RESUMEN

O avanço da tecnologia de digitalização de imagens e desenvolvimento de dispositivos de fresagem possibilitaram a otimização de diversos processos na Odontologia. O emprego do escaneamento intraoral e do sistema CAD-CAM (CAD - Computer Aided Design; CAM - Computer Aided Manufacturing) aprimorou a realização de reabilitações protéticas, permitindo a obtenção de peças de alta qualidade em tempo reduzido. O objetivo deste trabalho é relatar um caso de reabilitação de um incisivo central inferior a partir de escaneamento intraoral, com scanner Omnicam (DentsplySirona Charlotte, NC, EUA) e aplicação do sistema CAD-CAM.


The advancement of image digitization technology and the development of milling devices have made it possible to optimize various processes in Dentistry. The use of intraoral scanning and the CAD-CAM system (CAD - Computer Aided Design; CAM - Computer Aided Manufacturing) has improved the performance of prosthetic rehabilitations, allowing the obtaining of high quality parts in a reduced time. The objective of this work is to report a case of rehabilitation of a lower central incisor using intraoral scanning, with an Omnicam scanner (Dentsply Sirona) and application of the CAD-CAM system.

Purpose: To evaluate the preferences of dental practitioners for tooth-supported crowns dental ceramics by means of an online survey and to assess the influence of age, gender, years of experience, and dental specialty on those preferences. Materials and Methods: An anonymous questionnaire was delivered online through Google Surveys, targeting 796 dentists. It contained 17 dichotomous, closed questions with two sections. The first section dealt with general characteristics, including age, gender, years of experience, training in prosthodontics, and hours worked per week. The second section included questions regarding preferences of different materials for crowns, and the use of digital workflows. Analyses were carried out with Stata 14.0 software (StataCorp, TX, USA). A significance level of p=0.05 was adopted. Results: 248 surveys were answered. Practitioners in Chile preferred lithium disilicate for the anterior region (55.2%) but also for the posterior area (40.7%), regardless of their age, gender, years since graduation, hours worked per week or any training in prosthodontics. Chilean dentists over 50 years old considered zirconia almost 4 times more than those under 30 years old for anterior crowns (51.85%). 59.68% of dentists take digital impressions, and 37.10% have access to chairside CAD/CAM technology. In this group, 54.4% preferred feldspathic ceramic for anterior and 23.9% for posterior crowns. Conclusions: While there is a wide range of dental ceramic materials, these results provide a snapshot of the current trends in Chile where lithium disilicate is the most preferred ceramic material for tooth-supported crowns, and metal-ceramic is the least preferred material.

OBJECTIVE: To evaluate the effect of the deterioration of computer aided design/computer aided manufacturing (CAD/CAM) burs during zirconia milling, on surface roughness, contact angle, and fibroblast viability. MATERIALS AND METHODS: Ceramic blocks were milled and 75 ceramic disks (8 × 1.5 mm) made and allocated into three groups (n = 25): G1-brand new 2L and 1L burs, G2-2L bur at the end of lifetime and brand new 1L bur and G3-both burs at the end of their lifetimes. Roughness (Ra, Rq, and Rz) was evaluated using a 3D optical profilometer, the contact angle by the sessile drop method and the cell viability of the mouse NIH/3T3 fibroblast, using the Alamar Blue assay at intervals of 24, 48, and 72 h (ISO 10993-5). Data were analyzed by one-way ANOVA and Kruskal-Wallis tests (p ≤ 0.05). RESULTS: Roughness increased as the burs deteriorated and G3 (0.27 ± 0.04) presented a higher value for Ra (p < 0.001). The highest contact angle was observed in G3 (86.2 ± 2.66) when compared with G1 (63.7 ± 12.49) and G2 (75.3 ± 6.36) (p < 0.001). Alamar Blue indicated an increase in cell proliferation, with no significant differences among the groups at 24 and 72 h (p > 0.05). CONCLUSIONS: The deterioration of the burs increased the surface roughness and decreased the wettability, but did not interfere in cell viability and proliferation. CLINICAL SIGNIFICANCE: The use of custom zirconia abutments represents an effective strategy for single crowns restorations. Our findings suggest that these abutments can be efficiently milled using CAD/CAM burs within their recommended lifetime.

