RESUMEN

La creciente resistencia antimicrobiana asociada a la crisis en la producción de nuevos antibióticos y las consecuen-cias humanas y económicas de este fenómeno constituyen un complejo escenario que requiere el urgente desarrollo de estrategias antimicrobianas alternativas. Los bacterió-fagos son virus que infectan y lisan bacterias. Si bien se conocen desde hace más de un siglo, en las últimas dos décadas la administración de bacteriófagos ha ganado popularidad en todo el mundo. Existe un extenso cuerpo de evidencia preclínica y clínica que posiciona a la fago-terapia como una de las principales herramientas para el tratamiento de infecciones difíciles de tratar. Aunque esto es conceptualmente promisorio, su implementación está limitada por la escasez de datos clínicos de seguridad y efi-cacia, obtenidos acorde a los estándares científicos actua-les. Esta revisión describe los datos más relevantes acerca de la biología de los fagos, los aspectos farmacocinéticos y farmacodinámicos conocidos hasta la actualidad, los te-mas regulatorios y los resultados clínicos más relevantes


The rising antimicrobial resistance associated with the crisis in new antibiotics production and the human and economic consequences of this phenomenon constitute a complex scenario that requires the urgent development of alternative antimicrobial strategies. Bacteriophages are viruses that infect and lyse bacteria. They have been known for over a century but in the last two decades, phage administration has gained popularity worldwide. There is an extensive body of preclinical and clinical evidence that positions phage therapy as one of the main tools for the treatment of difficult-to-treat infections. Although this is conceptually promising, its implementation is limited by the paucity of clinical data on safety and efficacy, obtained according to current scientific standards. This review describes the most relevant data on phage biology, pharmacokinetic and pharmacodynamic aspects known to date, regulatory issues, and the most relevant clinical results

Interaction between severe acute respiratory coronavirus 2 (SARS-CoV-2) and IIEB remains under investigation. Objective: to compare IIEB incidence before and during COVID-19 pandemic, and assess incidence of coinfection with COVID-19 and case fatality. A cross-sectional study was performed on data from a centralized microbiology laboratory serving a network of healthcare centers comprising 713 pediatric and adult inpatient beds, expanded by 20% during the pandemic. Three periods were evaluated: (1) pre-pandemic: March 1, 2019-February 29, 2020; (2) pandemic year 1: March 1, 2020-February 28, 2021; (3) pandemic year 2: March 1, 2021-July 31, 2021. Descriptive statistical analysis was performed. 56 502 samples (96% blood cultures) from 27224 patients were analyzed. Of these, 54 samples (from 54 patients) were positive for encapsulated bacteria. IIEB incidence was: 167.4, 32.6, and 50.4 per 100000 samples for periods 1, 2, and 3, respectively. Twelve IIEB episodes occurred during the pandemic period: 10 Streptococcus pneumoniae, and 2 Haemophilus influenzae, of which 7 were SARS-CoV-2/S. pneumoniae coinfections, with an incidence of 5.68 per 10000 COVID-19-related hospitalizations (0.056%). IIEB case fatality was 31%, 29%, and 60% for each period, respectively, 3/7 patients with coinfection died (43%). Case fatality for invasive pneumococcal disease (IPD) in patients without COVID-19, was 32.5%. Significant reduction in IIEB incidence was observed during the pandemic, coinciding with implementation of containment measures. The incidence of SARS-CoV-2/S. pneumoniae coinfection was low, with higher case fatality than IPD patients without COVID-19.

Objetivo principal: Determinar la adherencia al lavado de manos en el personal de salud en los periodos pre-pandemia y pandemia SARS-CoV-2. Objetivos secundarios: Determinar la adherencia según categoría profesional, sectores de internación y momentos del lavado de manos. Determinar el grado de cumplimiento según una escala de referencia. Metodología: Estudio cuasi experimental que evaluó la adherencia al lavado de manos antes y después de intensificar medidas de prevención. Se comparó la adherencia de dos períodos, diciembre 2019 y agosto 2020. Resultados: Se observó que la adherencia en 2019 fue 67% (IC95%: 63%-71%) y en 2020 de 89% (IC95%: 86%-92%), lo que implica un aumento de 22% (IC95%: 0.17-0.27; p:0.000). En el análisis de subgrupos de momentos de la OMS se observó un aumento en los momentos agrupados como antes (65,7 a 81,2; p:0.001) y como después (67,7 a 94,3; p:0.000). En el análisis por sectores de internación se observó un aumento en los sectores de guardia (50,5 a 100; p:0.000) e internación (63,3 a 78,0; p:0.006) y en pediatría (94,2 a 96,5; p:0.336) y UTI/UCO (63,7 a 70,5; p:0.405), en estos dos últimos no significativo. En el análisis por categoría profesional se observó un aumento en las categorías enfermero (81,4 a 92,9; p:0.001), médicos (66,7 a 83,8; p:0.003), técnicos (57,8 a 93,2; p:0.000) y apoyo (49,4 a 83,9; p:0.000). Discusión: En nuestro estudio, la intensificación de las medidas de lavado de manos y la pandemia motivaron un incremento significativo en la adherencia a la higiene de manos


