RESUMEN

OBJECTIVE: To evaluate the cytotoxic effect of photodynamic therapy (PDT) combined with targeted therapy using cross-linked liposomes and gels (Ce6-PC-Tmab@A-Gel) loaded with photosensitizer Chlorin (Ce6) and the tumor-targeting drug Trastuzumab (Tmab) in drug-resistant HER2+ breast cancer cells. OBJECTIVE: Ce6-PC-Tmab liposomes were prepared using the thin-film hydration method. The general properties, encapsulation efficiency and near-infrared responsivity of the nanoparticles were evaluated. Ce6-PC-Tmab@A-Gel with a shear response was prepared by freeze drying and stirring crosslinking, and its microstructure was observed with scanning electron microscopy (SEM) and the shear response evaluated using a rheometer. The inhibitory effect of Ce6-PC-Tmab@A-Gel in drug-resistant HER2+ breast cancer SK-BR-3 cells was assessed with cytotoxicity assay (MTT assay) combined with near-infrared light. OBJECTIVE: The particle size of Ce6-PC-Tmab was 239.7±9.7 nm and the potential was -2.03±0.09 mV. The entrapment efficiency of Tmab by Ce6-PC-Tmab liposomes was (40.22± 0.73)%. The prepared Ce6-PC-Tmab@A-Gel had a good shear response with excellent drug release characteristics under nearinfrared light, and increased intensity and duration of near-infrared light exposure enhanced Tmab release from the gel. Ce6-PC-Tmab@A-Gel was stable at room temperature and in a simulated tumor microenvironment (pH 6.25). Cytotoxicity assay (MTT) showed that Ce6-PC-Tmab@A-Gel combined with near-infrared light resulted in a survival rate of (31.37±1.73)% in SKBR-3 cells, much lower than that in the control group and other treatment groups (P < 0.01); the combined treatment also had a high efficiency of ROS production, and ROS release reached (22.36 ± 0.11)% after 2 min of near-infrared light exposure. OBJECTIVE: The prepared Ce6-PC-Tmab@A-Gel has good near-infrared light response release characteristics to ensure effective targeted therapy with Tmab. The injectable gel system potentially allows long-term local drug release in the tumor to improve the treatment efficacy against drug-resistant breast cancer.

OBJECTIVE: To assess the effect of salt intake on residual renal function in rats and explore the possible mechanism. OBJECTIVE: SD rats were 5/6-nephrectomized to induce chronic renal failure followed by peritoneal dialysis for 4 weeks (n=18) or without dialysis treatment (control group; n=18). In both groups, the rats were divided into 3 subgroups and were given lowsalt diet (0.02% NaCl), normal salt diet (0.4% NaCl), and high-salt diet (4% NaCl). After 8 and 12weeks, blood pressure and creatinine and sodium levels in the blood, urine, and peritoneal dialysate of the rats were examined. Glomerular sclerosis, tubulointerstitial fibrosis, and protein expression levels of RAS components (ACE-1, AGT, and AT-1) in renal cortical tissue of the rats were evaluated. OBJECTIVE: The residual renal function of the rats all decreased especially in rats with high salt intake for 8and 12 weeks. In peritoneal dialysis group, the rats with high-salt diet showed signficiantly increased renal interstitial fibrosis score (P=0.036), glomerular sclerosis index (P=0.045), systolic blood pressure (P=0.004), diastolic blood pressure (P=0.048), and renal expressions of AGT, ACE-1, and AT1 (P < 0.05) as compared with those with normal salt intake. In the rats fed the same high-salt diet, the renal interstitial fibrosis score, glomerular sclerosis index, diastolic blood pressure increase, and renal AGT and ACE-1 expression levels were significantly lower in the peritoneal dialysis group than in the control group (P < 0.05). A positive correlation was noted between the reduction of residual renal function and sodium intake in the rats. OBJECTIVE: In rats with chronic renal failure, high salt intake promotes the activation of the renal RAS system, increases blood pressure, and agrevates renal fibrosis to accelerate the decline of residual renal function, and peritoneal dialysis partially reduces the damage of residual renal function induced by high-salt diets by removing excessive sodium.

