RESUMEN

Most women who have had a mastectomy and have not opted for breast reconstruction choose to use an external breast prosthesis. This study aimed to assess the impacts of external breast prostheses on the body postures of women after unilateral mastectomies. An additional aim was to identify whether postural asymmetry depended on the side of mastectomy. This study involved 52 women after unilateral mastectomy and consisted of two parts: (1) anthropometric measurement and (2) assessment of body posture using the moiré topography method. The posturometric indices showed that the body posture of the subjects in the sagittal plane is characterized by forward trunk inclination and a tendency to excessive kyphosis. There were no significant differences between parameters characterizing body posture with and without external breast prosthesis. The lack of external breast prosthesis had a significant effect only on excessive forward trunk inclination. Significant differences were found in the posturometric parameters in the transverse plane between the groups of patients after left- and right-sided mastectomy. The obtained results did not fully confirm the hypothesis that the external breast prosthesis affects the body posture of women after unilateral mastectomy.

BACKGROUND: The lifestyle of children has a significant impact on the future health of the whole society. Therefore, health education, prevention and monitoring of health determinants is important at every stage of ontogenesis. This requires a thorough knowledge of the schoolchild's environment, perceived as a wide set of stressors, including not only genetic but also epigenetic factors. One of them is the issue of the correct and abnormal body posture at school and on the way there. The goal of the study was to show the influence of the weight of the back carried container with school supplies on body posture. METHOD: The research was carried out as part of a project that examined the impact of carrying weights, which are school supplies, by children on their posture. The research material consisted of data obtained from a group of 65 students (35 girls, 30 boys) aged 7 years. Body posture tests were carried out, using the projection moiré method in 4 positions: 1-habitual posture, 2-posture after 10-min of asymmetric axial load, 3-a posture after 1 min of the load removal, 4-a posture after two minutes of the load removal. Physical fitness was measured with the Sekita test. The obtained data were statistically analyzed. RESULTS: The significance of differences between the 1st and 2nd measurements was analyzed to determine the impact of the backpack load and the correlation with physical fitness, and to study its influence on the value of the differences in posture features. Considering the differences in the volume of posture features among boys between the 1st and 2nd measurement, the Wilcoxon's rank test showed a statistically significant difference in the range of all analyzed variables, except for the torso bend angle to the right (KNT+), where no statistically significant change was noted. A statistically significant difference in the volume of all analyzed variables was observed among the girls. CONCLUSIONS: Carrying school supplies on the back induces significant changes in the value of the features describing the body posture in the frontal plane. The greater weight of the container and carrying time, and intensity of physical effort is the greater the changes will be. Physical fitness has a various and sex-dependent influence on the value of changes in body posture features because of carrying school supplies. Among boys it significantly affects the asymmetry of the torso bend, shoulder height, the waist triangles height and width, whereas among girls it affects the asymmetry of the shoulders and the distance of the angles of the lower shoulder blades from the line of the spinous processes of the spine. Among boys the changes in the value of posture features are mostly influenced by endurance and speed, but strength, power and agility are of lower influence, whereas among girls only agility matters.

BACKGROUND: The gold standard in the clinical assessment of the extent of scoliosis is the Cobb method but the analysis also covers the changes that occur within the spine and trunk in relation to the transverse and sagittal planes. OBJECTIVE: The study aimed to estimate changes in the sagittal plane of the spine in girls with right-sided thoracic adolescent idiopathic scoliosis. METHODS: The study involved 29 girls with scoliosis (13.6 ± 0.6 years) and 36 healthy girls as the comparison group. The tests included an interview, clinical examination (with the use of Adam's test and the Cobb angle), and examination of shape of the spine surface with the use of the photogrammetric method and the projection moire effect. RESULTS: Lower values of the thoracic kyphosis parameters were found in the scoliosis group. Significant differences were noted for the parameters of the lumbosacral section α (p= 0.02), thoracolumbar section ß (p< 0.001), the upper thoracic section γ (p< 0.001), thoracolumbar and upper thoracic section ß+γ (p< 0.001) and the total size of α+ß+γ (p< 0.001) curvatures. CONCLUSIONS: The study showed that with an increase in the angular value of the thoracic curvature, the thoracic kyphosis decreased.

