This study reviewed different country studies and noted that complaints in Brazil are more concentrated in complaints about being attended to and receiving access to services, rather than about clinical quality and safety issues. This paper explores the possible explanations for these differences based on the institutional logics theory and which logics actors privilege, and how they may play out in the healthcare field. To accomplish this undertaking, this study makes use of the healthcare complaint categorization developed by Reader and colleagues, which has been used by various studies. Next, a set of studies about healthcare complaints in different countries was examined to analyze the issues most common in the complaints and compare this information with the Brazilian data. This study identified three explanations why complaints about medical errors seldom occur. One group of studies highlights the hardships of local health systems. Another focuses on patient behavior. Finally, the third kind focuses on the issue of power to determine health orientation. The studies about a lack of resources do not directly explain why fewer complaints about clinical quality occur, thus helping to stress the management issues. Patient behavior studies indicate that patients may be afraid to point out medical errors or may be unaware of the procedures of how to do so, suggesting that family logic is left out of the decisions in the field. The third group of work highlights the prominence of the medical professional logic, both in terms of regulation and medical exercise.

Cognitive deficits play an important role in Bipolar Disorder (BPD). The Cognitive Problems and Strategies Assessment (CPSA) is a measure that evaluates the patient's perception of cognitive difficulties, and the spontaneous use of compensatory strategies and could thus have potential utility for clinical practice in patients with BPD. Our aim was to determine the validity and reliability of the Cognitive Problems and Strategies Assessment (CPSA) in Bipolar Disorder (BPD). Ninety-three BPD outpatients and 90 controls completed the Assessment of Problems with Thinking and Memory (APTM) questionnaire and the Assessment of Memory and Thinking Strategies (AMTS) questionnaire which constitute the CPSA, the Cognitive Complaints in Bipolar Disorder Rating Assessment (COBRA), as a measure of convergent validity, and general sociodemographic data. Cronbach's alpha coefficient, Spearman's correlation coefficient and independent sample t tests were used for Internal consistency, Convergent validity and Discriminant validity. The APTM had a Cronbach's alpha coefficient of 0.93 and the AMTS 0.90. The COBRA score and the APTM were significantly correlated. BPD patients exhibited higher scores on the APTM and lower scores on the AMTS than controls. The present instrument enriches the clinician's repertoire for rapid and inexpensive cognitive evaluation in BPD.

BACKGROUND: Caring for people with dementia (PwD) usually triggers stress and leads to mental and somatic health complaints (SHCs). Physical activity (PA) can provide burden relief in PwD caregivers, but it is not clear whether PA habits would contribute to reducing SHCs. This study aims to analyze the effect of PA on the relationship between burden and SHCs in a sample of family caregivers of PwD. METHODS: One hundred and fifty-seven caregivers of PwD reported their PA habits, and completed the Giessen's Subjective Health Complaints Questionnaire (GBB-8) and the Zarit Burden Interview (ZBI). The relationship between PA habits, burden of care (ZBI), and SHCs (B-GBB-8 scale) was examined. Subsequently, the moderating effect of PA habits on the relationship between burden and SHCs was tested. RESULTS: PA habit was inversely associated with ZBI (rbp = -0.242) and GBB-8 scores (rbp (Gastrointestinal) = -0.174; rbp (Musculoskeletal) = -0.195; rbp (Exhaustion) = -0.247; rbp (Cardiovascular) = -0.250; and rbp (Overall) = -0.257, respectively), whereas moderate positive correlations were found between ZBI and GBB-8 scores (r (Gastrointestinal) = 0.483; r (Musculoskeletal) = 0.536; r (Exhaustion) = 0.542; r (Cardiovascular) = 0.438; and r (Overall) = 0.598, respectively). The interaction effect of PA habit and burden was significant for the overall SHCs (b = -0.11; P < 0.05) and cardiovascular complaints (b = -0.06; P < 0.05). However, the association between burden and SHCs was significant (P < 0.001) only for sedentary caregivers. CONCLUSION: These findings indicate that maintaining an active lifestyle through regular PA could potentially help alleviate the adverse effects of caregiver burden on somatic health among caregivers of PwD. Encouraging and endorsing PA interventions for informal caregivers might yield substantial advantages for their health and general well-being.

