RESUMEN

Objective:To understand the status of knowledge, attitude and practice of metabolic syndrome(MS) among the elderly in the community and analyze its influencing factors, so as to provide guidance for the prevention and treatment of MS.Methods:From January 2021 to August 2021, permanent residents(residence duration ≥6 months)aged 65 and above with regular health check examination in Fangzhuang Community Health Service Center of Fengtai District, Beijing were investigated using self-designed MS knowledge, attitude and practice questionnaire.After excluding 34 incomplete questionnaires, the 491 valid questionnaires were recycled with questionnaire valid percentage of 93.5%(491/525). The survey results were analyzed by descriptive analysis, Mann-Whitney U test, Kruskal-Wallis H test and multiple linear regression analysis and multiple linear regression.Results:The scores of MS knowledge, attitude and practice of the elderly in the community were 7(5, 10), 9(8, 10)and 19(18, 21)respectively.The correct answer rate of the diagnostic criteria of MS components was less than 50%.The score of knowledge about MS in the elderly was positively correlated with the scores of attitudes( r=0.263)and practice( r=0.134)(all P<0.01). The results of univariate and multiple linear regression analysis showed that gender, attending educational lectures and the prevalence of MS were the influencing factors of MS knowledge score(all P<0.05). Leisure time, attending educational lectures, pre-retirement occupation and regular health check examination were the influencing factors for MS attitude score(all P<0.05); attending educational lectures, leisure time, regular health check examination and pre-retirement occupation were the influencing factors for MS practice score(all P<0.05). Conclusions:The elderly in the community have a low level of knowledge about the components and diagnostic criteria of MS, but have a positive attitude and practice towards the prevention and treatment of MS.The level of knowledge, attitude and practice of MS is affected by health education lectures, occupation, regular health check examination and other factors.Health education on the diagnostic criteria and the awareness of disease prevention for the elderly in the community should be strengthened to improve the levels of the knowledge, attitude and practice of MS.

RESUMEN

Objective: To compare the image quality of three-dimensional MR cholangiopancreatography with breath-hold gradient-spin echo (3D-Grase-MRCP) and respiratory gated triggering turbo-spin echo (3D-Tse-MRCP). Methods: Totally 96 patients with suspicious pancreatic or biliary tract diseases underwent 3D-Grase-MRCP and 3D-Tse-MRCP scanning. The image quality scores, lesion display and contrast noise ratio (CNR)of the common bile duct were compared and analyzed between 3D-Grase-MRCP and 3D-Tse-MRC. The 3D-Grase-MRCP images were divided into hold-breath group and poor hold-breath group, and the image quality scores were compared between 2 groups. Results: CNR of common bile duct on 3D-Tse-MRCP images (357.08 [209.73, 594.38]) were higher than that on 3D-Grase-MRCP (256.14 [141.54, 417.87],Z=-3.01, P<0.05). The imaging scores on 3D-Grase-MRCP of the gallbladder, ductus cysticus, common bile duct and intrahepatic bile duct were higher than those on 3D-Tse-MRCP (all P<0.01). 3D-Grase-MRCP showed gallstones (n=42) and cystic duct stones (n=7) more clearly than 3D-Tse-MRCP (both P<0.05). Imaging scores of gallbladder, ductus cysticus, common bile duct, pancreatic duct and intrahepatic bile duct on 3D-Grase-MRCP in hold-breath group (n=68) were higher than those in poor hold-breath group (n=28) (all P<0.01). Conclusion: The imaging quality and lesion display of breath-hold 3D-Grase-MRCP sequence are superior to those of respiratory gated triggering 3D-Tse-MRCP sequence with significantly shortened scanning time, which is expected to be used as a new method for MRCP.