OBJECTIVE: The purpose of this study was to test the validity and determine the accuracy of surface topography in relation to photogrammetry for measuring the thoracic kyphosis angle in patients with scoliosis. METHODS: This was a prospective, cross-sectional study of diagnostic accuracy that followed the guidelines recommended by the Standards for Reporting Diagnostic Accuracy. We consecutively included 51 participants aged 7 to 18 years. Exclusion criteria were surgical treatment of the spine, neurological disease, lower limb discrepancy greater than 1.5 cm, and body mass index above 29 kg/m². Each participant was evaluated using both a surface topography scan and photogrammetry in random order. The measurement obtained through photogrammetry was used as a reference in this study. For statistical purposes, Pearson's correlation test, Bland-Altman graphical analysis, and the receiver operating characteristic curve (P < .05) were performed. RESULTS: The correlation between the measurements was strong and significant (r = 0.76, P < .001) with an average difference of 0.4° in the Bland-Altman analysis. The receiver operating characteristic curve area was excellent for hypokyphosis (93.4%) and good for hyperkyphosis (86.4%), both being significant (P < .005). CONCLUSION: The agreement and strong correlation between the 2 methods indicate the validity of surface topography to measure the thoracic kyphosis angle. The surface topography provides accurate measures for the thoracic kyphosis angle with cutoff points for hypo- (33.3°) and hyperkyphosis (40.8°) for individuals with scoliosis.

O objetivo deste estudo foi examinar a exequibilidade do uso da Técnica de Moiré de Sombra (TMS) em triagens periódicas no ambiente escolar e estabelecer o estado da prevalência de desvios posturais em uma escola pública do município do Rio de Janeiro. A amostra se compôs por 304 alunos. O exame da TMS baseou-se na diferença do número de franjas nas regiões dorsal e cintura escapular. A exequibilidade foi identificada considerando-se parâmetros preestabelecidos. Identificaram-se 225 sujeitos com diferença de franjas na região dorsal, e 224 com diferença de franjas na cintura escapular. Quanto à exequibilidade, a amostra foi examinada em sete dias e não houve dificuldades na execução da técnica, no processamento das imagens e nos materiais requeridos. Considerou-se a TMS exequível para triagens populacionais, permitindo exames periódicos em larga escala. A TMS pode ser uma estratégia na implementação de programas de saúde pública na escola, objetivando a melhora da qualidade de vida.


This study aimed to examine the feasibility of the Shadow Moiré Technique (SMT) in carrying out periodic school screenings and to identify the prevalence of postural disorders in students from a public school in Rio de Janeiro. The sample was comprised of 304 students. The SMT exam was based on the difference in the number of fringes in the dorsal and scapulothoracic regions. The feasibility of the SMT was identified based on predetermined parameters. Two hundred and twenty-five subjects were identified with fringe difference in the dorsal region and 224 were identified with fringe difference in the scapulothoracic region. Regarding feasibility, the sample was examined over the period of 7 days. There were no difficulties in obtaining the materials, in the use of the technique or in the processing of the images. SMT was considered feasible to screen the population and it allows for large-scale periodic examination. The SMT can be a strategy for implementing public health programs at school aiming at improving the quality of life.

