RESUMEN

The paper is concerned with a study into the mechanisms of renal interstitial lesions of the secondary character seen in patients afflicted with chronic glomerulonephritis. 120 patients with mesangioproliferative and membranous proliferative glomerulonephritis were examined. The patients were distributed into groups depending on the disease acuity, the degree of glomerular sclerosis and the disease stage. Three major mechanisms of the augmentation of changes in the renal interstice have been revealed: 1) exacerbation of the underlying process; 2) the degree of fibroplastic and sclerotic lesions in the glomerulus; 3) drug sensitization.

RESUMEN

Morphological changes of the renal blood vessels were studied in 143 patients with different nephropathies and hypertensive disease. Vessels wall changes proper included: fibroelastosis, hyalinosis as well as antibodies to smooth muscle cells observed exclusively in the development of arterial hypertension. The changes were more pronounced in severe and durable hypertension. Perivascular sclerosis was found almost in all hypertensive patients and in some normotonic patients with aggravated heredity for arterial hypertension. The obtained data allow to frame a hypothesis on the role of formation of perivascular sclerosis as a reflection of repeated vascular spasm, subsequent increase of the peripheral resistance in the pathogenesis of development of arterial hypertension.

RESUMEN

A hypothesis of the development of drug-induced nephropathy has been reviewed. The authors believe that the body sensitization to different drugs underlies drug-induced nephropathy. The sensitization may develop in the presence of congenital or acquired abnormalities leading to hemodynamic disorders in the kidneys.

RESUMEN

Results of 62 reconstructive and restorative operations performed on 47 patients with scarry strictures and injuries of main bile ducts are analyzed. A group of 17 patients was chosen, a spare transhepatic drain being used in 18 operations. It was established that bougieurage, dilatation of the strictures and a continuous frame drainage with the stricture of hepaticocholedochus as long as 20 mm or recurrent strictures of hepaticojejunoanastomosis provided stabilization of scarring and epithelization of the stricture zone. A favorable result of the treatment is determined by the duration of the frame drainage not less than for 2 years.

RESUMEN

Histologic structure of Mueller's adenosarcoma has been studied in 5 patients subjected to uterine and its appendages extirpation. Microspectrophotometric assessment of the course of nuclear DNA accumulation, measurement of X-chromatin in two morphologic components of the tumor (epithelial and stromal) confirmed malignant morphological nature of the latter and benign one of the former. Basing on their experience with the 5 patients three of which died in the course of 5 years of relevant metastases, the authors believe the prognosis for uterine adenosarcoma to be poor.

RESUMEN

A clinicomorphological study was conducted over time in 62 patients with different morphological types of glomerulonephritis. An increase in vascular, interstitial and sclerotic changes was noted morphologically in most patients, the worst dynamics was observed in membranoproliferative glomerulonephritis. Disease progression was associated with 2 processes; sclerosis of all elements of the renal tissue or active inflammation.

RESUMEN

A 54-year old man was hospitalized with such acute complaints, as vomiting, fatigue, severe gastric pain, progressive weight loss. Gastric cancer was suspected, but the severity of the patient's state excluded X-ray examination of the stomach. The death occurred on day 9 of the hospital stay. It was only the microscopic pathoanatomic examination of the stomach and small intestine, that revealed trichinosis. Absence of Trichinella larvae in the gastric mucosa was suggestive of a massive alimentary infection in the patient.

RESUMEN

Light optical and electron-microscopic investigations were carried out on 42 dogs with experimental hypertensive-enzymatic acute pancreatitis. It was revealed that microcirculatory changes appeared already within the first hour of experimental pancreatitis. By the 4th hour of the experiment the changes in basal membrane and podocytes, as well as proliferation of mesangial cells and growth of mesangial matrix were noted. Thus, at the initial stages of acute pancreatitis morphological alterations in the kidneys correspond to the pattern of mesangial proliferative diffuse glomerulitis.

Based on an analysis of the clinicolaboratory findings and morphological examination of 93 patients with renal amyloidosis the proteinuric stage was diagnosed in 24, the nephrotic in 35, and the azotemic in 34 patients. Elevation of the arterial blood pressure was revealed in 40% of the patients. During the first year since proteinuria was diagnosed, the nephrotic syndrome developed in 50% of the patients; renal failure in 75% of cases developed over 9 years. The 5-year survival since proteinuria was diagnosed amounted to 72%, the 10-year survival to 34%. The 5-year survival among patients with the nephrotic syndrome constituted 49%, whereas the 10-year survival was attained only in single cases. One year after blood creatinine became elevated only 30% of the patients were still alive. As the disease progressed, there was a gradual increase in amyloid impregnation of the glomeruli alone, while amyloidosis of the tubules was most pronounced in the nephrotic stage. No complete correlation was found between the gravity of the morphological picture of glomerular involvement and blood creatinine level which is undoubtedly influenced by alterations in the exoglomerular structures.
