RESUMEN

Background: Despite the high incidence of lateral neck lymph node (LN) metastasis in papillary thyroid cancer (PTC), the management of the lateral neck remains controversial. We aimed to map the draining LNs in the lateral neck using carbon nanoparticles and explore its potential in neck evaluation. Methods: We conducted a multicenter, prospective study in PTC patients who had non-palpable yet suspicious metastatic lateral LNs on ultrasound and/or computed tomography (CT) but could not be confirmed by fine needle aspiration. Carbon nanoparticle suspension was injected peritumorally into the thyroid and modified lateral neck dissection was subsequently performed. Results: A total of 154 patients were enrolled for analysis. And 5,070 lateral LNs were removed, of which 1,079 (21.3%) were dyed. The median of dyed LNs was 6 per case (range, 1-33). The distribution of dyed LNs in neck compartments was IV > III > IIA > IIB/V, independent of tumor size, location, multifocality or microscopic extra-thyroidal extension (ETE). Compared with undyed LNs, the probabilities of metastasis in dyed LNs were significantly increased in compartment III, IV, V, and II-V (III: 29.3% vs. 15.4%, P<0.001; IV: 26.3% vs. 14.5%, P<0.001; V: 16.7% vs. 3.3%, P=0.005; II-V: 26.3% vs. 10.0%, P<0.001). The relative risks of metastasis in dyed LNs compared with undyed LNs were 1.90, 1.82, 5.04 and 2.62 in compartment III, IV, V, and II-V, respectively. Conclusions: It was the first prospective multicenter study to map the lateral neck LNs with carbon nanoparticles, which could help surgeons visualize the suspicious LNs during surgery. Instead of unguided LN biopsy, this method has a potential role in lateral neck assessment for indeterminate lateral LNs in PTC.

To study the effect of different cationic polyacrylamide organic dehydrating agents on sludge dewatering performance, eight commercially available cationic polyacrylamides of the same series with different properties were used. Based on the different cationic degree, they were named 9101, 9102, 9103, 9104, 9106, 9108, 9110, and 9112, respectively. Their properties were characterized by instruments and chemical analysis, and the indexes of sludge after treatment were also measured. The results showed that the properties of the eight organic dehydrants were different, among which the charge density, cationicity, viscosity, and Zeta potential had homologous trends, which all increased gradually from 9101 to 9112. The four indexes of 9112 were as high as 2.98 meq·L-1, 17.42%, 85.07 mPa·s, and 67.10 mV, respectively. The dewatering performance of sludge was improved by improving the specific resistance of filtration (SRF), floc properties, viscosity, Zeta potential, the bound water content, and the distribution of extracellular polymeric substances (EPS) after dosing organic dewatering agents. The results showed that the viscosity, charge density, cationic degree, and Zeta potential of the dewatering agents had a great influence on the sludge dewatering performance. The SRF of sludge was negatively correlated with the viscosity of the organic dewatering agent, and the correlation coefficient was as high as 0.92025, indicating that the sludge dewatering performance was improved mainly through the adsorption bridging effect of the organic dewatering agent in sludge dewatering.

OBJECTIVE: To explore the effect of oxycodone hydrochloride combined with dexmedetomidine on the recovery quality and stress response during anesthesia in patients undergoing laparoscopic cholecystectomy (LC). METHODS: Ninety patients who had LC in Cangzhou Hospital of Integrated TCM-WM of Hebei from December 2016 to December 2019 were selected and divided into dexmedetomidine group (DEX group), oxycodone hydrochloride group (Q group), dexmedetomidine + oxycodone hydrochloride group (DQ group) by a random number table method, with 30 cases in each group. At the time before anesthesia induction (T0), and immediately (T1), 1 min (T2), 10 min (T3) and 30 minutes (T4) after extubation, the general vital signs of three groups were observed, and plasma cortisol (COR), epinephrine (E), norepinephrine (NE) and blood glucose (GLU) were measured. The spontaneous respiration recovery time, wake-up time, VAS score of each time period after extubation, extubation quality score, and adverse event rate were recorded. RESULTS: The vital signs at each time point of extubation, recovery time of spontaneous respiration, wake-up time, and extubation quality of DQ group were better than those of DEX group and Q group (P<0.05). The incidence of agitation, VAS score at T2 and T3, plasma concentrations of Cor, E, NE and Glu at T1, T3 and T4 in DQ group were significantly lower than those in Q group and DEX group (P<0.05). CONCLUSION: Oxycodone hydrochloride combined with dexmedetomidine can improve the recovery quality and reduce stress response in patients with LC after anesthesia, and can be safely used in patients with LC.

MYB transcription factors play pivotal roles in hormone conduction signaling and abiotic stress response. In this study, 54 differentially expressed ZmMYB genes were identified and comprehensive analyses were conducted including gene's structure, chromosomal localization, phylogenetic tree, motif prediction, cis-elements and expression patterns. The results showed that 54 genes were unevenly distributed on 10 chromosomes and classified into eleven main subgroups by phylogenetic analysis, supported by motif and exon/intron analyses. The mainly stress-related cis-elements were ABRE, ARE, MBS and DRE-core. In addition, 8 core ZmMYB genes were identified by co-expression network. qRT-PCR results showed that the 8 ZmMYB genes exhibited different expression levels under different abiotic stresses, indicating that they were responsive to various abiotic stress. These results will provide insight for further functional investigation of ZmMYB genes. In the title compound, C(15)H(13)N(3)O(4), the pyridine and benzene rings are nearly coplanar [dihedral angle = 4.92 (12)°]. The maximum deviation from the best least-squares plane calculated for the main mol-ecular skeleton is 0.1722 (1) Šfor the carbonyl O atom. In the crystal, inter-molecular O-H⋯O hydrogen bonds connect the mol-ecules into a chain, while π-π stacking inter-actions between the pyridine and benzene rings [centroid-centroid distance = 3.9162 (8) Šand offset angle = 27.20°] complete a two-dimensional network.
