RESUMEN

Our study examines the effect of comorbidities on rotator cuff repair (RCR) postoperative costs and complication rates. A retrospective review of patients receiving RCRs from 2008-2016 using the PearlDiver database was performed. We evaluated the effect of major comorbidities on postoperative costs and complications within one year of surgery. In total, 8,747 patients underwent RCR. Patients with no comorbidities had a 6-month and 12-month postoperative cost of $3,534 and $3,853, respectively. Patients with one comorbidity had 6-month postoperative costs ranging from $2,623 to $3,466 and 1-year postoperative costs ranging from $2,992 to $5,906. Patients with 3+ comorbidities receiving arthroscopic RCR had the highest complication rates at 1-, 3- and 6-month intervals (11.8%, 19.7% and 26.8%, respectively) compared to those with no comorbidities (8.1% [p = 0.139], 12.7% [p = 0.022] and 15.9% [p = 0.001], respectively). Patients with isolated comorbidities likely require similar healthcare utilization to those without, but patients with 3+ comorbidities risk greater complications and higher post-operative costs. Increasing acetate and ß-hydroxybutyrate (BHB) supply to lactating cows will increase milk fat synthesis. However, the underlying molecular mechanism remains largely unknown. Cell death-inducing DNA fragmentation factor-α-like effector C (CIDEC) is a lipid droplet-associated protein that promotes intracellular triacylglycerol accumulation. In the present study, using gene overexpression and knockdown, we detected the contributions of CIDEC on milk fat synthesis in mammary epithelial cells of dairy cows in the presence of acetate and BHB. The results showed that knockdown of CIDEC decreased fatty acid synthase (FASN) expression and intracellular triacylglycerol content, whereas overexpression of CIDEC had the opposite effect. The transcription factor CCAAT/enhancer-binding protein ß (C/EBPß) regulates cell growth and differentiation in the mammary gland. We demonstrated that the FASN promoter had a canonical C/EBPß binding sequence. CEBPB overexpression upregulated FASN expression and milk fat synthesis, whereas CEBPB knockdown had the opposite effect. Moreover, knockdown of CEBPB attenuated the promoting effects of CIDEC on acetate- and BHB-induced FASN transcription. Taken together, our data showed that acetate and BHB induced FASN expression in mammary epithelial cells of dairy cows in a CIDEC-C/EBPß-dependent manner, which provides new insights into the understanding of the molecular events involved in milk fat synthesis.

BACKGROUND: Previous studies have reported that lymphocyte-to-monocyte ratio (LMR) had novel prognostic value in hepatocellular carcinoma (HCC). The purpose of this meta-analysis was to synthetically evaluate the prognostic role of preoperative LMR in HCC patients following curative resection. METHODS: Eligible studies were acquired through searching Pubmed, Web of Science, Cochrane Library and EMbase update to September 2019. Merged hazard ratios (HRs) and 95% confidence intervals (CIs) were applied as effect sizes. RESULTS: A total of ten studies containing 4,092 patients following liver resection were enrolled in this meta-analysis. The pooled results demonstrated that preoperative elevated LMR indicated superior survival outcome (HR: 0.58, 95% CI: 0.34-0.96, P = .035) and recurrence-free survival (RFS)/disease-free survival/time to recurrence (HR = 0.76, 95% CI: 0.58-0.98, P = .034). The significant prognostic role of preoperative LMR was detected in the subgroup of all publication year, country of origin, sample sizes <300, TNM stage of I-IV and LMR cut-off value ≤4. Furthermore, high LMR was significantly associated with male, high AFP, large tumor size, incomplete tumor capsule, advanced TNM stage and BCLC stage, and presence of PVTT. CONCLUSION: Elevated preoperative LMR indicated superior survival outcome in HCC patients following curative resection, and might serve as a novel prognostic biomarker.