Background Adolescent idiopathic scoliosis (AIS) is a potentially progressive deformity, and early detection is crucial for timely intervention. However, the methods and criteria justifying screening for pediatric scoliosis remain controversial. We have, therefore, independently developed a Digital Moiré (DM) as a tool for scoliosis screening. The purpose of this study was to assess the usefulness of DM for scoliosis screening. Methods From March 2016 to March 2017, 126 patients (18 boys, 108 girls, mean age: 13.2 ± 2.2 years) with AIS underwent radiographic imaging of their whole spine. We tested the accuracy and reliability of DM by categorizing the examination results as Class 0 (no abnormality), Class 1 (return visit in one year), and Class 2 (further examination needed) and determined the distribution of the population by Cobb angle. The intra/inter-rater reliability and receiver operating characteristic (ROC) analyses were used to categorize the patients with positive findings into Class 1 or 2. Results Regarding the population distribution per Cobb angle in each of the distributions, 11 patients (8.7%) were Class 0, of which nine and two patients had Cobb angle ≤ 10 ° and > 10 °, respectively. There were 20 (15.9% ) Class 1 cases, of which 17 and three had Cobb angle ≤ 10 ° and > 10 °, respectively. Of the 95 (75.4%) Class 2 cases, five and 90 had a Cobb angle of ≤ 10 ° and > 10 °, respectively. The receiver operating characteristic (ROC) analysis of patients with positive findings showed that the area under the curve (AUC), sensitivity, specificity, and false-positive rate were 0.76, 0.98, 0.53, and 0.47, respectively, when predicting Cobb angle > 10°. Intra-rater and inter-rater reliability were 0.73 and 0.70, respectively. Conclusions This study demonstrated the usefulness of DM for determining whether a child with AIS requires a follow-up observation such as radiograph. Our findings suggest that the novel DM shows high accuracy and reliability for scoliosis screening.

BACKGROUND: To quantify the morphological changes in the surface of the back of adolescents with idiopathic scoliosis as a result of treatment with braces and to correlate them with radiographic changes. MATERIAL AND METHODS: An analytical, cohort, prospective study on a sample of 31 adolescents with idiopathic scoliosis. We divided them into two groups: eleven treated with braces and twenty without them. Quantification of the deformity was performed on two separate occasions with an interval of one year using three systems: 1) angle of trunk rotation (scoliometer); 2) surface topography; 3) full spine X-rays. RESULTS: No statistically significant differences were detected between the initial and final assessment of the topographic and radiographic variables in the group with braces. In the untreated group, only in measures with the scoliometer significant differences were registered. A positive correlation was found between the Cobb angle difference of the main curve with two topographic variables that quantify the asymmetry in the axial and coronal plane, respectively. DISCUSSION: In following patients with scoliosis treated with braces, we should consider and evaluate not only radiographic parameters such as the Cobb angle, but also clinical and topographic parameters that quantify the external deformity of the back, as there is a clinical-radiographic discrepancy amply demonstrated in the literature. The improvement of the external shape of the back is a very important factor for the patient, and can influence a better completion of the orthopedic treatment. In our study, the scoliotic curve and external deformity of the back remained stable during the follow-up period in both treated and untreated patients.

En el grupo no tratado, sólo hubo diferencias significativas para las medidas del escoliómetro. Se encontró una correlación positiva entre la diferencia del ángulo de Cobb de la curva principal con la de dos variables topográficas que cuantifican la asimetría en el plano axial y en el coronal, respectivamente. DISCUSIÓN: En el seguimiento de pacientes con escoliosis tratados con corsé, se deben tener en cuenta y valorar no sólo parámetros radiográficos como el ángulo de Cobb, sino también parámetros clínicos y topográficos que cuantifiquen la deformidad externa de la espalda, ya que existe una discrepancia clínico-radiográfica demostrada de manera amplia en la literatura. La mejoría de la forma externa de la espalda es un factor muy importante para el paciente y es lo que va a percibir fundamentalmente, lo que puede influir en una mejor cumplimentación del tratamiento ortopédico. En nuestro estudio, la curva escoliótica, así como la deformidad externa de la espalda, se mantuvo estable en el período de seguimiento tanto en los pacientes tratados con corsé como en los no tratados.

BACKGROUND: Standing postural alignment in children with cerebral palsy is usually altered by central postural control disorders. The primary aim of this study is to describe body alignment in a quiet standing position in ambulatory children with bilateral cerebral palsy compared with children with typical development. METHODS: Fifty-eight children with bilateral cerebral palsy (aged 7-13years) and 45 age-matched children with typical development underwent a surface topography examination based on Moiré topography and were classified according to their sagittal postural profiles. FINDINGS: The following eight grouping variables were extracted using a data reduction technique: angle of trunk inclination, pelvic tilt, and lordosis, the difference between kyphosis and lordosis, angle of vertebral lateral curvature, shoulder inclination, and shoulder and pelvic rotation. According to the cluster analysis results, 25% of the participants were classified into Cluster 1, 9% into Cluster 2, 49% in Cluster 3, and 17% in Cluster 4. INTERPRETATION: Three different postural patterns emerged in accordance with the sagittal postural profiles in children with bilateral cerebral palsy and were defined as follows: 1) a lordotic postural pattern corresponding to forward-leaning posture; 2) a swayback postural pattern corresponding to backward-leaning posture; and 3) a balanced postural pattern corresponding to balanced posture.

