RESUMEN

Investigated the relationship between pulmonary artery pressure (P(LA)) and the oxygen saturation of mixed venous blood (S(V)) in 12 group's of surveyed individuals (1750 men and 1026 women). We have identified a function (P(LA)) between P(LA) = f(S(V)), and a function (S(V)) S(V) = f(P(LA)) was estimated for each group based on direct measurements of P(LA) and S(V). We found, that factors were subordinated to the dependences for a P(LA) = f(S(V)), P(LA) = a x (S(V))(-b), where b = = -0.2284a + 0.6564 men - and b = -0.285a + 1.2947 in women and the other for -S(V) = f(P(LA)), S(V) = c x (P(LA))(-d) where d = -0.251311n(c) + 1.0212; (R2 = 0.8993) men and d = -1.96451n(c) + 2.852; (R2 = 0.9674) women. Each group occupies a position on the curves represented by equations. The subjects with a diagnosis of functional murmur in the heart and patients with congenital stenos is of the aortic valve form a group, provisionally designated as "group norms", which is characterized by its dependence P(LA) = f(S(V)), and -S(V) = f(P(LA)). The men in "group norms" additionally include patients with coronary heart disease. The equation - CO = Cons.O2/(KEK(S(A) - (c x (P(LA))(-d). It relates the P(LA), caused by different reasons, with the corresponding saturation of mixed venous blood, and when the saturation of mixed venous blood is also caused by various factors, set the corresponding P(LA). Interdependent changes in physiological parameters of blood circulation and gas exchange in humans is established equilibrium between systemic and pulmonary circulation.

RESUMEN

Laser doppler flowmetry was used for the intraoperative study of myocardial blood flow before and after revascularization in 116 patients with 2-3 vessel coronary artery disease and class II-IV angina. In patients without myocardial infarction, with microfocal myocardial infarction, or operated early after myocardial infarction revascularization caused no significant increase of myocardial blood flow. In patients with macrofocal infarction surgery was associated with significant increase of myocardial blood flow. Efficacy of revascularization could be assessed by lowering of flow gradients between various regions of the myocardium.

RESUMEN

The main aim of intensive care in acute head injury and in the postoperative period is to prevent, detect, and revert a secondary neuronal injury. To maintain the optimum systemic and cerebrovascular functions can substantially promote this aim achievement. There are some new neuroprotective interventions that are currently under investigation. Although the major focus of recent cerebral protection researches has been on the development of receptor-specific drugs, this effort has resulted in better outcomes. At present, patients are well served by using more traditional techniques to prevent and treat cerebral ischemic events. Initial therapy should include interventions to improve cerebral perfusion and the oxygen-carrying capacity of blood. Once this is accomplished, measures should be made to monitor blood glucose concentrations and to treat fever. General anesthetic choice may be of great importance in monitoring intracranial pressure and seizure activity. However, if direct cerebral protection is desired, a barbiturate should be the anesthetic of choice. All these measures should increase the chance of patients to neurologically recover following hypoxia and ischemia.

The electric properties of normal human blood were measured at 1.0-2.0 MHz frequency range. The dependence of relaxation time of the erythrocyte membranes on the degree of hemoglobin molecules saturation with oxygen was obtained.

RESUMEN

The gaseous composition of the blood has been determined in cardiac cavities and major vessels in 180 practically healthy individuals aged from 1 to 30 years, during diagnostic catheterization of the heart. Physiological norms of gaseous composition of the blood in cardiac cavities and in the major blood vessels have been determined. Blood saturation with oxygen in the identical cardiac cavities and the major blood vessels are the same in men and women. The arterial blood having oxygenation below 93% is considered hypoxaemic. Normal figures of oxygenation for venous blood are from 75-84%. Exponential decrease of blood oxygenation with age (arterial and mixed venous) is roughly the same, due to this the arteriovenous difference of oxygenation at rest remains constant and is roughly equal to 20%. Minimum arterio-venous difference in oxygenation is 10%, maximum 40%.

RESUMEN

A relationship between erythrocytic antigens of the ABO and Rh blood systems and cardiovascular pathology was revealed by comparing the distribution of blood groups in 13,175 patients and 7,800 donors. Prevalence of A gene and Rh+ phenotype in congenital and acquired heart diseases and ischemic heart disease was found. The frequency of B gene is increased in patients with acquired heart diseases.

The results of direct pressure measurement in pulmonary circulation in 452 patients with ventricular septal defect (216 males and 236 females), whose age ranged from 12 months to 37 years, were analysed. The findings of diagnostic catheterization of the heart in 150 practically healthy persons 12 months to 40 years of age were used to elaborate the normal values of pressure in pulmonary circulation. The results of pressure measurement in pulmonary circulation repeated at intervals of 1 to 6 years in 46 patients with ventricular septal defect, 18 months to 23 years of age, were analysed. Reverse evolution of pressure in pulmonary circulation after surgical rehabilitation was followed up in 85 patients in 1, 2, 3, 5, and 7 years after the operation. At the age of 12 months to 7 years, systolic pressure in the right ventricle and pulmonary artery comes down to a possible minimum due to expenditure of the adaptational reserve. In large ventricular septal defects, however, the organism proves incapable of reducing the pressure in the early and first periods of childhood as a rule, and so the pressure increases throughout life.

RESUMEN

The results of functional pharmacological tests with nitroglycerine and carbon dioxide gas are presented for patients with congenital heart diseases and pulmonary hypertension, as shown during right heart catheterization. It was established that nitroglycerine, taken sublingually, does not produce any direct effect on the pulmonary artery pressure, and the delayed changes in haemodynamics are a consequence of the altered regimen of pressure in the general circulation. When carbon dioxide gas was administered into the pulmonary artery, it proved to produce a direct effect upon the receptors of the pulmonary arterial bed. However, along with the hypotensive effect, in some cases the carbon dioxide gas increases the pressure in the pulmonary artery due to the embologenic effect of the gas bubbles. Both agents affect the arterial pressure irrespective of the patient's age, the nature of the heart disease, and the degree of pulmonary hypertension.