Main objective. To determine the adherence to hand washing in health care staff in pre-pandemic and SARS-CoV-2 pandemic periods, after intensifying the measures of a program. Secondary objectives. Determine adherence according to professional category, hospitalization sectors and moments of hand washing. Methodology. Quasi-experimental study that evaluated adherence to hand washing before and after intensifying measures. The adherence of 2 periods was compared, December 2019 and August 2020. Outcomes. It was observed that adherence in 2019 was 67% (95% IC: 63% -71%) and in 2020 it was 89% (95% IC: 86% -92%), which implies an increase of 22% (95% IC: 0.17-0.27; p: 0.000). In the subgroup analysis of WHO moments, an increase was observed in the moments grouped as before (65.7 to 81.2; p: 0.001) and as after (67.7 to 94.3; p: 0.000). In the analysis by hospitalization sectors, an increase was observed in emergency sectors (50.5 to 100; p: 0.000) and hospitalization (63.3 to 78.0; p: 0.006) and not significant in pediatrics (94.2 to 96.5; p : 0.336) and UTI / UCO (63.7 to 70.5; p: 0.405). In the analysis by professional category, a significant increase was observed in the categories nurse (81.4 to 92.9; p: 0.001), doctors (66.7 to 83.8; p: 0.003), technicians (57.8 to 93.2; p: 0.000) and support (49.4 to 83.9; p: 0.000). Conclusion. In our study, the intensification of hand washing measures and the pandemic led to a significant increase in adherence to hand hygien

ntroducción: La información sobre la evolución de la infección por COVID-19 en personas gestantes (PG) continúa en desarrollo.Objetivos: Describir la presentación de la infección por Sars-CoV-2 en PG y determinar variables asociadas a mayor gravedad.Materiales y métodos: Estudio observacional retrospectivo. Periodo: 01/03/2020-31/07/2021. Se incluyeron PG con diagnóstico de COVID-19 asistidas en una maternidad de gestión pública: se clasificaron según gravedad y se dividieron en dos grupos: Grup o1 leve y de manejo ambulatorio; Grupo 2 moderado, severo y crítico, con internación. Se analizó la relación entre gravedad y obesidad, DBT, hipertensión inducida por el embarazo (HIE), edad gestacional, edad materna, vacunación antigripal. Recién nacidos (RN) de madres infectadas se estudiaron con PCR para Sars-CoV-2 24-48 hs postnacimiento. Análisis estadístico: Chi-cuadrado o test exacto de Fisher. Significancia= p<0,05. Aprobado porComité Ética Institucional.Resultados: 52 PG con diagnóstico de COVID-19. Edadmediana 29,6 años. Grupo 1: 29 PG (55,5%). Grupo 2: 23 PG(44%), 19 (36,5%) moderados, 2 (4%) severos y 2 (4%) críticos. No hubo fallecimientos maternos ni fetales. Edad gestacional ≥ 28 semanas fue la única variable asociada a mayor gravedad,p=0,00004. 48% de los embarazos finalizaron por cesárea.48/52 RN fueron estudiados con PCR para Sars-CoV-2, siendo 1 (2%) positivo (fue el único RN sintomático).Conclusiones: La infección por COVID-19 en PG se asoció a presentaciones clínicas más graves cuando la infecciónse cursó en el tercer trimestre de gestación y se asociócon mayor incidencia de cesáreas