OBJECTIVE: To explore the relationship between childhood trauma and suicidal risk in prisoners and the mediating effect of alexithymia in this relationship. OBJECTIVE: We investigated a total of 362 male prisoners in Guangdong Province using Childhood Trauma Questionnaire-Short Form (CTQ-SF), the Twenty-item Toronto Alexithymia Scale (TAS-20) and Suicidal Behaviors-Revised Questionnaire (SBQ-R). OBJECTIVE: Thirty-five (9.7%) prisoners had a high risk of suicide. The total score and the dimension scores of the CTQ-SF was positively correlated with the total score of SBQ-R (r=0.065-0.358, P < 0.01) and the total score of TAS-20 (except for the dimension score of sexual abuse) (r=0.156-0.255, P < 0.01). The total score of TAS-20, the dimensions scores for difficulty in identifying and describing feeling and the dimension score for externally oriented thinking were positively correlated with the total score of SBQ-R (r=0.330, P < 0.01; r=0.294-0.305, P < 0.01; and r=0.130, P < 0.05, respectively). The results of structural equation analysis showed that alexithymia had a partial mediating effect on childhood trauma and suicide risks (ß=0.07, 95%CI: 0.023-0.12, P < 0.05). OBJECTIVE: Childhood trauma affects suicidal risk in prisoners not only directly but also indirectly through the mediating effect of alexithymia.

The Staphylococcus aureus Tet38 membrane protein has distinct functions, including drug efflux and host cell attachment and internalization mediated by interaction with host cell CD36. Using structural modeling and site-directed mutagenesis, we identified key amino acids involved in different functions. Tet38, a member of the major facilitator superfamily is predicted to have 14 transmembrane segments (TMS), 6 cytoplasmic loops, and 7 external loops. Cysteine substitutions of arginine 106 situated at the junction of TMS 4 and external loop 2, and glycine 151 of motif C on TMS 5, resulted in complete or near complete (8- to 16-fold) reductions in Tet38-mediated resistance to tetracycline, with minimal to no effect on A549 host cell internalization. In contrast, a three-amino acid deletion, F411P412G413, in external loop L7 situated between TMS 13 and 14 led to a decrease of fourfold in S. aureus internalization by A549 cells and a partial effect on tetracycline resistance (4-fold reduction). A three-amino acid deletion, D38D39L40, in external loop L1 situated between TMS 1 and 2, had a similar partial effect on tetracycline resistance but did not affect cell internalization. Using a Ni column retention assay, we showed further that the L7, but not the L1, deletion impaired binding to CD36. Thus, the L7 domain of Tet38 is key for interaction with CD36 and host cell internalization, and amino acids R106 and G151 (TMSs 4 and 5) are particularly important for tetracycline resistance without affecting internalization.

A high-fat diet (HFD) has been previously associated with the development of diseases such as chronic colitis. While chlorogenic acid (CGA) is known to exhibit potent antioxidant, antibacterial, and anti-inflammatory properties, little is known about its effects on intestinal inflammation. In this study, we investigated the effects of CGA on intestinal inflammation in an HFD-induced obesity rat model and assessed whether these effects were related to changes in gut microbiota composition. This was achieved by examining physiological and biochemical indicators, the liver transcriptome, and the structure of the fecal microflora. CGA treatment significantly reduced HFD-induced internal organ weight gain, promoted colon tissue repair, downregulated the expression of inflammatory cytokines, and promoted the accumulation of the tight junction protein. KEGG enrichment analysis of differentially expressed genes, applied to data from the RNA-seq of rat liver tissue, revealed that CGA treatment significantly affected amino acid and lipid metabolism in the liver. Furthermore, CGA decreased the abundance of bacteria belonging to the genera Blautia, Sutterella, and Akkermansia and increased butyric acid levels, which were positively correlated with the abundance of Ruminococcus (butyric acid producer). Moreover, the beneficial changes observed in the HFD group were not as pronounced as those in the CGA treatment group. In summary, CGA can alleviate colitis in HFD-induced obesity through its anti-inflammatory effects associated with changes in gut microbiota composition and an increase in the production of short-chain fatty acids and thus can be used as a potential drug for the treatment of this pathology.

