RESUMEN

The ventriculoperitoneal shunt placement procedure is common throughout the world, with a prevalence of approximately 30,000 procedures per year in the United States (5). However, the rate of complications is still important and must be remembered, with a percentage of 11 to 25% after the first year of implantation. Abdominal complications are described in 5-47% of the cases (6), and the abdominal pseudocyst is one of them. We present the case of a patient colonized by multidrug-resistant bacteria, who underwent exteriorization surgery of a ventriculoperitoneal shunt. Intraoperatively, a blue coloration was observed in the catheter, a phenomenon previously described in urinary infections (1)(2) but not before in pathologies of the central nervous system. Our objective is to present an atypical case of dysfunction of the ventriculoperitoneal shunt, whose diagnosis couldbe evidenced through the coloration of the catheter exteriorized in surgery.

BACKGROUND: Individuals from the community who volunteer within prisons are an understudied population, despite previous research indicating the increase in involvement of the penal voluntary sector and benefits to both prisons and prisoners from effective implementation of volunteer programmes. OBJECTIVES: This study aimed to identify the characteristics, motivations and experiences of individuals who volunteer in prisons. STUDY DESIGN: This was a systematic review conducted according to Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines. METHODS: Peer-reviewed publications were identified through searchers of five electronic databases (MEDLINE, PsycINFO, Scopus, Applied Social Sciences Index & Abstracts and Social Sciences Database) without date restrictions, supplemented by hand searching and reference checking of retrieved articles. Explicit inclusion and exclusion criteria determined study eligibility. Study quality was appraised using standard tools. A narrative synthesis was conducted, and motivations were organised according to the Volunteer Function Inventory. RESULTS: Eight studies (five qualitative and three quantitative) reported a total of 764 volunteers across five countries. More than half of the included studies investigated individuals providing primarily religious volunteer support; volunteers in these studies were typically middle aged, White and female. Prison volunteers frequently described motivations related to altruistic or humanitarian values, as well as social reasons. Positive experiences of volunteering were related to personal benefits to volunteers. Negative experiences were related to a lack of support and challenges in volunteers' relationships with prison staff. CONCLUSIONS: Prison volunteer programmes have the ability to improve the psychological health of prisoners and provide a range of potential benefits to penal systems and volunteers themselves, but research on individuals who volunteer in prisons is limited. Difficulties in the volunteer role could be mitigated by developing formal induction and training packages, promoting closer integration with paid prison staff and providing ongoing supervision. Interventions to improve the volunteer experience should be developed and evaluated.

OBJECTIVES: To describe the appearance and size of the ganglionic eminence (GE) in normal fetuses on midtrimester three-dimensional (3D) neurosonography and to report on the association between GE alterations (cavitation or enlargement) and malformation of cortical development (MCD). METHODS: This was a prospective multicenter cohort study of normal fetuses and a retrospective analysis of pathological cases with MCD. From January 2022 to June 2022, patients attending our tertiary centers for an expert fetal brain scan were recruited for the purpose of the study. A 3D volume of the fetal head, starting from the sagittal plane, was acquired in apparently normal fetuses using a transabdominal or transvaginal approach. Stored volume datasets were then evaluated independently by two expert operators. Two measurements (longitudinal diameter and transverse diameter) of the GE in the coronal view were obtained twice by each operator. Intra- and interobserver measurement variation was calculated. Reference ranges for GE measurements were calculated in the normal population. A previously stored volume dataset of 60 cases with MCD was also analyzed independently by the two operators using the same method in order to assess if GE abnormalities (cavitation or enlargement) were present. Postnatal follow-up was obtained in all cases. RESULTS: In the study period, 160 normal fetuses between 19 and 22 weeks of gestation were included in the study. The GE was visible in the coronal plane on 3D neurosonography in 144 (90%) cases and was not clearly visible in the remaining 16 (10%) cases. The intra- and interobserver agreement was almost perfect for the longitudinal diameter, with an intraclass correlation coefficient (ICC) of 0.90 (95% CI, 0.83-0.93) and 0.90 (95% CI, 0.86-0.92), respectively, and substantial for the transverse diameter, with an ICC of 0.80 (95% CI, 0.70-0.87) and 0.64 (95% CI, 0.53-0.72), respectively. A retrospective analysis of 50 cases with MCD in the second trimester showed that GE enlargement was present in 12 cases and GE cavitation was present in four cases. CONCLUSIONS: Systematic assessment of the GE in fetuses at 19-22 weeks of gestation is feasible on 3D neurosonography, with good reproducibility in normal cases. Cavitation or enlargement of the GE can be demonstrated in fetuses with MCD. © 2023 The Authors. Prototheca spp. cause numerous infections in a wide variety of species, including treatment-unresponsive mastitis. Thus, the search for an effective therapy is essential. Silver nanoparticles are compounds with high therapeutic potential. This study aimed to evaluate the susceptibility profile and morphological changes in Prototheca spp. treated with biogenic silver nanoparticles (Bio-AgNP). The algaecide activity was evaluated in microplates by microdilution method, resulting in a MIC50 of 30 µg ml-1 and a MIC90 of 60 µg ml-1 . Scanning electron microscopy demonstrated changes in the surface of Prototheca bovis cells following treatment. The algaecide activity of Bio-AgNP suggests a therapeutic potential as a novel approach for the control of Prototheca spp. in bovine mastitis.