Constructing moderate surface roughness is a widely used, non-toxic, cost-effective, and outcome-predictable approach to accelerate implant osteointegration in clinical settings. MicroRNAs (miRNAs) play vital regulatory roles in the osteogenic differentiation of bone marrow stem cells (BMSCs). However, their specific contribution to the influence of surface roughness on osteoblastic behavior remains unknown. Therefore, applying the smooth titanium surface as a control, a typical titanium surface with moderate roughness was prepared here to reveal the mechanism through which surface roughness regulates cell osteogenic behavior by altering miRNA expression. First, the morphology and roughness of two surfaces were characterized, and the enhanced osteogenic differentiation of BMSCs on rough surfaces was verified. Then, twenty-nine differentially expressed miRNAs in BMSCs cultured on different surfaces were selected via miRNA chip and corresponding functional prediction. After verifying the expression of these miRNAs using quantitative real-time polymerase chain reaction, four were considered eligible candidates. Among these, only miR-181d-5p significantly affected RUNX2 gene expression based on overexpression and knockdown experiments. From the osteogenesis-related gene and protein expression, as well as alkaline phosphatase and alizarin red experiments, we further confirmed that the downregulation of miR-181d-5p promoted osteogenic differentiation of BMSCs, and vice versa. In addition, rescue assays showed that the knockdown of miR-181d-5p improved the inferior osteogenesis observed on smooth surfaces, whereas the overexpression of miR-181d-5p suppressed the superior osteogenesis observed on rough surfaces. These results indicate that the moderate surface roughness of the implant stimulates the osteogenic differentiation of BMSCs by remarkably downregulating miR-181d-5p. These findings provide helpful information and a theoretical basis for the development of advanced implant materials for fast osteointegration.

Recently, the prevalence trend of pulmonary fungal infection (PFI) has rapidly increased. Changes in the risk factors for, distributions of underlying diseases associated with and clinical characteristics of some individual PFIs have been reported in the past decade. However, data regarding PFIs remain uncertain. This study reports the epidemiological characteristics and trends of PFIs over time in recent years. We applied an automated natural language processing (NLP) system to extract clinically relevant information from the electronic health records (EHRs) of PFI patients at the First Affiliated Hospital of Guangzhou Medical University. Then, a trend analysis was performed. From January 1, 2013, to December 31, 2019, 40,504 inpatients and 219,414 outpatients with respiratory diseases were screened, in which 1368 inpatients and 1313 outpatients with PFI were identified. These patients were from throughout the country, but most patients were from southern China. Upward trends in PFIs were observed in both hospitalized patients and outpatients (P<0.05). The stratification by age showed that the incidence of hospitalized patients aged 14-30 years exhibited the most obvious upward trend, increasing from 9.5 per 1000 patients in 2013 to 88.3 per 1000 patients in 2019. Aspergillosis (56.69%) was the most common PFI, but notably, the incidence rates of Talaromyces marneffei, which used to be considered uncommon, exhibited the most rapid increases. In younger PFI patients, the incidence and trend of PFIs have increased. Infection by previously uncommon pathogens has also gradually increased. Increased attention should be paid to young PFI patients and uncommon PFI pathogen infections.

