RESUMEN

Gallbladder abdominal wall fistula is usually due to the acute cholecystitis with-out timely treatment, which leads the formation of abscess around the gallbladder, the gallbladder adhering to the abdominal wall and the abscess infiltrating into the skin to form a spontaneous abdominal wall fistula. Patient with gallbladder abdominal wall fistula may have the symptoms of cholecystolithiasis and acute cholecystitis. Ultrasound examination can detect the situation of gallbladder conveniently, including the internal echo after formation of abscess, the connection between the gallbladder and abdominal cavity, and the blood flow signal, to clarify the diagnosis for the subsequent treatment. The authors share the diagnosis and treatment experiences of an elderly patient with gallbladder abdominal wall fistula.

RESUMEN

Tubulin affects platelets count through the control of mitosis and the formation of pro-platelets during the maturation of megakaryoblast to platelets. Tubulin is involved in maintaining the integrity of platelet skeleton, and also participates in the change of platelet morphology during platelet activation. Some new anti-tumor drugs targeting cell mitosis are trying to reduce the effect on tubulin in order to reduce the side effect of drugs on platelet formation. In some patients with thrombocytopenia, the variation and polymorphism of the tubulin gene affect the structure of microtubule multimers, which leads to the decrease of platelet formation. This review summarized the latest progresses of tubulin in the regulation of megakaryopoiesis and thrombopoiesis.

RESUMEN

OBJECTIVES@#To study the association of the anti-oxidative damage factors nuclear factor erythroid 2-related factor 2 (Nrf2), heme oxygenase-1 (HO-1), and NAD(P)H:quinone oxidoreductase-1 (NQO1) with preterm premature rupture of membranes (PPROM).@*METHODS@#A prospective study was conducted. The neonates who were hospitalized in Yanbian Hospital from 2019 to 2020 were enrolled as subjects, among whom there were 30 infants with PPROM, 32 infants with term premature rupture of membranes (TPROM), and 35 full-term infants without premature rupture of membranes (PROM). Hematoxylin and eosin staining was used to observe the inflammatory changes of placental tissue. Immunohistochemical staining was used to measure the expression of Nrf2, HO-1, and NQO1 in placental tissue. Western blot was used to measure the protein expression levels of Nrf2, HO-1, and NQO1 in placental tissue.@*RESULTS@#Compared with the PPROM group, the TPROM group and the non-PROM full-term group had significantly higher positive expression rates and relative protein expression levels of Nrf2, HO-1, and NQO1 in placental tissue (P<0.05). There were no significant differences in the positive expression rates and relative protein expression levels of Nrf2, HO-1, and NQO1 in placental tissue between the TPROM and non-PROM full-term groups (P>0.05).@*CONCLUSIONS@#The low expression levels of Nrf2, HO-1, and NQO1 in placental tissue may be associated with PPROM, suggesting that anti-oxidative damage is one of the directions to prevent PPROM.