STUDY DESIGN: Optical cross-sectional study. PURPOSE: To study the correlation between asymmetry of the back (measured by means of surface topography) and deformity of the spine (quantified by the Cobb angle). OVERVIEW OF LITERATURE: The Cobb angle is considered the gold standard in diagnosis and follow-up of scoliosis but does not correctly characterize the three-dimensional deformity of scoliosis. Furthermore, the exposure to ionizing radiation may cause harmful effects particularly during the growth stage, including breast cancer and other tumors. METHODS: Patients aged 13.15±1.96 years (range, 7-17 years; n=88) with Cobb angle greater than 10° were evaluated with X-rays and our back surface topography method through three variables: axial plane (DHOPI), coronal plane (POTSI), and profile plane (PC). Pearson's correlation was applied to determine the correlation between topographic and radiographic variables. One-way analysis of variance and Bonferroni correction were used to compare groups with different grades of scoliosis. Significance was set at p<0.01 and, in some cases, at p<0.05. RESULTS: We detected a positive, statistically significant correlation between Cobb angle with DHOPI (r=0.810) and POTSI (r=0.629) and between PC variables with thoracic kyphosis angle (r=0.453) and lordosis lumbar angle (r=0.275). In addition, we found statistically significant differences for DHOPI and POTSI variables according to the grade of scoliosis. CONCLUSIONS: Although the back surface topography method cannot substitute for radiographs in the diagnosis of scoliosis, correlations between radiographic and topographic parameters suggest that it offers additional quantitative data that may complement radiologic study.

[Purpose] This study aimed to develop and validate an assessment tool for Moiré topogram made specifically with the Shadow Moiré technique of the dorsum. [Subjects and Methods] In the analysis of topograms, frontal, sagittal, and transverse planes of individuals' dorsal were considered. Two instructor evaluators analyzed the topograms at different times in the day and on different days. The measurements of intra- and inter- (intra- and interday) reliability were calculated. [Results] Of the three planes analyzed, in all analyses, good (0.61 to 0.80) and/or excellent (0.81 to 1) concordances were observed. [Conclusion] This analysis framework can be recommended to evaluate the topograms obtained with SMT.

PURPOSE: The aim of this study is to compare the accuracy and reliability of spinal curvatures and vertebral rotation data based on patient-specific 3D models created by 3D imaging system or by bi-planar imaging coupled with Moiré-Fringe projections. METHODS AND MATERIALS: Sixty-two consecutive patients from a single institution were prospectively included. For each patient, frontal and sagittal calibrated low-dose bi-planar X-rays were performed and coupled simultaneously with an optical Moiré back surface-based technology. The 3D reconstructions of spine and pelvis were performed independently by one radiologist and one technician in radiology using two different semi-automatic methods using 3D radio-imaging system (method 1) or bi-planar imaging coupled with Moiré projections (method 2). Both methods were compared using Bland-Altman analysis, and reliability using intraclass correlation coefficient (ICC). RESULTS: ICC showed good to very good agreement. Between the two techniques, the maximum 95 % prediction limits was -4.9° degrees for the measurements of spinal coronal curves and less than 5° for other parameters. Inter-rater reliability was excellent for all parameters across both methods, except for axial rotation with method 2 for which ICC was fair. Method 1 was faster for reconstruction time than method 2 for both readers (13.4 vs. 20.7 min and 10.6 vs. 13.9 min; p = 0.0001). CONCLUSION: While a lower accuracy was observed for the evaluation of the axial rotation, bi-planar imaging coupled with Moiré-Fringe projections may be an accurate and reliable tool to perform 3D reconstructions of the spine and pelvis.