BACKGROUND/AIM: Dental trauma is a highly prevalent problem worldwide. The aim of this study was to describe characteristics of trauma to anterior primary teeth and its sequelae for primary and successor teeth over a 10-year period of assistance at the paediatric dental traumatology clinic in the State University of Rio de Janeiro, Brazil. MATERIALS AND METHODS: The data were obtained from the dental records of children assisted from 2006 to 2016. Inclusion criteria were trauma to anterior primary teeth; aged <1 to 9 years at the time of trauma; and at least one follow-up appointment. The data concerning traumatized teeth, child's age, soft tissue injuries, types of traumatic dental injuries and sequelae in both dentitions were collected. RESULTS: Of the 1230 dental records, 483 (39.3%) were eligible, consisting of 815 teeth. The mean age at the time of trauma was 37.4 months (239 males and 244 females). The most important characteristics were: tooth 51 was most affected; falls, at home, on a ceramic floor, were the main cause; gingival laceration, enamel fracture and intrusive luxation were the most frequent types of trauma; premature tooth loss of primary teeth and enamel discoloration in their successor were the most frequent sequelae; and injuries in the periodontal tissue were demonstrated as the highest risk for sequelae in successors (OR, 3.798; p< 0.001). CONCLUSIONS: Most traumatic injuries occur in young children, in their home environment. Consequences for the primary teeth were more frequent compared to the permanent successor tooth. Traumatized primary teeth involving periodontal tissue have more chances to result in sequelae to their successor teeth.

PURPOSE: Identification of potential predictive factors for keratoconus progression after treatment by accelerated Cross-linking (A-CXL) SECONDARY OBJECTIVES: Evaluation of clinical and topographic outcomes for two years following accelerated cross-linking treatment for progressive keratoconus including: best spectacle corrected visual acuity (BSCVA), thinnest pachymetry, maximum keratometry (Kmax), cylinder. STUDY: Prospective, interventional, monocentric study. SITE: Metz-Thionville Regional Medical Center, Lorraine University, Mercy Hospital, Metz, France. PATIENTS AND METHODS: We included 82 eyes of 60 patients between March 2014 and June 2016 who underwent accelerated corneal cross-linking (A-CXL) with epithelial debridement for progressive keratoconus, with a minimum follow-up of 2 years. A complete clinical evaluation and corneal topography were performed before cross-linking, and subsequently at 6, 12 and 24 months post-procedure. The following parameters were monitored during follow-up: best spectacle corrected visual acuity (BSCVA), minimal pachymetry, maximum keratometry (Kmax), mean anterior and posterior curvatures, maximum posterior curvature, presence of optical aberrations, subdivided into spherical aberration, coma, astigmatism, higher order optical aberrations and residual optical aberrations. After a 2-year follow-up, two groups, defined as "responders" and "non-responders" to treatment, were separated for analysis, and their initial characteristics were compared. RESULTS: Data for 82 eyes of 60 patients with progressive keratoconus with a mean age of 24±7 years were studied. Fourteen eyes (17.1%) showed signs of progression after treatment by A-CXL (non-responders), and 68 eyes (82.9%) showed stabilization of the disease (responders). Characteristics of non-responding eyes after A-CXL included a younger mean age (20±5 vs. 25±7 years) (P=0.04) and a lower initial mean BCVA for non-responders of 0.44±0.16 logMAR vs. 0.29±0.19 logMAR (P=0.03). Non-responders also had a higher mean maximal posterior curvature (AKB) of -10.84±1.72D vs. -9.46± 1.12D (P=0.03). They also showed more higher order optical aberrations (3.84±1.72D vs. 2.4±1.02D; P=0.01), including coma (3.85±1.81D vs. 2.1±1.01D; P=0.03) and more residual aberrations than responders (1.05±0.44D vs. 0.45±0.6D; P=0.005). No significant differences were found between responders and non-responders for the other parameters in our study. CONCLUSION: Eyes with progressive keratoconus who did not respond to A-CXL treatment were the most aggressive cases in the youngest patients, with highest maximum corneal curvatures and most pronounced optical aberrations. These patients should be informed in advance of the high risk of non-response to A-CXL treatment, and of the potential need for additional treatment in the future.