Purpose: To evaluate, in vitro, the efficiency of a novel apparatus to test the adherence and penetration of bacteria on different membranes for guided regeneration. Methodology: To create the 3D device, Computer Aided Design/Computer Aided Manufacturing (CAD/CAM) systems were used. Three types of biomaterials were tested (n = 6): (DT) a collagen membrane; (DS) a polymer membrane; and (LP) a dense polytetrafluoroethylene barrier. The biomaterials were adapted to the apparatuses and challenged with two different monospecies bacterial culture of A. actinomycetemcomitans b and S. mutans. After 2 h, bacterial adherence and penetration were quantified by counting the number of colony-forming units (CFUs). Two specimens from each group were used for image analysis using Confocal Laser Scanning Microscopy. Statistical analysis was performed. Findings: The DS group had a higher adherence of S. mutans compared to A. actinomycetemcomitans b (p = 0.05). There was less adherence of A. actinomycetemcomitans b in the DS group, compared to the LP (p = 0.011) and DT (p < 0.001) groups. Only the membranes allowed penetration, which was blocked by barriers. The DT group allowed a greater penetration of S. mutans to occur compared to A. actinomycetemcomitans b (p = 0.009), which showed a higher penetration into the DS membranes compared to S. mutans (p = 0.016). The penetration of A. actinomycetemcomitans b through DS was higher compared to its penetration through DT and LP (p < 0.01 for both). DT and DS allowed a greater penetration of S. mutans to occur compared to LP, which prevented both bacterial species from penetrating. Conclusion: The apparatus allowed for the settlement and complete sealing of the biomaterials, enabling standardization.

The development of intelligent transportation systems (ITS), vehicular ad hoc networks (VANETs), and autonomous driving (AD) has progressed rapidly in recent years, driven by artificial intelligence (AI), the internet of things (IoT), and their integration with dedicated short-range communications (DSRC) systems and fifth-generation (5G) networks. This has led to improved mobility conditions in different road propagation environments: urban, suburban, rural, and highway. The use of these communication technologies has enabled drivers and pedestrians to be more aware of the need to improve their behavior and decision making in adverse traffic conditions by sharing information from cameras, radars, and sensors widely deployed in vehicles and road infrastructure. However, wireless data transmission in VANETs is affected by the specific conditions of the propagation environment, weather, terrain, traffic density, and frequency bands used. In this paper, we characterize the path loss based on the extensive measurement campaign carrier out in vehicular environments at 700 MHz and 5.9 GHz under realistic road traffic conditions. From a linear dual-slope path loss propagation model, the results of the path loss exponents and the standard deviations of the shadowing are reported. This study focused on three different environments, i.e., urban with high traffic density (U-HD), urban with moderate/low traffic density (U-LD), and suburban (SU). The results presented here can be easily incorporated into VANET simulators to develop, evaluate, and validate new protocols and system architecture configurations under more realistic propagation conditions.