RESUMEN

ObjectiveThis study investigated the mechanism of Wenjingtang in the prevention and treatment of endometriosis (EMT) from the perspective of regulating hypoxia stress and mitochondrial function. MethodPrimary human endometrial stromal cells (ESCs) form ectopic endometrial tissues were isolated and cultured, the cells were divided into control group (Control), 5% control serum group (5% KBXQ), 10% control serum group (10% KBXQ), 5% Wenjingtang serum group (5% WJTXQ) and 10% Wenjingtang serum group (10% WJTXQ). ESCs in different groups were detected for proliferation by methyl thiazolyl tetrazolium (MTT) assay, mRNA and protein expression of hypoxia inducible factor-1α (HIF-1α) by real-time fluorescent quantitative polymerase chain reaction (Real-time PCR) and Western blot analysis, mitochondrial ultrastructure by transmission electron microscope, mitochondrial function [mitochondrial membrane potential, mitochondrial membrane potential(MMP) and cytochrome C(Cyt C) content] and apoptosis (cell membrane permeability, nuclear fluorescence intensity, nuclear size and cell counts) by high content screening (HCS) assay, apoptosis rate by flow cytometry, and proteins of B-cell lymphoma-2 (Bcl-2) associated X (Bax), Bcl-2 and cleaved cysteinyl aspartate specific proteinase-3 (cleaved Caspase-3) by Western blot. ResultCompared with Control group, the 5% KBXQ and 10% KBXQ groups showed increased cell viability (P<0.01), there was no significant change in HIF-1α mRNA and protein expression, transmission electron microscopy showed that the mitochondrial cristae were obvious and the inner and outer membranes of mitochondria were clear, HCS multichannel fluorescence staining showed that there were no significant changes in the expression of MMP, Cyt C and cell membrane permeability, and the nuclei showed uniform light staining, there were no significant changes in apoptosis rate, cleaved Caspase-3 protein expression and Bax/Bcl-2 ratio. Compared with Control group and corresponding concentration KBXQ group, the 5% WJTXQ and 10% WJTXQ group showed decreased cell viability (P<0.01) and HIF-1α mRNA and protein levels (P<0.05,P<0.01), the ultrastructure of mitochondria was destroyed, some mitochondria were swollen and the cristae were blurred, moreover, decreased MMP and up-regulated Cyt C release (P<0.05,P<0.01), increased cell membrane permeability (P<0.01), and apoptosis characteristics included nuclear pyknosis, DNA agglutination in nucleus and decrease of cell numbers were observed (P<0.05,P<0.01), increased apoptosis rate (P<0.01), which was consistent with the results of HCS analysis, and up-regulated expression levels of cleaved Caspase-3 protein and Bax/Bcl-2 ratio (P<0.05,P<0.01). ConclusionIn conclusion, the results suggest that Wenjingtang can improve hypoxia stress via down-regulating HIF-1α expression in ectopic ESCs, and inhibit cell proliferation, reduce mitochondrial biological activity and induce apoptosis, which might be the internal mechanism of Wenjingtang in preventing and treating EMT.

RESUMEN

BACKGROUND: Recent studies have revealed that gut microbiota play an important part in the regulation of the immune function. With the development of newer detection methods, our cognition of the human gut microbiota continues to evolve with startling speed, but our understanding of the changes in the structure and function of gut microbiota before and after renal transplantation and the practical applications of this knowledge are still in their infancy. METHODS: We prospectively recruited 10 renal transplant recipients and collected serial fecal specimens (N = 30) before the operation, and on the 7th and 30th day after the operation, and characterized their gut microbiota structure through deep sequencing of the 16S rRNA V4-V5 variable region and analyzed the presence of metabolites using LC-MS methods. RESULTS: A decrease in the relative abundance of overall gut microbiota was detected in post-transplantation samples compared to that in pre-transplantation samples. Principal coordinate analysis (PCoA) inhibited a obvious separation between the three groups, and the linear discriminant analysis effect size (LEfSe) method showed that Clostridiales, Clostridia, Ruminococcaceae, Faecalibacterium, and Veillonellaceae were all significantly more abundant in the fecal specimens from the pre-transplantation group while Bacilli, Enterococcaceae, and Enterococcus were significantly more abundant in the fecal specimens from the four weeks post-transplantation group. Anaerostipes and Clostridia-bacterium were detected in the fecal samples from the one week post-transplantation group. Analysis of community composition did not reveal any significant difference between the pre-transplantation group and the post-transplantation group. The metabolic profiling of the volunteers before renal transplantation were distinct from the post-transplantation profiling, which gather together in PCA (Fig. 4A). After renal transplantation, the metabolic profiling of post-transplantation specimens revealed marked diversity and complexity. CONCLUSIONS: Our research indicated remarkable variations in the gut microbiota and metabolites following renal transplantation, and that the gut microbiota and metabolites of patients with uremia were relatively stable and showed reasonable concordance. Distinct microbial compositions and metabolites were observed in patients after transplantation.

