RESUMEN

The iron-modified coal gasification slag (Fe-CGS) material has excellent performance in purifying heavy-metal-contaminated water due to its good surface properties and adsorption capacities. However, it is unclear whether it can provide long-term simultaneous stabilization of Cd and As in composite-contaminated soils in extreme environments. This study investigated the long-term stabilization of Cd and As in acidic (JLG) and alkaline (QD) soils by simulating prolonged heavy rainfall with the addition of Fe-CGS. Multiple extraction methods were used to analyze the immobilization mechanisms of Cd and As in soil and their effects on bioavailability. The results indicate that the stabilization efficiency was related to the dosage of Fe-CGS. The concentrations of Cd and As in the JLG soil leachate were reduced by 77.6% (2.0 wt%) and 87.8% (1.0 wt%), respectively. Additionally, the availability of Cd and As decreased by 46.7% (2.0 wt%) and 53.0% (1.0 wt%), respectively. In the QD soil leachate, the concentration of Cd did not significantly change, while the concentration of As decreased by 92.3% (2.0 wt%). Furthermore, the availability of Cd and As decreased by 22.1% (2.0 wt%) and 40.2% (1.0 wt%), respectively. Continuous extraction revealed that Fe-CGS facilitated the conversion of unstable, acid-soluble Cd into oxidizable Cd and acid-soluble Cd. Additionally, it promoted the transformation of both non-specifically and specifically adsorbed As into amorphous iron oxide-bound and residual As. Fe-CGS effectively improved the soil pH, reduced the bioavailability of Cd and As, and blocked the migration of Cd and As under extreme rainfall leaching conditions. It also promoted the transformation of Cd and As into more stable forms, exhibiting satisfactory long-term stabilization performance for Cd and As.

OBJECTIVE: The function of Bcl-6 in T follicular helper (Tfh) cell maturation is indispensable, and Tfh cells play a pivotal role in asthma. This study investigated the impact of Bcl-6 on asthmatic traits. METHODS: The microscopic pathological alterations, airway resistance (AR), and lung compliance (LC) were determined in asthmatic mice and Bcl-6 interference mice. The surface molecular markers of Tfh cells and the Bcl-6 mRNA and protein expression were determined by flow cytometry, RT-qPCR, and Western blotting, respectively. The relationships between the Tfh cell ratio and the IgE and IgG1 concentrations in peripheral blood mononuclear cells (PBMCs) and bronchoalveolar lavage fluid (BALF) were determined. RESULTS: Asthmatic inflammatory changes were observed in the lung tissue and were attenuated by Bcl-6 siRNA and dexamethasone (DXM). Asthmatic mice exhibited an increased AR and a decreased LC, while Bcl-6 siRNA or DXM mitigated these changes. The percentages of Tfh cells and eosinophils were significantly increased in the asthmatic mice, and they significantly decreased after Bcl-6 inhibition or DXM treatment. RT-qPCR and Western blotting analyses revealed that the Bcl-6 expression level in PBMCs was significantly higher in asthmatic mice, and it decreased following Bcl-6 inhibition or DXM treatment. The IgE expression in the serum and BALF and the B cell expression in PBMCs exhibited a similar trend. In asthmatic mice, the ratio of Tfh cells in the peripheral blood showed a strong positive correlation with the IgE levels in the serum and BALF, but not with the IgG1 levels. CONCLUSION: The amelioration of airway inflammation and airway hyper-responsiveness is achieved through Bcl-6 suppression, which effectively hinders Tfh cell differentiation, ultimately resulting in a concurrent reduction in IgE production.

