Objective: Distorted loudness perception is one of the main complaints of hearing aid users. Measuring loudness perception in the clinic as experienced in everyday listening situations is important for loudness-based hearing aid fitting. Little research has been done comparing loudness perception in the field and in the laboratory.Design: Participants rated the loudness in the field and in the laboratory of 36 driving actions. The field measurements were recorded with a 360° camera and a tetrahedral microphone. The recorded stimuli, which are openly accessible, were presented in three conditions in the laboratory: 360° video recordings with a head-mounted display, video recordings with a desktop monitor and audio-only.Study samples: Thirteen normal-hearing participants and 18 hearing-impaired participants with hearing aids.Results: The driving actions were rated as louder in the laboratory than in the field for the condition with a desktop monitor and for the audio-only condition. The less realistic a laboratory condition was, the more likely it was for a participant to rate a driving action as louder. The field-laboratory loudness differences were bigger for louder sounds.Conclusion: The results of this experiment indicate the importance of increasing realism and immersion when measuring loudness in the clinic.

An issue commonly expressed by hearing aid users is a difficulty to understand speech in complex hearing scenarios, that is, when speech is presented together with background noise or in situations with multiple speakers. Conventional hearing aids are already designed with these issues in mind, using beamforming to only enhance sound from a specific direction, but these are limited in solving these issues as they can only modulate incoming sound at the cochlear level. However, evidence exists that age-related hearing loss might partially be caused later in the hearing processes due to brain processes slowing down and becoming less efficient. In this study, we tested whether it would be possible to improve the hearing process at the cortical level by improving neural tracking of speech. The speech envelopes of target sentences were transformed into an electrical signal and stimulated onto elderly participants' cortices using transcranial alternating current stimulation (tACS). We compared 2 different signal to noise ratios (SNRs) with 5 different delays between sound presentation and stimulation ranging from 50 ms to 150 ms, and the differences in effects between elderly normal hearing and elderly hearing impaired participants. When the task was performed at a high SNR, hearing impaired participants appeared to gain more from envelope-tACS compared to when the task was performed at a lower SNR. This was not the case for normal hearing participants. Furthermore, a post-hoc analysis of the different time-lags suggest that elderly were significantly better at a stimulation time-lag of 150 ms when the task was presented at a high SNR. In this paper, we outline why these effects are worth exploring further, and what they tell us about the optimal tACS time-lag.

The aim of the study was to compare the effect of different spatial noise-processing algorithms in hearing aids on listening effort and memory effort on a subjective, behavioral, and neurophysiological level using electroencephalography (EEG). Two types of directional microphone (DM) technologies for spatial noise processing were chosen: one with a wide directionality (wide DM) and another with a narrower directionality (narrow DM) to accentuate the speech source. Participants with a severe hearing loss were fitted with hearing aids and participated in two EEG experiments. In the first one, participants listened to sentences in cafeteria noise and were asked to rate the experienced listening effort. The second EEG experiment was a listening span task during which participants had to repeat sentence material and then recall the final words of the last four sentences. Subjective listening effort was lower with narrow than wide DM and EEG alpha power was reduced for the narrow DM. The results of the listening span task indicated a reduction in experienced memory effort and better memory performance. During the memory retention phase, EEG alpha level for the narrow relative to the wide DM was reduced. This effect was more pronounced during linguistically difficult sentences. This study extends previous findings, as it reveals a benefit for narrow DM in terms of cognitive performance and memory effort also on a neural level, and when speech intelligibility is almost 100%. Together, this indicates that a narrow and focused DM allows for a more efficient neurocognitive processing than a wide DM.

In recent years, several studies have reported beneficial effects of transcranial alternating current stimulation (tACS) in experiments regarding sound and speech perception. A new development in this field is envelope-tACS: The goal of this method is to improve cortical entrainment to the speech signal by stimulating with a waveform based on the speech envelope. One challenge of this stimulation method is timing; the electrical stimulation needs to be phase-aligned with the naturally occurring cortical entrainment to the auditory stimuli. Due to individual differences in anatomy and processing speed, the optimal time-lag between presentation of sound and applying envelope-tACS varies between participants. To better investigate the effects of envelope-tACS, we performed a speech comprehension task with a larger amount of time-lags than previous experiments, as well as an equal amount of sham conditions. No significant difference between optimal stimulation time-lag condition and best sham condition was found. Further investigation of the data revealed a significant difference between the positive and negative half-cycles of the stimulation conditions but not for sham. However, we also found a significant learning effect over the course of the experiment which was of comparable size to the effects of envelope-tACS found in previous auditory tACS studies. In this article, we discuss possible explanations for why our findings did not match up with those of previous studies and the issues that come with researching and developing envelope-tACS.

