RESUMEN

To determine the effect of valsartan on the androgen status and erectile function in hypertensive patients. 60 hypertensive patients of 40-65 years of age were included in the study. All patients filled in the questionnaire on aging male symptoms scale and international index of erectile function before and 3 months after the course of antihypertensive therapy. Patients of the first group received angiotensin II receptor antagonist (valsartan) as monotherapy. Valsacor was administered starting from the first 24 hours after destabilization of blood pressure, and the dose was titrated from 80 up to 160 mg/day. Traditional treatment of hypertension including angiotensin converting enzyme inhibitors, calcium antagonists, diuretics and beta-blockers was prescribed to controls. Valsacor treatment reduced the intensity of the symptoms of erectile dysfunction in hypertensive males (by 11,3 against 2,2% in the control group, p<0,05). In addition, this therapy led to a decrease in androgen deficiency symptoms (20,2 against 12,1%, respectively, p < 0,05). Systolic and diastolic blood pressure reduction was comparable in both groups. There was an increase in the number of "dippers" at valsacor treatment, while the number of other categories ("over-dipper", "non-dipper", "night-peaker") decreased (p<0,05). In the control group, circadian blood pressure profile was not changed. Thus, therapy with valsartan normalizes diurnal variations in blood pressure, reduces the symptoms of androgen deficiency and does not contribute to erectile dysfunction.

In 96 patients with coronary heart disease and stage II circulatory failure, the chronopharmacodynamics of strophanthin, corglycon, and nitrosorbide was studied by using acute clinical and pharmacological tests in the morning (8.00 a.m.), in the afternoon (2.00 p.m.), and in the evening (8.00 p.m.). Central and peripheral hemodynamic parameters were measured prior to and following 5, 15, 30, 60, and 120 min of intravenous injection of cardiac glycosides or sublingual administration of nitrosorbide in a dose of 10 mg for 240 min. The investigations showed that there were the most profound hemodynamic changes in the morning if strophanthin was used and in the afternoon if corglycon was given. The maximal reduction in heart pre- and afterload was seen in the morning when nitrosorbide was applied, the most improvement in central hemodynamic parameters was observed in the evening when nitrosorbide was used.

RESUMEN

One of the ways of individualization of treating patients with hypertensive disease (HD) is an attempt of an individual action on peripheral blood circulation depending on the initially increased pre- or postloading of the heart. The clinicopharmacological studies showed that the course treatment with low dose anaprilin decreased the pre- and postloading of the heart in HD patients, intravenous administration of isoptin (5 mg) reduced the cardiac preloading and intravenous labetalol (100 mg) decreased the cardiac postloading.

RESUMEN

During course therapy with nonachlazine of patients with ischemic heart disease, stable angina pectoris a negative inotropic effect of the drug in persons with eukinetic and in particular hyperkinetic type of hemodynamics was revealed. Enhancement of beta-adrenoblocking effect of nonachlazine with an increase of its daily dose was found. Nonachlazine effect on hemodynamic parameters was shown to be mediated through adrenoreactive structures.

RESUMEN

The influence of a course therapy with anaprilin alone on the peripheral circulation in patients with essential hypertension of the early stages was investigated. Anaprilin was shown to have a significant effect on the peripheral blood circulation in the skeletal muscles expressed as veno- and artery dilatation, with the predominance of the former.

RESUMEN

The results of the study of adrenal cortical function in 94 patients (including 20 patients with acute myocardial infarction and pulmonary edema) and in 20 practically healthy persons are discussed. Marked activation of adrenal cortical hormones was noted in patients with acute myocardial infarction and edema of the lungs.