Cadmium (Cd) and arsenic (As) co-contamination has become increasingly serious in China agricultural soil due to rapid industrialization and urbanization. The opposite geochemical behaviors of Cd and As pose huge challenges for developing a material for their simultaneous immobilization in soils. Coal gasification slag (CGS) as a by-product of coal gasification process, is always dumped into a local landfill, which has a negative impact on environment. Few reports have been available on applying CGS as a material to immobilize simultaneously multiple soil heavy metals. A series of iron-modified coal gasification slag (IGS) composites IGS3/5/7/9/11 (with different pH values) were synthesized by alkali fusion and iron impregnation. After modification, carboxyl groups were activated, and Fe was successfully loaded onto the surface of IGS in the form of FeO and Fe2O3. The IGS7 exhibited the best adsorption capacity with the maximum Cd and As adsorption capacity of 42.72 mg/g and 35.29 mg/g, respectively. The Cd was mainly adsorbed through electrostatic attraction and precipitation, while the As through complexation with iron (hydr)oxides. IGS7 significantly reduced the bioavailability of Cd and As in soil with Cd bioavailability reduced from 1.17 mg/kg to 0.69 mg/kg and As bioavailability reduced from 10.59 mg/kg to 6.86 mg/kg at 1 % IGS7 addition. The Cd and As were all transformed to more stable fractions after IGS7 addition. The acid soluble and reducible Cd fractions were transformed into oxidizable and residual Cd fractions, and the non-specifically and specifically adsorbed As fractions were transformed to amorphous iron oxide-bound As fraction. This study provides valuable references for the application of CGS to the remediation of Cd and As co-contaminated soil.

Objectives: To evaluate the impact of angiotensin-converting enzyme inhibitors (ACEIs) or angiotensin receptor blockers (ARBs) on the inflammatory response and viral clearance in coronavirus disease 2019 (COVID-19) patients. Methods: We included 229 patients with confirmed COVID-19 in a multicenter, retrospective cohort study. Propensity score matching at a ratio of 1:3 was introduced to eliminate potential confounders. Patients were assigned to the ACEI/ARB group (n = 38) or control group (n = 114) according to whether they were current users of medication. Results: Compared to the control group, patients in the ACEI/ARB group had lower levels of plasma IL-1ß [(6.20 ± 0.38) vs. (9.30 ± 0.31) pg/ml, P = 0.020], IL-6 [(31.86 ± 4.07) vs. (48.47 ± 3.11) pg/ml, P = 0.041], IL-8 [(34.66 ± 1.90) vs. (47.93 ± 1.21) pg/ml, P = 0.027], and TNF-α [(6.11 ± 0.88) vs. (12.73 ± 0.26) pg/ml, P < 0.01]. Current users of ACEIs/ARBs seemed to have a higher rate of vasoconstrictive agents (20 vs. 6%, P < 0.01) than the control group. Decreased lymphocyte counts [(0.76 ± 0.31) vs. (1.01 ± 0.45)*109/L, P = 0.027] and elevated plasma levels of IL-10 [(9.91 ± 0.42) vs. (5.26 ± 0.21) pg/ml, P = 0.012] were also important discoveries in the ACEI/ARB group. Patients in the ACEI/ARB group had a prolonged duration of viral shedding [(24 ± 5) vs. (18 ± 5) days, P = 0.034] and increased length of hospitalization [(24 ± 11) vs. (15 ± 7) days, P < 0.01]. These trends were similar in patients with hypertension. Conclusions: Our findings did not provide evidence for a significant association between ACEI/ARB treatment and COVID-19 mortality. ACEIs/ARBs might decrease proinflammatory cytokines, but antiviral treatment should be enforced, and hemodynamics should be monitored closely. Since the limited influence on the ACEI/ARB treatment, they should not be withdrawn if there was no formal contraindication.