RESUMEN

The aim of the work was to investigate the correlation between serum TSH (thyrotropin) levels within normal range and serum lipids. A total of 1962 subjects with normal thyroid function were enrolled. The subjects were divided into four groups according to the quartiles of serum normal TSH levels, [Q1 (0.27-1.68) mIU/l, Q2 (1.69-2.35) mIU/l, Q3 (2.36-3.07) mIU/l, and Q4 (3.08-4.20) mIU/l]. The effect of serum normal TSH levels on serum lipid profiles of different age or gender was analyzed. The total cholesterol (TC), triglyceride (TG), and low-density lipoprotein cholesterol (LDL-C) levels of the Q3 group and TG levels of the Q4 group were higher than those of the Q1 group in youth (p <0.05). The TC levels of the Q3 group were higher than those of the Q2 group in middle age (p <0.05). The LDL-C levels of middle age or elderly were higher than those of youth at the same TSH levels (p <0.05), while the TC levels of middle age were higher than those of youth in Q1, Q3, or Q4 group (p <0.05), and the TC and HDL-C levels of elderly were higher than those of youth in the Q2 group (p <0.05). The TG levels of the Q3 group were higher than those of Q1 group in males (p <0.05). The LDL-C levels of the Q3 group were higher than those of the Q1 group in females (p <0.05). In conclusion, the normal serum TSH levels were found to be closely related to serum lipid profiles, and with increasing TSH levels, serum lipids levels increased gradually.

RESUMEN

OBJECTIVE@#To explore the prognostic factors of young and middle-aged patients with acute myeloid leukemia (AML) and the predictive value of minimal residual disease (MRD) before consolidation therapy.@*METHODS@#The clinical data of 262 middle-risk young and middle-aged patients with AML treated in our hospital from January 2010 to December 2018 were selected retrospectively. All the patients were reached morphological leukemia-free state (MLFS) after induction chemotherapy, the overall and subgroup clinical data of the selected patients were analyzed. Cox regression model was used to evaluate the independent prognostic factors of middle-risk newly diagnosed young and middle-aged patients.@*RESULTS@#Among the patients less than 40 years old treated by consolidation therapy with PR-CT and allo-HSCT regimens, the 5-year cumulative leukemia-free survival(LFS) rates were 40.92% and 63.51%(P=0.01)respectively, while those over 40 years old were 23.61% and 49.14%(P=0.00), respectively. The 5-year cumulative LFS rates of the patients treated by chemotherapy and achieved early remission and late remission were 63.51% and 41.33% (P=0.01), respectively. The 5-year cumulative overall survival(OS) rates of the patients treated by PR-CT and allo-HSCT regimens were 23.65% and 69.32% (P=0.00), respectively, and the 5-year cumulative LFS rates were 26.44% and 52.30% (P=0.01). Among the patients treated by PR-CT consolidation treatment, the MRD-negative and MRD-positive cases were 74 and 60 cases, respectively. The 5-year cumulative incidence of relapse rate in the MRD-negative subgroup was significantly lower than those in the MRD-positive subgroup (P<0.05), the 5-year LFS rate and OS rate of the patients in MRD-negative subgroup were significantly higher than those in MRD-positive subgroup (P<0.05). For the patients treated by allo-HSCT consolidation treatment, the MRD-negative and MRD-positive cases were 66 and 62 cases, respectively. The 5-year cumulative incidence of relapse rate of the patients in MRD-negative subgroup was significantly lower than those in MRD-positive subgroup(P<0.05), and the 5-year LFS and OS rates of the patients in MRD-negative subgroup were significantly higher than those in MRD-positive subgroup (P<0.05). The univariate analysis results showed that age, chromosome karyotype, MRD status after reaching MLFS, and consolidation treatment regime were all related to the prognosis of patients (P<0.05). The multivariate analysis results showed that age, MRD status after reaching MLFS, and consolidation therapy were the independent factors affecting the cumulative OS rate of the patients (P<0.05). Chromosome karyotype was an independent factor affecting the cumulative LFS rate of the patients (P<0.05). MRD status and consolidation treatment plan after reaching MLFS were the independent factors affecting the cumulative recurrence rate of the patients (P<0.05).@*CONCLUSION@#The OS rate of middle-risk young and middle-aged patients with newly diagnosed AML is independently related to age, MRD status after MLFS and consolidation therapy, while chromosome karyotype is independently related to cumulative LFS, and allo-HSCT consolidation therapy is recommended for middle-risk young and middle-aged AML patients after induction chemotherapy for MLFS, especially for those less than 40 years old and MRD positive before consolidation therapy.