It remains unknown whether dairy cows with more reactive temperament produce more enteric methane (CH4) and are less bioenergetically efficient than the calmer ones. The objectives of this study were (a) to evaluate the relationship between cattle temperament assessed by traditionally used tests with energetic metabolism and enteric CH4 emissions by crossbred dairy cows; (b) to assess how cows' restlessness in respiration chambers affects energetic metabolism and enteric CH4 emissions. Temperament indicators were evaluated for 28 primiparous F1 Holstein-Gyr cows tested singly in the handling corral (entrance time, crush score, flight speed, and flight distance) and during milking (steps, kicks, defecation, rumination, and kick the milking cluster off). Cows' behaviors within respiration chambers were also recorded for each individual kept singly. Digestibility and calorimetry trials were performed to obtain energy partitioning and CH4 measures. Cows with more reactive temperament in milking (the ones that kicked the milking cluster off more frequently) spent 25.24% less net energy on lactation (P = 0.04) and emitted 36.77% more enteric CH4/kg of milk (P = 0.03). Furthermore, cows that showed a higher frequency of rumination at milking parlor allocated 57.93% more net energy for milk production (P < 0.01), spent 50.00% more metabolizable energy for milk production (P < 0.01) and 37.10% less CH4/kg of milk (P = 0.04). Regarding the handling temperament, most reactive cows according to flight speed, lost 29.16% less energy as urine (P = 0.05) and tended to have 14.30% more enteric CH4 production (P = 0.08), as well as cows with a lower entrance time (most reactive) that also lost 13.29% more energy as enteric CH4 (P = 0.04). Temperament and restless behavior of Holstein-Gyr cows were related to metabolic efficiency and enteric CH4 emissions. Cows' reactivity and rumination in the milking parlor, in addition to flight speed and entrance time in the squeeze chute during handling in the corral, could be useful measures to predict animals more prone to metabolic inefficiency, which could negatively affect the sustainability of dairy systems.

There is a trend to reduce the space allowance per animal in cattle feedlot, despite its potential negative impact on animal welfare. Aiming to evaluate the effects of space allowance per animal in outdoor feedlots on beef cattle welfare, a total of 1350 Nellore bulls (450 pure and 900 crossbred) were confined for 12 weeks using three space allowances: 6 (SA6), 12 (SA12) and 24 (SA24) m2/animal (n = 450 per treatment). Bulls were housed in three pens per treatment (n = 150 per pen). The first 6 weeks in the feedlot were defined as 'dry' and the last as 'rainy' period, according to the accumulated precipitation. Animal-based (body cleanliness, health indicators and maintenance behaviour) and environmental-based indicators (mud depth and air dust concentration) were assessed weekly during the feedlot period. Most of the health indicators (nasal and ocular discharge, hoof and locomotion alterations, diarrhoea, bloated rumen and breathing difficulty) were assessed in a subset of 15 animals randomly selected from each pen. Coughs and sneezes were counted in each pen. Maintenance behaviours (number of animals lying and attending the feed bunk) were recorded with scan sampling and instantaneous recording at 20-min intervals. Postmortem assessments were carried out in all animals by recording the frequencies of macroscopic signs of bronchitis, pulmonary emphysema, nephritis and urinary cyst and by measuring the weight and cortical and medullar areas of adrenal glands (n = 30 per pen). Compared with SA12 and SA24, SA6 showed a greater number of sneezes per minute during the dry period and a greater percentage of animals with locomotion alterations during the rainy period. Coughing, diarrhoea and nasal discharge affected a larger number of animals in the SA6 relative to the other two groups. During the rainy period, there was a lower percentage of animals with nasal and ocular discharge, and a greater percentage of animals with abnormal hoof and lying. A lower percentage of animals in SA6 and SA12 (but not SA24) attended the feed bunk during the rainy relative to the dry period. A mud depth score of 0 (no mud) was most frequent in SA24 pens, followed by SA12 and then SA6. Adrenal gland weight and cortical area were lower in SA24 animals compared with those in SA6 and SA12. The results show that decreasing the space allowance for beef cattle in outdoor feedlots degrades the feedlot environment and impoverishes animal welfare.