STATEMENT OF PROBLEM: Intraoral scanning has benefits over conventional impression making, but whether scanning is sufficiently accurate for multiple implants is unclear. PURPOSE: The purpose of this in vitro study was to compare the trueness of digital scans acquired by using intraoral scanners from a small range to a complete arch with the conventional impression technique and to determine the influence of 2 different evaluation methods (best-fit algorithm versus absolute linear deviation) on the outcomes of accuracy assessment. MATERIAL AND METHODS: A mandibular model with 8 implants (A-H) around an edentulous arch was used as the master model. Open-format standard tessellation language (STL) data sets (1 reference file from a highly accurate dental laboratory scanner, 10 files from an intraoral scanner, and 10 files from digitized conventional impressions at room temperature) were imported to a metrology software program, and 5 groups of scanning ranges (AB, FGH, CDEF, BCDEFG, and ABCDEFGH) were identified simulating different clinical situations. Two evaluation methods-root mean square values calculated from the best-fit algorithm and average value of linear discrepancies from absolute linear deviation-were used to describe the trueness values. The impacts of different scanning or impression methods, ranges, and evaluation methods were tested by using a 3-way ANOVA. The effect of the scanning range on accuracy was further identified with 1-way ANOVA. The paired-sample t test was used to determine the differences of trueness values between the 2 methods in different groups. RESULTS: The trueness values of the implant impressions were significantly affected by different scanning or impression methods (P<.001), evaluation methods (P<.001), and scanning ranges (P<.001) as independent variables. With use of the best-fit algorithm, deviations from the digital scans were significantly greater than those from the conventional impressions in cross-arch situations (groups CDEF, BCDEFG, and ABCDEFGH). With use of the absolute linear deviation method, statistically significant lower accuracy was found when larger areas were encountered (groups BCDEFG and ABCDEFGH). Use of the absolute linear deviation method resulted in a higher mean score of inaccuracy than that from the best-fit algorithm method in most situations. CONCLUSIONS: Scanning or impression methods, ranges, and evaluation methods affected the dimensional accuracy (trueness) of scans or impressions with multiple implants. Digital scans had worse trueness values compared with those made with the conventional splinting open-tray technique when cross-arch implant impressions were acquired.

The LBD (Lateral Organ Boundaries Domain) family are a new group of plant-specific genes, which encode a class of transcription factors containing conserved Lateral Organization Boundary (LOB) domains, and play an important role in regulating the adaxial-abaxial polarity of plant leaves. In Arabidopsis thaliana, ASYMMETRIC LEAVES 2 (AS2) has a typical LOB domain and is involved in determining the adaxial cell fate. In this study, we isolated the BcAS2 gene from the pak choi cultivar "NHCC001", and analyzed its expression pattern. The results showed that the BcAS2 encoded a protein made up of 202 amino acid residues which were located in the nucleus and cytomembrane. The Yeast two-hybrid system (Y2H) assay indicated that BcAS2 interacts with BcAS1-1 and BcAS1-2 (the homologous genes of AS1 gene in pak choi). In the transgenic Arabidopsis thaliana that overexpressed BcAS2 gene, it presented an abnormal phenotype with a curly shape. Taken together, our findings not only validate the function of BcAS2 in leaf development in Arabidopsis thaliana, but also contribute in unravelling the molecular regulatory mechanism of BcAS2, which fulfills a special role by forming complexes with BcAS1-1/2 in the establishment of the adaxial-abaxial polarity of the lateral organs in pak choi.

Circular RNAs (circRNAs) have confirmed to participate in diverse biological functions in cancer. However, the expression patterns of circRNAs on hepatocellular carcinoma (HCC) remains unclear. In the present study, we clarified that hsa_circRNA_104348 was dramatically upregulated in HCC tissues and cells. Patients with HCC displaying high hsa_circRNA_104348 level possessed poor prognosis. Has_circ_104348 facilitated proliferation, migration, and invasion, meanwhile suppressed apoptosis of HCC cell. Furthermore, hsa_circRNA_104348 directly targeted miR-187-3p, could regulate miR-187-3p to affect proliferation, migration, invasion, and apoptosis of HCC cells, and may have effect on Wnt/ß-catenin signaling pathway. Moreover, RTKN2 could be a direct target of miR-187-3p. In addition, knockdown of hsa_circRNA_104348 attenuated HCC tumorigenesis and lung metastasis in vivo. Taken together, these findings indicated that circular RNA hsa_circRNA_104348 might function as a competing endogenous RNA (ceRNA) to promotes HCC progression by targeting miR-187-3p/RTKN2 axis and activating Wnt/ß-catenin pathway.