OBJECTIVES: An ongoing outbreak of coronavirus disease 2019 is spreading globally. Acute hypoxemic respiratory failure is the most common complication of coronavirus disease 2019. However, the clinical effectiveness of early high-flow nasal oxygen treatment in patients with coronavirus disease 2019 with acute hypoxemic respiratory failure has not been explored. This study aimed to analyze the effectiveness of high-flow nasal oxygen treatment and to identify the variables predicting high-flow nasal oxygen treatment failure in coronavirus disease 2019 patients with acute hypoxemic respiratory failure. DESIGN: A multicenter, retrospective cohort study. SETTING: Three tertiary hospitals in Wuhan, China. PATIENTS: Forty-three confirmed coronavirus disease 2019 adult patients with acute hypoxemic respiratory failure treated with high-flow nasal oxygen. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Mean age of the enrolled patients was 63.0 ± 9.7 years; female patients accounted for 41.9%. High-flow nasal oxygen failure (defined as upgrading respiratory support to positive pressure ventilation or death) was observed in 20 patients (46.5%), of which 13 (30.2%) required endotracheal intubation. Patients with high-flow nasal oxygen success had a higher median oxygen saturation (96.0% vs 93.0%; p < 0.001) at admission than those with high-flow nasal oxygen failure. High-flow nasal oxygen failure was more likely in patients who were older (p = 0.030) and male (p = 0.037), had a significant increase in respiratory rate and a significant decrease in the ratio of oxygen saturation/FIO2 to respiratory rate index within 3 days of high-flow nasal oxygen treatment. In a multivariate logistic regression analysis model, male and lower oxygen saturation at admission remained independent predictors of high-flow nasal oxygen failure. The hospital mortality rate of the cohort was 32.5%; however, the hospital mortality rate in patients with high-flow nasal oxygen failure was 65%. CONCLUSIONS: High-flow nasal oxygen may be effective for treating coronavirus disease 2019 patients with mild to moderate acute hypoxemic respiratory failure. However, high-flow nasal oxygen failure was associated with a poor prognosis. Male and lower oxygenation at admission were the two strong predictors of high-flow nasal oxygen failure.

BACKGROUND: Several previously healthy young adults have developed Coronavirus Disease 2019 (COVID-19), and a few of them progressed to the severe stage. However, the factors are not yet determined. METHOD: We retrospectively analyzed 123 previously healthy young adults diagnosed with COVID-19 from January to March 2020 in a tertiary hospital in Wuhan. Patients were classified as having mild or severe COVID-19 based on their respiratory rate, SpO2, and PaO2/FiO2 levels. Patients' symptoms, computer tomography (CT) images, preadmission drugs received, and the serum biochemical examination on admission were compared between the mild and severe groups. Significant variables were enrolled into logistic regression model to predict the factors affecting disease severity. A receiver operating characteristic (ROC) curve was applied to validate the predictive value of predictors. RESULT: Age; temperature; anorexia; and white blood cell count, neutrophil percentage, platelet count, lymphocyte count, C-reactive protein, aspartate transaminase, creatine kinase, albumin, and fibrinogen values were significantly different between patients with mild and severe COVID-19 (P < 0.05). Logistic regression analysis confirmed that lymphopenia (P = 0.010) indicated severe prognosis in previously healthy young adults with COVID-19, with the area under the curve (AUC) was 0.791(95% Confidence Interval (CI) 0.704-0.877)(P < 0.001). CONCLUSION: For previously healthy young adults with COVID-19, lymphopenia on admission can predict severe prognosis.

Increasing concentrations of metals in soil have posed a serious threat to the soil environment. The control and evaluation of soil metal hazards demand the establishment of soil ecological criteria, which is mainly based on the obtainment of toxicity thresholds. As the most typical representative of soil-dwelling springtails, Folsomia candida performs numerous essential ecological functions in soil and has been extensively used to investigate metal toxicity effects and thresholds. This review outlined the current state of knowledge on the metal toxicity thresholds to Folsomia candida, including (1) toxicity thresholds of soil metals for the different endpoints, (2) the influence factors of metal toxicity thresholds including the test conditions, the chemical forms of metal, the soil physicochemical properties, aging time and leaching, (3) the bioavailable fractions predicting metal toxicity thresholds, (4) the internal threshold of metals. To conclude, several recommendations for future research are given to obtain the more reliable toxicity thresholds and further supplement the toxicity data of metals to Folsomia candida.

Nickel (Ni) is a toxic metal, but studies on Ni toxicity to soil-dwelling springtail are fairly limited, and did not consider the effects of various soil properties and long aging time. To address this, the chronic toxicity of Ni to model organism-Folsomia candida in relation to soil properties and aging time were evaluated in the laboratory study. The results showed that compared to the soils aged only for 7 d, the concentrations causing 50% mortality (LC50) and inhibiting 50% reproduction (EC50) basing measured total Ni in four soils aged for 120 d increased by 1.30-1.94 fold and 1.27-1.82 fold, respectively. Furthermore, the aging effects significantly correlated with soil pH. The toxicity values of Ni differed in ten soils aged for 120 d, the LC50 values were 279-4025 mg/kg and the EC50 values were 133-1148 mg/kg. When calculating the toxicity values basing water soluble and CaCl2 extracted Ni, the variations in LC50 values between ten soils decreased, while the variations in EC50 values increased. Regression analysis indicated that soil pH was the most important single factor predicting soil Ni toxicity to springtail, the combination of soil pH and OM could best explain Ni toxicity variance in ten soils (89.1% of the variance in LC50 values and 89.6% of the variance in EC50 values).

