RESUMEN

PURPOSE: To retrospectively compare interpretations of Doppler ultrasound (US) in newborns with confirmed perinatal testicular torsion (PTT) by an experienced faculty (staff) pediatric radiologist (SPR), pediatric radiology fellow (PRF), pediatric urology fellow (PUF) and staff pediatric urologist (SPU). METHODS: US images of 27 consecutive males with PTT between May 2000 and July 2020 were retrieved. The testicles were classified as affected or non-affected by PTT. We performed a blinded comparison of interpretation by four assessors (SPR, PRF, PUF, SPU), with respect to the US features of PTT. Paired inter-rater agreement was calculated using Cohen's Kappa (κ) and overall agreement was assessed using Fleiss' kappa. RESULTS: Overall comparison using Fleiss' kappa found fair agreement for most features except testicular echogenicity and echogenic foci at interface for which there was poor agreement. Paired comparisons revealed better agreement between the SPR and PRF compared to the remaining two pairs, suggesting a need for the pediatric urologists (PUF and SPU) to acquaint themselves with testicular ultrasonography as this may have an impact on patient risk stratification and the quality of information given to parents. CONCLUSION: This study highlights the need for focused training program for pediatric urologists to attain similar agreement as the radiologists, suggesting a need for the pediatric urologists (PUF and SPU) to acquaint themselves with testicular ultrasonography as this may have an impact on patient risk stratification and the quality of information given to parents.

RESUMEN

Juvenile idiopathic arthritis (JIA) is the most common chronic arthropathy in the pediatric population. Although the diagnosis is essentially clinical for many affected joints, MR imaging has become an important tool for the assessment of joints that are difficult to evaluate clinically, such as temporomandibular and sacroiliac joints, and for screening of inflammatory changes in the entire body by whole body MRI (WBMRI) assessment. The utilization of MR imaging is challenging in the pediatric population given the need for discrimination between pathological and physiological changes in the growing skeleton. Several multicentric multidisciplinary organizations have made major efforts over the past decades to standardize, quantify, and validate scoring systems to measure joint changes both cross-sectionally and longitudinally according to rigorous methodological standards. In this paper, we (1) discuss current trends for the diagnosis and management of JIA, (2) review challenges for detecting real pathological changes in growing joints, (3) summarize the current status of standardization of MRI protocols for data acquisition and the quantification of joint pathology in JIA by means of scoring systems, and (4) outline novel MR imaging techniques for the evaluation of anatomy and function of joints in JIA. Optimizing the role of MRI as a robust biomarker and outcome measure remains a priority of future research in this field.

BACKGROUND: Pre-diabetic state is a major risk factor for the development of diabetes and cardiovascular events. Admission glucose, fasting glucose and HbA1c levels have an effect on prognosis in patients with pre-diabetes and in non-diabetic individuals. The aim of the present study was to investigate which of the following glucometabolic markers (admission glucose, fasting glucose and HbA1c levels) is correlated with the severity of coronary artery disease (CAD) in non-diabetic patients. METHODS: CAD severity according to SYNTAX score was prospectively evaluated in 226 non-diabetic patients hospitalized with myocardial infarction or stable angina and underwent coronary angiography. Glucose intolerance was assessed by serum admission glucose, fasting glucose and HbA1c levels. Logistic regression analysis was used to evaluate which glucometabolic factor has the strongest correlation with CAD severity. RESULTS: HbA1c was the only glucometabolic factor associated with SYNTAX score above 22 (OR = 3.03, CI 95% 1.03-8.9, p = 0.04). HbA1c was also significantly associated with CAD severity in subgroup analysis (MI and stable angina). CONCLUSIONS: In non-diabetic patients with myocardial infarction or stable angina, HbA1c levels correlate with CAD severity as measured by the SYNTAX score. No correlation was found between admission glucose or fasting glucose levels and CAD severity.