BACKGROUND: The outcomes, therapeutic strategies and risk factors of congenital corrected transposition of great arteries (ccTGA) unoperated before adulthood are unclear. METHODS: From October 2009 to January 2018, 117 adult ccTGA patients, classified into ccTGA with intact ventricular septum, ventricular septum defect and pulmonary valve or subpulmonary outflow tract stenosis (PS) groups, were reviewed. Statistical analysis was performed with SPSS 19.0 RESULTS: At the first visit, no patients suffered operation. PS group had the least systemic atrioventricular valve regurgitation and the greatest systemic ventricular ejection fraction. All 49 patients underwent surgery. From the first visit to last follow-up time, systemic ventricular ejection fraction of unoperated ccTGA decreased significantly. In the intact ventricular septum group, patients receiving systemic atrioventricular valve replacement/valvuloplasty had a significantly increased systemic ventricular ejection fraction and statistically more freedom from death and transplant than unoperated. In ventricular septum defect group the late systemic ventricular ejection fraction of operated patients were not statistically different from their basic data at first visit. In PS group patients receiving physiologic repair had significantly decreased systemic ventricular ejection fractions. Severe systemic atrioventricular valve regurgitation, physiological repair and systemic ventricular dysfunction (ejection fraction <40%) were risk factors for mortality, transplant and congestive heart failure. CONCLUSIONS: PS protects against systemic atrioventricular valve regurgitation and ventricular dysfunction. Systemic atrioventricular valve replacement/valvuloplasty improved systemic ventricular function for ccTGA with an intact ventricular septum. Physiological repair was not ideal for ccTGA with PS. Severe systemic atrioventricular valve regurgitation and systemic ventricular dysfunction were associated with suboptimal outcomes.

BACKGROUND: Left main coronary arterial atresia is a rare coronary arterial anomaly with extremely limited data on the optimal management. We aimed to report our single surgeon experience of the ostioplasty in patients with left main coronary arterial atresia. METHODS: From Jul 2018 to Dec 2019, pediatric patients who presented with left main coronary arterial atresia and subsequently underwent surgical coronary ostioplasty were recruited into this retrospective study. Concomitant mitral repair was applied when the regurgitation was moderate or more. RESULTS: A total of 9 patients diagnosed as left coronary arterial atresia were included. Mitral regurgitation was found in all of them, including 6 (66.7%) severe and 1 (11.1%) moderate and 2 (22.2%) mild. In addition to ischemic lesions which was found in 7 (77.8%) patients, structural mitral problem was also common (presented in 7 (77.8%) patients). All the patients underwent coronary ostioplasty with autologous pulmonary arterial patch augmenting anterior wall of the neo-ostium. Mean Aortic cross clamp time and CPB time was 88.1 ± 18.9 and 124.6 ± 23.6 min, respectively. During a median of 10.9 (range, 3.3 - 17.2) months follow-up, there was only 1 death at 5 months after surgery. All survivors were recovered uneventfully with normal left ventricular function, however, with 4 (50.0%) significant recurrence of MR. CONCLUSIONS: With favorable surgical outcomes, coronary ostioplasty for LMCA atresia may be an option of revascularization. Structural mitral problems presented in majority patients, resulting in the requirement of concomitant mitral repair. However, the optimal technique of mitral repair remains unclear.

Myasthenia gravis (MG) is a prototypic organ-specific autoimmune disorder that, in most cases, is mainly mediated by antibodies against the acetylcholine receptor. Evidence implicates CD4+ T helper (Th) cells in the development of MG, whereas regulatory T cells (Tregs) are associated with disease resolution. Melatonin has important immunoregulatory effects in many T cell-mediated autoimmune diseases. However, there are few studies on the role of melatonin in MG. In the present study, we investigated serum melatonin levels and melatonin receptor expression in MG patients and healthy controls (HCs). We also evaluated the impact of melatonin administration on peripheral CD4+ Th cells and related cytokine production. Serum melatonin levels were lower in MG patients than in HCs, and MT1 expression was lower in PBMCs from MG patients than in those from HCs. Administration of melatonin significantly decreased Th1 and Th17 cell responses and proinflammatory cytokine production. Further investigation in vitro revealed that melatonin administration increased FoxP3 and IL-10 expression in CD4+ T cells from MG patients and enhanced the suppressive function of Tregs. These findings indicate that melatonin exerts immunoregulatory activity in MG by balancing effector and regulatory Th cell populations as well as by suppressing proinflammatory cytokine production.