OBJECTIVE: To evaluate the effect of polishing and bleaching on the recovery of lightness, color, whiteness, and relative translucency parameter (RTP) in CAD/CAM materials and changes in these properties when another staining in coffee was conducted after the treatments. MATERIALS AND METHODS: Disks of Lava Ultimate (LU), Vita Enamic (VE), IPS Empress CAD (EMP), IPS e.max CAD (EMAX), and Vita Suprinity (VS) were (1) not treated (control), (2) polished with Proxyt or (3) Ceramisté, (4) bleached with Opalescence PF or (5) Whiteness HP Blue, and (6) air polished with Clinpro Prophy Powder. CIE L*a*b* color coordinates were registered at baseline (R0), after staining with coffee for 30 min daily for 36.5 days and treatment (R1), and after another staining (R2). Differences (R1-R0 and R2-R0) in lightness (ΔL00), color (ΔE00), RTP (ΔRTP00), and whiteness (ΔWID) were evaluated by mixed repeated measures ANOVA and 95% confidence intervals (α = 0.05) and interpreted in function of their respective 50:50% PT and AT thresholds. Topography was evaluated by scanning electron microscopy (SEM). RESULTS: In LU, Opalescence PF and Proxyt decreased Δ L 00 R 1 - R 0 $$ {\Delta L}_{00\left({R}_1\hbox{--} {R}_0\right)} $$ , Δ E 00 R 1 - R 0 $$ {\Delta E}_{00\left({R}_1\hbox{--} {\mathrm{R}}_0\right)} $$ , and ΔWI D R 1 - R 0 $$ {\Delta \mathrm{WI}}_{\mathrm{D}\left({R}_1\hbox{--} {R}_0\right)} $$ and showed lower Δ L 00 R 2 - R 0 $$ {\Delta L}_{00\left({R}_2\hbox{--} {R}_0\right)} $$ , Δ E 00 R 2 - R 0 $$ {\Delta E}_{00\left({R}_2\hbox{--} {R}_0\right)} $$ , and ΔWI D R 2 - R 0 $$ {\Delta \mathrm{WI}}_{\mathrm{D}\left({R}_2\hbox{--} {R}_0\right)} $$ . Δ E 00 R 2 - R 0 $$ {\Delta E}_{00\left({R}_2\hbox{--} {R}_0\right)} $$ of Ceramisté and ΔWI D R 2 - R 0 $$ {\Delta \mathrm{WI}}_{\mathrm{D}\left({R}_2\hbox{--} {R}_0\right)} $$ and Δ R T P 00 R 2 - R 0 $$ \varDelta RT{P}_{00\left({R}_2\hbox{--} {R}_0\right)} $$ of Proxyt were lower than those of the control group. CONCLUSIONS: The most suitable treatment to recover the lightness, color, whiteness, and RTP without changing these properties after another coffee exposure is material-dependent. CLINICAL SIGNIFICANCE: Although the effectiveness of the treatment was material-dependent, Proxyt was the only treatment that promoted clinically acceptable changes for both LU and VE, while for purely ceramic materials, this condition was observed with Ceramisté and Clinpro Prophy Powder.

A technique is outlined for utilising a polymeric composite reinforced with glass fibres in a three-dimensional mesh as a post-core in aesthetic cases. The clinical procedure involves obtaining an impression of the root canal space, scanning the definitive cast, and milling a fibre-reinforced composite post-core. Subsequently, the intra-radicular post-core is cemented using an adhesive resin cement. The use of custom-made computer-aided design-computer-aided manufacturing (CAD-CAM) fibre-reinforced composite post-core facilitates repairability, provides better adaptation to the root canal space, avoids uneven cement thickness, ensures chemical adhesion to resin cement, and promotes favourable aesthetics when combined with all-ceramic crowns.

OBJECTIVES: Evaluate the efficacy of denture cleaners on the adhesion of Candida albicans and their effects on the surface, optical, and mechanical properties of resins for conventional, milled, and 3D-printed denture bases. MATERIALS AND METHODS: A total of 240 resin samples were made, 120 for testing Candida albicans adhesion, optical stabilities (ΔE00), roughness (Ra), hydrophilicity (°), surface free energy (Owens-Wendt) and 120 samples for testing Candida albicans adhesion, surface microhardness (Knoop), flexural strength and modulus of elasticity in a three-point test, in which they were divided into 3 groups of denture resin (n = 40) and subdivided into 5 cleaners of dentures (n = 8). Data were evaluated by two-way ANOVA and Tukey's test for multiple comparisons (α = 0.05). RESULTS: Denture cleaners with an alkaline solution and dilute acid composition were those that showed the greatest effectiveness in reducing Candida albicans (P < 0.001), however 1% NaOCl significantly affected the properties of the resins (P < 0.05). Denture 3D-printed showed that the surface microhardness was significantly lower for all cleansers (P < 0.05). CONCLUSIONS: Listerine demonstrated superior efficacy in reducing Candida albicans with minimal effect on denture properties, whereas 1% NaOCl had a significant negative impact on the properties. The mechanical properties were significantly lower in 3D-printed resin than in other resins for all denture cleansers. CLINICAL RELEVANCE: Denture base materials are being sold to adapt to the CAD/CAM system, increasing the number of users of dentures manufactured with this system. Despite this, there is little investigation into denture cleaners regarding the adhesion capacity of microorganisms and the optical, surface and mechanical properties of dentures, thus requiring further investigation.

