RESUMEN

A 27-year-old female visited our hospital with a history of asthma, peripheral blood eosinophilia, increased total IgE, Aspergillus fumigatus specific IgE, reversible mild bronchiectasis, sinusitis, bronchial mucus plugs and cultivation of Aspergillus from BALF. Glucocorticoids therapy is effective. These results met the diagnostic criteria for both allergic bronchopulmonary aspergillosis (ABPM) and eosinophilic granulomatosis with polyangiitis (EGPA). Special attention should be paid to the possibility of both diseases coexisting in the disease process.

BACKGROUND: Neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) are closely related to the prognosis of patients with non-small cell lung cancer, but their effect on extensive-stage small cell lung cancer (ES-SCLC) remains uncertain. METHODS: This retrospective study was conducted in ES-SCLC patients treated with first-line atezolizumab or durvalumab and platinum-etoposide. Clinical data from three hospitals were analyzed. Significant risk factors for survival were identified using descriptive statistics and Cox regression. Homogeneity was assessed using t-tests or nonparametric tests. Kaplan-Meier analysis revealed an association between high NLR level and median PFS and OS. RESULTS: A total of 300 ES-SCLC patients were included in the study. Cox regression analysis revealed that an elevated NLR level after the second treatment cycle (defined as NLRT2) was an independent prognostic factor for survival. Stratifying patients based on median NLRT2 showed significant differences in both PFS (HR: 1.863, 95% CI: 1.62-2.12, p < 0.001) and OS (HR: 2.581, 95% CI: 2.19-3.04, p < 0.001) between NLR ≥ 1.75 and NLR < 1.75 groups. mPFS and mOS were 8.2 versus 6.1 months and 13.7 versus 9.5 months, respectively. NLR was also associated with treatment efficacy and occurrence of irAEs. Further stratification based on NLR and irAEs showed that in the NLR < 1.75 group, patients with irAEs had prolonged mPFS and mOS. In the NLR ≥ 1.75 group, only mPFS showed a significant difference between patients with and without irAEs. CONCLUSION: NLRT2 and irAEs can predict the prognosis of ES-SCLC patients with first-line ES-SCLC receiving PD-L1 inhibitors combined with chemotherapy.

The bio-based coatings of cellulose fabrics (cotton) had attracted increasing attention for multifunction and sustainability but suffered from poor durability and low efficiency. Here, the aldehyde-free and durable coatings for cotton fabrics (CPZ@CF) with satisfactory flame retardancy, antibacteria as well as wearing performance were prepared through the interfacial coordination effect where the well-organized zinc phytate complex were in situ grew on the pre-treated surface of cotton fabrics with chitosan (CS) and Zn2+. The CZP@CF exhibited excellent antibacterial activity for Staphylococcus aureus (S. aureus) and Escherichia coli (E. coli) with 99.99 % antibacterial rates benefiting from the synergistic effect between Zn2+ and CS. Meanwhile, even the CPZ coatings loading was only 1.5 wt%, the fire safety of CZP@CF remarkably enhanced owing to the excellent synergistic catalytic charring and free radical capture. More importantly, the antibacterial rates of CZP@CF for S. aureus and E. coli still reached 99.99 % and 91.67 % after 50 washing cycles. Additionally, this treatment method did not deteriorate the fabrics properties, including mechanical and breathability as well as wearing performance, which provided the approach to fabricate the flame retardant and antibacterial textiles with well durability and wearing performance.

Lighting assessment in special operating environments like enclosed spaces is of great research significance and value. In addition to investigating the visual ergonomics of workers, the emotional monitoring and guidance of workers in enclosed spaces is also a research focus. Based on the Circumplex emotion model theory, this paper designs an experiment to assess emotions in an enclosed space with 6 different lighting settings (2 correlated color temperature (CCT) × 3 illuminance levels). From the perspective of subjective assessment, participants used a rapid sensory analysis method (Check-all-that-apply, CATA) and a Subjective Coordinate Scale (SCS) method for rapid ambience perception checking and emotional self-reporting of lighting settings. From the perspective of objective evaluation, the participants' facial expressions were recorded during the experiment using a camera, and then the recorded facial expressions were automatically analyzed and predicted using FaceReader (FRE) software. The CATA and SCS showed similar results, with the 3100 K × 600 lx, 3100 K × 1000 lx and 6500 K × 600 lx lighting settings creating a relaxed, pleasant emotion in participants, the 6500 K × 1000 lx setting creating an excited, tense atmosphere, and the low illumination level settings of 3100 K × 250 lx and 6500 K × 250 lx made participants feel tired and frustrated. The results of the objective emotion analysis indicate that the FRE was able to effectively identify differences in participants' emotions in response to different lighting settings and was consistent with the results of participants' subjective emotion reports. This laboratory study validates that the three methods can effectively assess the enclosed space lighting settings, and provides a reference for further research on the enclosed space lighting settings and emotional monitoring of workers in the future.