OBJECTIVE: In this article, we describe a new safe entry point for the posterolateral pons. METHODS: To show the adjacent anatomy and measure the part of the interpeduncular sulcus that can be safely accessed, we first performed a review of the literature regarding the pons anatomy and its surgical approaches. Thereafter, 1 human cadaveric head and 15 (30 sides) human brainstems with attached cerebellums were bilaterally dissected with the fiber microdissection technique. A clinical correlation was made with an illustrative case of a dorsolateral pontine World Health Organization grade I astrocytoma. RESULTS: The safe distance for accessing the interpeduncular sulcus was found to extend from the caudal end of the lateral mesencephalic sulcus to the point at which the intrapontine segment of the trigeminal nerve crosses the interpeduncular sulcus. The mean distance was 8.2 mm (range, 7.15-8.85 mm). Our interpeduncular sulcus safe entry zone can be exposed through a paramedian infratentorial supracerebellar approach. When additional exposure is required, the superior portion of the quadrangular lobule of the cerebellar hemispheric tentorial surface can be removed. In the presented case, surgical resection of the tumor was performed achieving a gross total resection, and the patient was discharged without neurologic deficit. CONCLUSIONS: The interpeduncular sulcus safe entry zone provides an alternative direct route for treating intrinsic pathologic entities situated in the posterolateral tegmen of the pons between the superior and middle cerebellar peduncles. The surgical corridor provided by this entry point avoids most eloquent neural structures, thereby preventing surgical complications.

Aim The aim of this study is to assess the impact of EBUS on the concordance of clinical and pathological NSCLC staging in our center. Methods Data was collected retrospectively from the hospital database regarding patients who underwent surgical resection for early stage NSCLC between 2012 and 2017. Results A total of 251 patients were included. The mean age was 67 (±9), 55% (n=137) were male and 83% (n=209) were current/former smokers. In group A (n=154, 61%) clinical nodal stage (cN) was established from a combination of CT, PET CT and mediastinoscopy. Group B underwent additional EBUS (n=97, 39%). cN and pathological nodal staging (pN) were concordant in 78% (n=120) in group A versus 62% (n=60) in group B (p=0.009). Conclusion This study demonstrated higher rates of nodal discordance in patients who underwent EBUS which contrasts existing data that demonstrates improved concordance with EBUS. We describe these findings and potential explanations further in this study.

The BW has been largely used as a selection criterion in genetic selection programmes; however, increases in BW can affect animal metabolism and metabolites. The knowledge of how genetic potential for growth affects the metabolites can give a footprint of growth metabolism. This research aimed to evaluate the effect of genetic potential for post-weaning growth (GG) on performance, carcass traits and serum metabolome of non-castrated Nellore males during the finishing phase. Forty-eight Nellore non-castrated males, with divergent potential for post-weaning growth, were selected and divided into two groups: high potential for post-weaning growth (HG; n = 24) and low potential for post-weaning growth (LG; n = 24). Animals were kept and fed for 90 days where performance and ultrasound carcass traits were evaluated. Blood samples were collected at the beginning and end of feeding period to analyse serum metabolites concentration. The hot carcass weight and dressing percentage were recorded at slaughter. The feedlot performance and carcass traits were not affected by genetic potential. The HG animals had a lower glucose (P = 0.039), glutamate (P = 0.038), glutamine (P = 0.004), greater betaine (P = 0.039) and pyruvate (P = 0.039) compared to the LG group at the beginning of feedlot. In addition, higher creatine phosphate concentrations were observed at the beginning of feeding period, compared to final, for both groups (P = 0.039). In conclusion, the genetic potential for post-weaning growth does not affect performance and carcass traits during the finishing period. Differences in metabolite concentrations can be better found at the beginning of feedlot, providing a footprint of growth metabolism, but similar metabolite concentration at the end of finishing period.

In the Amazon and Orinoco basins, mercury has been released from artisanal and industrial gold mining since the Colonial time, as well as a result of deforestation and burning of primary forest, that release natural deposits of methyl mercury, affecting the local aquatic vertebrate fauna. This study reports the presence of mercury in river dolphins' genera Inia and Sotalia. Mercury concentrations were analysed in muscle tissue samples collected from 46 individuals at the Arauca and Orinoco Rivers (Colombia), the Amazon River (Colombia), a tributary of the Itenez River (Bolivia) and from the Tapajos River (Brazil). Ranges of total mercury (Hg) concentration in muscle tissue of the four different taxa sampled were: I. geoffrensis humboldtiana 0.003-3.99 mg kg-1 ww (n = 21, Me = 0.4), I. g. geoffrensis 0.1-2.6 mg kg-1 ww (n = 15, Me = 0.55), I. boliviensis 0.03-0.4 mg kg-1 ww (n = 8, Me = 0.1) and S. fluviatilis 0.1-0.87 mg kg-1 ww (n = 2, Me = 0.5). The highest Hg concentration in our study was obtained at the Orinoco basin, recorded from a juvenile male of I. g. humboldtiana (3.99 mg kg-1 ww). At the Amazon basin, higher concentrations of mercury were recorded in the Tapajos River (Brazil) from an adult male of I. g. geoffrensis (2.6 mg kg-1 ww) and the Amazon River from an adult female of S. fluviatilis (0.87 mg kg-1 ww). Our data support the presence of total Hg in river dolphins distributed across the evaluated basins, evidencing the role of these cetaceans as sentinel species and bioindicators of the presence of this heavy metal in natural aquatic environments.