RESUMO O presente artigo relata a experiência de estágio em Psicologia Escolar e Educacional em um serviço-escola de Psicologia, localizado em uma universidade em Natal/RN. O projeto interventivo desenvolvido ao longo do período de estágio proporcionou encontros grupais abertos para as demandas dos estudantes. Assim, foram estabelecidas três etapas: 1) divulgação do projeto; 2) realização de três encontros interventivos; 3) momento avaliativo das ações executadas por parte da equipe de estágio. As intervenções elaboradas explicitaram a importância de a universidade proporcionar o acolhimento às queixas do cotidiano acadêmico, trazendo não apenas a ideia da qualidade de vida dentro da universidade, como também oferecendo suporte considerável para atender às necessidades dos estudantes.
RESUMEN En el presente artículo se busca relatar la experiencia de pasantía en Psicología Escolar y Educacional en un servicio-escuela de Psicología, localizado en una universidad en Natal/RN. El proyecto intervenido desarrollado a lo largo de la pasantía buscó proporcionar encuentros grupales abiertos a las demandas de los estudiantes. Así, se establecieron tres etapas: 1) divulgación del proyecto; 2) realización de tres encuentros intervenidos; 3) momento evaluativo de las acciones ejecutadas por parte del equipo de la pasantía. Las intervenciones elaboradas explicitaron la importancia de la universidad en proporcionar el acogimiento a las quejas del cotidiano académico, trayendo no solo la idea de la cualidad de vida dentro de la universidad, como también dando suporte considerable para atender a las necesidades de los estudiantes.
ABSTRACT This article seeks to report the internship experience in School and Educational Psychology at a Psychology school service, located at a university in Natal/RN. The interventional project that was developed throughout the internship period sought to provide open group meetings for the students' demands. Thus, three steps were established: 1) dissemination of the project; 2) three intervention meetings; 3) evaluative moment of the actions performed by the internship team. The elaborated interventions highlighted the importance of the university in providing the reception to the complaints of academic daily life, bringing not only the idea of quality of life inside the university, but also offering considerable support to meet the needs of the students.

BACKGROUND: Subjective memory complaints (SMCs) are a possible prodrome of cognitive decline but are understudied in low- and middle-income countries (LMIC). We aimed to estimate the prevalence of SMCs in a large, nationally representative sample of older adults from Brazil and to identify sociodemographic and health-related factors that are associated with SMCs independently of objective memory. METHODS: Baseline data (n = 7831) from the ELSI-Brazil study, a national representative sample of adults aged 50 and over. They were asked to rate their memory and then divided into two groups - having or not having SMCs. Logistic regression models were used to estimate the association of demographic characteristics, health related factors, objective memory performance and disability in basic (b-ADL), instrumental (i-ADL), and advanced (a-ADL) activities of daily living associated with SMCs. Whether sex was an effect modifier of the association between age and objective memory performance and SMCs was also tested. RESULTS: Of the sample, 42% (95% CI; 39.9-43.9) had SMCs, and it was higher among women (46.9%) than men (35.9%). SMC prevalence decreased with age among women and increased among men, and for both it decreased with better cognitive performance. Fully adjusted logistic regression model showed that older age, higher education, higher b-ADL scores, and better cognitive performance were associated with decreased SMCs, while being female, with higher number of chronic conditions, higher i-ADL scores, worst self-rated health, and an increased number of depressive symptoms were associated with increased SMCs. However, the interaction test (p < 0.001) confirmed that increased age was associated with decreased SMCs only among women, and that better objective memory performance was associated with decreased SMCs only among men. CONCLUSIONS: SMCs are common in the Brazilian older population and are associated with health and sociodemographic factors, with different patterns between men and women. There is a need for future studies looking at the cognitive trajectory and dementia risk in older adults with subjective cognitive complaints.