In this study, a personal acoustic comfort prediction model (PACPM) for exploring the acoustic comfort of oceanauts in a deep-sea manned submersible cabin was proposed, and an oceanauts' task performance model (OTPM) was constructed in this study. Based on oceanauts' comfort and task performance, the change characteristics in six different pure-noise environments (the sound pressure levels of the noise audio are 40 dB (A), 45 dB (A), 50 dB (A), 55 dB (A), 60 dB (A), and 65 dB (A) respectively) were analysed. An effective method for improving acoustic comfort was proposed. According to the analysis, personal comfort at 40 and 45 dB(A) was higher than that at other noise levels. The oceanaut's comfort and task performance of normal-weight people were significantly higher than those of thin people. Meanwhile, a comprehensive consideration of the demographic characteristics and physiological responses can effectively improve the prediction accuracy of the personnel acoustic comfort. Furthermore, the 45 dB (A) pure-noise environment overlaid with 40 dB(A) fast-paced light music effectively improves oceanauts' comfort. Practitioner summary: This study provides a convenient and available method for analysing acoustic comfort in the cabins of deep-sea manned submersibles, including a quantitative prediction model and an effective method for sound environment improvements. These can be used to improve the comfort, task performance, and working efficiency of manned submersibles.

BACKGROUND: The cabin of the manned submersible is a narrow and sealed space. The long-time work of oceanauts is easy to cause muscle fatigue and stiffness, which reduces efficiency. This paper explored the relative more effective way to relieve leg muscle fatigue of the oceanauts in the manned submersible. MATERIAL AND METHODS: Twenty healthy young people with an average age of 24 years were selected as the participants, while the usual natural relaxation and the stretching method proposed by the surveys were selected as the relieve method for muscle fatigue. This study compared the effects of the 2 recovery ways using the electromyography data from the quadriceps femoris and biceps femoris, and the subjective scales. RESULTS: With the support of data of integral electromyogram (iEMG), root-mean-square (RMS), subjective scales, authors finally found that the 2 muscles of the leg are fatigued significantly, and biceps femoris has more accumulation of fatigue. Although the 2 recovery ways have a significant relief effect on muscle fatigue, the stretching method can reduce more muscle fatigue in a short period of time, especially for biceps femoris. The stretching method is advocated for the oceanauts. CONCLUSIONS: The findings suggested that the stretching method was optimal for relieving the muscle fatigue of the oceanauts during the operation. Med Pr. 2022;73(2):95-107.

BACKGROUND: Non-small cell lung cancer (NSCLC) accounts for 85% of lung cancer which is the most frequently diagnosed malignancy in China. Colon cancer associated transcript 1 (CCAT1) acts as an oncogene in enhancing tumor progression. However, the effects of CCAT1 in NSCLC remain unclear. The purpose of this study was to explore the role of CCAT1 in NSCLC. METHODS: Wound healing and transwell assays were performed to measure cell migration. RT-qPCR was employed to calculate the mRNA level of CCAT1 and miR-490. RESULTS: High expression of CCAT1 was observed in NSCLC tissues and cells, with low expression of miR-490. CCAT1 promoted the proliferation and metastasis of H1299 and A549 cells, while miR-490 had the opposite effect. CCAT1 could specifically bind to miR-490 and regulate its expression. MiR-490 partially reversed the inhibitory effect of CCAT1 on cell proliferation and metastasis. CONCLUSIONS: The CCAT1/miR-490 molecular axis has been shown to be important for the treatment of NSCLC.

Solitary fibrous tumor of the pleura (SFTP) is a rare disease, and most published case reports are in patients over 40 years old. We report a case of SFTP in a 22-year-old woman. The imaging features were observed using contrast-enhanced computed tomography (CT), and histomorphological features were evaluated using pathology and immunohistochemistry. The CT showed a mass in the pleura inside the ninth rib on the left. Pathological results of percutaneous puncture in the chest suggested the possibility of solitary fibroma. The patient underwent surgical resection, and the tumor measured 2.5 × 1.5 × 1.5 cm with an intact capsule. Pathological examination revealed a spindle cell tumor, and immunohistochemistry showed strong positive staining for CD34 and STAT6, consistent with typical solitary fibroma. Although SFTP is rare in young patients, early diagnosis and intervention are needed to avoid the possibility of future complications.

