BACKGROUND: For patients with prior intra-abdominal surgery or multiple arteries, the retroperitoneal robot-assisted partial nephrectomy (rRAPN) is a better choice. The renal ventral tumor poses an additional challenge due to poor tumor exposure. This study is determined to assess the feasibility of an internal traction technique (ITT) in rRAPN for the management of renal ventral tumors. METHODS: From November 2019 to March 2021, a total of 28 patients with renal ventral tumor underwent rRAPN. All patients had prior abdominal surgery or multiple arteries. The ITT group (20 patients), which improved the tumor exposure by traction of the kidney with suture, was compared with the traditional technique group (8 patients) in terms of warm ischemia time, estimated blood loss and postoperative hospital stay, retroperitoneal drainage, R.E.N.A.L. score, and serum creatinine. Differences were considered significant when P < 0.05. RESULTS: All rRAPN surgeries were successful without conversion to radical nephrectomy or open partial nephrectomy. The warm ischemia time was lower in the ITT group (17.10 min vs. 24.63 min; P < 0.05). Estimated blood loss in the traditional technique group was 324.88 ± 79.42 mL, and in the ITT group, it was 117.45±35.25 mL (P < 0.05). No significant differences with regard to postoperative hospital stay, retroperitoneal drainage, R.E.N.A.L. score, and serum creatinine were observed between both groups. Surgical margins were negative and no intraoperative complications occurred in all the patients. After 10 months of follow-up, no recurrence or metastasis occurred in all cases. CONCLUSION: ITT is a feasible, safe, and valid procedure in rRAPN for renal ventral tumors. Application of ITT improved the exposure and reduces warm ischemic time in comparison with the conventional procedure.

Inspired by electronic transistors, electric field gating has been adopted to manipulate ionic currents of smart nanofluidic devices. Here, we report a PNP nanofluidic bipolar junction transistor (nBJT) consisting of one polyaniline (PANI) layer sandwiched between two polyethylene terephthalate (PET) nanoporous membranes. The PNP nBJT exhibits three different responses of currents (quasi-linear, rectification, and sigmoid) due to the counterbalance between surface charge distribution and base voltage applied in the nanofluidic channels; thus, they can be switched by base voltage. Four operating modes (cutoff, active, saturation, and breakdown mode) occur in the collector response currents. Under optimal conditions, the PNP nBJT exhibits an average current gain of up to 95 in 100 mM KCl solution at a low base voltage of 0.2 V. The present nBJT is promising for fabrication of nanofluidic devices with logical-control functions for analysis of single molecules.

An unpredictable ghost peak was intermittently observed during the impurity separation of cefaclor and formulation by high performance liquid chromatography (HPLC) with a content from below the reported threshold to approximately 0.3% in different laboratories. Through a series of investigations, the ghost peak was identified as an unusual on-column degradant of cefaclor formed under elevated column temperature but was not an actual sample impurity. The chemical structure of the degradant was determined by spectroscopic methods, including high resolution mass spectrometry (HRMS) and 1H-NMR. Consequently, the unknown peak was identified as a C-4 oxidative decarboxylation analog of cefaclor. The formation mechanism of the analog is proposed, and it is suggested that elevated column temperature during HPLC analysis has a profound effect on the degradation. Dissolved oxygen in the mobile phase may promote the formation of the ghost peak. The degradation can be suppressed by using a column temperature below 30 °C. Moreover, several other prevention measures are suggested based upon the results of the investigation.

OBJECTIVE: To observe the effect of different acupuncture intervention time on the improvement of the symptoms of acute facial neuritis and the length of recovery time, and to find the best interventional opportunity for acute facial neuritis. METHODS: A total of 120 patients with acute facial neuritis (all unilateral onset) were randomly divided into 4 groups, namely group A, group B, group C and group D, 30 cases in each group. Acupuncture performed within 3 days after onset in the group A; oral mecobalamin dispersible tablets and multivitamin B tablets were received within 3 days after onset, and acupuncture was started from the 4th day in the group B; oral mecobalamin dispersible tablets and multivitamin B tablets were given within 5 days after onset, and acupuncture was administered from the 6th day in the group C; oral mecobalamin dispersible tablets and multivitamin B tablets were given within 7 days after onset, and acupuncture was applied from the 8th day in the group D. Penetration needling was the main acupuncture treatment, with infrared radiation on the cheeks, once a day, 6 days a week and 3 weeks were given. After treatment, the symptom scores (Portmann scores) of the 7th, 14th and 28th day after onset were compared. The time of improvement and the clinical effect of each group were compared. RESULTS: On the 7th, 14th and 28th day after onset, the Portmann scores of the 4 groups were higher than those before treatment (all P<0.05). On the 7th, 14th and 28th day after onset, the Portmann scores in the group A were higher than those in the other 3 groups (all P<0.05). There was significant difference between both pairs of group B, group C and group D (all P<0.05). The total effective rate in the group A was 96.7% (29/30), which was higher than 93.3% (28/30), 86.7% (26/30) and 83.3% (25/30) in the other 3 groups (all P<0.05). The difference between the 4 groups in the beginning of improvement, significant improvement time and clinical recovery time was statistically significant (all P<0.05), and the group A was optimal. CONCLUSION: Early intervention of acupuncture with acute facial neuritis can appropriately slow the progression of facial nerve injury, improve the therapeutic effect, and shorten the clinical recovery time.

