RESUMEN

Sepsis is the main cause of death in intensive care unit (ICU). Sepsis and septic shock seriously affect the prognosis of patients and increase the mortality and re-morbidity of patients. Early and timely intervention can reduce the mortality and recurrence rate of patients with sepsis. The occurrence of sepsis may be related to the phosphorylation of connexin 43 (Cx43), which needs to be realized through various signal pathways. The related sites of connexin 43 are phosphorylated through different signal pathways to achieve the precise regulation of sepsis, these sites may be related targets for the treatment of sepsis and provide a direction for accurate treatment of sepsis. This paper mainly analyzes the role of Cx43-related signal pathways such as protein kinase A (PKA), protein kinase C (PKC), mitogen-activated protein kinase (MAPK) and phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt) in the pathogenesis of sepsis.

RESUMEN

OBJECTIVE: To analyze the effect of primary nursing on the improvement of potential risk and pain degree in elderly patients with cancer pain. METHODS: Altogether 187 patients with cancer pain from June 2018 to November 2019 were selected as the research participants and grouped into two groups according to different nursing intervention methods, including 102 cases in the research group (RG) and 85 cases in the control group (CG). The pain relief and medication compliance were evaluated. General Self-efficacy Scale (GSES) and Exercise of Self-Care Agency (ESCA) were used to evaluate the self-efficacy and self-care ability. Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) were used to evaluate the anxiety and depression. Pittsburgh Sleep Quality Index (PSQI) was used to evaluate sleep quality. SF-36 Quality of Life Scale was used to evaluate the quality of life. Self-made nursing satisfaction questionnaire was used to evaluate the nursing satisfaction. RESULTS: The improvement of pain relief in the RG was evidently higher than that in the CG. The scores of SAS and SDS in the RG after intervention were evidently lower than those in the CG. GSES and ESCA scores of patients in the RG after intervention were evidently higher than those in the CG. The compliance of patients in the RG after intervention was evidently higher than that in the CG. The PSQI scores of patients in the RG after intervention were evidently better than those in the CG. The scores of SF-36 and nursing satisfaction in the RG were evidently higher than those in the CG. CONCLUSION: The implementation of primary nursing intervention for elderly patients with cancer pain can effectively relieve their pain and improve their bad psychological state and medication compliance, thus improving patients' quality of life.

Acute lung injury caused by sepsis remains one of the most difficult challenges faced by patients in intensive care units and is associated with a high mortality rate. The aim of the present study was to investigate whether programmed cell death (PD)-1/programmed cell death-ligand 1 (PD-L1) inhibitors reduce alveolar macrophage apoptosis, reduce inflammatory factor release and relieve inflammation. For this purpose, murine alveolar macrophages, MH-S, were cultured and divided into control, lipopolysaccharide (LPS) and LPS+BMS-1 (PD-1/PD-L1 inhibitors) groups. LPS (10 ng/ml) was added to the LPS and LPS+BMS-1 groups for 24 h and PD-1/PD-L1 inhibitor BMS-1 (1 µmol/l) was added to the LPS+BMS-1 group for 72 h. PD-1 mRNA expression was detected using reverse transcription-quantitative PCR and PD-1 protein expression was detected using western blotting in the control, LPS and LPS+BMS-1 groups of macrophages. MH-S apoptosis was detected using flow cytometry with Annexin V/PI staining. The levels of the inflammatory factors interleukin (IL)-1ß, IL-6, tumor necrosis factor (TNF)-α and IL-10 were detected by ELISA. Murine alveolar macrophages expressed PD-1 at both the molecular and protein levels and PD-1 expression was increased in MH-S cells stimulated with LPS. Compared with the LPS group, the expression of PD-1 in the LPS+BMS-1 group was significantly decreased. Flow cytometry demonstrated that there was increased apoptosis of alveolar macrophages in the LPS group compared with the control group, whereas, alveolar macrophages notably decreased apoptosis in the LPS+BMS-1 group compared with the LPS group. There was no statistical difference between the control group and the LPS+BMS-1 group. IL-1ß, IL-6, TNF-α and IL-10 were increased in the LPS group compared with the control group. The levels of IL-1ß, IL-6 and TNF-α in the LPS+BMS-1 group were lower compared with those in the LPS group whereas IL-10 was further increased. In vitro, the PD-1/PD-L1 inhibitor, BMS-1, decreases alveolar macrophage apoptosis compared with the LPS group to maintain effective immune clearance and reduce inflammatory factor release. This decreased the inflammatory response and reduced acute lung injury caused by sepsis. Therefore, PD-1/PD-L1 inhibitors may be a potential therapeutic target for acute lung injury in patients with sepsis.