ntroduction: The information concerning the impact of COVID-19 infection in pregnant people (PP) continues to be established.Aim: to describe the evolution of the Sars-CoV-2 infection in pregnant people and to determine variables associated with clinical severity.Materials and Methods: Retrospective observational study. Period: 01-03-2020 to 31-07-2021. We included PP with diagnosis of COVID-19, assisted in a public maternity hospital. The cases were classified according to clinical severity based on the NIH guidelines. The patients were divided into 2 groups: Group 1: mild (ambulatory manage-ment). Group 2: moderate, severe and critical (requiring hospitalization). The relationship between variables and clinical severity was analyzed. Variables studied: obesity, DBT, gestational hypertension, gestational age, maternal age, influenza vaccination. Newborns of infected mothers were studied with PCR for Sars-CoV-2 24 to 48 hours af-ter birth. Statistical analysis: Chi-square or Fisher's exact test, significance = p <0.05. Study approved by the Institu-tional Ethics Committee.Results: 52 PP with diagnosis of COVID-19 were includ-ed. Median age 29.6 years. 23 patients (44%) required hospitalization and 2 (4%) MRA (mechanical respiratory assistance). 29 (55.5%) were mild, 19 (36.5%) moderate, 2 (4%) severe, and 2 (4%) critical. There were no mater-nal or fetal deaths. Gestational age ≥ 28 weeks was the only variable associated with more severe clinical forms, p = 0.0001. 48% of the pregnancies ended by cesarean section. 48/52 newborns were studied with PCR for Sars-CoV-2, with only 1 (2%) being positive. This was the only symptomatic newborn.Conclusions: In our study, Sars-CoV-2 infection during pregnancy was associated with more severe clinical pre-sentations when the infection occurred in the 3rd trimes-ter of pregnancy. Introducción: La neumonía en adultos mayores institucionalizados en geriátricos presenta elevada incidencia y mortalidad. Conocer sus características permite optimizar conductas terapéuticas. Objetivo: Comparar severidad, etiología, regímenes antibióticos utilizados y evolución de una cohorte de pacientes hospitalizados por neumonía provenientes de geriátrico, con un grupo de pacientes de igual edad no residentes en geriátricos. Material y métodos: Estudio multicéntrico, retrospectivo, caso-control, sobre la base de datos internacional CAPO. Se incluyeron 534 pacientes internados en hospitales de Argentina por neumonía, periodo 6/2001-12/2007; 30 pacientes provinieron de geriátrico (casos); por cada caso se seleccionaron 3 controles de edad similar. Análisis estadístico: ?2 y F-ANOVA. Se aplicó análisis univariado y multivariado. Resultados: Edad media 85,7 (DS 6.46) y 84.8 (DS 5.8) años para casos y controles respectivamente, pNS. Los casos presentaron categorías de PSI significativamente mayores (p < 0.0001). La etiología se determinó en 17% de casos y 27% de controles. Los antibióticos utilizados fueron similares en ambos grupos (p = 0.7). No hubo diferencias significativas en tiempo a la estabilidad clínica ni duración de estadía hospitalaria. La mortalidad global en los casos fue 50% vs 26% en controles; (p = 0.02) y la atribuible 43.3% vs 15.5%; (p = 0.002). Análisis multivariado: Accidente cerebro vascular (ACV) (p = 0.05), estado mental alterado (EMA) (p = 0.01) e hipotensión arterial (p = 0.04) fueron las variables significativamente asociadas a mortalidad. Conclusiones: los pacientes provenientes de geriátrico presentaron neumonías más severas y mayor mortalidad. Esta se asoció con la presencia de ACV previo, EMA e hipotensión arterial al ingreso, y no con la condición de provenir de geriátrico.(AU)

Defining its characteristics will contribute to optimize therapeutic measures. Objective: To compare severity, etiology, antibiotic regimens used and outcome of a cohort of nursing home residents with CAP with a group of age-matched patients who are not resident at a nursing home. Methods: Multicenter, retrospective, case-control study. Information was taken from the international database CAPO. We included 534 patients hospitalized for CAP in Argentina, from June 2001 to December 2007. Among them, 30 cases were residents in nursing homes (cases). For each case, three controls of similar age were selected. Statistical analysis: ?2 and F-ANOVA. Univariate and multivariate analyses were performed. Results: Mean age was 85.7 (SD 6.46) and 84.8 (SD 5.8) years for cases and controls respectively, p NS. Cases had significantly higher PSI scores (p < 0.0001). The etiology was determined in 17% of cases and 27% of controls. The antibiotics used were similar in both groups (p = 0.7). There was no significant difference in time to reach clinical stability or in length of hospital stay. Overall mortality was 50% in cases vs 26% in controls, (p = 0.02) and attributable mortality was 43.3% and 15.5% respectively (p = 0.002). In the multivariate analysis, stroke (p = 0.05), confusion (p = 0.01) and hypotension (p = 0.04) were associated significantly with mortality. Conclusions: nursing home residents presented more frequently severe pneumonia with increased mortality. This incidence was associated with the presence of previous stroke, confusion and hypotension on admission; it was not associated with residence in a nursing home.(AU)