Objective: To evaluate the short-term clinical efficacy and safety of transvaginal pelvic floor reconstruction with TiLOOP mesh, a titanized polypropylene lightweight mesh. Methods: From November 2017 to July 2019, 50 patients underwent surgery, who were with anterior vaginal prolapse stage Ⅲ-Ⅳ and (or) apical prolapse and posterior vaginal prolapse (stage Ⅰ-Ⅱ); 37 patients underwent self-cut TiLOOP mesh and 13 patients underwent pre-cut mesh-kit procedure. The subjective and objective parameters (including surgical success) of them were evaluated. Results: The mean follow-up time was (18±6) months. The overall surgical success rate was 90% (45/50). Prolapse recurrence rates were isolated anterior 2% (1/50), isolated apical 2% (1/50) and isolated posterior 6% (3/50). None of recurrent patients underwent re-treatment, including either surgery or use of a pessary at last follow-up. According to patient global impression of improvement (PGI-I), 46 patients (92%, 46/50) were very much better, and 4 patients (8%, 4/50) were much better. After the operation, pelvic floor distress inventory-short form 20 (PFDI-20) and pelvic floor impact questionnaire-short form 7 (PFIQ-7) scores were significantly lower than those before operation (all P<0.05); pelvic organ prolapse-urinary incontinence sexual questionnaire 12 (PISQ-12) score was no significant difference before and after operation (P>0.05), but without dyspareunia. The vaginal mesh erosion rate was 12% (6/50) and the stress urinary incontinence was observed in 2 cases (4%, 2/47). Conclusion: Transvaginal pelvic floor reconstruction with TiLOOP mesh is a safe and effective surgery, short-term efficacy is acceptable, how to reduce the mesh erosion still need to be explored.

OBJECTIVE: HIF-1α and Runx2 expression usually increase in chondrocytes (CHs) during osteoarthritis (OA), which involves the changes in glycolytic metabolism. However, the molecular regulation of HIF-1α related to the CHs glycolytic metabolism is still unclear. In this study, we aimed to reveal the mediation of HIF-1α by Runx2 and its effect on the glycolytic metabolism of degenerative CHs. PATIENTS AND METHODS: The expression of HIF-1α, Runx2, and the degenerative markers of CHs in both natural conditions from the OA patients and IL-1ß treated in vitro model was analyzed by a Western blot or real-time polymerase chain reaction (RT-PCR). The glycolytic metabolism was determined by the intracellular glucose uptake and adenosine triphosphate (ATP) generation. Transfection of siRNA coding HIF-1α or Runx2 was used to clear the function between HIF-1α and Runx2 in the glycolytic metabolism of degenerated CHs caused by IL-1ß. Chromatin immunoprecipitation (ChIP) and Luciferase reporter gene assay were used to verify the Runx2 protein binds to the promoter of HIF-1α and promote its expression. RESULTS: HIF-1α and Runx2 were increased, and glucose uptake and ATP generation were decreased in the degenerative CHs from both OA and IL-1ß conditions. Under the stimulation of IL-1ß, Runx2 silencing rejected the upregulation of HIF-1α and further aggravated the glycolytic metabolism. When HIF-1α was silenced, the glycolytic metabolism of CHs was also suppressed. Besides, Runx2 protein could regulate HIF-1α expression in the transcriptional level by binding to its promoter. CONCLUSIONS: OHIF-1α plays a role in the self-repair of the glycolytic metabolism of degenerative CHs via the transcriptional regulation of Runx2.