Desynchronization of circadian rhythms and sleep-wake patterns impacts biochemical, physiological, and behavioral functions, including mental processes. The complex relationship between circadian rhythms and mental health makes it challenging to determine causality between circadian desynchronization and mental disorders. Regarding the fact that psychologists act as the front line for initial mental health care, we aimed to assess the knowledge and use of sleep science and basic chronobiology by professional psychologists in Brazil. Data were collected via an online questionnaire completed by 1384 professional psychologists between October 2018 and May 2019. Our findings revealed that ±80% of psychologists reported that at least half of their patients presented some sleep-related complaints; however, only ±27% routinely inquired about sleep quality even in the absence of patient complaints. Additionally, only ±66% initiated treatments to understand these complaints, potentially influenced by the lack of prior academic exposure to biological rhythms as reported by ±76% of Brazilian psychologists interviewed. Importantly, ±15% did not believe in an association between mental health and biological rhythms, and even a significant ±67% were unfamiliar with the term chronobiology and ±63% were not able to describe any other biological rhythm except for the sleep-wake cycle. These results demonstrate that fundamental concepts in chronobiology and sleep science are unknown to a substantial proportion of Brazilian psychologists. In conclusion, we propose that this subject could be more effectively integrated into psychologists' academic training, potentially promoting benefits through the incorporation of a chronobiological approach in mental health practice.

OBJECTIVE: To estimate prefrailty and frailty prevalence and associated factors in people living with HIV (PLHIV) from a sex perspective. METHODS: Cross-sectional study on PLHIV at specialized public health centres in Brazil. Data were obtained from individuals aged ≥50 years using antiretroviral therapy (ART) and with an undetectable viral load through personal interviews, clinical evaluations and medical records. Frailty and prefrailty were characterized using the Fried Frailty Phenotype tool. Multinomial regression models were performed, and the associated factors were selected through the backward stepwise method. RESULTS: Among 670 patients, 373 men and 297 women were included. The prevalence of frailty and prefrailty was significantly higher for women (16.2% and 56.2%, respectively) than for men (11.5% and 46.4%, respectively). Low socioeconomic and educational level, multimorbidity, depression, subjective cognitive complaints, and low scores on the Mini-Mental State Exam (MMSE) were associated (P < 0.05) with frailty for both sexes. However, in the sex-specific analysis, while smoking (OR = 3.66, 95% CI: 1.58-8.48) and a history of low adherence to ART (OR = 3.10, 95% CI: 1.33-7.23) were associated with frailty in men, depression (OR = 3.39, 95% CI: 1.36- 8.44) and the absence of functional dentition (OR = 3.77, 95% CI: 1.36- 10.43) were associated with frailty in women. CONCLUSIONS: This study adds self-reported cognitive complaints as a potential predictor of frailty in both sexes and supports the known deleterious effect of multimorbidity on frailty in adults living with HIV. Furthermore, it suggests that other possible predictors, such as depression, oral health status and adherence to ART, may be sex-specific.

Approximately 30% of COVID-19 cases may experience chronic symptoms, known as post-COVID-19 syndrome (PCS). Common PCS symptoms can include fatigue, cognitive impairment, and persistent physical, neurological, and neuropsychiatric complaints. To improve healthcare and management of the current and future pandemics, we highlight the need for establishing interdisciplinary post-viral outpatient clinics comprised of specialists in fields such as psychiatry, psychotherapy, neurology, cardiology, pneumology, and immunology. In this way, PCS patients with a high health burden can receive modern diagnostics and targeted therapeutic recommendations. A key objective is to distinguish the "sick recovered" from the "healthy recovered." Our hypothesis is that there is a PCS subgroup with autoimmune-mediated systemic and brain-vascular dysregulation, which may lead to circulatory disorders, fatigue, cognitive impairment, depression, and anxiety. This can be clarified using a combination of specific antibody diagnostics and precise clinical, psychological, and apparative testing.