The aim of this study was to investigate the associations of serum levels of neuron-specific enolase (NSE), pro-gastrin releasing peptide (ProGRP), and lactate dehydrogenase (LDH) with clinical response and survival in small cell lung cancer (SCLC) patients receiving first-line platinum-based chemotherapy.One hundred thirty-six patients with SCLC were recruited in this study. All the patients received first-line platinum-based chemotherapy. Clinical efficacy was assessed according to Response Evaluation Criteria in Solid Tumors v1.1 criteria. Serum samples were collected from SCLC patients before chemotherapy. NSE, ProGRP, and LDH levels were measured by commercial electrochemiluminescence immunoassay, enzyme-linked immune sorbent assay, and kinetic spectrophotometric method, respectively.Overall response rate was 71.3% with 97 patients who achieved complete response (CR) +  partial response (PR). NSE and LDH level declined in patients who achieved CR + PR compared with patients in stable disease (SD) and progress disease (PD). Multivariate logistic regression analysis revealed that NSE > 50.324 ng/mL, stage ED, and distant metastases were independent risk factors for patients achieving CR + PR, and chemotherapy > 4 cycles was an independent protective factor in predicting CR + PR. Receiver operating characteristic (ROC) curves presented that expression of NSE, ProGRP, and LDH are of good predicting value for patients achieving CR + PR. Patients with a higher level of NSE and LDH presented worse progression-free survival and overall survival. In addition, multivariate Cox regression analysis showed that NSE level > 50.324 ng/mL and distant metastasis were independently correlated with worse OS.Serum NSE and LDH could be promising biomarkers for predicting therapy response and survival of SCLC patients receiving first-line platinum-based chemotherapy.

RESUMEN

Tracheoesophageal fistulas (TEFs) often occur with esophageal or bronchial carcinoma. Currently, we rely on implantation of delicate devices, such as self-expanding and silicone stents, in the esophagus or trachea to cover the fistula and expand the stenosis in order to relieve patient pain. However, because each case is different, our approach may not be effective for every patient. Consequently, new devices and technology have emerged to address these situations, such as degradable stents, Amplatzer® devices, endobronchial one-way umbrella-shaped valves, and transplantation of mesenchymal stem cells. Although some studies have shown such alternatives can be reasonable solutions in special cases, further development of other new and effectual techniques is of utmost importance.

The competitive adsorption of bisphenol A (BPA) and17α-ethinylestradiol (EE2) with different endocrine disrupting compounds (EDCs), such as estrone (E1), ß-estradiol (E2), and estriol (E3) was investigated in the water-sediment system. The primary and interaction effects of coexisted EDCs on the adsorption of BPA and EE2 were studied in binary and multiple systems. The adsorption selectivity of sediment at different initial concentrations of EDCs was also considered, based on the distribution coefficient (ß). In binary systems, coexisted EDCs exhibited a positive effect on the adsorption of BPA, while E3 showed a negative effect on the adsorption of EE2. In ternary systems, the interaction of E1*E3 and E2*BPA showed a synergistic effect on the sorption of BPA and EE2, respectively. In quaternary systems, the interaction of E1*E2*E3 showed a synergistic effect on the adsorption of both BPA and EE2. In the quinary system, coexisted EDCs all showed an antagonistic effect on the adsorption of BPA and EE2, which indicated that the coexisted EDCs competed for adsorption with BPA and EE2. EDCs in the E2-EE2-BPA system presented a superior selectivity of sediment with ß values of 43.48-87.86. The order of sediment selectivity (E1 > EE2 > E2 > E3 > BPA) in binary systems was in agreement with EDCs' adsorption capacity, which suggested that the adsorption was dominated by partition adsorption.
