RESUMEN

PURPOSE: To assess outcomes of bilateral single-session percutaneous nephrolithotomy (PCNL) with minimally invasive techniques in pediatric population. MATERIALS AND METHODS: From August 2015 to July 2021, 45 children (including 12 infants) were treated with bilateral single-session PCNL, which included miniPCNL (12-16-Fr) and Microperc (4.8-Fr). Patient, stone and operation-related characteristics, stone-free rate (SFR) and complication rate (CR) were compared using ANOVA. Independent predictors were determined using multivariate linear regression. RESULTS: The mean stone burden was 3.2 cm in sum diameter for both kidneys. For bilateral kidneys, the mean operative time was 61.6min and SFR was 93.3%; CR was 53.3%, of which complications of Clavien grade 1 and 2 accounted for 46.7%. Bilateral Microperc, bilateral miniPCNL and Microperc plus miniPCNL was performed in 19, 14 and 12 children respectively. Both irrigation volume and postoperative stay were less in groups with Microperc. Both SFRs and CRs were satisfactory for the three groups. Self-limiting hematuria represented the most common complication of all cases (33.3%), especially in groups with miniPCNL. The stone burden was the only independent predictor for operative time (P < .001) and the postoperative complication (P = .008). Children with older age (P = .009), higher body mass index (P = .016) or a higher stone burden (P < .001) received larger irrigated fluid volume. Microperc was associated with less irrigated fluid volume (P = .001). Children with Clavien grade 3 complications (P = .004) spent prolonged postoperative hospital stay. CONCLUSION: With favourable SFR and acceptable CR, bilateral single-session PCNL with minimally invasive techniques might be an effective and safe procedure for pediatric nephrolithiasis.

INTRODUCTION: Pediatric urolithiasis is a less common disease with a steadily increasing incidence of 10.6 % per year. The comprehensive management of pediatric urolithiasis is more challenging than in adults, and published studies of endourological treatment in children have small sample sizes and older mean ages, which may not be representative of the overall pediatric population, especially in infants and toddlers. This study aimed to report results of retrograde ureteroscopy for treating pediatric ureteral stones in infants and toddlers (<3 years) from a single center in China. METHODS: Demographic and surgical data of infants and toddlers with ureteral stones who underwent retrograde URS were retrospectively analyzed from January 2015 to September 2022. RESULTS: The mean age of 100 infants (73 boys and 27 girls) was 19.6 ± 7.6 months and stone burden was 0.27 (0.11-0.52) cm³. 111 procedures were performed for all children due to 11 patients with bilateral ureteric stones were simultaneously treated. Among them, 70.3 % were semi-rigid URS, 12.6 % were Micro-URS and 17.1 % were flexible URS. SFR was up to 96 %, the median operation time and hospitalization days were 35.0 (25.0-50.0) minutes and 6.0 (5.0-6.0) days, respectively. 15 (15.0 %) infants had postoperative fever (Grade I), and complications above grade I were not observed. DISCUSSIONS: This research reported the largest sample size of infant and toddler ureteric stones (under 3 years old) and shared optimal management strategies for these special populations. Compared to other related studies, we had a higher SFR due to diverse management strategies and reasonable application of pre-operative ureteric stents. The most important potential reason why our complication rate was relatively higher might be the younger population under 3 years old. In additionally, the reason of long hospitalization was maybe some patients underwent staged surgeries during one hospitalization. Those results indicated the robust effectiveness of URS for infants. This study has limitations, including its retrospective single-center design, absence of long-term follow-up data, and potential variability in surgical outcomes due to differences in surgeons' experience. CONCLUSIONS: The experience of endourologic procedures for 100 infant and toddler patients with ureteric stones from a Chinese single center was reported. And URS showed its effectiveness and safety for those special populations based on its high SFR and low complication rate.

BACKGROUND: Transient receptor potential (TRP) channels are involved in various physiological, pathological, and tumorigenesis-related processes. However, only a few studies have comprehensively analyzed TRP family members and their association with prognosis and tumor microenvironment (TME) in various cancers. Thus, in this study, we focused on TRP channels in pan-cancer and screened two typical TRP channels, TRPV4 and TRPC4, as examples. METHODS: Based on the latest public databases, we evaluated the expression level and prognostic value of TRP family genes in pan-cancer tissues via various bioinformatic analytical methods, and investigated the relationship between the expression of TRP family genes with TME, stemness score, immune subtype, drug sensitivity, and immunotherapy outcome in pan-cancer tissues. RESULTS: Pan-cancer analysis revealed that the TRP family genes were differentially expressed in tumor and para-carcinoma tissues. A significant correlation existed between the expression of TRP family genes and prognosis. The expression of TRP family genes was significantly correlated with stromal, immune, RNA stemness, and DNA stemness scores in pan-cancer tissues. Our results indicated that the expression of TRP family genes correlated with the sensitivity to various drugs including PLX-4720, SB-590885, and HYPOTHEMYCIN, immunotherapy outcome, and immune-activation-related genes. Immunohistochemical analysis revealed significant differential expression of TRPV4 in bladder and para-carcinoma tissues. CONCLUSIONS: Our study elucidated the possible role of TRP family genes in cancer progression and provided insights for further studies on TRP family genes as potential pan-cancer targets to develop diagnostic and therapeutic strategies.

