RESUMEN

OBJECTIVES: Chronic liver disease is often associated with testosterone deficiency. However, testosterone replacement does not improve hepatic function or survival with diseased liver. So far, to our knowledge, testosterone replacement therapy after successful livertransplantforfunctional sarcopenia has not been studied. We had 3 goals: (1) define postoperative functional sarcopenia afterlivertransplant with serum testosterone level; (2) examine the role of short-term testosterone replacement therapy with active in-bed exercise of upper and lower extremity joints; and (3) correlate functional sarcopenia with skeletal muscle index and skeletal muscle density in relation to ascites, pleural effusion subtracted body mass index. MATERIALS AND METHODS: We evaluated 16 liver transplant recipients who had been receiving posttransplanttestosterone replacementtherapy with functional sarcopenia. Preoperative and postoperative demographics and laboratory and radiological data were retrieved; body mass index, skeletal muscle index, and skeletal muscle density were calculated. For this retrospective study, institutional review board approval was obtained before the electronic database was reviewed and analyzed. RESULTS: Mean testosterone level was 28.3 ng/dL (<5% of expected). Twelve patients received 1 dose, and the remaining 4 patients received >1 dose oftestosterone cypionate, 200 mg. Mean hospital stay was 26 days. Seven patients were discharged home, with the remaining patients to a rehabilitation facility or nursing home. One patient died from a cardiac event, and another patient died from recurrent metastatic malignancy. The 1-year and 5-year actuarial patient and graft survival rates were 93.8% and 87.5%, respectively. Overall, 5 patients were sarcopenic by skeletal muscle index, and 6 patients had poor muscle quality by skeletal muscle density. CONCLUSIONS: Testosterone deficiency after liver transplant exists with functional sarcopenia. Two- thirds of such recipients have low skeletal muscle index and/or have low skeletal muscle density. Short- term testosterone replacement therapy with in-bed active exercise provides 5-year patient and graft survival of 87.5%.

The purpose of this study was to evaluate the feasibility of translation of RadLex lexicon from English to German performed by Google Translate, using the RadLex ontology as ground truth. The same comparison was also performed for German to English translations. We determined the concordance rate of the Google Translate-rendered translations (for both English to German and German to English translations) to the official German RadLex (translations provided by the German Radiological Society) and English RadLex terms via character-by-character concordance analysis (string matching). Specific term characteristics of term character count and word count were compared between concordant and discordant translations using t-tests. Google Translate-rendered translations originally considered incongruent (2482 English terms and 2500 German terms) were then reviewed by German and English-speaking radiologists to further evaluate clinical utility. Overall success rates of both methods were calculated by adding the percentage of terms marked correct by string comparison to the percentage marked correct during manual review extrapolated to the terms that had been initially marked incorrect during string analysis. 64,632 English and 47,425 German RadLex terms were analyzed. 3507 (5.4%) of the Google Translate-rendered English to German translations were concordant with the official German RadLex terms when evaluated via character-by-character concordance. 3288 (6.9%) of the Google Translate-rendered German to English translations matched the corresponding English RadLex terms. Human review of a random sample of non-concordant machine translations revealed that 95.5% of such English to German translations were understandable, whereas 43.9% of such German to English translations were understandable. Combining both string matching and human review resulted in an overall Google Translate success rate of 95.7% for English to German translations and 47.8% for German to English translations. For certain radiologic text translation tasks, Google Translate may be a useful tool for translating multi-language radiology reports into a common language for natural language processing and subsequent labeling of datasets for machine learning. Indeed, string matching analysis alone is an incomplete method for evaluating machine translation. However, when human review of automated translation is also incorporated, measured performance improves. Additional evaluation using longer text samples and full imaging reports is needed. An apparent discordance between English to German versus German to English translation suggests that the direction of translation affects accuracy.

Epidemiological data of renal diseases have great geographic variability throughout the world. Due to the lack of a national renal data registry system, there is no information on the prevalence rate, clinical and pathological features of various glomerulonephritis (GN) in the United Arab Emirates (UAE). In a retrospective cross-sectional study, we analyzed 158 renal biopsies done in Dubai Hospital, UAE, between the years of 2005 and September 2014, with an aim to determine the prevalence rate and frequency of different pathological patterns of GN in adult patients who presented with proteinuria ± hematuria. In our study, primary GN still remains more common than secondary GN (66.4% vs. 33.5%). Among the primary GN in our analysis, minimal change disease was the most common primary GN affecting 20% of the study population (13.2% of the total GN causes) followed with membranous GN (18.2%), then membrano- proliferative GN (15.3%) and focal segmental glomerulosclerosis (13.46%), while among the secondary causes lupus nephritis (LN) is the most prevalent GN in UAE, predominantly in the Emirati national population whom constituted 48% of total biopsies. Indeed, LN had the highest incidence among all types of GN even the primary ones, constituting 23.4% of total GN in Dubai (74% of the total secondary causes). Furthermore, systemic lupus erythematosus was the most common GN in women while the minimal change was widely affecting male patients. Among elderly, the most common pathology was diabetic glomerulosclerosis followed by amyloidosis.
