BACKGROUND: Primary pulmonary lymphoepithelioma-like carcinoma (PPLELC) is an uncommon subtype of squamous cell carcinoma (SCC) of the lung, closely associated with Epstein-Barr virus (EBV) infection. The pathological features of PPLELC closely resemble those of SCC, which makes it prone to misdiagnosis. Surgical intervention constitutes the primary treatment approach for PPLELC. CASE SUMMARY: This report describes a 44-year-old woman who was hospitalized for 1 mo due to left chest pain. Computed tomography revealed a mass shadow in the anterior basal segment of the left lower lobe, and a subsequent needle biopsy suggested SCC. The patient underwent radical tumor resection in the lower left lobe of the lung, and postoperative pathological examination indicated lymphoepithelial carcinoma, and the test for EBV encoded small RNA was positive. Following surgery, the patient was scheduled to receive four cycles of adjuvant chemotherapy, using the paclitaxel + carboplatin regimen, but the patient refused further treatment. CONCLUSION: PPLELC is an exceptionally rare subtype of lung SCC and is prone to misdiagnosis.

Background: Geiparvarin (GN) is a natural compound with anticancer activity. However, the effect of GN on osteosarcoma (OS) and the anticancer mechanism of GN are still unclear. Methods: Cell viability was measured by MTT assay. Invasion and migration were measured by transwell assay. The miRNAs, genes, and signaling pathways affected by GN were confirmed by whole-genome sequencing and bioinformatics analysis. The expression level of mRNA and protein was measured by qRT-PCR and western blot. Animal experiment was performed for confirming the GN anticancer effect and side effect in vivo. Results: Our results show that GN significantly inhibits OS cell growth and metastasis in vitro. In vivo experiment also showed that GN dramatically suppressed OS lung metastasis and no side effects were found. GN treatment inhibited OS metastasis through upregulating the ANGPTL4 expression. In addition, GN inhibited the expression of miR-3912-3p, which targets ANGPTL4. Conclusion: Our data clearly indicate that GN is a candidate drug for OS treatment, and GN plays its role through miR-3912-3p/ANGPTL4 in OS.

In order to further understand the influence of high temperature shock on the microbial community structure of activated sludge during the process of nitrite oxidation, the enriched nitrifying activated sludge under different NO2--N concentration was taken as the research object in this study. 16S rRNA high-throughput sequencing technology was used to analyze the changes in the microbial community abundance and structural characteristics of activated sludge by changing the environmental temperature. The results of high-throughput sequencing showed that microorganisms were more likely to grow at 25℃, and the diversity of the microbial community in the activated sludge was the most abundant. With increased temperature, the richness, evenness, and diversity of the flora in the system decreased. In addition, it was found that the main nitrifying bacterium in the system was Nitrospira of Nitrospirae, whereby 35℃ was more suitable for its growth. Meanwhile, a higher temperature also caused differences in the structure of non-nitrifying functional microorganisms (e.g., Bacteroidetes, Chlorofulexi, Halomonas, and Pseudomonas) in the activated sludge. The results of this study provide some theoretical reference for the investigation of the distribution characteristics of microbial flora during the process of nitrite oxidation under high temperature shock, and can also be used as reference for relevant high temperature shock tests.

BACKGROUND: At present, minimally invasive endoscopic treatment is mostly used for patients with actively bleeding Dieulafoy's lesions, , as it has the advantages of minimal trauma, short operation time and good hemostatic effect, although bleeding can easily recur postoperatively. Recently, extensive gastric cuneiform resection has been advocated for use in these patients because the constant-diameter artery follows a long path to the gastric mucosa. CASE SUMMARY: A 47-year-old man was admitted to the hospital for repeated hematemesis and black stool, and he was diagnosed with Dieulafoy's disease. We chose a method that not only simulates surgical gastric cuneiform resection but also reduces trauma. We performed enlarged local endoscopic full-thickness resection of the gastric wall and abdominal constant-diameter artery and sutured the gastric wall. Postoperative follow-up showed that the constant-diameter artery had been resected from the gastric wall, which was confirmed to have no blood flow signals by endoscopic ultrasonography. CONCLUSION: Endoscopic full-thickness resection of the gastric wall and abdominal constant-diameter artery with suturing of the gastric wall has demonstrated potential as a new treatment for Dieulafoy's disease.

BACKGROUND: Paclitaxel treatment is a major cause of chemotherapy-induced peripheral neuropathy. The sodium channel Nav1.7 plays a critical role in pain perception. However, whether Nav1.7 in the dorsal root ganglion (DRG) is involved in paclitaxel-induced peripheral neuropathy remains unclear. Thus, our study aimed to evaluate whether Nav1.7 participates in the pathogenesis of paclitaxel-induced neuropathy. METHODS: Paclitaxel-induced peripheral neuropathy was generated by intraperitoneal administration of paclitaxel on four alternate days. RESULTS: The results showed that DRG mRNA and protein expression levels of Nav1.7 were upregulated between days 7 and 21 after the administration of paclitaxel. Besides, paclitaxel upregulated extracellular signal-regulated kinase (ERK1/2) phosphorylation in DRG. Intrathecal injection of U0126 (a MEK inhibitor) blocking ERK1/2 phosphorylation blunted up-regulation of Nav1.7 in the DRG and correspondingly attenuated hyperalgesia. CONCLUSION: These results indicated that the sodium channel Nav1.7 in the DRG exerted an important function in paclitaxel-induced neuropathy, which was associated with ERK phosphorylation in neurons.

