RESUMEN

BACKGROUND: In laparoscopic colorectal surgery, accurate localization of a tumor is essential for ensuring an adequate ablative margin. Therefore, a new method, near-infrared laparoscopy combined with intraoperative colonoscopy, was developed for visualizing the contour of a cecal tumor from outside of the bowel. The method was used after it was verified on a model that employed a silicone tube. CASE PRESENTATION: The patient was a 77-year-old man with a cecal tumor near the appendiceal orifice. Laparoscopy was used to clamp of the terminal ileum, and a colonoscope was then inserted through the anus to the cecum. The laparoscope in the normal light mode could not be used to identify the cecal tumor. However, a laparoscope in the near-infrared ray mode could clearly visualize the contour of the cecal tumor from outside of the bowel, and the tumor could be safely resected by a stapler. The histopathological diagnosis of the resected specimen was adenocarcinoma with an invasion depth of M and a clear negative margin. CONCLUSIONS: This is the first report of the laparoscopic detection of the contour of a cecal tumor from outside the bowel. This technique is useful and safe for contouring tumors in laparoscopic colorectal surgery and can be used in various surgeries that combine endoscopy and laparoscopy.

RESUMEN

In recent years, flourodeoxyglucose-positron emission tomography/computed tomography (FDG PET/CT) has been used intensively in the field of oncology. However, an increase in FDG uptake has been observed both in malignant tissues, and inflammatory processes. Therefore false-positive results have appeared. We present a 70-year-old male patient who presented to the hospital with right lower quadrant pain. A right lower quadrant mass was observed with conventional methods, and PET/CT was performed which revealed a hypermetabolic mass in the right lower quadrant. The patient was referred to the surgery with a suspect malignant mass whose histopathological report indicated plastron appendicitis. Although FDG PET/CT is a reliable method in the evaluation of oncological cases, false-positivities should be taken into consideration in inflammatory processes.

RESUMEN

An adenocarcinoid of the cecum is a tumor that has seldom been reported in the literature. Its has the histologic features of both an adenocarcinoma and a carcinoid tumor. However, the malignancy potential and the prognostic behavior are still unclear. We treated one patient with this unusual tumor. A 64-year-old man was admitted with a complaint of right lower quadrant abdominal pain for 2 months. On colonoscopy and histological examination, the patient was diagnosed as having a cecal adenocarcinoma. A radical right hemicolectomy was done. Postoperative histologic analysis revealed that the tumor was composed of well-differentiated adenocacinoma and carcinoid components. Lymph node metastases were found in the three lymph nodes with only the carcinoid component.

RESUMEN

We report a case of intussusception of the cecum and appendix arising in pericecal endometriosis that are mistaken as a cecal tumor preoperatively. At the other OBGY clinic, the 1st lapalotomy was performed for management of leiomyoma but pelvic organs were adhesion severely and palpation mass in the cecum and appendix. The patient was transfer to our hospital for evaluation and treatment of mass. We investigated to intestine closely and diagnosis was cecal tumor colonoscopically. Total hystrectomy and iliocecectomy was performed successfully and discovered intussusception of the cecum and appendix due to endometriosis mistaken as a cecal tumor in operative field. Postoperatively, this case was diagnosed in pericecal endometriosis histologically.