OBJECTIVE: This study aimed to investigate the expression of MMP-9 (matrix metalloproteinases 9) and P53 (tumor suppressor proteins, tumor protein P53) in serum of patients with lung cancer. The relation between the two factors and their clinical pathological features were also explored. PATIENTS AND METHODS: 150 patients were selected as the experimental group. They were diagnosed with lung cancer in Shandong Province Chest Hospital from January 2014 to January 2017. Meanwhile, 90 healthy subjects were selected as the control group. The expression levels of MMP-9 and P53 in serum were detected by fluorescence quantitative Real-Time Polymerase Chain Reaction (RT-PCR). The diagnostic value of MMP-9 and P53 in patients with lung cancer was analyzed by receiver operating characteristic curve (ROC). Pearson test was used to analyze the correlation between MMP-9 and P53 in the experimental group. The patients were divided into high expression and low expression groups according to the median of MMP-9 and P53 expressions. RESULTS: The expressions of MMP-9 and P53 in patients with lung cancer were higher than the normal level (p<0.05). MMP-9 and P53 in serum of the experimental group were closely related to patients' TNM-stage, degree of differentiation, lymph node metastases, smoking history and pattern of organization (p<0.05). The expressions of MMP-9 and P53 were positively correlated (p<0.001). CONCLUSIONS: The expressions of MMP-9 and P53 played important roles in lung cancer and were closely related to clinicopathology and three-year survival rate. They could provide help for the diagnosis and treatment of clinical lung cancer.

OBJECTIVE: Biliary and hyperlipidemic acute pancreatitis (AP) has become the second most common AP in China. Currently, AP is exclusively diagnosed as biliary or hyperlipidemic AP. However, as suggested by some reports, biliary and hyperlipidemic AP might coexist in a single patient. Moreover, acute lipotoxicity was shown to regulate the severity of biliary AP in the mouse model. Thus, whether these two etiologies coexist in AP patients and potentially worsen the clinical course remains unclear. To elucidate the clinical feature of a new complex type of acute pancreatitis with both biliary and hyperlipidemic etiologies. PATIENTS AND METHODS: This retrospective study included AP patients who were admitted into our department within 7 days after the onset of the disease. 267 AP patients were enrolled in this study and were classified as BAP (biliary acute pancreatitis, n=153), HLAP (hyperlipidemic acute pancreatitis, n=65) and BHAP (biliary-hyperlipidemic acute pancreatitis, n=49). All the enrolled patients met the classification criteria of biliary etiology, hyperlipidemic etiology, and both etiologies, respectively. BHAP was compared with BAP and HLAP in terms of general information, inflammatory biomarkers, organ dysfunction, disease severity and clinical outcomes. RESULTS: BHAP (41 vs. 53) patients were younger than BAP patients. Serum procalcitonin of BHAP patients was higher than BAP and HLAP patients. Serum CRP of BHAP patients was higher than BAP patients. BHAP patients had the highest diagnosis rate of severe acute pancreatitis (SAP) (46.9% vs. 17.6% or 21.5%) compared to BAP and HLAP. Prevalences of persistent respiratory, acute renal, and circulatory failure were highest in BHAP patients (44.9%, 28.6%, 12.2%, respectively). Requirements for mechanical ventilation, renal replacement therapy and vasoactive agents were also highest in BHAP patients (36.7%, 34.7%, 12.2%, respectively). Hospital stay was longer in BHAP patients (33 days) compared with BAP patients (24 days). CONCLUSIONS: Patients with both biliary and hyperlipidemic etiologies suffer from more severe clinical course of the disease and have worse prognosis than single-etiology BAP or HLAP patients in the early stage of AP (within 7 days). It should be recognized as a new etiological type named biliary-hyperlipidemic acute pancreatitis (BHAP).

Objective: To explore the value of night pulse oximetry monitoring in the prediction and classification of obstructive sleep apnea hypopnea syndrome (OSAHS). Methods: From January 2018 to December 2019, 580 snoring patients admitted to the Sleep Center of Tianjin Medical University General Hospital were analyzed retrospectively. There were 418 males and 162 females, aging 13-85(49±14) years. All subjects underwent polysomnography, and the apnea hypopnea index (AHI)was 0-101.4(43.06±27.47) times/hour. There were 52 cases in the non-OSAHS group (AHI<5 times/h), 69 cases in the mild OSAHS group (5 times/h30 times/h) was 94.74%±0.76%. Conclusion: Based on the signals recorded by the SpO2 monitor, the methods of using MLR model for AHI prediction and using BPNN model for multi-classification may have higher value for the prediction and classification of OSAHS.