RESUMEN

To establish the method for determining non-volatile ingredients of neochlorogenic acid, chlorogenic acid, cryptochlorogenic acid, caffeic acid, isochlorogenic acid A, rosmarinic acid, ferulic acid, rutin, luteoloside, isoquercitrin, hesperidin, diosmin, diosmetin, luteolin, acacetin and linarin in Menthae Haplocalycis Herba formula granules and traditional herbal pieces by UPLC-MS/MS, and analyze the correlation of non-volatile ingredients in Menthae Haplocalycis Herba formula granules and traditional herbal pieces. Shim-pack GIST C_(18) column(2.1 mm×100 mm, 2 μm) was adopted with acetonitrile-0.1% formic acid aqueous solution as the mobile phase for gradient elution at the flow rate of 0.4 mL·min~(-1). The column temperature was set at 35 ℃. The quantitative analysis was performed using the electrospray ionization source and the multiple reaction monitoring. The linear relationship, resolution, repeatability and recovery of the 16 chemical components all met the requirements. The 16 non-volatile ingredients in traditional herbal pieces of Menthae Haplocalycis Herba could be tracked in formula granules. There were certain differences of the 16 chemical components among Menthae Haplocalycis Herba formula granules of different manufacturers and traditional herbal pieces of different producing areas. The UPLC-MS/MS method was simple, rapid and accurate, and could be used for the quality control of non-volatile ingredients in Menthae Haplocalycis Herba formula granules and traditional herbal pieces.

RESUMEN

Notoginsenosides, the saponins extracted from Panax notoginseng, have many pharmacological effects, such as anti-inflammation, anti-oxidation, anti-tumor, nervous system and cardiovascular system protection, microcirculation improvement and calcium overload inhibition. At present, notoginsenosides are widely used clinically for treating many diseases with good efficacy, especially for nervous system diseases such as stroke, stroke sequelae and Alzheimer's disease. In recent years, the mechanism underlying their neuroprotective effect has been continuously explored. To advance the applied research on notoginsenosides in the prevention and treatment of central nervous system diseases, this paper, combined with the latest reports, summarizes their neuroprotective effect and mechanisms in terms of regulating voltage-gated ion channels, protecting nerve cells and neurovascular unit, inhibiting oxidative stress and inflammatory reaction, promoting angiogenesis and reducing excitatory neurotoxicity. Although the protective mechanism of notoginsenosides for the nervous system mainly involves the above several aspects, some of them still remain to be fully elucidated, which necessitates the further exploration of neuroprotective effect of notoginsenosides with molecular biology, metabolomics, proteomics and other technologies.

RESUMEN

Diabetes is a worldwide public health problem that seriously threats human health. Long-term metabolic disorders, as the main cause of multi-system complications and death in the later stage of diabetes, can cause multi-system damage, leading to chronic progressive lesions in the eyes, kidneys, nerves, heart, blood vessels and other tissues and organs, as well as functional decline and failure. The low risk of side effects and new treatment strategies remain an area to be explored in clinical treatment of diabetes. Salviae Miltiorrhizae Radix et Rhizoma (SM) is one of the commonly used herbs in traditional Chinese medicine (TCM), with the main effect of activating blood circulation and removing blood stasis. In recent years, it has been found that SM shows good performance in lowering blood sugar and treating diabetes complications. Data mining information has also shown that the drugs of activating blood circulation and removing blood stasis are now common drugs in clinical treatment of diabetes, and SM has the highest use frequency, with significant curative effect. In addition, TCM is a kind of treatment with composite components and multiple targets, and so people are increasingly interested in its effective components and carry out extensive researches. This article summarized the experimental verification of SM extract and its components (tanshinone A, tanshinone B, tanshinone ⅡA, tanshinone I, protocatechuic aldehyde, polysaccharide, and total polyphenol acid) in various diabetes models in improving glucolipid metabolism, improving heart function in patients with diabetes, alleviating the motor and sensory deficits caused by diabetes, preventing the occurence of the diabetic retinopathy, recovery of liver and kidney structure and function damage in diabetic patients, and helping to resist high sugar-induced atrophic cavitation potential. It may inhibit hyperglycemia-induced vascular injury with polyol pathway activation, reduce the formation of advanced glycation end products, inhibit protein kinase C pathway activation and hexosamine pathway activation, and alleviate oxidative stress caused by excessive production of peroxides in mitochondrial electron transport chain during hyperglycemia to play a role of treatment, and provide reference for clinical application.