La presente investigación consiste en evaluar y comparar estadísticamente el número de casos de violencia intrafamiliar en Colombia entre los años 2010-2021, con base en los reportes de la Policía Nacional. La base de datos está compuesta por 896.017 casos de violencia denunciados. Se empleó distribución de frecuencias, análisis comparativo por el método GLM y proyección Biplot. Cundinamarca diverge estadísticamente respecto a las demás regiones evaluadas, donde las mujeres presentan el mayor porcentaje de violencia intrafamiliar. Factores como las restricciones de movilidad, la incertidumbre, el estrés entre otros componentes, son la raíz de situaciones complejas familiares.
The present research consists of evaluating and statistically comparing the number of cases of intrafamily violence in Colombia between the years 2010-2021, based on the reports of the National Police. The database is made up of 896,017 reported cases of violence. Frequency distribution, comparative analysis by the GLM method and Biplot projection were used. Cundinamarca differs statically with respect to the other regions evaluated, where women present the highest percentage of domestic violence. Factors, such as mobility restrictions, uncertainty, stress, among other components, are at the root of complex family situations.

R E S U M E N El artículo tiene como objetivo mostrar un análisis conciso de la importancia, dimensiones y relaciones de la innovación en el control gubernamental y la gestión de las denuncias ciudadanas, presente en revistas científicas y publicaciones de entidades como OLACEFS y diversas Contralorías, desde el 2018 al 2022. Para ello, se desarrolló una investigación no experimental, descriptiva y transversal; basada en una revisión bibliográfica en Redalyc, Scielo y Scopus, sobre artículos científicos relacionados a las variables mencionadas, obteniendo 53 publicaciones. Dentro de los principales resultados se encuentra la relación de herramientas de innovación con las variables de control gubernamental y denuncias ciudadanas, obteniendo 22 temáticas, siendo transversal el uso de TIC. Finalmente, se concluyó que la innovación es una variable relacionada con el control gubernamental y la gestión pública, contemplando la gestión de denuncias ciudadanas. Si bien es cierto que, no hay investigaciones puntuales que relacionen estas variables, hay muchas publicaciones que usan términos como TIC, innovación o gobierno abierto para mantener una correlación; contemplando a la participación ciudadana como parte del control social que ayuda al control gubernamental.
A B S T R A C T The article aims to show a concise analysis of innovation's importance, dimensions, and relationships in government control and the management of citizen complaints, present in scientific journals and publications of entities such as OLACEFS and various Comptrollerships, from 2018 to 2022. For this, a non-experimental, descriptive, and cross-sectional investigation was developed; based on a bibliographic review in Redalyc, Scielo, and Scopus, of scientific articles related to the mentioned variables, obtaining 53 publications. Among the main results is the relationship of innovation tools with the variables of government control and citizen complaints, getting 22 themes, the use of ICT being transversal. Finally it was concluded that innovation is a variable related to government control and public management, contemplating the direction of citizen complaints. Although it is true that there is no specific research that relates these variables, there are many publications that use terms such as ICT, innovation, or open government to maintain a correlation; contemplating citizen participation as part of the social control that helps government control.

Abstract Background and objectives: Anesthesiologists and hospitals are increasingly confronted with costs associated with the complications of Peripheral Nerve Blocks (PNB) procedures. The objective of our study was to identify the incidence of the main adverse events associated with regional anesthesia, particularly during anesthetic PNB, and to evaluate the associated healthcare and social costs. Methods: According to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we conducted a systematic search on EMBASE and PubMed with the following search strategy: (‟regional anesthesia" OR ‟nerve block") AND (‟complications" OR ‟nerve lesion" OR ‟nerve damage" OR ‟nerve injury"). Studies on patients undergoing a regional anesthesia procedure other than spinal or epidural were included. Targeted data of the selected studies were extracted and further analyzed. Results: Literature search revealed 487 articles, 21 of which met the criteria to be included in our analysis. Ten of them were included in the qualitative and 11 articles in the quantitative synthesis. The analysis of costs included data from four studies and 2,034 claims over 51,242 cases. The median claim consisted in 39,524 dollars in the United States and 22,750 pounds in the United Kingdom. The analysis of incidence included data from seven studies involving 424,169 patients with an overall estimated incidence of 137/10,000. Conclusion: Despite limitations, we proposed a simple model of cost calculation. We found that, despite the relatively low incidence of adverse events following PNB, their associated costs were relevant and should be carefully considered by healthcare managers and decision makers.