Although advancements in CAD/CAM technology allow for more personalized treatments, it is not clear how modifications in the CAD/CAM milling process could affect the restoration surface conditions and their mechanical behavior. The objective of this study was to evaluate the effect of different CAD/CAM milling protocols on the topography and fracture behavior of zirconia monolithic crowns (3Y-PSZ) subjected to a chewing simulation. Monolithic 3Y-PSZ premolar crowns were milled using three protocols (n = 13) (slow (S), normal (N), and fast (F)). Crowns were cemented on a dentin analog abutment and subjected to mechanical aging (200 N, 2 Hz, 1,500,000 cycles, 37 °C water). Surviving crowns were subjected to compressive load test and analyzed using fractography. Fracture load data were analyzed with two-parameter Weibull analysis. The surface topography of the crowns was examined with a stereomicroscope and a 3D non-contact profiler. All crowns survived the chewing simulation. Crowns milled using the F protocol had the greatest characteristic fracture load, while crowns produced with the S protocol showed high Weibull modulus. Groups N and S had a more uniform surface and detailed occlusal anatomy than group F. The CAD/CAM milling protocol affected the topography and mechanical behavior of 3Y-PSZ monolithic crowns.

Severe tooth wear is related to substantial loss of tooth structure, with dentin exposure and significant loss (≥1/3) of the clinical crown. The objective of this systematic review was to summarize and analyze the scientific evidence regarding the mechanical performance of computer-aided design/computer-aided manufacturing (CAD/CAM) composite resin and CAD/CAM lithium disilicate ceramic occlusal veneers, in terms of fatigue and fracture resistance, on severely worn posterior teeth. Currently, occlusal veneers are an alternative for treating worn posterior teeth. Although scientific evidence demonstrates the good performance of lithium disilicate occlusal veneers, there are less brittle materials with a modulus of elasticity more similar to dentin than ceramics, such as resin CAD/CAM blocks. Therefore, it is important to identify which type of material is best for restoring teeth with occlusal wear defects and which material can provide better clinical performance. This review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. A comprehensive search of the PubMed, Embase, Web of Science, Scopus, Cochrane, OpenGrey, Redalyc, DSpace, and Grey Literature Report databases was conducted and supplemented by a manual search, with no time or language limitations, until January 2022. We aimed to identify studies evaluating the fatigue and fracture resistance of CAD/CAM composite resin and ceramic occlusal veneers. The quality of the full-text articles was evaluated according to the modified Consolidated Standards of Reporting Trials (CONSORT) criteria for in vitro studies, and 400 articles were initially identified. After removing duplicates and applying the selection criteria, 6 studies were included in the review. The results demonstrated that the mechanical performance of CAD/CAM composite resin occlusal veneers is comparable to that of CAD/CAM lithium disilicate occlusal veneers in terms of fatigue and fracture resistance.

The aim of this in vitro study is to compare the performance of digital and conventional methods in the manufacture of single copings in terms of the internal and marginal fit. Twenty-four prefabricated titanium Cone Morse Exact abutments of the lateral anatomical type were used to manufacture 24 lithium disilicate copings in the CAD-CAM Ceramill (n = 12) and heat-press (n = 12) systems. The copings were cemented using a self-adhesive resin cement (RelyX U-200; 3M ESPE) and then cut vertically. The cement line was photographed using an optical microscope at ×100 and ×200 magnification and then the internal and marginal regions were measured. The mean values of fits, for the CAD-CAM and heat-press techniques, respectively, were (µm): angular regions, 81.8 and 75.3; linear regions, 63.1 and 60.1; incisal regions, 171.1 and 114.7; marginal discrepancy, 74.1 and 75.2; and absolute marginal discrepancy, 99.5 and 96.2. MANOVA test showed that there is an effect of the techniques on the regions (p < .05). The effects of laboratories and the interaction between laboratories and techniques on the regions were not statistically significant (p > .05). The techniques evaluated presented clinically acceptable results for the marginal fit. However, the conventional method performed better for the internal fit. RESEARCH HIGHLIGHTS: Scientific evidence regarding the methods of making prosthesis can help the dental professional in decision-making. Digital and conventional methods is both good in the manufacture of single copings in terms of the internal and marginal fit.