Tibetyrus formicarius Yin Lin, gen. et sp. nov., a new myrmecophilous genus and species of the pselaphine tribe Tyrini, is described from southeastern Xizang, China. Three adults were collected in nests of a Formica ant species (Hymenoptera: Formicidae) at an altitude of 3,485 m. Tibetyrus is similar to Megatyrus Hlavác Nomura in sharing the basally pedunculate maxillary palpi that lack an apical cone on the fourth palpomeres. Nevertheless, it can be readily separated from the latter and all other genera of the tribe by its unique suite of morphological features as well as a highly recognizable habitus.

To overcome the deficiency of the volume expansion of MoS2 as the anode material for lithium-ion batteries (LIBs), an effective strategy was developed to design hierarchical porous MoS2/carbon nanospheres via a facile, easy-operated hydrothermal method followed by annealing. FESEM and TEM images clearly showed that nanospheres are composed of ultra-thin MoS2/C nanosheets coated with carbon layer and possess an expanded interlayer spacing of 0.98 nm. As anodes for LIBs, MoS2/carbon nanospheres deliver an initial discharge capacity of 1307.77 mAh g-1 at a current density of 0.1 A g-1. Moreover, a reversible capacity of 612 mAh g-1 was obtained even at 2 A g-1 and a capacity retention of 439 mAh g-1 after 500 cycles at 1 A g-1. The improved electrochemical performance is ascribed to the hierarchical porous structure as well as the intercalation of carbon into lattice spacing of MoS2, which offers fast channels for ion/electron transport, relieves the influence of volume change and increases electrical conductivity of electrode. Meanwhile, the expanded interlayer spacing of MoS2 in MoS2/C can decrease the ion diffusion resistance and alleviate the volumetric expansion during discharge/charge cycles.

BACKGROUND: Circular RNAs (circRNAs) have shown crucial regulatory roles in cancer biology. We aimed to uncover the role and underlying mechanism of circ_0091581 in hepatocellular carcinoma (HCC) progression. METHODS: The abundance of circ_0091581, microRNA-591 (miR-591) and FOS like 2, AP-1 transcription factor subunit (FOSL2) was measured by quantitative real-time polymerase chain reaction. Cell viability, colony formation ability, and invasion ability were assessed by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay, colony formation assay, and transwell invasion assay. The migration ability was analyzed by transwell migration assay and wound healing assay. Flow cytometry was used to evaluate the cell cycle and apoptosis of HCC cells. The interaction between miR-591 and circ_0091581 or FOSL2 was predicted by Circular RNA Interactome database or TargetScan database and confirmed by dual-luciferase reporter assay and RNA immune co-precipitation assay. FOSL2 protein expression was measured by Western blot assay. Xenograft tumor assay was conducted to analyze the role of circ_0091581 in HCC tumor growth in vivo. RESULTS: Circ_0091581 was highly expressed in HCC tissue samples and cell lines in contrast to that in adjacent normal tissue samples and THLE-2 cell line. Circ_0091581 accelerated the viability, colony formation, metastasis, and cell cycle, while it impeded the apoptosis of HCC cells. MiR-591 bound to circ_0091581, and circ_0091581 knockdown-mediated effects in HCC cells were largely overturned by miR-591 silencing. FOSL2 was a target of miR-591, and FOSL2 overexpression largely reversed miR-591 accumulation-induced influences in HCC cells. FOSL2 protein expression was down-regulated by circ_0091581 silencing, and the addition of miR-591 inhibitor partly recovered the expression of FOSL2 in HCC cells. Circ_0091581 interference notably suppressed HCC tumor growth in vivo. CONCLUSION: Circ_0091581 acted as an oncogene to enhance the viability, colony formation, metastasis and cell cycle and inhibit the apoptosis of HCC cells through targeting miR-591/FOSL2 axis.