PURPOSE: Musculoskeletal complaints (MSCs), a leading contributor to disability worldwide, have a major impact on health-related quality of life (HRQoL). Poor general health related to lifestyle factors such as smoking, alcohol consumption and physical inactivity can lead to a higher risk to suffer MSCs. For minority groups in Suriname such as the Maroons and the Indigenous peoples no research has been conducted regarding prevalence of MSCs, HRQoL and various lifestyle factors. The aims were to determine the prevalence of MSCs and HRQoL in two rural tribal villages in the forested interior of Suriname and to identify various lifestyle factors associated with HRQoL in these communities. METHOD: This was a cross-sectional community-based study using the Community Oriented Program for the Control of Rheumatic Diseases stage 1, phase 1 & 2 methodology in Goejaba, a Maroon village and Galibi, an Indigenous rural village. Sociodemographic data, self-reported comorbidities, past MSCs (for longer than seven days), lifestyle factors including smoking, alcohol use, body mass index (BMI) and physical activity (PA), and HRQoL (using the 36-item Short Form Survey (SF-36)) data were gathered among 153 Indigenous individuals in Galibi, and 516 Maroons in Goejaba. Regression models were constructed to explore associations between presence of MSCs, lifestyle factors and HRQoL. RESULTS: High prevalence rates for past MSCs were reported in Galibi (72.4%) and Goejaba (58.3%). In both communities, respondents with MSCs reported significantly worse HRQoL than persons without MSCs. MSCs and the presence of comorbidities had a strong negative association with HRQoL, whereas PA positively influenced the physical and mental health domains of the SF-36. Smoking, alcohol use and BMI showed no association with HRQoL. CONCLUSIONS: In this first study, a high prevalence for MSCs was reported in an Indigenous and Maroon rural community in Suriname. MSCs and comorbidities had a significant negative impact on HRQoL. PA was associated with higher self-reported HRQoL.

INTRODUCTION: The Memory Complaint Scale (MCS-15) is a 15-item instrument to explore frequent forgetfulness in daily life in people with possible cognitive impairment. However, knowledge about its psychometric performance is limited. OBJECTIVE: The objective of this study was to know the dimensionality and internal consistency of the MCS-15 in Colombian older adults. METHODS: A probabilistic sample of 1,957 older adults from the general Colombian population was taken, aged between 60 and 98 years (mean = 71.0 ± 7.9), and 62.2% were women. Internal consistency (Cronbach's alpha and McDonald's omega) and dimensionality (exploratory and confirmatory factor analysis) were calculated for the original and ten-item versions. RESULTS: The 15-item version showed Cronbach's alpha and McDonald's omega of 0.91, and one dimension accounted for 45.3% of the variance. A version of ten items showed Cronbach's alpha and McDonald's omega of 0.89 and a single factor that explained 50.9% of the variance with better indicators in the confirmatory factor analysis. Convergence with the shortened Mini-Mental State Examination was rs = 0.43 (p < 0.001), and the Montreal Cognitive Assessment test was rs = 0.38 (p < 0.001). The nomological validity with the geriatric depression scale was rs = 0.44 (p < 0.001), and women scored higher than men (p < 0.001). CONCLUSIONS: The MCS-15 shows high internal consistency with poor dimensionality. However, a ten-item version shows high internal consistency and a clear one-dimensional structure. More research is needed: testing the performance against a structured interview for major cognitive impairment.

BACKGROUND: Several factors have been found to defend against pathologic cognitive decline in aging (i.e., cognitive reserve [CR]); however, other factors, including subjective memory complaints (SMC) and decreased functionality are considered early indicators of underlying neurocognitive dysfunction. Despite these known associations, the relationship between the presence of CR and SMC remains equivocal. This study sought to determine the relationship between objectively measured CR and SMC in a sample of functionally independent older women. METHODS: This cross-sectional study recruited women aged ≥60 years who attended fitness or continuing education programs at the University for Seniors in Mexico City. Participants underwent a battery of physical and cognitive evaluations, including the Cognitive Reserve Questionnaire (CRQ), and were asked probing questions used to identify the presence of SMC. RESULTS: The 269 participants had a median age of 69 years; most were single (40.5 %), lived alone (32.7 %), retired (58.2 %), well-educated (≥12 years of education), and functionally independent (89.2 %). 62 % scored "high" on the CRQ, while 9.3 % scored "low". After adjusting for multiple covariates, an independent association between CRQ score and the probability to have SMC was found (adjusted OR = 0.87, 95% CI 0.80-0.95, p-value = 0.002). CONCLUSIONS: This study identified a relationship between low CR and the presence of SMC, independently of the cognitive function and motoric marker of muscle strength (i.e., low gait speed and handgrip strength) in functionally independent older women over 60y. This relationship remains independent of other variables such as age, symptoms of depression and instrumented activities of daily living.