RESUMEN

Paeoniae Radix Rubra is a traditional Chinese medicine commonly used in clinical practice, it is mostly wild and widely distributed in different areas of China. In addition, the plant of Paeoniae Radix Rubra also has ornamental value. Modern phytochemical researches showed that the chemical constituents of Paeoniae Radix Rubra were complex. Up to now, more than 300 chemical constituents have been found, mainly including monoterpene glycosides, triterpenoids, flavonoids, tannins, phenolic acids, saccharides, steroids, volatile oils and so on. Among them, the content of monoterpene glycosides was the highest, and the types of volatile oil were the most. Paeoniae Radix Rubra has a wide range of pharmacological effects, exerting different curative effects in multiple systems such as blood, cardiovascular, nervous and digestive system. It can protect myocardial cells and nerve cells, stabilize microcirculation, anti-endotoxin, anti-atherosclerosis, reduce pulmonary hypertension, anti-depression, protect liver, anti-gastric ulcer, anti-tumor, slow down aging, treat Parkinson's syndrome and diabetes and its complications, anti-radiation, anti-inflammatory, anti-virus and so on. Through reviewing the literature on chemical constituents and pharmacological effects of Paeoniae Radix Rubra, it was found that total glycosides and monomers such as paeoniflorin, albiflorin, benzoylpaeoniflorin and gallic acid may be the main active components of Paeoniae Radix Rubra. At present, the research on Paeoniae Radix Rubra mainly focused on monoterpene glycosides, while the research on flavonoids and volatile oil in Paeoniae Radix Rubra was less. It is suggested that research on these two components should be strengthened in the future.

RESUMEN

Objective:To observe the effect of Shaofu Zhuyutang on nuclear factor erythroid-2-related factor 2 (Nrf2) /antioxidant response element (ARE) signaling pathway in blood vessels by establishing the model of rats with cold coagulation and blood stasis syndrome, and to explore the protective effect and mechanism of Shaofu Zhuyutang on vascular endothelial injury. Method:The 50 SPF rats were randomly divided into high dose group (4.8 g·kg-1), middle dose group (2.4 g·kg-1), low dose group (1.2 g·kg-1), model group and normal group (ten of each group). The rat model of cold coagulation and blood stasis syndrome was established by subcutaneous injection of epinephrine hydrochloride combined with ice bath. At the same time of modeling, the drug was administered by gavage. After 28 days of continuous administration, the hemorheology indexes were detected by automatic hemorheology instrument. Levels of nitric oxide (NO), endothelin (ET)-1, superoxide dismutase (SOD), glutathione (GSH-Px), intercellular adhesion molecule-1(ICAM-1), vascular cell adhesion molecule-1(VCAM-1), von Willebrand factor (vWF) in serum were determined by ELISA. Hematoxylin and eosin (HE) staining was used to observe the endothelial injury of vascular tissue of thoracic aorta. The protein expression of Nrf2 and HO-1 in vascular tissue of thoracic aorta was detected by Western blot. Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR)was used to observe the expression of Nrf2 and heme oxygenase-1 (HO-1) mRNA in vascular tissue of thoracic aorta. Result:Compared with the blank group, model group rats whole blood viscosity and plasma viscosity were significantly increased (P<0.05,P<0.01), vWF, ICAM 1, VCAM 1 content increased significantly (P<0.01), NO, SOD, gsh-px levels decreased significantly (P<0.01), significantly increased the content of ET-1(P<0.01), thoracic aorta vascular tissue Nrf2, HO-1 mRNA expression was significantly increased (P<0.01), Nrf2 protein expression in the cell nucleus increased significantly (P<0.05), The protein expression level of Nrf2 in cytoplasm was significantly decreased (P<0.05), while the protein expression level of HO-1 was significantly increased (P<0.01). Compared with model group, the whole blood viscosity (high and middle cut), plasma viscosity, were significantly reduced in high and meduim-dose Shaofu Zhuyutang groups(P<0.05,P<0.01). The levels of vWF, ICAM-1, VCAM-1 and ET-1 in serum were significantly reduced (P<0.05,P<0.01), NO, SOD and GSH-Px increased significantly (P<0.05,P<0.01). The pathological changes such as hyperplasia, swelling and shedding of endothelial cells of thoracic aorta, rupture of internal elastic membrane and disorder of smooth muscle arrangement were improved. The expression levels of Nrf2, HO-1 protein and gene were significantly increased in vascular tissue of thoracic aorta (P<0.01). Conclusion:Shaofu Zhuyutang has a protective effect on vascular endothelial injury in rats with cold coagulation and blood stasis syndrome. The mechanism of action is related to the activation of Nrf2/ARE signaling pathway, which leading to the increased expression of antioxidant enzymes and decreased the expression of adhesion factors.