Remote microphones (RMs) have been developed to support hearing aid (HA) users in understanding distant talkers. In traditional clinical applications, a drawback of these systems is the deteriorated speech intelligibility in the near field. This study investigates advantages and disadvantages of clinical RM usage and the effects of different directionality settings of the HAs in complex listening situations in the laboratory. Speech intelligibility was investigated in 15 experienced severely hearing impaired participants in a noisy environment using a dual-task test paradigm where the tasks were presented from either a near field or a far field loudspeaker. Primary and secondary tasks were presented simultaneously so attention had to be shared on both tasks. In a second experiment, two speech intelligibility tests were presented from either the near field or the far field loudspeaker. The tests were interleaved to simulate a complex listening situation with shifting attention. Directional HA microphones yielded better performance than omnidirectional microphones (both combined with a RM) in near field when analyzing both tasks of the dual-task experiment separately. Furthermore, the integrated dual-task test results showed better performance with directional HA microphones compared with the omnidirectional setting (both cases in combination with a RM). These findings were confirmed by the results of the interleaved speech intelligibility test.

OBJECTIVE: To report the development of a standardized German version of a reading span test (RST) with a dual task design. Special attention was paid to psycholinguistic control of the test items and time-sensitive scoring. We aim to establish our RST version to use for determining an individual's working memory in the framework of hearing research in German contexts. DESIGN: RST stimuli were controlled and pretested for psycholinguistic factors. The RST task was to read sentences, quickly determine their plausibility, and later recall certain words to determine a listener's individual reading span. RST results were correlated with outcomes of additional sentence-in-noise tests measured in an aided and an unaided listening condition, each at two reception thresholds. STUDY SAMPLE: Item plausibility was pre-determined by 28 native German participants. An additional 62 listeners (45-86 years, M = 69.8) with mild-to-moderate hearing loss were tested for speech intelligibility and reading span in a multicenter study. RESULTS: The reading span test significantly correlated with speech intelligibility at both speech reception thresholds in the aided listening condition. CONCLUSION: Our German RST is standardized with respect to psycholinguistic construction principles of the stimuli, and is a cognitive correlate of intelligibility in a German matrix speech-in-noise test.

OBJECTIVE: This paper describes the composition and international multi-centre evaluation of a battery of tests termed the preliminary auditory profile. It includes measures of loudness perception, listening effort, speech perception, spectral and temporal resolution, spatial hearing, self-reported disability and handicap, and cognition. Clinical applicability and comparability across different centres are investigated. DESIGN: Headphone tests were conducted in five centres divided over four countries. Effects of test-retest, ear, and centre were investigated. Results for normally-hearing (NH) and hearing-impaired (HI) listeners are presented. STUDY SAMPLE: Thirty NH listeners aged 19-39 years, and 72 HI listeners aged 22-91 years with a broad range of hearing losses were included. RESULTS: Test-retest reliability was generally good and there were very few right/left ear effects. Results of all tests were comparable across centres for NH listeners after baseline correction to account for necessary differences between test materials. For HI listeners, results were comparable across centres for the language-independent tests. CONCLUSIONS: The auditory profile forms a clinical test battery that is applicable in four different languages. Even after baseline correction, differences between test materials have to be taken into account when interpreting results of language-dependent tests in HI listeners.

In the framework of the European HearCom project, promising signal enhancement algorithms were developed and evaluated for future use in hearing instruments. To assess the algorithms' performance, five of the algorithms were selected and implemented on a common real-time hardware/software platform. Four test centers in Belgium, The Netherlands, Germany, and Switzerland perceptually evaluated the algorithms. Listening tests were performed with large numbers of normal-hearing and hearing-impaired subjects. Three perceptual measures were used: speech reception threshold (SRT), listening effort scaling, and preference rating. Tests were carried out in two types of rooms. Speech was presented in multitalker babble arriving from one or three loudspeakers. In a pseudo-diffuse noise scenario, only one algorithm, the spatially preprocessed speech-distortion-weighted multi-channel Wiener filtering, provided a SRT improvement relative to the unprocessed condition. Despite the general lack of improvement in SRT, some algorithms were preferred over the unprocessed condition at all tested signal-to-noise ratios (SNRs). These effects were found across different subject groups and test sites. The listening effort scores were less consistent over test sites. For the algorithms that did not affect speech intelligibility, a reduction in listening effort was observed at 0 dB SNR.