Objective@#To investigate the clinicopathologic characteristics, molecular and genetic features, differential diagnoses and prognosis of fumarate hydratase-deficient renal cell carcinoma (FH-RCC).@*Methods@#The immunohistochemical (IHC) expression of FH in 391 renal neoplasms in tissue chips collected from the Affiliated Hospital of Qingdao University and 971 Hospital of PLA Navy from January 2011 to December 2017 was evaluated. The clinicopathologic data of eight FH negative cases were collected.Polymerase chain reaction (PCR) and sequencing were used to detect the changes in FH gene in three cases. Interphase FISH with a dual color and break-apart probe was applied to detect the TFE3 gene alteration in the cases showing TFE3 protein expression.@*Results@#Among the eight patients, seven were male and one was female, and age ranged from 28 to 50 years (mean 39 years). Tumor size ranged from 3.5 cm to 12.0 cm (mean 7.9 cm). Renal pelvis invasion was identified in six cases, and the tumor emboli in renal vein and inferior vena cava were found in four patients. The cut surface of most tumors was solid, colorful, grayish white or yellow with no clear border showing invasive growth pattern. Microscopically, the tumors showed different proportions of papillary, tubular cystic, cribriform and solid structures. The tumor cells were rounded or polygonal with eosinophilic or amphotropic cytoplasm, round or oval nuclei, and focal large and prominent nucleoli (WHO/ISUP grade 3-4). Two cases had sarcomatoid or rhabdoid components. Intravascular tumor emboli were found in five cases. IHC staining showed most tumors expressed PAX8(7/8), CK19(7/8), vimentin (6/8) and P504s(8/8). However, other immunomarkers including CK7, CD10, CD117, RCC, 34βE12, HMB45 and Melan A were all negative. Sequencing showed all three cases had FH gene mutations in exon 1. FISH revealed no TFE3 gene translocation or amplification in the two cases with TFE3 IHC expression. Follow-up data were available in seven patients with the follow-up period from 11 to 66 months. Among them, five patients died between 11 to 31 months after the surgery because of extensive distant metastases of the tumor to the lung, liver and lymph nodes. The other two patients were alive at the 36th and 66th month after the surgery.@*Conclusions@#Morphologically, FH-RCC overlaps with papillary RCC, collecting duct carcinoma and tubular-cystic RCC, showing a mixture of papillary, tubular cystic, cribriform or tubular papillary structures with at least focal large and prominent nucleoli. The negative expression of FH and the detection of FH gene mutation could facilitate the diagnosis of the tumor. FH-RCC is a high aggressive tumor, prone to metastasize, and is associated with poor prognosis. The timely diagnosis of FH-RCC could benefit the patients and their relatives as well.

Inflammation participates in the construction of tumor mi-croenvironments (TMEs) by changing the homeostasis of tumor tissues. The interaction of cytokines secreted by TME-infiltrating cells forms a complex network of cytokines that play a role in tumor progression through various mechanisms such as inflammation promo-tion, immune editing, and immune escape. RESUMEN

Aging is a biological process and state characterized by morphological change, hypofunction and metabolic disorder, often accompanied by severe degenerative diseases, including amyotrophy, atherosclerosis, neurodegeneration, and so on. Age-related diseases not only decrease the life quality of the elderly, but also impose great burden on family and society. As population aging, we are increasingly coming to realize that the longevity we want should be healthy aging but not simple increase of lifespan. By researching on the yeast, C. elegans, drosophila, mice and humans, we get to know the mechanisms of aging and methods for anti-aging, such as dietary restriction and use of anti-aging drugs like rapamycin. In this review, we summarized mainly the studies on mechanisms of aging and elaborate associated anti-aging strategies and underlying mechanisms.

Objective To analyze the impact of lymphocyte subsets on chemotherapy efficacy and long-term survival of patients with advanced non-small cell lung cancer(NSCLC).Methods Totally 125 NSCLC patients who had received first-line chemotherapy including paclitaxel and pemetrexed with/without platinum were enrolled in this study.Lymphocytes from peripheral blood were collected before and after two cycles of first-line chemotherapy.Flow cytometry was performed to determine the expressions of 21 fluorescence-labeled lymphocyte subsets.Based on the imaging findings,chemotherapy efficacy was evaluated,and impact of the lymphocyte subsets on progression-free survival(PFS)and overall survival(OS)were analyzed.Results The baseline peripheral lymphocyte subsets showed no significant difference among groups receiving different treatment protocols(all P>0.05).After 2 cycles of chemotherapy,the percentage of CD4CD29lymphocytes was(16.87±5.28)% in progressive disease group,which was significantly lower than those in complete remission+partial remission group [(22.42±7.88)%,P=0.013] and stable disease group [(21.88±6.81)%,P=0.009].The median PFS was 7.07 months and median OS was 23.00 months.Cox multivariable regression analysis showed that the percentages of HLA-DR(HR:1.03,95%CI:1.01-1.05,P<0.001) and CDHLA-DRlymphocytes (HR:1.05,95%CI:1.01-1.08,P<0.001)were positively correlated with OS.Conclusions The rise of CD4CD29T lymphocytes in patients after chemotherapy indicates good chemotherapy efficacy.Higher percentage of HLA-DRand CD3HLA-DRlymphocytes in peripheral blood before chemotherapy predicts favorable prognosis.

BACKGROUND: The aim of this study was to investigate whether the combination of conventional treatment and Panax notoginseng (PN group) is superior to conventional treatment alone (CG group) in reducing the clinical symptoms of patients with multiple fractured ribs and pulmonary contusions. PATIENTS AND METHODS: We retrospectively analyzed the medical records of patients treated for multiple fractured ribs and pulmonary contusions with either conventional treatment (n = 17) or P. notoginseng (n = 18). Visual analog scale (VAS) pain scores and arterial oxygen saturation were measured at baseline and at 1 and 2 weeks following treatment. The duration of mechanical ventilation, systemic analgesics, and hospital stay were also recorded. RESULTS: VAS scores in the PN group were lower than in the CG group at 1 week (p < 0.01) and at 2 weeks (p < 0.05). Arterial oxygen saturation in both groups was higher after treatment than at baseline (p < 0.05), but there was no statistically significant difference between the 2 groups (p > 0.05). The duration of mechanical ventilation, systemic analgesics administration, and hospital stay in the PN group was remarkably decreased as compared to the CG group (p < 0.05). CONCLUSION: Combining conventional treatment and P. notoginseng seems to be an efficient method that can improve the clinical symptoms of multiple fractured ribs and pulmonary contusions.