BACKGROUND: Nasopharyngeal carcinoma (NPC) is a malignant head and neck tumor arising in the nasopharynx. MicroRNAs (miRNAs) are elucidated to exert tumor-suppressing function in human cancers. Numerous studies have manifested that miR-30a-5p serves as an anti-oncogene in various cancers. OBJECTIVE: To research the biological function and molecular mechanism of miR-30a-5p in NPC. METHODS: The morphology of NPC tissues was revealed by H&E staining. Transwell and wound healing assays were applied to investigate the effects of miR-30a-5p on NPC cell migration. The binding interaction between miR-30a-5p and nucleobindin 2 (NUCB2) was identified by luciferase reporter assay. Xenograft nude mice were used to detect the influence of miR-30a-5p on NPC tumor growth. RESULTS: MiR-30a-5p was downregulated in NPC tissues and cells. The overexpression ofmiR-30a-5p inhibited proliferation, migration and invasion abilities of NPC cells. Moreover, NUCB2 was revealed to be a downstream target gene of miR-30a-5p, and knockdown of NUCB2 repressed the malignant behaviors of NPC cells and tumor growth. Additionally, rescue experiments revealed that miR-30a-5p suppressed the proliferation, migration and invasion of NPC cells via targeting NUCB2 in vitro. Meanwhile, in vivo assays depicted that NUCB2 overexpression rescued the effects induced by miR-30a-5p upregulation on tumor growth. CONCLUSION: MiR-30a-5p modulates NPC progression by targeting NUCB2. These findings lay a foundation for exploring the clinical treatment of NPC.

Hepatic sinusoidal obstruction syndrome (HSOS) is the most common early complication in patients with hematopoietic stem cell transplantation (HSCT). According to reports, the average incidence of HSOS post-HSCT is 13.7% (0~62%). HSOS may be accompanied by multiple organ failure, and the mortality rate can exceed 80% at 100 days of onset. Defibrotide is currently the safest and most effective drug used to prevent and treat HSOS post-HSCT. It is currently the only drug approved in the United States for the treatment of HSOS with lung/renal dysfunction post-HSCT. In addition, it has been approved in the European Union for the treatment of severe HSOS cases post-HSCT. This article briefly describes and summarizes the research progress, clinical application, pharmacokinetics, efficacy, safety, dose and treatment of defibrotide in HSOS post-HSCT.

It has been recently recognized that biomechanics plays a crucial role in medicine. Corneal biomechanics is closely related to multiple aspects of corneal refractive surgery, such as indications for refractive surgery, choice of surgical methods, design of surgical parameters, management of surgical complications, prediction of surgical correction effect and long-term stability. By analyzing the various roles of biomechanics in corneal refractive surgery, this article aims to emphasize the possible influence of mechanics on corneal tissue, as well as the importance of biomechanics in corneal refractive surgery, so as to further improve the surgical safety, efficacy and predictability. Objective: To explore the rule of corneal biomechanical alteration with residual stromal thickness (RST) and percent tissue altered (PTA) after small incision lenticule extraction (SMILE) and to determine the factors influencing postoperative corneal biomechanical properties. Methods: In this retrospective study, a total of 184 patients (184 right eyes) who underwent SMILE in Tianjin Eye Hospital Refractive Surgery Center from January 2019 to January 2020 were enrolled. There were 83 males and 101 females with age of (24.6±5.8) years. Corneal biomechanical parameters, including DA ratio, stiffness parameter at the first applanation (SPA1) and integrated radius (IR), were measured with Corvis ST preoperatively and at 3 months postoperatively. The association between PTA, RST and the changes of DA ratio, SPA1 and IR was assessed by linear and nonlinear regression analyses. Stepwise multivariate regression analyses were conducted to explore the factors associated with postoperative corneal biomechanical parameters with age, sex, anterior mean keratometry, spherical equivalent, postoperative central corneal thickness (CCT) and preoperative corneal biomechanical parameters as covariates. Preoperative and postoperative data were compared using the paired t test. Correlations were determined by the Pearson or Spearman analysis. Results: The alterations at 3 months postoperatively of DA ratio, SPA1 and IR were 1.33 (30.0%), 28.05 (26.0%) and 2.56 (34.0%), respectively. The changes before and after surgery were statistically significant (t=35.52, -28.00, 36.95, P<0.01). The best-fit curve showed that the changes of DA ratio, SPA1 and IR increased with the decrease of RST or increase of PTA. When the RST was<280 µm or the PTA was>28%, the slope of the change of DA ratio curve was significantly increased. Multivariate regression models showed that the factors with the greatest influence on postoperative DA ratio, SPA1 and IR were preoperative DA ratio (Sß=0.489, P<0.01), preoperative SPA1 (Sß=0.483, P<0.01) and preoperative IR (Sß=0.471, P<0.01), respectively. The CCT was the second factor that influenced postoperative DA ratio and SPA1 (Sß=-0.238, P<0.01; Sß=0.326, P<0.01). Conclusions: The changes of DA ratio, SPA1 and IR following SMILE increased with the decrease of RST or increase of PTA. With the RST<280 µm or the PTA>28%, the alteration of DA ratio significantly accelerated. Preoperative corneal biomechanical properties and postoperative CCT were main factors influencing corneal biomechanical properties after SMILE. (Chin J Ophthalmol, 2021, 57: 104-112).