The present study aimed to investigate the factors associating with the presence of residual thrombosis in patients with acute pulmonary embolism (APE) after at least 3-month anticoagulant therapy. Demographic and clinical data of 180 cases in the affiliated hospital of Qingdao University from January 2005 to June 2015 were retrospectively analyzed. APE in all patients were confirmed by computed tomography pulmonary angiography (CTPA). Patients were then detected for the presence of residual thrombosis according to a second CTPA. After appropriate comparison test, multivariate logistic regression analysis was performed to identify predictors for residual thrombosis. Among 180 patients, complete clearance of thrombosis occurred in 115 (63.9%) patients. Residual thrombosis remained in 65 (36.1%) patients. The independent factors associating with residual thrombosis include unprovoked APE (OR 0.231, 95% CI 0.062-0.861) and fibrinogen level in acute phase (OR 1.958, 95% CI 1.282-2.911). Furthermore, these two variables were both associated with the presence of residual thrombosis in patients receiving different parenteral anticoagulants (unfractionated heparin or low-molecular-weight heparin). Pulmonary thrombosis in some patients with APE are not completely dissolved after at least 3-month treatment. Additionally, unprovoked APE is positive predictor of decreased residual thrombosis and fibrinogen level in acute phase is a risk factor of the presence of residual thrombosis.

The imbalance of Th17/Treg cells plays an important role in the pathogenesis of chronic obstructive pulmonary disease (COPD). Caveolin-1 (Cav-1) has been regarded as a potential critical regulatory protein in pathological mechanisms of chronic inflammatory respiratory diseases. Therefore, we investigated whether the loss of Cav-1 is involved in the homeostasis of Th17/Treg cells in COPD. We examined the expressions of plasma Cav-1 and circulating Th17, Treg cells, and the related cytokines in patients with COPD. Enzyme-linked immunosorbent assay (ELISA) analyses showed a significant reduction of plasma Cav-1 levels in patients with stable COPD (SCOPD) and acutely exacerbated COPD (AECOPD) compared to smokers without COPD. This loss was associated with an increase in frequency of Treg and decreased in frequency of Th17 cells. To further identify the role of Cav-1, we studied the effects of Cav-1 overexpression or downregulation on frequencies of Treg and Th17 cells in peripheral blood mononuclear cells (PBMCs) from subjects. Interestingly, small interfering RNA (siRNA) downregulation of Cav-1 was accompanied by an augmentation of Treg and reduction of Th17 expression. Together, our study demonstrated that the loss of Cav-1 contributed to the imbalance of Th17/Treg cells in patients with COPD.

OBJECTIVE: This study evaluated the expression level of high mobility group box-B1 (HMGB-1) and matrix metalloproteinase-9 (MMP-9) in non-small cell lung cancer (NSCLC) inmorder to reveal any relation with development and prognosis. METHODS: NSCLC and normal tissues were selected from 30 patients at age of 30- 73, and used for RT-PCR and Western blot analyses of HMGB-1. A total of 100 paraffin embedded NSCLC tissues were also isolated from patients through surgical resection, and used for detection of HMGB-1 by immunohistochemistry. In addition, 50 samples were also applied for MMP-9 detection, and 30 normal tissues were considered as controls. Correlation analysis of HMGB-1 and MMP-9 was carried out by Pearsons correlation coefficient. RESULTS: The average expression level of HMGB-1 in NSCLC patients was significantly higher than in normal lung tissues. In addition, patients in III-IV period exhibit significantly higher positive rate of HMGB- 1 when compared with I-II period cases. Furthermore, a positive correlation with HMGB-1 was found in the expression of MPP-9. CONCLUSION: HMGB-1 was highly expressed in NSCLC, which may become a prognostic and predictive marker for NSCLC. Besides, MPP-9 was positively correlated with HMGB-1.

OBJECTIVE: To evaluate the incidence of pulmonary embolism in patients with acute exacerbations of chronic obstructive pulmonary disease (COPD). METHODS: Comprehensive searches as of June 2012 were performed in PubMed (1966-), Embase (1974-), Chinese Biomedical Literature Database (1978-), Chinese Journal Full-text Database (1979-) and VIP Database (1989-) for literatures on the incidence of pulmonary embolism in patients with acute exacerbations of COPD. Meta-analysis was conducted with Stata version 11.0. RESULTS: Among 2273 articles identified, 5 studies met the inclusion criteria (4 in English, 1 in Chinese). The total sample size was 762 patients, among whom 145 were diagnosed with pulmonary embolism. The incidence of pulmonary embolism ranged from 3.3% to 33.0%. Meta-analysis showed that the combined incidence was 15.8% (95%CI: 5.1%-26.4%). Among patients with acute exacerbations of COPD of unknown etiology, the incidence was 29.0% (95%CI: 20.8%-37.1%). CONCLUSIONS: There is a high incidence of pulmonary embolism in patients with acute exacerbations of COPD, especially among those of an unknown etiology. More attention should be paid to this population.