METHODS: According to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we conducted a systematic search on EMBASE and PubMed with the following search strategy: ("regional anesthesia" OR "nerve block") AND ("complications" OR "nerve lesion" OR "nerve damage" OR "nerve injury"). Studies on patients undergoing a regional anesthesia procedure other than spinal or epidural were included. Targeted data of the selected studies were extracted and further analyzed. RESULTS: Literature search revealed 487 articles, 21 of which met the criteria to be included in our analysis. Ten of them were included in the qualitative and 11 articles in the quantitative synthesis. The analysis of costs included data from four studies and 2,034 claims over 51,242 cases. The median claim consisted in 39,524 dollars in the United States and 22,750 pounds in the United Kingdom. The analysis of incidence included data from seven studies involving 424,169 patients with an overall estimated incidence of 137/10,000. CONCLUSIONS: Despite limitations, we proposed a simple model of cost calculation. We found that, despite the relatively low incidence of adverse events following PNB, their associated costs were relevant and should be carefully considered by healthcare managers and decision makers.

Background: Subjective cognitive complaints (SCC) are associated with higher risk of mild cognitive impairment (MCI) and dementia. Cardiovascular risk factors (CVRF) have been also associated with cognitive decline, MCI, and dementia. Few studies have examined the associated of CVRF and SCC. Methods: Participants were cognitively normal Mexican Americans from the HABLE study. Participants were categorized as with and without SCC, and SCC was also measured as a continuous variable. CVRF diagnosis were ascertained during consensus review. Cognitive measures used were MMSE, Trails B, SEVLT, and digit span. Logistic regression and linear regression were used to asses the association of SCC with CVRF and cognitive scores. Results: A total of 673 participants [mean age 63.3 (SD=7.71), 69.2% female] were included. SCC was present in 323 participants (47.99%). Dyslipidemia and depression were associated with SCC. Individuals with dyslipidemia had 1.72 times the odds (95% CI = 1.20 to 2.47) of SCC, and those with depression had 3.15 times the odds (95% CI = 2.16 to 4.59) of self-reporting SCC. Higher SCC scores, were significantly associated with MMSE (B = 0.07; SE = 0.03; p = 0.02), and SEVLT immediate and delayed (B= -0.03; SE = 0.00; p = 0.000 and B = -0.03; SE = 0.00; p = 0.000, respectively). Conclusions: In a cognitively normal Mexican Americans sample of older adults, depression and dyslipidemia were correlated with self-reported SCC. A greater self-perception of cognitive decline correlated with lower scores on the MMSE and SEVLT.

Background and objectives: An increase in emotional disturbances and complaints about cognitive performance has been observed in Latin American healthcare workers during the SARS-CoV-2 pandemic, which can affect attention capacity and increase the levels of stress and burnout of these professionals. The objective was to analyse subjective cognitive complaints (SCC) and associated factors in health personnel during the COVID-19 pandemic in five Latin American countries.Methods: Multicentre cross-sectional study, which included 3,738 professionals from Colombia, Chile, Argentina, Ecuador, Bolivia, and Peru. The Generalized Anxiety Disorder Scale (GAD-7) and the Patient Health Questionnaire (PHQ-9) was used to assess depression, and the Mini-Z to assess Burnout. For the SCC, an item on cognitive concerns in attention and memory was used.Results: The prevalence of cognitive complaints was 69.2%. The factors associated with a higher risk of SCC were the scores in the GAD-7, PHQ and Mini-Z, in addition to being part of the Ecuadorian health personnel.Conclusions: There is a high prevalence of SCC in health personnel, which is modulated by emotional states and stress.