Objective To explore the expression of eukaryocyte initiation factors-4E (eIF-4E) in normal esophageal epithelium and different esophageal lesions tissue and its relationship with vascular endothelial growth factor (VEGF).Methods Thirty normal esophageal incisal margin tissues,32 atypical hyperplasia tissues,20 carcinoma in situ tissues and 117 esophageal invasive carcinoma tissues were selected from the biopsy specimens of the Department of Pathology,the First Affiliated Hospital of Xinxiang Medical University from March 2014 to March 2016.The expression of eIF-4E and VEGF were detected by immunohistochemistry method and the correlation between them was analyzed.Results The positive expression rate of eIF-4E in normal esophageal epithelium,atypical hyperplasia,carcinoma in situ and invasive carcinoma tissues was 0.0% (0/ 30),9.4% (3/32),45.0% (9/20) and 80.3% (94/117) respectively.There was no statistic difference in the positive expression rate of eIF-4E in normal esophageal epithelium and atypical hyperplasia tissues (P > 0.05);there was statistic difference in the positive expression rate of eIF-4E in the other esophageal tissues(P < 0.05).In 117 invasive carcinoma tissues,the positive expression rate of eIF-4E in metastatic carcinoma tissues (93.2%,55/59) was significantly higher than that in the non metastatic carcinoma tissues (67.2%,39/58) (P < 0.05).The positive expression rate of eIF-4E in esophageal carcinoma tissues which invaded into shallow muscle layer,deep muscular layer and adventitia was 70.0% (28/40),73.8% (31/42) and 100.0% (35/ 35) respectively;there was no statistic difference in positive expression rate of eIF-4E in the carcinoma tissues which invaded into shallow muscle layer and deep muscular layer(P > 0.05);the positive expression rate of eIF-4E in the carcinoma tissues which invaded into shallow muscle layer and deep muscular layer was significantly lower than that in the carcinoma tissues which invaded into adventitia(P <0.01).The expression of eIF-4E in the esophageal invasive carcinoma tissues was positive VEGF(x2 =51.460,P < O.05).Conclusion eIF-4E play an important role in the canceration of normal esophageal and the invasion,metastasis of esophageal carcinoma.The expression of eIF-4E is correlate with VEGF in esophageal carcinoma tissues.

The asymmetric unit of the title compound, [Ni(2)(C(10)H(2)O(8))(C(12)H(8)N(2))(2)(H(2)O)(4)]·0.67H(2)O, contains one complete binuclear complex and one half-mol-ecule, the latter being completed by crystallographic inversion symmetry, and 0.67 of a solvent water molecule. Each Ni(2+) cation is coordinated by a 1,10-phenanthroline ligand, a bidentate benzene-1,2,4,5-tetra-carboxyl-ate (btec) tetra-anion and two water mol-ecules to generate a distorted cis-NiN(2)O(4) octa-hedral coordination geometry. The btec species bridges the metal ions. In the crystal, the clusters and uncoordinated water mol-ecules are linked by O-H⋯O hydrogen bonds and π-π inter-actions [shortest centroid-centroid separation = 3.596 (2) Å] to form a three-dimensional network.

A series of metal-organic complexes based on d(10) metals and the ligand H(4)bbh (H(4)bbh = benzene-1, 2, 4, 5-biformhydrazide), formed through hydrothermal in situ acylate reaction of H(4)bta (H(4)bta = benzene-1, 2, 4, 5-tetracarboxylic acid) with hydrazine hydrate (N(2)H(4)·H(2)O), have been prepared and structurally characterized by single-crystal X-ray diffraction. Compounds [Zn(µ(2)-H(2)bbh)(phen)(H(2)O)](2) (1) (phen = 1, 10-phenanthroline) and [Zn(µ(2)-H(2)bbh)(2, 2'-bpy)](2) (2) (2, 2'-bpy = 2, 2'-bipyridine) are both dinuclear complexes in which bridging ligands H(2)bbh(2-) display different µ(2)- coordination modes. [Zn(µ(2)-H(2)bbh)(1/2)(µ(2)-H(2)bbh)(1/2)(H(2)O)](n) (3) exhibits a two-dimensional (2-D) layer structure containing simultaneously two kinds of different coordination modes of H(2)bbh(2-): µ(2)-bidentate and µ(4)-tetradentate. [Cd(µ(3)-H(2)bbh)(phen)](n) (4) consists of one-dimensional (1-D) double-metal chains. The crystal structures of these compounds are stabilized by hydrogen bonds and π···π interactions, forming three-dimensional supramolecular networks. All of the compounds were characterized by IR, UV-vis spectra and elemental analysis and they show good fluorescence properties in the solid state at room temperature. In order to understand the emission mechanism, we carried out TDDFT calculations on the excited electronic states of compound 2.

The multiple-channel reactions OH + CH3NHC(O)OCH3 --> products are investigated by direct dynamics method. The optimized geometries, frequencies, and minimum energy path are all obtained at the MP2/6-311+G(d,p) level, and energetic information is further refined by the BMC-CCSD (single-point) method. The rate constants for every reaction channels, R1, R2, R3, and R4, are calculated by canonical variational transition state theory with small-curvature tunneling correction over the temperature range 200-1000 K. The total rate constants are in good agreement with the available experimental data and the two-parameter expression k(T) = 3.95 x 10(-12) exp(15.41/T) cm3 molecule(-1) s(-1) over the temperature range 200-1000 K is given. Our calculations indicate that hydrogen abstraction channels R1 and R2 are the major channels due to the smaller barrier height among four channels considered, and the other two channels to yield CH3NC(O)OCH3 + H2O and CH3NHC(O)(OH)OCH3 + H2O are minor channels over the whole temperature range.
