RESUMEN

Venous Ulcers (VU) are a serious health problem that affect the Quality of Life (QoL). They are evaluated by many different scales in the literature. We aimed to analyze the correlation between the Medical Outcomes Short-Form Health QoL (SF-36) and Charing Cross Venous Ulcer Questionnaire (CCVUQ) scales. This is a cross-sectional study conducted in a Brazilian center specializing in chronic VU of the Primary Health Care (PHC) provided to patients with active VU. The general QoL instrument SF-36 and the CCVUQ, specific for people with VU, were used. Spearman's Rho Test determined the correlation between the variables analyzed. Our sample had a total of 150 patients. We found a direct correlation between the domestic activities division (CCVUQ) aspect and the SF-36 Physical role functioning (strong), and Physical functioning (moderate) domains. The Social interaction division (CCVUQ) aspect presented moderate correlation with the domains of the SF-36 Physical role functioning and Physical functioning. The Vitality domain (SF-36) showed moderate correlation with the aspects of CCVUQ Cosmesis division and Emotional status division. The greatest forces of direct correlation were observed between the physical, functional and vitality aspects of SF-36 with those represented by domestic activities and social interaction in the CCVUQ.

This study is aimed at describing a protocol for a systematic review and meta-analysis to assess the effect of nontechnical skills training on the acquisition of knowledge, skills or attitudes, and changes in behavior at the workplace, of healthcare professionals working in intensive care units (ICUs), as well as the effect on outcomes at an organizational level. We will search for original studies in the PubMed/Medline, Scopus, Web of Science, Science Direct, EMBASE and PsycINFO databases. Studies with a clinical trial or quasi-experimental design will be included. Two reviewers will independently screen and assess the included studies, with any disagreements being resolved by a third reviewer. We will summarize the findings using a narrative approach and, if possible, conduct a quantitative synthesis (meta-analysis). We will conduct the protocol following the Preferred Reporting Items for Systematic Review and Meta-Analyses Protocols (PRISMA-P) guidelines. The review will summarize the current evidence on nontechnical skills training in ICUs, examining satisfaction with the training program, improvements in knowledge about nontechnical skills and the adoption of safety behaviors, as well as improvement in outcomes for the organization, such as mortality rates, length of stay and cost indicators. We expect that the systematic review could indicate effective strategies for training ICU professionals in nontechnical skills and also determine whether these strategies really improve the safety culture and professional knowledge and behaviors, as well as patient outcomes and safety.

Disruptive behavior in the healthcare context has an impact on patient care, healthcare personnel, and the health organization, and it also influences the therapeutic relationship, communication process, and adverse events. However, there is a lack of instruments that could be used for its analysis in the hospital care environment in the Spanish context. The objective of the study was to culturally adapt and perform a content validation of the tool "Nurse−Physician Relationship Survey: Impact of Disruptive Behavior on Patient Care", to the Spanish content (Spain). An instrumental study was conducted, which included an analysis of conceptual and semantic equivalence. A panel of experts analyzed the translations, by analyzing the Content Validity Index (CVI) of the group of items in the scale through the Relevance Index (RI) and the Pertinence Index (PI). Only a single item obtained an RI value of 0.72, although with PI value of 0.81, with consensus reached for not deleting this item. The CVI of all the items was >0.80 for the mean value of the RI, as well as the PI. The instrument was adapted to the Spanish context and is adequate for evaluating the disruptive behaviors on nurse−physician relationships and its impact on patient care. However, the importance of continuing the analysis of the rest of the psychometric properties in future studies is underlined.

The objective of the present study was to determine the subjective impact of the pandemic due to COVID-19 on communication, as perceived by nurses working at emergency services and Intensive Care Units at various hospitals in the Region of Murcia (Spain). A qualitative study was conducted based on the content analysis of 12 semi-structured individual interviews. The participant recruitment process was performed through a snowball sampling technique. Four main dimensions, eleven categories, and two sub-categories were obtained: (1) communication (communicative expressions, both verbal and non-verbal-, and limitations); (2) emotional aspects (positive, negative); (3) overload (first wave, second wave, and third wave); and (4) relationships (health professionals-patients, healthcare professionals, patients-family, and family-health professionals). The main findings of the study show that communication was slightly affected during the pandemic, especially the non-verbal kind, with verbal communication maintained and, in some occasions, strengthened. The lack of training in communication skills and its influence on the management of difficult periods was another important finding. Communication in general deteriorated during the pandemic, especially during the initial waves. Non-verbal communication was more affected due to the use of Personal Protective Equipment and the initial fear of infection, with this finding strongly observed in departments such as emergencies or critical care. The nurses who were interviewed underlined negative emotional aspects associated with a deficit in communication. The positive aspects described were associated with the creation of mutual support spaces and the group cohesion of the work teams during the pandemic. As an implication for current and future clinical practice, we recommend a coordinated institutional response to mitigate the potential emotional effects on workers by designing appropriate communication and emotional expression protocols.