RESUMEN

To a certain extent, put forward the concept of " component of traditional Chinese medicine (TCM)" simplifies the complexity of multi-component and multi-target of TCM, which provides a possibility for the clarification of the material basis of the efficacy of TCM, and also provides a new direction for promoting the modernization and industrialization of TCM, promots the high quality development of TCM. The correlation between prescription and disease syndrome has made rapid progress, both basic research and clinical application are fruitful. However, the correlation between components and disease syndrome still needs to be further studied. The syndrome of blood stasis is a common syndrome of TCM science, and it is more common in various diseases, especially cardiovascular and cerebrovascular diseases, kidney disease, diabetes and hyperlipoidemia. A large number of studies have shown that some specific components contained in TCM or TCM compound can improve the related indexes of patients or experimental animal model with blood stasis syndrome. It is manifested in reducing blood viscosity, inhibiting platelet activation and adhesion aggregation, changing erythrocyte deformability index, inhibiting thrombosis and so on. Blood stasis is not only the pathogenic factor of many diseases, but also the pathological product of many kinds of diseases, which involves a wide range of diseases. Therefore, this study will study the progress of different components of TCM in the prevention and treatment of blood stasis syndrome, focusing on saponins, flavonoids, organic acids, polysaccharides, alkaloids and other active components in improving hemorheological abnormalities, hypercoagulability, platelet activation and adhesion aggregation, thrombosis. Based on the thought of component-disease syndrome, this paper searches the relevant literature in recent 20 years, classifies and summarizes the achievements of different components in the prevention and treatment of blood stasis syndrome, and hopes to provide some ideas for the further study of the pharmacological action of TCM components, the study of compatibility of TCM components and the research of TCM components.

The affecting factors of urinary iodine levels were analyzed by multiple linear regression models.@*Results@#Totally 1 964 subjects were recruited, including 1 099 males, and 865 females. The age was (41.23 ± 14.75) years old. The median urinary iodine of the selected group was 225.60 (158.80, 311.58) μg/L. The urinary iodine level of rural residents was significantly higher than that of urban residents [μg/L: 249.80 (180.58, 336.88) vs 206.25 (148.03, 280.33), Z=-8.015, P < 0.05], and there was no significant difference between the urinary iodine levels of male and female [μg/L: 222.40 (161.60, 298.90) vs 230.60 (158.00, 326.40), P > 0.05]. The composition ratio of < 100 μg/L group in rural areas was lower than that in urban areas (5.14% vs 8.27%, χ2= 7.45, P < 0.05), and the composition ratio of ≥300 μg/L group was higher than that in urban areas (34.93% vs 21.51%, χ2= 43.87, P < 0.05). The composition ratio of ≥300 μg/L group in male was lower than that in female (24.57% vs 31.21%, χ2= 10.73, P < 0.05). The multiple linear regression analysis showed that lived in rural areas, age, moderate and severe salt intaking, student, and weight were independently correlated with urinary iodine levels (β= 0.074, -0.001, 0.059, 0.034, 0.096, -0.003, P < 0.05). And the urinary iodine levels were decreased with age and weight increased (P < 0.05).@*Conclusions@#At present, the iodine nutrition level of urban and rural residents in some areas of Gansu Province is generally over the adequate level. The urinary iodine level of rural residents is higher than that of urban residents. In the future, we should pay attention to the impact of different living areas, salt intaking and body weight on urinary iodine levels to prevent thyroid diseases.

RESUMEN

Bronchopulmonary dysplasia (BPD) is one of the common respiratory diseases in premature infants.Recently, the survival rate of VLBW and ELBW infants is increasing, and the incidence of BPD is also increasing year by year. However, the treatment of BPD still lacks a unified clinical standard. The main treatments include permissive hypercapnia, glucocorticoid, bronchiectator, caffeine and inhaled nitric oxide. The treatment of glucocorticoids, including systemic and local medication,is the focus of clinical research. How to use hormones rationally to prevent and treat BPD and minimize adverse reactions is a hot issue.