ABSTRACT BACKGROUND: Dermatitis herpetiformis (DH) is considered a skin celiac disease (CD). The individuals can be seen by primary care professionals or by dermatologists that could refer the patient to a gastroenterologist. OBJECTIVE: The study aimed to investigate the clinical profile of patients diagnosed with DH and referred to a gastroenterologist and evaluate the treatment response. METHODS: We retrospectively studied patients with DH referred to the same gastroenterologist at a private office in Curitiba, Brazil, between January 2010 to December 2019. We included adult patients with a confirmed DH diagnosis. Symptoms, clinical signs, laboratory and histological data, as well as treatment response, were collected. RESULTS: Thirty-three patients were studied (60.6% women, mean age at diagnosis 40.8±12.61 years). The median delay for DH diagnosis was four years. Skin involvement was mild in 33.3%, moderate in 18.2%, and severe in 48.5%. The more frequent gastrointestinal complaints were abdominal distension (78.8%), flatulence (75.7%), and gastroesophageal reflux (51.5%). Depression and anxiety were observed in 81.8% and anemia in 51.1%. A higher prevalence of bone disorders was associated with higher age at DH diagnosis (P=0.035). Duodenal biopsy showed changes in all patients. Improvement after treatment only with a gluten-free diet (GFD) plus dapsone was verified in 81.2%. CONCLUSION: Patients with DH referred to a gastroenterologist showed a high frequency of gluten intolerance and systemic complaints. Duodenal histological alterations were found in all the cases. The treatment based on GFD plus dapsone was effective in most patients.
RESUMO CONTEXTO: A dermatite herpetiforme (DH) é considerada como a doença celíaca (DC) da pele. Os pacientes podem ser atendidos por profissionais do atendimento primário ou por dermatologistas que podem encaminhar o paciente a um gastroenterologista. OBJETIVO: Os objetivos do estudo foram investigar o perfil clínico dos pacientes com diagnóstico de DH encaminhados a um gastroenterologista e avaliar a resposta ao tratamento. MÉTODOS: Foram investigados retrospectivamente pacientes com DH encaminhados ao mesmo gastroenterologista em consultório particular em Curitiba, Brasil, entre janeiro de 2010 a dezembro de 2019. Foram incluídos pacientes adultos com diagnóstico confirmado de DH. Dados sobre sintomas e sinais clínicos, dados laboratoriais, histológicos e resposta ao tratamento foram coletados. RESULTADOS: Foram estudados 33 pacientes (60,6% mulheres, média de idade 40,8±12,61 anos). O atraso médio para o diagnóstico de DH foi de 4 anos. O envolvimento cutâneo foi considerado leve em 33,3%, moderado em 18,2% e grave em 48,5%. As queixas gastrointestinais mais frequentes foram distensão abdominal (78,8%), flatulência (75,7%) e refluxo gastroesofágico (51,5%). Depressão e ansiedade foram observadas em 81,8% e anemia em 51,1%. Maior prevalência de alterações ósseas foi associada à maior idade ao diagnóstico de DH (P=0,035). A biópsia duodenal mostrou alterações em todos os pacientes. A melhora após o tratamento apenas com dieta sem glúten e/ou dapsona foi verificada em 81,2%. CONCLUSÃO: Pacientes com DH encaminhados ao gastroenterologista apresentaram alta frequência de queixas gastrointestinais e sistêmicas. Alterações histológicas duodenais foram encontradas em todos os casos. O tratamento à base de dieta sem glúten e/ou dapsona foi eficaz na maioria dos pacientes.