OBJECTIVES: The purpose of this scoping review was to identify different methods employed for recording the maxillomandibular relationship (MMR) for computer-aided designed and manufactured (CAD-CAM) complete dentures (CDs). MATERIALS AND METHODS: This scoping review followed the PRISMA-ScR guidelines and was developed according to Arksey and O'Malley and The Joanna Briggs Institute protocol. The methods were registered on the Open Science Framework (< osf.io/rf4xm> ). The focus question was: "What are the different techniques for recording the maxillomandibular relationship in the digital workflow used in CECDs?" Two investigators searched 3 online databases [MEDLINE (PubMed), Scopus, and Science Direct] independently. The inclusion criteria were clinical studies and reviews that assessed techniques for recording MMR using digital workflow for manufacturing of CECDs. A descriptive analysis was performed considering the study design, manufacturing system, clinical steps, and tools for the determination of MMR, and the difficulty level of procedures. RESULTS: 4779 articles were identified in the electronic search and 10 studies were included for data analysis. The review identified 4 commercially available CAD-CAM denture systems and 3 innovative methods suitable for abbreviating the number of appointments (2 to 4 visits). The trial denture is inherent to the procedure for the Baltic System and 3 innovative techniques. Three techniques (2 innovative and WholeYouNexteeth) demonstrated lower difficulty levels for performing the clinical procedures, regardless of the professional skills. CONCLUSIONS: The commercially available and innovative techniques for the recording of MMR may provide predictability of the treatment. The techniques are effective, however, rely on the learning curve and the patient's clinical condition. CLINICAL RELEVANCE: Recording of the maxillomandibular relationship is paramount for the manufacturing and functionality of complete dentures. Clinicians should be aware of the different tools and techniques described for registering the jaw relationship.

OBJECTIVES: This study aimed to assess the influence of translucent monolithic versus bilayered crowns and whether the use of a CoCr base abutments affects the fatigue and fracture resistance of screwed implant-supported single crowns with external connections under mechanical cycling. MATERIALS AND METHODS: Fifty specimens were divided into groups: (1) metal-ceramic (MC) crown, (2) veneered zirconia crown (Zr), (3) veneered zirconia crown with a CoCr base abutment (ZrB), (4) monolithic translucent zirconia crown (MZr), and (5) monolithic translucent zirconia crown with a CoCr base abutment (MZrB). Specimens underwent mechanical cycling (5 × 106 cycles; 150 N) evaluating fatigue resistance (number of failures) and those that failed were subsequently subjected to fractographic analyses (stereomicroscope and scanning electron microscope) to evaluate failure location and area, and maximum fracture load was also measured. RESULTS: The failure-related survival rate (100%) and maximum fracture resistance of the MZrB were significantly higher than those of MC and Zr (50%; p < 0.05). There were no significant differences in the failure rate and fracture resistance when a CoCr base abutment was used or not in the translucent monolithic Zr groups (p > 0.05;MZrB vs. MZr). Failure location, with MC crowns' fractures, noted at the screw area (p = 0.043), while all-ceramic crowns were mostly in the cuspid and to failure area, the Zr group had the largest mean (15.55 ± 9.17 mm2) among the groups, significant difference only when compared with MC (1.62 ± 0.81 mm2) (p = 0.025). CONCLUSIONS: Translucent monolithic zirconia crowns exhibited significantly higher fatigue and fracture resistance compared with conventional MC and bilayered crowns. CLINICAL SIGNIFICANCE: The appropriate choice of material and manufacturing technique is crucial for predicting the higher clinical performance of single crowns. Enhanced mechanical resistance in terms of fatigue and fracture resistance can be achieved by replacing MC and bilayered restorations with computer-aided design and computer-aided manufacturing monolithic zirconia.

Introduction: A Computer-Assisted Detection (CAD) System for classification into malignant-benign classes using CT images is proposed. Methods: Two methods that use the fractal dimension (FD) as a measure of the lung nodule contour irregularities (Box counting and Power spectrum) were implemented. The LIDC-IDRI database was used for this study. Of these, 100 slices belonging to 100 patients were analyzed with both methods. Results: The performance between both methods was similar with an accuracy higher than 90%. Little overlap was obtained between FD ranges for the different malignancy grades with both methods, being slightly better in Power spectrum. Box counting had one more false positive than Power spectrum. Discussion: Both methods are able to establish a boundary between the high and low malignancy degree. To further validate these results and enhance the performance of the CAD system, additional studies will be necessary.

