivers who completed questionnaires that assessed psychological morbidity, satisfaction with social support, coping, burden, unmet needs, and QoL.Results: High psychological morbidity, burden and information, financial and emotional unmet needs were associated with lower QoL, while higher satisfaction with social support and more effective use of coping strategies were associated with better QoL. Women caregivers reported more satisfaction with social support and those who did not choose to care reported greater financial unmet needs and more use of coping strategies. The relationship between caregivers' psychological morbidity/social support and QoL was mediated by emotional needs and double mediated by coping and burden. The caregivers' age moderated the relationship between psychological morbidity/social support and emotional needs.Conclusion: Interventions to support the caregiver's emotional needs to promote their QoL are needed. These should be particularly tailored for older caregivers reporting greater psychological morbidity and younger caregivers less satisfied with their social support, as they have a negative indirect impact on their QoL.

OBJECTIVES: This study examined the mediator role of mindfulness and spirituality in the relationship between psychological morbidity, awareness of the disease, functionality, social support, family satisfaction, and quality of life (QoL) in patients with mild AD. METHOD: The sample consisted of 128 patients who answered the Cognitive and Affective Mindfulness Scale-Revised (CAMS-R), the Assessment Scale of Psychosocial Impact of the Diagnosis of Dementia (ASPIDD), the Hospital Anxiety and Depression Scales (HADS), the Satisfaction with Social Support Scale (SSSS), the Family Satisfaction Scale (FSS), the Spiritual and Religious Attitudes in Dealing with Illness (SpREUK), the Index of Barthel, and the Quality of Life-Alzheimer's Disease (QoL-AD). RESULTS: Mindfulness and spirituality mediated the relationship between functionality, awareness of the disease, family satisfaction and QoL. Psychological morbidity had a direct negative impact on QoL and was negatively associated with awareness of the disease, family satisfaction and social support. Mindfulness was negatively associated with spirituality and the latter was negatively associated with QoL. More social support was associated with greater awareness of the disease and family satisfaction. More functionality, awareness of the disease and family satisfaction contributed to more QoL and this relationship was mediated by mindfulness and spirituality. CONCLUSION: Interventions directed at the promotion of the QoL of patients with mild AD should focus on the promotion of mindfulness skills in AD patients, in addition to the reduction of psychological morbidity and the promotion of functionality, awareness of the disease, family relationships and social support.

OBJECTIVE: Cancer diagnosis affects patients, their families, and their caregivers in particular. This study focused on the validation of the CareGiver Oncology Quality of Life (CarGOQoL) questionnaire in Portuguese caregivers of patients with multiple myeloma, from the caregiver's point of view. METHOD: This was a cross-sectional study with 146 caregivers of patients with multiple myeloma from outpatient medical oncology and clinical hematology consultations from five hospitals in north and central Portugal. Participants were assessed on quality of life (QoL), psychological morbidity and social support. RESULT: The Portuguese version maintains 17 of the original 29 items version, maintaining general coherence and a dimensional structure that is clinically interpretable. Reliability findings indicated good internal consistency for the total scale (0.86) and respective subscales (0.75 to 0.88), which is in agreement with the alpha values from the previous CarGOQoL validation study for the corresponding subscales (0.74 to 0.89) and total scale (0.90). SIGNIFICANCE OF RESULTS: The CarGOQoL is a reliable and valid tool for clinical trials and intervention programs to assess QoL in caregivers of myeloma patients. Future studies should validate the adapted version in caregivers of other types of cancer patients including other chronic diseases.

OBJECTIVE: The aim of this study was the validation of the European Organization for Research and Treatment of Cancer's Multiple Myeloma Module (QLQ-MY20) in Portuguese myeloma patients. METHODS: A total of 213 Portuguese patients diagnosed with multiple myeloma participated in this study and were assessed with the European Organization for Research and Treatment of Cancer's (EORTC) Questionnaire C30 (QLQ-C30), the EORTC Multiple Myeloma Module (QLQ-MY20), the Hospital Anxiety and Depression Scale (HADS) and the Satisfaction with Social Support Scale (SSSS). RESULTS: The validated version includes 17 items presenting good global adjustment and good internal consistency. Overall, the Portuguese validation maintains the original model with the exception of three items that were excluded. The instrument also showed good reliability and good convergent and divergent validity. CONCLUSION: The Portuguese version of the EORTC Multiple Myeloma Module questionnaire seems to be a valid instrument for myeloma patients to help monitor interventions in this population focused on the promotion of quality of life.

This study reports on the validity of the 15-item Portuguese version of the Systemic Clinical Outcome Routine Evaluation (SCORE-15; Vilaça, Silva, & Relvas, 2014), a brief and comprehensive measure of family functioning. Previous studies with SCORE-15 show that this version replicates the three-factor solution found for the original English version: Family strengths, Family communication and Family difficulties. In addition to reviewing previous studies, this article analyses the discriminant, convergent and predictive validity of the Portuguese SCORE-15. To do so, the SCORE-15 was administered to family members attending systemic family or couple's therapy at the start of the first and fourth sessions and also to a group of non-clinical individuals. Overall, data are reported from 618 participants, including 136 from families attending systemic therapy and 482 community family members. Comparisons of community and clinical samples (discriminant validity) showed statistically significant differences for the total scale and subscales (p < .001), with the community participants presenting healthier family functioning than the clinical ones. Analyses using SCORE-15 and the Quality of Life - adult version, another family measure applied simultaneously (convergent validity), indicate that both scales are significantly (p < .01) and moderately (r = -.47) correlated. Mean score analysis of SCORE-15's therapeutic sensitivity to change (predictive validity) showed that only the Family communication subscale was sensitive to statistically significant improvement (p < .05) from session 1 to session 4, whereas the SCORE-15's reliability change index points to its ability to detect clinical improvements (RCI = 14%).

RESUMEN

This study reports on the validity of the 15-item Portuguese version of the Systemic Clinical Outcome Routine Evaluation (SCORE-15; Vilaça, Silva, & Relvas, 2014), a brief and comprehensive measure of family functioning. Previous studies with SCORE-15 show that this version replicates the three-factor solution found for the original English version: Family strengths, Family communication and Family difficulties. In addition to reviewing previous studies, this article analyses the discriminant, convergent and predictive validity of the Portuguese SCORE-15. To do so, the SCORE-15 was administered to family members attending systemic family or couple's therapy at the start of the first and fourth sessions and also to a group of non-clinical individuals. Overall, data are reported from 618 participants, including 136 from families attending systemic therapy and 482 community family members. Comparisons of community and clinical samples (discriminant validity) showed statistically significant differences for the total scale and subscales (p < .001), with the community participants presenting healthier family functioning than the clinical ones. Analyses using SCORE-15 and the Quality of Life - adult version, another family measure applied simultaneously (convergent validity), indicate that both scales are significantly (p < .01) and moderately (r = -.47) correlated. Mean score analysis of SCORE-15's therapeutic sensitivity to change (predictive validity) showed that only the Family communication subscale was sensitive to statistically significant improvement (p < .05) from session 1 to session 4, whereas the SCORE-15's reliability change index points to its ability to detect clinical improvements (RCI = 14%) (AU)