OBJECTIVE: To investigate the independent risk factors, outcomes and genotypes associated with carbapenem-non-susceptible K. pneumoniae bloodstream infections (BSIs) in northern China from 2014 to 2016. METHODS: Over a three-year period, a total of 289 K. pneumoniae BSI patients were identified. Medical records were extracted to obtain the clinical information. Polymerase chain reactions (PCRs) were performed to analyse the multilocus sequence typing (MLST) types, Klebsiella pneumoniae carbapenemase (KPC) and metallo-ß-lactamases (MBL) genes, for replicon typing of the 10 randomly selected carbapenem-non-susceptible K. pneumoniae. RESULTS: A total of 59 carbapenem-non-susceptible K. pneumoniae strains were identified. Resistance rates to imipenem, meropenem, ertapenem and amikacin were low. Multivariate analyses showed that a central venous catheter odds ratio (OR) of 4.021 (CI 1.002-16.134); mechanical ventilation of 7.587 (2.856-20.156); Pitt bacteraemia score of 1.481 (CI 1.218-1.800); hospitalization prior to culture of 1.026 (CI 1.001-1.053); and some antibiotic use 30 days prior to K. pneumoniae bacteremia, including carbapenem of 9.123 (CI 2.995-27.791), aminoglycoside of 34.079 (2.091-555.396), and tigecycline of 5.065 (CI 1.261-20.339) were associated with carbapenem-non-susceptible K. pneumoniae bacteremia. Sequence type 11 (ST11) was the most predominant MLST type, which accounted for 50% of the isolates. Eighty per cent of the isolates harbored the KPC-2 gene. The overall 28-day mortality rates of carbapenem-non-susceptible and carbapenem-susceptible K. pneumoniae were 54.24% and 19.56%, respectively. CONCLUSION: Central venous catheter, mechanical ventilation, high Pitt bacteraemia score, hospitalization prior to culture, and prior antibiotic use (carbapenem, aminoglycoside and tigecycline) were identified as independent risk factors for carbapenem-non-susceptible K. pneumoniae BSI, which was mostly caused by KPC-2 in northern China.

BACKGROUND & OBJECTIVE: Although the roles of CD40 in B cells have been intensively studied, little is known on the function of CD40 in lung cancer cell lines. This study was to investigate biological effects of soluble CD40 ligand (sCD44L) on lung cancer cell line A549 (CD40 positive), and its possible mechanism. METHODS: A549 cells were co-incubated with sCD40L, cell proliferation was detected by MTT assay and 3H-TdR incorporation method. Immunofluorescence technique and flow cytometry (FCM) were used to evaluate changes in cell phenotypes and cell cycle. Cell apoptosis, and expression changes of Bcl-2 and Bax were observed by FCM, reverse transcriptase-polymerase chain reaction (RT-PCR), and Western blot. RESULTS: Compared with control cells, proliferation of A549 cells co-incubated with sCD40L was inhibited (P< 0.05). Positive rates of cell surface molecules, CD49e, CD54, TNFRI, and CD95L, in A549 cells co-incubated with sCD40L for 72 h were (61.2+/-4.8)%, (31.2+/-6.1)%,(42.7+/-5.9)%, and (38.2+/-3.4)%, respectively, while those in control cells were (34.7+/-2.1)%, (7.1+/-1.6)%, (15.2+/-4.1)%, and (10.1+/-2.3)%, respectively (P< 0.05). However, positive rate of TNFRII in A549 cells co-incubated with sCD40L[(8.7+/-0.8)%] was lower than that in control cells [(58.1+/-3.6)%] (P< 0.05). G1 phase of A549 cells treated with sCD40L for 72 h was (76.0+/-9.1)%, more than that of control cells [(56.7+/-6.9)%], while S phase of sCD40L-treated A549 cells [(10.3+/-5.7)%] was less than that of control cells [(32.7+/-5.5)%]. No significant apoptosis of A549 cells was observed after co-incubated with sCD40L for 72 h, but Bax expression was up-regulated. CONCLUSION: sCD40L may inhibit cell proliferation, cause changes in phenotype and cell cycle of A549 cells, and alter expression of apoptosis-associated gene, such as Bax.

RESUMEN

This study aims to modify the surgical procedure of conventional trabeculectomy for glaucoma by incorporating scleral flap preparation with crescent tunnel knife under surface anesthesia. A total of 41 patients (48 eyes) received this modified surgical procedure, with the anesthesia being successfully implemented in all the cases. The time consumption by the operation on each eye averaged 17 min, shortened by 37% compared with the conventional procedure. The postoperative intraocular pressure was well under control without any complications arising either during or after the operation, showing the safety, rapidness and efficacy of this surgical approach in the treatment of glaucoma.