The present work aims to develop a production method of pre-sintered zirconia-toughened-alumina (ZTA) composite blocks for machining in a computer-aided design and computer-aided manufacturing (CAD-CAM) system. The ZTA composite comprised of 80% Al2O3 and 20% ZrO2 was synthesized, uniaxially and isostatically pressed to generate machinable CAD-CAM blocks. Fourteen green-body blocks were prepared and pre-sintered at 1000 °C. After cooling and holder gluing, a stereolithography (STL) file was designed and uploaded to manufacture disk-shaped specimens projected to comply with ISO 6872:2015. Seventy specimens were produced through machining of the blocks, samples were sintered at 1600 °C and two-sided polished. Half of the samples were subjected to accelerated autoclave hydrothermal aging (20h at 134 °C and 2.2 bar). Immediate and aged samples were characterized by scanning electron microscopy (SEM) and X-ray diffraction (XRD). Optical and mechanical properties were assessed by reflectance tests and by biaxial flexural strength test, Vickers indentation and fracture toughness, respectively. Samples produced by machining presented high density and smooth surfaces at SEM evaluation with few microstructural defects. XRD evaluation depicted characteristic peaks of alpha alumina and tetragonal zirconia and autoclave aging had no effect on the crystalline spectra of the composite. Optical and mechanical evaluations demonstrated a high masking ability for the composite and a characteristic strength of 464 MPa and Weibull modulus of 17, with no significant alterations after aging. The milled composite exhibited a hardness of 17.61 GPa and fracture toughness of 5.63 MPa m1/2, which remained unaltered after aging. The synthesis of ZTA blocks for CAD-CAM was successful and allowed for the milling of disk-shaped specimens using the grinding method of the CAD-CAM system. ZTA composite properties were unaffected by hydrothermal autoclave aging and present a promising alternative for the manufacture of infrastructures of fixed dental prostheses.

INTRODUCTION: CAD (MIM*114010) encodes a large multifunctional protein with the enzymatic activity of the first three enzymes initiating and controlling the de novo pyrimidine biosynthesis pathway. Biallelic pathogenic variants in CAD cause the autosomal recessive developmental and epileptic encephalopathy 50 (MIM #616457) or CAD deficiency presenting with epilepsy, status epilepticus (SE), neurological deterioration and anemia with anisopoikilocytosis. Mortality is around 9% of patients, mainly related to the no use of its specific treatment with uridine. Majority of reported cases have an early onset during infancy, with some few starting later in childhood. CASE REPORT: Here we report a deceased female patient with CAD deficiency whose epilepsy started at 14 years. She showed a rapid neurologic deterioration including cognitive decline, electroencephalographic background slowing which later evolved to a fatal refractory SE and supra and infratentorial atrophy on neuroimaging. Anemia developed after SE onset. METHODS AND RESULTS: her post-mortem whole exome sequencing identified biallelic missense variants in CAD (NM_004341.5): c.[2944G > A];[5366G > A] p.[(Asp982Asn)];[(Arg1789Gln)]. Our review of twenty-eight reported cases (2015-2023) revealed an epilepsy age onset from neonatal period to 7 years and the SE prevalence of 46 %. DISCUSSION: With our case, we highlight the relevance of suspecting this treatable condition in older patients and in SE with no evident etiology.

Abstract The purpose of this study was to examine the effect of different decontamination agents on the bond strength of saliva-contaminated CAD/CAM blocks and repair composite materials. Two hunderd eighty 3-mm-thick specimens were prepared from four different CAD/CAM materials: Katana Zirconia UTML disc, IPS e.max CAD block, Shofu block, and Vita Enamic block. Each material was divided into seven different subgroups (N=70). Group 1 had a clean surface. The other groups were comprised of the samples, which were contaminated with human saliva: group 2: negative control (non-cleaned); group 3: cleaned with water spray; group 4: cleaned with 70% ethanol; group 5: cleaned with Ivoclean; group 6: cleaned with Katana Cleaner; and group 7: cleaned with phosphoric acid. After the decontamination protocols, the resin composite cylinders were bonded to the CAD/CAM surfaces with a thin layer of dual-cured resin cement. The samples were stored for 24 hours at 37°C in distilled water. Then, they were subjected to a shear bond strength test (SBS). The values were recorded, and fracture types were evaluated using a microscope. Data were analyzed using two-way ANOVA and Tukey's post-hoc test. Generally, all decontaminating agents improved the SBS of composites to Katana Zirconia UTML, IPS e.max, and Vita Enamic materials (p<0.05). However, for Shofu materials, group 6 samples exhibited significantly higher bond strength values as compared with group 2 samples (p=0.026). The highest SBS values were seen in the phosphoric acid-treated group in Katana zirconia materials (26.45 ± 9.38 MPa), whereas the lowest values were seen in group 2 samples in Shofu materials (13.17±3.40 MPa). Each decontaminant agent improved the bond strength of composites to the contaminated CAD/CAM materials. If saliva is not cleaned before adhesive procedure, SBS values may decrease. All decontamination agents can be used safely on zirconia, lithium-disilicate glass-ceramic, hybrid ceramic, and polymer-infiltrated ceramic surfaces.