BACKGROUND: Microfluidic chip detection technology is considered a potent tool for many bioanalytic applications. Rapid detection of foodborne pathogens in the early stages is imperative to prevent the outbreak of foodborne diseases, known as a severe threat to human health. Conventional bacterial culture methods for detecting foodborne pathogens are time-consuming, laborious, and lacking in pathogen diagnosis. To overcome this problem, we have created an embedded paper-based microchip based on isothermal loop amplification (LAMP), which can rapidly and sensitively detect foodborne pathogens. RESULTS: We embed paper impregnated with LAMP reagent and specific primers in multiple reaction chambers of the microchip. The solution containing the target pathogen was injected into the center chamber and uniformly distributed into the reaction chamber by centrifugal force. The purified DNA of Escherichia coli O157:H7, Salmonella spp., Staphylococcus aureus, and Vibrio parahaemolyticus has been successfully amplified and directly detected on the microchip. The E. coli O157:H7 DNA was identified as low as 0.0134 ng µL- 1. Besides, the potential of this microchip in point-of-care testing was further tested by combining the on-chip sample purification module and using milk spiked with Salmonella spp.. The pyrolyzed milk sample was filtered through a polydopamine-coated paper embedded in the inside of the sample chamber. It was transported to the reaction chamber by centrifugal force for LAMP amplification. Then direct chip detection was performed in the reaction chamber embedded with calcein-soaked paper. The detection limit of Salmonella spp. in the sample measured by the microchip was approximately 12 CFU mL- 1. CONCLUSION: The paper embedded LAMP microchip offers inexpensive, user-friendly, and highly selective pathogen detection capabilities. It is expected to have great potential as a quick, efficient, and cost-effective solution for future foodborne pathogen detection.

As a commonly used organophosphorus flame retardant (OPFR), tris (1,3-dichloro-2-propyl) phosphate (TDCIPP) has become an environmental pollutant. Studies have shown that TDCIPP exposure has many toxic effects, such as neurotoxicity, reproductive development toxicity and endocrine disturbance. CircRNAs are circular noncoding RNAs that have been found to play important roles in the occurrence of a variety of diseases. However, it is unclear whether circRNAs play a role in the cytotoxicity induced by TDCIPP exposure. In this study, we analysed the circRNA microarray results of a control group and a TDCIPP exposure group of PC12 cells and detected 3432 differentially expressed circRNAs (P < 0.05), of which 1682 were upregulated and 1750 were downregulated in the TDCIPP-exposure group. The expression levels of 2 upregulated and 3 downregulated circRNAs were verified by real-time quantitative PCR, and the results were consistent with the microarray results. Then, ceRNA analysis was performed on several kinds of circRNAs to predict the possible binding miRNAs and binding sites. The target genes were analysed by Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis. Through predictive analysis, it was found that rno_circRNA_013845, rno-miR-361-3p, and rno-miR-702-3p may be involved in the regulation of Traf2 expression, thereby affecting the expression of the downstream NF-κB signalling pathway and causing apoptosis.

Small short-arc xenon flash lamps, yielding high energy and ultraviolet radiation without cooling structures, provide ideal analytic-instrument light. Improving instrument designs requires accurately determining spatial optical-flux distribution. Thus, this paper presents rapid scanning of a xenon lamp's central light intensity using a high-sensitivity photodiode to capture short light pulses. Results show two-dimensional optical illumination patterns. In the horizontal mode, the anode and cathode lie in the target plane. In the vertical mode, the two electrodes are centered in a circle. Thus, because the xenon lamp's spatial light flux varies by orientation and main driving voltage, we recommend sampling light horizontally in front of the lamp across small angles.