Introduction and objectives: Omalizumab is present in international guidelines for the control of severe asthma, but data on the long-term effects in children are limited. Our objective was to perform a 'eal-life' long-term trial of omalizumab in children with allergic asthma. Materials and methods: An observational single center 'real-life' study was performed. Data for treatment, lung function, side effect, asthma exacerbations and hospitalizations were recorded at six months and annually. Results: Forty-eight patients <18 years of age were enrolled. Median treatment period was 2.9 (0.5-6). Fluticasone dose for the maintenance treatment decreases significantly at six months (452mcg/day to 329.89 mcg/day, respectively). This difference was maintained throughout the follow-up. Nobody used oral corticosteroid after six months. The rate of hospital admissions and visits to the emergency department for asthma exacerbations decreased significantly in the third years and fourth years follow-up, respectively. There was an improvement in lung function. Mean values of FEV1 and FEF25-75% before treatment were 79.88 and 62.94, respectively; after six months of treatment a statistically significant change was seen with a mean FEV1 of 92.29 and FEF25-75% of 76.31 (p = 0.0001). Lung function values were above normal throughout the six years of treatment. No side effects were reported. Conclusions: Overall in 'real life' omalizumab in children reduces asthma exacerbations and hospitalizations, improves lung function, and decreases the maintenance therapy. It is shown to be safe for up to six years of treatment in children

INTRODUCTION AND OBJECTIVES: Omalizumab is present in international guidelines for the control of severe asthma, but data on the long-term effects in children are limited. In this paper we studied the reversal magnetization of La1-x Sr x Fe0.5Cr0.5O3-δ (x = 0, 0.1 and 0.2) samples produced by combustion synthesis. The structural analysis was carried out by x-ray diffraction with Rietveld analysis. These analyses revealed that all samples have an orthorhombic structure with space group Pbnm (62) and that the Sr-doping induces a decrease of the lattice parameter. The x-ray photoelectron spectroscopy analysis indicates that the Sr-doping favor the change of the valence states of the Fe3+ to Fe4+. The magnetization as a function of the temperature reveals an unusual magnetic behavior with a reversal of magnetization. The increase of the Sr content induces a decrease of the temperature where occurs an inversion of the magnetization and do the value of the magnetization at 5 K more negative. This effect is attributed to the increase of the concentration of Fe4+ with increasing of the Sr content. The Fe and Cr with a valence of 4+ act as paramagnetic impurities in the antiferromagnetic lattice and are responsible for the changes in the magnetic behavior.

The clinical recognition of CSF fistula is a clinical challenge. We report the case of a young woman, who presented with a late orthostatic headache 20 months after epidural anesthesia. She developed a lumbar dural fistula of CSF confirmed in myelography CT scanning and treated successfully with epidural blood patch.

Four floor pen studies were carried out to evaluate the effects of nicarbazin (NIC) administration on blood glucose concentrations and the onset of hypoglycemia in broiler chickens. All tests involved continuous NIC feeding at 0, 100, or 125 ppm to 28 days of age. In each study, birds were reared at both standard environmental temperatures and at 3 C below this level. In addition, two studies were conducted in the presence of coccidial infection and two were carried out in noninfected broilers. At 26 days of age in each test, two birds per pen were bled by puncture of the brachial vein, and whole blood glucose concentrations were determined. Results indicated that the administration of NIC to broilers for 26 days had no effect on blood glucose concentrations, although graded levels of NIC tended to increase these values. In addition, no evidence of hypoglycemia was recorded in any of the trials. In a similar fashion, blood glucose was unaffected by environmental temperature and coccidial challenge. These findings support previous work showing that NIC administration does not influence blood glucose levels and indicate that the product is not involved when field diagnoses use reduced blood glucose and hypoglycemia as indicators of production anomalies.