BACKGROUND: Although small perforation of the maxillary sinus schneiderian membrane is a well-documented complication during lateral sinus floor elevation (LSFE), complete perforations larger than 10 mm often result in discontinuation of surgery. Reports on reentry LSFE and its long-term outcomes are sparse. PURPOSE: To evaluate the long-term outcomes of reentry LSFE following complete membrane perforation to elucidate the technical details of the reentry procedure. MATERIALS AND METHODS: We assessed the medical records of all patients receiving LSFE from 2008 to 2017 in the Department of Oral Implantology, Peking University Hospital of Stomatology. Twenty-two patients receiving reentry LSFE after complete membrane perforation were enrolled. Data were recorded using cone beam computer tomography: including the residual bone height, membrane thickness of the sinus prior to surgery (MT1 ), and before reentry (MT2 ), and height of the bone graft during the reentry procedure (HBG). Cumulative survival rate of implants (CSR), marginal bone loss (MBL), and subsequent complications were also recorded. RESULTS: From 2008 to 2017, 2023 consecutive patients (2262 sinuses) who underwent LSFE were screened. Complete membrane perforation occurred in 28 patients and resulted in discontinuation of surgery (1.2%). Twenty two patients were enrolled and received reentry LSFE within 3-6 months. Two patients undergoing the reentry procedure were suspended due to excessive membrane perforation, while the other 20 finished reentry sinus bone graft. In the reentry procedure, the HBG was 9.73 ± 2.67 mm with 34 implant placements. The MT1 and MT2 were 1.03 ± 0.43 and 1.91 ± 1.45 mm, respectively, showing a statistically significant difference (P < .05). After a follow-up of 2-10 years, CSR was 97.1%, and MBL was 0.64 ± 0.50 mm. CONCLUSIONS: The long-term outcome of reentry LSFE is predictable and reentry LSFE offers a reliable alternative following complete membrane perforation. However, the procedure is relatively sensitive and should be performed by experienced surgeons.

BACKGROUND: Event-related potentials (ERP) data are widely used in brain studies that measure brain responses to specific stimuli using electroencephalogram (EEG) with multiple electrodes. Previous ERP data analyses haven't accounted for the structured correlation among observations in ERP data from multiple electrodes, and therefore ignored the electrode-specific information and variation among the electrodes on the scalp. Our objective was to evaluate the impact of early adversity on brain connectivity by identifying risk factors and early-stage biomarkers associated with the ERP responses while properly accounting for structured correlation. METHODS: In this study, we extend a penalized generalized estimating equation (PGEE) method to accommodate structured correlation of ERPs that accounts for electrode-specific data and to enable group selection, such that grouped covariates can be evaluated together for their association with brain development in a birth cohort of urban-dwelling Bangladeshi children. The primary ERP responses of interest in our study are N290 amplitude and the difference in N290 amplitude. RESULTS: The selected early-stage biomarkers associated with the N290 responses are representatives of enteric inflammation (days of diarrhea, MIP1b, retinol binding protein (RBP), Zinc, myeloperoxidase (MPO), calprotectin, and neopterin), systemic inflammation (IL-5, IL-10, ferritin, C Reactive Protein (CRP)), socioeconomic status (household expenditure), maternal health (mother height) and sanitation (water treatment). CONCLUSIONS: Our proposed group penalized GEE estimator with structured correlation matrix can properly model the complex ERP data and simultaneously identify informative biomarkers associated with such brain connectivity. The selected early-stage biomarkers offer a potential explanation for the adversity of neurocognitive development in low-income countries and facilitate early identification of infants at risk, as well as potential pathways for intervention. In a cohort of infants, we found that lack of the Lewis histo-blood group antigen was associated with increased susceptibility to shigellosis. Broadly inhibiting fucosylation in epithelial cells in vitro decreased invasion by Shigella flexneri. These results support a role for fucosylated glycans in susceptibility to shigellosis.