Xenon flash lamp is used by most analytical instruments and medical equipments as light sources whose spectral characteristics exert a tremendous influence on the property of instruments and medical equipments. In order to study the effects of pulsed Xenon discharge circuit parameters and geometric parameters on the spectral characteristics, theoretical analysis has been conducted to Xenon lamp emitting process with the gas discharge theory. The spectroscopic detection system has been designed to conduct experimental detection to the spectral characteristics of pulsed Xenon lamp with different parameters. The experimental results show that the emission spectra of Xenon flash lamp contain both line and continuous radiation. The line spectrum results from the electron energy gap transition, while the cw radiation related to ionic recombination and bremsstrahlung. The relative spectral intensity increases linearly with the discharge voltage, and has a positive correlation with storage capacitor. When low discharge voltage, the shorter the arc length, the higher the relative spectral intensity .while the discharge voltage becomes higher, the result is reversed. The work in this paper has great significance for xenon flashlamp on the parameters selection and industrial application.

Using the UV absorption spectrum to detect Organic pollutants content in water has become one of the most important methods for real-time online monitoring in the field of water quality inspection, however, the water complex and unstable components often bring much uncertain offset to the standard test. In this paper, water samples were classified firstly by analyzing UV absorption spectrum ranging from 200 nm to 400 µm including the organic substances, through the way of combining principal component analysis (PCA) with Euclidean distance. In this paper, we compared the Principal component analysis combined with partial least squares regression (PCA-PLSR) and the direct multi-wavelength absorption models combined with partial least squares regression (MWA-PLSR), not only for the real water sample but also for the analysis of different concentrations of COD standard solution. The result indicates that the measurement errors of the PCA is less than 5%, it is the smallest by using the first and second principal components as regression parameters for PLSR. Using the methods above can simultaneously achieve to classify of water samples and to measure the concentration of water quality parameters more accurately.

BACKGROUND: The strategy of using fecal microbiota transplantation (FMT) for refractory ulcerative colitis (UC) remains unclear if single FMT failed to induce remission. This study aimed to evaluate the efficacy and safety of a designed step-up FMT strategy for the steroid-dependent UC. METHODS: Fifteen patients with steroid-dependent UC were enrolled, and treated with step-up FMT strategy. Follow-up clinical data was collected for a minimum of 3 months. Fecal microbiota composition before and post FMT of patients and related donors were analyzed by 16S rRNA sequencing. RESULTS: Eight of fourteen (57.1 %) patients achieved clinical improvement and were able to discontinue steroids following step-up FMT. One patient was lost to follow-up. Among the 8 patients who responded, five (35.7 %) received one FMT therapy, one (7.1 %) received two FMTs, and two (14.2 %) received two FMTs plus a scheduled course of steroids. Four (28.6 %) of the 8 patients who responded maintained long-term remission during follow-up (3-18 months). Six patients (42.9 %) failed to meet the criteria of clinical improvement and maintained steroid dependence, though three experienced transient or partial improvement. Microbiota analysis showed that FMT altered the composition greatly, and a microbiota composition highly similar to that of the donor emerged in the patients with successful treatment. No severe adverse events occurred during treatment and follow-up. CONCLUSIONS: Step-up FMT strategy shows promise as a therapeutic strategy for patients with steroid-dependent UC, likely due to the successful restructuring of gut microbial composition. The Xenon flash lamp is a new type of light source for analytical instrument. The present paper analyzed the discharge process of xenon flash lamp, presented the discharge test system, and conducted experimental measurement of the voltage, current and optical pulse signal in the process of discharge. The results show that in the preliminary discharge, the free electron concentration was at a low level, so the energy was at a low level, then following the gas discharge, numerous free electrons formed in the lamp, resultin in the increase in the concentration of free electrons, therefore discharge current rised rapidly and voltage reduced. The lamp released photons to generate light pulse in the moment of ionic recombination, The pulse xenon lamp light energy output and spectral characteristic is related to electron energy in recombination and combination level of xenon, if the input energy and the energy consumption of the xenon lamp is inconsistent, it will lead to repeated capacitor charging and discharging and produce oscillation waveform. This paper is very useful for understanding the process of xenon lamp discharge, optimizing the driver circuit and the production of xenon flash lamp.

The objective of this study was to adapt a novel near-infrared diffuse correlation spectroscopy (DCS) flow-oximeter for simultaneous and continuous monitoring of relative changes in cerebral blood flow (rCBF) and cerebral oxygenation (i.e. oxygenated/deoxygenated/total hemoglobin concentration: Δ[HbO2]/Δ[Hb]/ΔTHC) during overnight nocturnal polysomnography (NPSG) diagnostic test for obstructive sleep apnea-hypopnea (OSAH). A fiber-optic probe was fixed on subject's frontal head and connected to the DCS flow-oximeter through a custom-designed fiber-optic connector, which allowed us to easily connect/detach the optical probe from the device when the subject went to bathroom. To minimize the disturbance to the subject, the DCS flow-oximeter was remotely operated by a desktop located in the control room. The results showed that apneic events caused significant variations in rCBF and ΔTHC. Moreover, the degrees of variations in all measured cerebral variables were significantly correlated with the severity of OSAH as determined by the apnea-hypopnea index (AHI), demonstrating the OSAH influence on both CBF and cerebral oxygenation. Large variations in arterial blood oxygen saturation (SaO2) were also found during OSAH. Since frequent variations/disturbances in cerebral hemodynamics may adversely impact brain function, future study will investigate the correlations between these cerebral variations and functional impairments for better understanding of OSAH pathophysiology.