Resumen El propósito de este estudio fue examinar el efecto de diferentes agentes descontaminantes sobre la fuerza de unión de bloques CAD/CAM contaminados con saliva y materiales compuestos de reparación. Se prepararon doscientas ochenta muestras de 3 mm de espesor a partir de cuatro materiales CAD/CAM diferentes: disco Katana Zirconia UTML, bloque IPS e.max CAD, bloque Shofu y bloque Vita Enamic. Cada material se dividió en siete subgrupos diferentes (N=70). El grupo 1 tenía una superficie limpia. Los otros grupos estaban compuestos por muestras que estaban contaminadas con saliva humana: grupo 2: control negativo (no limpio); grupo 3: limpiado con agua pulverizada; grupo 4: limpiado con etanol al 70%; grupo 5: limpiado con Ivoclean; grupo 6: limpiado con Katana Cleaner; y grupo 7: limpiado con ácido fosfórico. Después de los protocolos de descontaminación, los cilindros de composite de resina se adhirieron a las superficies CAD/CAM con una fina capa de cemento de resina de curado dual. Las muestras se almacenaron durante 24 horas a 37°C en agua destilada. Luego, fueron sometidos a una prueba de resistencia al corte (SBS). Se registraron los valores y se evaluaron los tipos de fracturas utilizando un microscopio. Los datos se analizaron mediante ANOVA de dos factores y la prueba post-hoc de Tukey. En general, todos los agentes descontaminantes mejoraron el SBS de los composites con los materiales Katana Zirconia UTML, IPS e.max y Vita Enamic (p<0,05). Sin embargo, para los materiales Shofu, las muestras del grupo 6 exhibieron valores de fuerza de unión significativamente más altos en comparación con las muestras del grupo 2 (p=0,026). Los valores más altos de SBS se observaron en el grupo tratado con ácido fosfórico en materiales de circonio Katana (26,45 ± 9,38 MPa), mientras que los valores más bajos se observaron en las muestras del grupo 2 en materiales Shofu (13,17 ± 3,40 MPa). Cada agente descontaminante mejoró la fuerza de unión de los composites a los materiales CAD/CAM contaminados. The aim of this study was to compare the influence of heat treatment on fracture resistance (FR) of different ceramic materials used for CAD/CAM systems. METHODS: Eighty monolithic restorations were designed using the same parameters and milled with a CAD/CAM system (CEREC SW 5.0, PrimeMill, Dentsply-Sirona™, Bensheim, Germany), forming five study groups: Group 1 (n = 10), CEREC Tessera (Dentsply-Sirona™, Bensheim, Germany) crystallized (CCT), Group 2 (n = 10), CEREC Tessera uncrystallized (UCT), Group 3 (n = 20), Emax-CAD (Ivoclar Vivadent, Schaan, Liechtenstein) (CEC), Group 4 (n = 20), Vita Suprinity (Vita Zahnfabrik, Bad Säckingen, Germany) (CVS), and Group 5 (n = 20) Cameo (Aidite, Qinhuangdao, China) (CC). RESULTS: The average FR was similar for CCT, CC, and CEC at above 400 N, while CVS and UCT had the lowest values at 389,677 N and 343,212 N, respectively. CONCLUSION: Among the three ceramic materials that exhibited an FR above 400 N, CCT was considered the first recommended choice for CAD/CAM systems. This material not only demonstrated the highest FR but also exhibited outstanding consistency in the related measurements without the presence of outliers. Although the CC material showed high FR, its high dispersion revealed inconsistencies in the repetitions, suggesting caution in its use.