Disruptive behavior creates a dysfunctional culture that has a negative impact on work relations and influences the quality of care and safety of the patient. The objective of the present work is to provide the best methodological quality scientific evidence available on disruptive behavior at a hospital, the aspect associated with the safety of the patient, and its impact on quality of care. For this, we included studies that addressed the prevalence of disruptive behaviors observed in the area of hospital health and its professionals. The selection, eligibility, data extraction and evaluation of the risk of bias stages were conducted by two researchers, and any discrepancies were solved by a third researcher. The data presented show that disruptive behaviors are frequently observed in the daily life of health professionals, and compromise the quality of care, the safety of the patient, and can lead to adverse effects. The results presented indicate that the appearance of disruptive behaviors compromises the quality of care, the safety of the patient, and the appearance of adverse effects, and can also affect the physical and mental health of the health professionals. El término seguridad viene siendo cada vez más discutido en el ámbito sanitario. Estando la Enfermería implicada en este proceso como promotora directa de acciones de seguridad por medio de sus prácticas de cuidado. Este trabajo tiene como objetivo investigar cómo el término seguridad viene siendo abordado en el escenario del cuidado de enfermería en el ámbito nacional e internacional. Investigación bibliográfica basada en artículos científicos publicados en revistas nacionales e internacionales indexadas en bases de datos Scientific Library (Scielo), Medical Literature Analysis and Retrieval System Online (MEDLINE) y SCOPUS, y publicadas en el período de 2007 a 2012. La recogida de datos ocurrió en el período de octubre de 2012. En total, fueron seleccionados 24 artículos, publicados en la lengua portuguesa, inglesa y española. Los fallos relatados con comprometimiento de seguridad del paciente fueron: en la comunicación oral o escrita entre profesionales del equipo y entre profesionales y paciente; terapéutica medicamentosa; realización de procedimientos de salud; y factores humanos relacionados al trabajo, como cansancio, falta de motivación, sobrecarga y/o (in)satisfacción. Como promotores de seguridad están: cuidado centrado en el paciente/familia, comunicación y diálogo, comunicación abierta sobre error; promoción de políticas de prevención, medición y evaluación de los errores y estandarización de las prácticas. La enfermería desempeña un papel crucial en la promoción de la seguridad del paciente por presentarse de forma más constante y directa en el cuidado al paciente. En contrapartida, si dicho cuidado no es realizado con calidad, puede ser generador de errores y comprometer la seguridad del cuidado (AU)


O termo segurança vem sendo cada vez mais discutido no âmbito sanitario. Estando a enfermagem implicada nesse processo como promotora direta de ações de segurança por meio de suas práticas de cuidado. Este trabalho objetivou-se investigar como o termo segurança vem sendo abordado no cenário do cuidado de enfermagem no âmbito nacional e internacional. Trata-se de uma revisão integrativa baseada em artigos científicos publicados em periódicos nacionais e internacionais indexados nas bases de dados Scientific Library (Scielo), Medical Literature Analysis and Retrieval System Online (MEDLINE) e SCOPUS e publicados no período de 2007 a 2012. A coleta de dados ocorreu no período de outubro de 2012. No total, foram selecionados 24 artigos, publicados na língua portuguesa, inglesa e espanhola. As falhas relatadas com comprometimento da segurança do paciente, foram: na comunicação oral ou escrita entre profissionais da equipe e entre profissional e paciente; terapêutica medicamentosa; realização de procedimentos de saúde; e fatores humanos relacionados ao trabalho, como cansaço, falta de motivação, sobrecarga e/ou (in)satisfação. Como promotores de segurança estão: cuidado centrado no paciente/família, comunicação e diálogo, comunicação aberta sobre o erro; promoção de gestão de pessoas; melhoria do sistema de prestação da assistência sanitária; promoção de políticas de prevenção, medição e avaliação dos erros e uniformização das práticas. A enfermagem desempenha um papel crucial na promoção da segurança do paciente por apresentar-se de forma mais constate e direta no cuidado ao paciente. The term safety is being increasingly discussed in the sanitary fields. Nursery is involved in this process as a direct promoter of security actions through their care practices. This study aimed to investigate how the term safety has been addressed in the national and international scenario of nursing care. Literature search based on scientific papers published in national and international journals indexed in databases Scientific Library (SciELO), Medical Literature Analysis and Retrieval System Online (MEDLINE) and SCOPUS published from 2007 to 2012. Data collection occurred in October 2012. In total, we selected 24 articles published in Portuguese, English and Spanish. The reported failures with compromised patient safety, were on oral or written communication between health professionals and between professionals and patients; drug therapy; procedures of health, and human factors related to work, such as fatigue, lack of motivation, overload and / or (dis) satisfaction. As promoters of safety are: patient-centered care / family communication and dialogue, open communication about the error; promotion of personnel management; improvement of the delivery system of health care, promoting prevention policies, measurement and evaluation of errors and standardization of practices Nursing plays a crucial role in promoting patient safety by presenting up and finding more direct patient care. However, if this is not done carefully, it may generate errors and compromise the safety of care (AU)