Distinguishing periprosthetic crystalline arthropathy from periprosthetic joint infection (PJI) remains a diagnostic challenge as both symptom presentation and diagnostic tests overlap. Accurate differentiation is important as treatment plans vary significantly. We sought to systematically review all cases of total knee arthroplasty (TKA) periprosthetic crystalline arthropathy reported in the literature and summarize clinical, diagnostic, and operative findings in the context of guidelines for diagnosing PJI. The goal of this systematic review is to determine the amount of diagnostic overlap and to identify best practices for differentiating between these two diagnoses. MEDLINE and Google Scholar were searched to identify cases of crystalline arthropathy following TKA. Case reports were reviewed for patient characteristics, clinical symptoms, physical exam, laboratory results, and treatment outcomes. These findings were summarized across patients and dichotomized based on current thresholds for diagnosing PJI according to Musculoskeletal Infection Society criteria. Twenty-six articles were identified which included 42 cases of periprosthetic crystalline arthropathy (17 gout, 16 pseudogout, one both, and eight not specified). Of these cases, 25 presented over 1 year after their index arthroplasty and 15 had no prior history of crystalline arthropathy. Only six cases had a superimposed infection based on aspiration or intraoperative cultures. For cases without a culture-positive infection, several diagnostic tests overlap with PJI thresholds: 95% of patients had C-reactive protein greater than 1 mg/dL, 76% had an erythrocyte sedimentation rate greater than 30 mm/hour, 91% had a synovial white blood cell greater than 3,000 cells, and 76% had a synovial polymorphonuclear cells percent greater than 80%. Patients without co-infection were managed with non-steroidal anti-inflammatory drugs, colchicine, allopurinol, steroids, or a combination of these treatments and most had complete resolution of symptoms within 1 week. Commonly used markers of PJI fail to reliably distinguish periprosthetic crystalline arthropathy from infection. Though clinical judgement and consideration of the implications of delayed treatment for acute PJI remain paramount, in the setting of synovial crystals, surgeons may wish to consider this alternate etiology as the source of the patient's clinical symptoms.

Objective To explore the therapeutic effects of trimetazidine (TMZ) on diabetic patients with coronary heart diseases.Methods We conducted a comprehensive electronic search of PubMed, EMBASE, and Cochrane databases between the inception dates of databases and May 2019 (last search conducted on 30 May 2019) to identify randomized controlled trials. The evaluation method recommended by Cochrane Collaboration for bias risk assessment was employed for quality assessment. Random or fixed models were used to investigate pooled mean differences in left ventricular function, serum glucose metabolism, serum lipid profile, myocardial ischemia episodes and exercise tolerance with effect size indicated by the 95% confidence interval (CI).Results Additional TMZ treatment contributed to considerable improvement of left ventricular ejection fraction (WMD=4.39, 95%CI: 3.83, 4.95, P<0.00001), left ventricular end diastolic diameter (WMD=-3.17, 95%CI: -4.90, -1.44, P=0.0003) and left ventricular end systolic diameter (WMD=-4.69, 95%CI: -8.66, -0.72, P=0.02). TMZ administration also significantly decreased fasting blood glucose (SMD=-0.43, 95%CI: -0.70, -0.17, P=0.001), glycosylated hemoglobin level (WMD=-0.59, 95%CI: -0.95, -0.24, P=0.001), serum level of total cholesterol (WMD=-20.36, 95%CI: -39.80, -0.92, P=0.04), low-density lipoprotein cholesterol (WMD=-20.12, 95%CI: -32.95, -7.30, P=0.002) and incidence of myocardial ischemia episodes (SMD=-0.84, 95%CI: -1.50, -0.18, P=0.01). However, there were no significant differences in serum triglyceride level, high-density lipoprotein cholesterol, exercise tolerance between the TMZ group and the control group. Conclusion TMZ treatment in diabetic patients with coronary heart disease is effective to improve cardiac function, serum glucose and lipid metabolism and clinical symptoms.

We herein report a highly efficient strategy for the stereoselective synthesis of structurally complex bispiro[oxindole-thiazolidinone-hexahydroxanthone]s. This squaramide-catalyzed domino Michael addition afforded these products in good to excellent yields with excellent stereoselectivities bearing five contiguous stereocenters with two spiroquaternary stereocenters. Meanwhile, both gram-scale synthesis and further transformation experiments have been also demonstrated.