Endocrowns have emerged as a promising option for restoring endodontically treated teeth, offeringa restorable fracture scenario. However, regarding the choice of material and its fracture resistance, there are gaps in the literature regarding the best indication. The objective of this research was to evaluate and compare the effect of restorative materials for CAD-CAM in the manufacture of endocrown restorations, through an in Vitro study on hardness and fracture resistance. For the study, CAD-CAM blocks were transformed into discs 12 mm in diameter and 1.2 mm thick (specimens). Three restorative materials were evaluated and distributed into experimental groups (N=12 specimens): Leucita- Reinforced Ceramic/ IPS Empress CAD (MRleu), Lithium Disilicate/ IPS Emax CAD (MRdis) and Nanoceramic Resin /Lava Ultimate (MRres). These restorative materials were evaluated for morphology (N=1) by Scanning Electron Microscopy (SEM) and surface chemistry (N=1) by dispersive energy spectroscopy (EDS). The specimens were evaluated when the Vikers micro hardness (N=1) with a load of 1kg and 10 indentations, as well as the resistance to biaxial flexion (N=10) at a test speed of 0.5 mm/min. After the fracture occurred, the fragments were examined under a stereomicroscope. The results were tabulated and analyzed using the Minitab statistical program. The results showed that the MRdis material demonstrated superior results in relation to hardness (P=0.000) and biaxial bending resistance (P=0.000), followed by MRleu and finally the MRres. The presence of inorganic particles on an organic matrix and the presence of Zirconium (Zr) stands out in Lava Ultimate. As endocrowns surgiram como uma alternativa favorável para a restauração de dentes tratados endodonticamente, e se apresentam em um cenário de fratura restaurável. No entanto, em relação ao material de escolha e sua resistência à fratura existem lacunas na literature sobre a melhor indicação. O objetivo dessa pesquisa foi avaliar e comparar o efeito de materiais restauradores para CAD-CAM na confecção de restaurações endocrowns, através de um estudo in vitro sobre dureza e resistência à fratura. Para realização do estudo, blocos para CAD-CAM foram transformados em discos com 12 mm de diâmetro e 1,2 mm de espessura (espécimes). Três materiais restauradores foram avaliados e distribuídos em grupos experimentais (N=12 espécimes): Cerâmica Reforçada por Leucita/ IPS Empress CAD (MRleu), Dissilicato de Lítio/ IPS Emax CAD (MRdis) e Resina Nanocerâmica /Lava Ultimate (MRres). Estes materiais restauradores foram avaliados quanto à morfologia (N=1) através de Microscopia Eletrônica de Varredura (MEV) e química superficial (N=1) pela Espectroscopia de energia dispersiva (EDS). Os espécimes foram avaliados quanto à microdureza Vikers (N=1) com uma carga de 1kg e 10 indentações, como também em relação a resistência à flexão biaxial (N=10) em uma velocidade de ensaio de 0,5 mm/min. Os fragmentos após a fratura foram observados em estereomicroscópio. Os dados obtidos foram tabulados e analisados no programa estatístico Minitab. Os resultados observados mostraram que o material MRdis obteve resultados superiores em relação a dureza (P=0,000) e a resistência à flexão biaxial (P=0,000), seguido pelo MRleu e por fim o MRres. Destaca-se na Lava Ultimate a presença de partículas inorgânicas sobre uma matriz orgânica, além da presença de Zircônio (Zr). Conclui-se que materiais restauradores para CAD-CAM na confecção de restaurações Endocrowns apresentam efeito significativo quanto a dureza e resistência mecânica. (AU)

Despite the advancements in indirect monolithic restorations, technical complications may occur during function. To overcome this issues, intraoral repair using resin composite is a practical and low-cost procedure, being able to increase the restoration's longevity. This review aimed to evaluate the need for repair and suggest a standardized repair protocol to the main indirect restorative materials. For this, studies were surveyed from PubMed with no language or date restriction, to investigate the scientific evidence of indirect monolithic restoration repair with direct resin composite. A classification to guide clinical decisions was made based on the FDI World Dental Federation criteria about defective indirect restorations considering esthetic and functional standards, along with the patient's view, to decide when polishing, repairing or replacing a defective restoration. Based on 38 surveyed studies, different resin composite intraoral repair protocols, that included mechanical and chemical aspects, were defined depending on the substrate considering resin-based, glass-ceramic or zirconia restorations. The presented criteria and protocols were developed to guide the clinician's decision-making process regarding defective indirect monolithic restorations, prolonging longevity and increasing clinical success.