RESUMEN

Retinopathy of prematurity (ROP) has become the leading cause of childhood blindness worldwide. It is the key point of ROP prevention and treatment to screen out and deal with severe ROP that require treatment. However, more than 70% ROP regressed spontaneously without treatment. It is useful in deeply understanding the natural course of ROP and is helpful to develop strategies of ROP screening and prevention, that the characteristics of spontaneous regression of ROP are investigated comprehensively. The epidemic characteristics, clinical features, time course, related factors and visual impairment of ROP regression will be systematically summarized in this review.(Chin J Ophthalmol, 2021, 57: 150-155).

Lactose and somatic cell score (SCS) are major economic traits of milk. However, for many countries, they are typically not directly considered in the national genetic evaluation of Simmental cattle. This study aimed to estimate the genetic relationships between lactose, SCS, and growth traits of Simmental cattle to provide information for the national genetic evaluation of the selection of traits of this cattle population. The data of 1781 animals with 6519 records obtained over a period of 41 years (1975-2016) were collected from Xinjiang Hutubi Farm, China. The analyzed traits included 305 days of milk yield (305MY), milk fat percentage (MFP), milk protein percentage (MPP), milk lactose percentage (MLP), total solids (TS), SCS, body height (BH), body length (BL), chest girth (CG), abdominal circumference (AC), rump width (RW), rump length (RL), leg circumference (LC), and cannon circumference (CC). The multiple-trait repeatability model was adopted to estimate (co)variance components using the average information-restricted maximum likelihood method implemented using the DMU statistical package. The heritability estimates for milk components and growth traits ranged from 0.09 (SCS) to 0.51 (BH). Genetic correlations for milk components ranged from 0.03 ±â€¯0.14 (MFP and MLP) to 0.81 ±â€¯0.08 (MFP and MPP). Genetic correlation between MLP and SCS was moderate and negative (-0.50 ±â€¯0.15) compared with that among other traits. Genetic correlations between the milk components and growth traits ranged from 0.00 ±â€¯0.07 (305MY and RW) to -0.64 ±â€¯0.15 (MLP and BL). Genetic correlations of BL, LC, RW, and RL with MLP were moderate to high and negative ranging from -0.39 to -0.64. Somatic cell score showed the highest correlation with BL (0.41) followed by LC (0.21). An increase in MLP would result in an increase in 305MY or TS and a decrease in BL, LC, RW, and RL. Additionally, a decrease in SCS would occur with the selection of increased MLP and reduced BL. We conclude that selection based on easily and inexpensively measured growth traits could improve the milk quality from Simmental cattle.