Objective To evaluate the effects of micro-screw on the intrusion of lower incisors and the leveling of curve of Spee in the process of orthotherapy.Methods Fifteen patients (5 males,10 females) with deep-bite were selected,aged from 11.3 to 28.8 years,with average of 17.2 years.All of them were Angle Class Ⅱ Classification 1 malocclusion and had a curve of Spee larger than 3 mm.Orthotherapy of extraction was adopted for the first premolars of two upper mandible and second premolars of two lower mandible.All these patients had micro-screws implanted at root apex area between lower lateral incisor and canine bilaterally,and when the 0.457 mm× 0.635 mm (0.018 in ×0.025 in) Ni-Tisquare wire was internalized,the strength of intrusion from micro-screws was used.Lateral cephalometric radiographs were taken before loading the micro-screw and immediately when the curve of Spee was leveled.Two linear and two angular measurements were selected for cephalometric analysis.Results The following results were noted: the lower incisors were significantly intruded for a mean of (4.10±0.98) mm; the mandibular molars were not significantly extruded for a mean of (0.91±0.69) mm; some lower incisors proclined and some lower incisors retroclined for a mean of (-0.09±4.12)°.After bite opening,the mandibular plane angle was minimally altered for a mean of (0.25±0.70)°; the ratio of lower incisor intrusioin to lower molar extrusion was 4.5: 1.0.Conclusions The combined use of micro screw with Ni-Ti wire is capable of enhancing the intrusion of lower incisors and leveling the curve of Spee,and it has minimal side effects on the posterior teeth.

Objectives To investigate the overall prevalence of cardiovascular disease (CVD) in subjects hospitalized for chronic obstructive pulmonary disease (COPD),and explore the prevalence of the major CVD complications and trends in patients with COPD over a 10-year period.Methods Medical records in the PLA General Hospital,Beijing Union Medical College Hospital,and Beijing Hospital from 2000/01/01 to 2010/03/03 were retrospectively reviewed.A total of 4960 patients with COPD were reviewed in the study (3570 males,mean age,72.2 ± 10.5 years; 1390 females,mean age,72.0 ± 10.4 years).Results The prevalence of CVD in COPD patients was 51.7%.The three most prevalent CVDs were ischemic heart disease (28.9%),heart failure (19.6%),and arrhythmia (12.6%).During the 10-year study period,the prevalence of various CVDs in COPD patients showed a gradual increasing trend with increasing age.There was higher morbidity due to ischemic heart disease (P < 0.01) in male COPD patients than in the female counterparts.However,heart failure (P < 0.01)and hypertension (P < 0.01) occurred less frequently in male COPD patients than in female COPD patients.Furthermore,the prevalence of ischemic heart disease decreased year by year.In addition to heart failure,various types of CVD complications in COPD patients tended to occur in younger subjects.The prevalence of all major types of CVD in women tended to increase year by year.Conclusions The prevalence of CVD in patients hospitalized for COPD in Beijing was high.Age,sex and CVD trends,as well as life style changes,should be considered when prevention and control strategies are formulated.

RESUMEN

Objective To establish the differential templates of juvenile and children in Shandong with normal occlusion, according to the group of skeletal age. Methods 212 juvenile and children at the age of 8-16 years (107 males and 105 females) with normal occlusion in Shandong natives were took cephalomatric radiographs, and divided into different groups by cervical vertebrae skeletal age. Then these cephalomatric radiographs were scanned on the computer and 14 skeletal landmarks were vectorizated. An analytical method of Ricketts and McNamara with WinCept 7.0 was used to make statistics and variance analysis among gender and every group of cervical vertebrae skeletal age,and then established the templates. Results The juvenile and children in Shandong natives with normal occlusion had different templates. Conclusion By overlapping the same skeletal age of templates by SN plane, we can see that male face outline is greater than female and male mandibular plane angle is smaller than female. By overlapping the same gender of templates, there is a developmental trend of mandibular bone to be forward and downward, and to revolve anticlockwise with age.

RESUMEN

Objective To study MRI manifestation of lymphoma of bone marrow. Methods Ten patients(8 males and 2 females) with lymphoma of bone were performed MRI. Two cases were primary (1 HD and 1 NHL) and 8 were secondary NHL. Results (1)Location: All cases involved axial bones; (2)Number: 9 cases involved multiple bones, only 1 primary NHL involved solitary bone; (3)Signal Intensity: The marrow infiltration were showed low and high signal intensity on T 1WI and T 2WI respectively; (4)Shape: 9 cases were showed multiple focal features, another one showed diffuse changes; (5)Pathological Fracture: 3 cases associated with pathological fractures of vertebrae. Conclusion The sensitivity of MRI was excellent for detecting marrow infiltration of lymphoma, but the specificity was poor.