BACKGROUND: The association between sleep duration and frailty remains unconclusive since most of the studies have been cross-sectional. Therefore, this study aimed to analyze the association between sleep duration, sleep complaints, and incident frailty. METHODS: A community-based cohort study from rural areas in Mexico with 309 older adults aged 70 and over. Data from waves two and three of the Rural Frailty Study were used. We operationalized the Fried frailty phenotype to describe prevalent and incident frailty at two consecutive waves. Sleep duration was classified as: ≤ 5 h, 6 h, 7-8 h, and ≥ 9 h; and the self-reported sleep complaints as a dichotomous variable. Analyses were performed using Poison regression models. RESULTS: The average age was 76.2 years and 55.3% were women; the incidence of frailty was 30.4%; 13.3% slept ≤5 h, and 38.5% ≥ 9 h. Compared with the group that slept 7-8 h, the risk of frailty at 4.4 years of follow-up was significantly higher among those who slept ≤5 h (adjusted RR 1.80, 95% CI: 1.04-3.11) and among those who slept ≥9 h (adjusted RR 1.69, 95% CI: 1.10-2.58). Sleep complaints were not associated with incident frailty (adjusted RR 1.41, 95% CI: 0.94-2.12). CONCLUSIONS: Our results show that short and long sleep duration are associated with the incidence of frailty. Studies that objectively evaluate sleep duration are needed to clarify whether meeting the recommended hours of sleep decreases frailty incidence.

Este artigo é resultado de uma pesquisa bibliográfica sobre a produção nacional a respeito do encaminhamento de crianças em processo de escolarização a profissionais de saúde, como expressão do processo de medicalização da educação. Realizou-se levantamento bibliográfico na base de dados Biblioteca Virtual em Saúde-Psicologia Brasil/BVS-Psi com os termos: medicalização, medicalização da educação, professores e encaminhamentos, queixa escolar, medicalização na rede pública, queixa escolar e medicalização e medicalização da queixa escolar. Selecionaram-se os textos que apresentavam uma perspectiva crítica a respeito dos processos de medicalização da/na educação. Os resultados desta pesquisa indicam que os encaminhamentos são realizados partindo-se de uma concepção de que as dificuldades apresentadas pelas crianças no processo de escolarização são de caráter individual e, dessa forma, passíveis de resolução no campo da saúde. Entretanto, há uma marcante falta de comunicação entre esses serviços e a escola, o que contribui para o recurso ao uso de drogas psicotrópicas como uma das principais medidas para intervir junto às queixas escolares, em detrimento do recurso a novas práticas institucionais da escola.
This article is the result of a bibliographical research on the Brazilian bibliographical production regarding the referral of children in schooling process to health professionals, as an expression of the medicalization process of education. A bibliographical survey was carried out in the database of the Virtual Library of Health/ Psychology Brazil/BVS-Psi under the terms: medicalization, medicalization of education, teachers and referrals, School complaint and medicalization and medicalization of the school complaint. The researchers selected the texts that presented a critical perspective regarding the medicalization processes of the education. The results indicate that the production of referrals rely on a conception that the difficulties presented by the children in the schooling process, as the school identifies them, are individual in nature and, therefore, can be resolved in the health field. However, there is a lack of communication between these services and the school, which contributes to the use of psychotropic drugs as one of the main measures to intervene with school complaints, rather than making new school practices in order to enhance the schooling process.
Este artículo es el resultado de una investigación bibliográfica sobre la producción brasileña acerca de la derivación de niños en proceso de escolarización a profesionales de salud, como expresión del proceso de medicalización de la educación. Se realizó una revisión de la literatura en la base de datos Biblioteca Virtual en Salud-Psicología Brasil / BVS-Psi con los términos: medicalización, medicalización de la educación, profesores y derivaciones, queja escolar, medicalización en la red pública, Queja escolar y medicalización y medicalización de la queja escolar . Se seleccionaron los textos que presentaban una perspectiva crítica acerca de los procesos de medicalización de la educación. Los resultados de esta investigación indican que las derivaciones se realizan partiendo de una concepción de que las dificultades presentadas por los niños en el proceso de escolarización son de carácter individual y de esa forma susceptibles de resolución en el campo de la salud. Sin embargo, hay una marcada falta de comunicación entre estos servicios y la escuela, lo que contribuye al uso de drogas psicotrópicas como una de las principales medidas para intervenir junto a las quejas escolares, en detrimento del recurso a nuevas prácticas institucionales de la escuela.