RESUMEN

A novel technology to measure oil concentration in water is presented, and mie-scattering theory was creatively introduced to oil-in-water detection in the present study. A variety of oil samples were examined to obtain both scattering and infrared absorption spectra, and the linear correlation between light scattering intensity and measures with infrared spectrophotometry is more than 0. 98. In addition, compared to the conventional methods, light scattering method requires no chemical reagent, and it is suitable for in situ measurement of oil in water.

The harm assessment and responsibility of marine mineral oil pollution require the content of mineral oil to be obtained by real-time online monitoring. In the present paper, the method of online monitoring of the oil content in water is presented, a creative technology based on optical scattering theory was proposed. Firstly the particle distribution of oil drops was scanned by Marvin laser particle meter, then with the help of the scattering signal, the crude, diesel of different concentrations from 0 to 30 mg x L(-1) was measured in experiment, the linearity is more than 0.99. This paper presents a feasible setup for the online monitoring of the content of mineral oil.

Water pollution control and prevention require that the water quality testing is of real time, online and portability. The presejt paper provides a water detecting system based on UV-visible spectra. The R2 of the linear regression is greater than 0.99 between absorbance and COD of the samples. Single measurement is within 1 s. At the same time, two methods were presented for direct comparison and normalization of the spectra and their corresponding indicator parameters. By a large number of experiments with standard solution of the potassium hydrogen phthalate and water samples, we found that these methods can be used to classify samples, find suitable mathematical model of the test water from the database, to improve the universal ability of the UV water testing technology, and get other water quality indices.

Arterial revascularization in patients with peripheral arterial disease (PAD) reestablishes large arterial blood supply to the ischemic muscles in lower extremities via bypass grafts or percutaneous transluminal angioplasty (PTA). Currently no gold standard is available for assessment of revascularization effects in lower extremity muscles. This study tests a novel near-infrared diffuse correlation spectroscopy flow-oximeter for monitoring of blood flow and oxygenation changes in medial gastrocnemius (calf) muscles during arterial revascularization. Twelve limbs with PAD undergoing revascularization were measured using a sterilized fiber-optic probe taped on top of the calf muscle. The optical measurement demonstrated sensitivity to dynamic physiological events, such as arterial clamping/releasing during bypass graft and balloon inflation/deflation during PTA. Significant elevations in calf muscle blood flow were observed after revascularization in patients with bypass graft (+48.1 ± 17.5%) and patients with PTA (+43.2 ± 11.0%), whereas acute post-revascularization effects in muscle oxygenation were not evident. The decoupling of flow and oxygenation after revascularization emphasizes the need for simultaneous measurement of both parameters. The acute elevations/improvements in calf muscle blood flow were associated with significant improvements in symptoms and functions. In total, the investigation corroborates potential of the optical methods for objectively assessing the success of arterial revascularization.

A portable diffuse correlation spectroscopy (DCS) flowmeter has been extended to measure both tissue blood flow and oxygenation (namely, DCS flow oximeter). For validation purposes, calf muscle blood oxygenation during cuff inflation and deflation was measured concurrently using the DCS flow oximeter and a commercial tissue oximeter. The oxygenation traces from the two measurements exhibited similar dynamic responses, and data were highly correlated (r(mean)>0.9, P<10(-5), n=10). The portable, inexpensive, and easy-to-use DCS flow oximeter holds promise for bedside monitoring of tissue blood flow and oxygenation in clinics.

To measure and analyze the spectrum of a small skin region for laser therapy of port wine stain is very useful for clinical doctor to understand the optical characters of patient's pigmented skin, to adjust the output parameters of lasers and then to improve the therapy effects. Based on the analysis of the forming mechanism of port wine stains and clinical operation of the disease, the present paper presents an automatic spectral measurement and analysis system for port wine stain therapy. It can accomplish the spectral detection in the range from 380 to 780 nm, with a spectral resolution of 1 nm. Experiments were performed for patients with various age and pigment diseases; finally, some influencing factors and relevant solutions are enumerated and discussed.