OBJECTIVE: The aim of this study was to assess existing differences in the isokinetic trunk muscle strength in males and females aged between 10 and 11 years depending on body posture. METHODS: The study included 145 children (67 males and 78 females) divided into 2 age groups: 10-year-old males (x¯ = 9.98 ± 2.34 years) and females (x¯ = 9.85 ± 2.94 years) and 11-year-old males (x¯ = 11.14 ± 2.22 years) and females (x¯ = 11.15 ± 2.32 years). Posture in the sagittal plane was assessed by photogrammetry using the moiré projection technique. Based on a classification system, the participants were divided into subgroups of males and females with normal and abnormal postures. Trunk muscle strength was measured using isokinetic dynamometry. RESULTS: A high prevalence of abnormal posture in children aged between 10 and 11 years was observed, primarily represented by an excessive curvature of the spine in the sagittal plane. The males and females with poor posture recorded lower values in isokinetic trunk muscle strength. CONCLUSION: The results of the study point to the need for the application of suitable physiotherapy treatment (corrective measures/exercises) to treat musculoskeletal disorders to compensate for the loss of trunk flexor muscle strength in children with improper posture.

STUDY DESIGN: Retrospective cross-sectional study. PURPOSE: To determine the prevalence of idiopathic scoliosis, define the distribution of the curve magnitude, evaluate the accuracy of Moiré topography as a screening tool, and investigate the cost-effectiveness of our screening system. OVERVIEW OF LITERATURE: Early detection of idiopathic scoliosis provides the opportunity for conservative treatment before the deformity is noticeable. We believe that scoliosis screening in schools is useful for detection; however, screening programs are controversial owing to over referral of students who do not require further testing or follow-up. In Japan, school scoliosis screening programs are mandated by law with individual policies determined by local educational committees. We selected Moiré topography as the scoliosis screening tool for schools in Nara City. METHODS: We selected Moiré topography as the scoliosis screening tool for schools in Nara City. We screened boys and girls aged 11-14 years and reviewed the school scoliosis screening results from 1990 to 2012. RESULTS: A total of 195,149 children aged 11-14 years were screened. The prevalence of scoliosis (defined as ≥10° curvature) was 0.057%, 0.010%, and 0.059% in fifth, sixth, and seventh grade boys and 0.337%, 0.369%, and 0.727% in fifth, sixth, and seventh grade girls, respectively. The false-positive rate of our Moiré topography was 66.7%. The minimum cost incurred for scoliosis detection in one student was 2,000 USD. CONCLUSIONS: The overall prevalence of scoliosis was low in the students of Nara City schools. Over 23 years, the prevalence of scoliosis in girls increased compared to that in the first decade of the study.

BACKGROUND: Several different strategies for maintaining upright standing posture in children with cerebral palsy (CP) were observed. PURPOSE: The purpose of the present study was to define two different postural patterns in children with unilateral CP, using moiré topography (MT) parameters. Additionally, another focus of this article was to outline some implications for managing physiotherapy in children with hemiplegia. PATIENTS AND METHODS: The study included 45 outpatients with unilateral CP. MT examinations were performed using a CQ Elektronik System device. In addition, a weight distribution analysis on the base of support between unaffected and affected body sides was performed simultaneously. A force plate pressure distribution measurement system (PDM-S) with Foot Print software was used for these measurements. RESULTS: THE CLUSTER ANALYSIS REVEALED FOUR GROUPS: cluster 1 (n=19; 42.22%); cluster 2 (n=7; 15.56%); cluster 3 (n=9; 20.00%); and cluster 4 (n=10; 22.22%). CONCLUSION: BASED ON THE MT PARAMETERS (EXTRACTED USING A DATA REDUCTION TECHNIQUE), TWO POSTURAL PATTERNS WERE DESCRIBED: 1) the pro-gravitational postural pattern; and 2) the anti-gravitational pattern.

Moire's Topography (MT) is a method that detects scoliosis through shadows assymetry arised in the back. Few studies have discussed time of application, interpretation and operating cost. The aim of this study was to analyze operating aspescts of MT in students and to verify postural alterations by MT in the same group. 58 boys were analyzed in this study and through MT, the presence of shadow (fringes) in the back were evaluated where each assymetry corresponds to about 10º in Cobb's angle. MT marks were used to determine possible deviation. Data of interest received descriptive statistic analysis in variables such as total body mass, body mass index (BMI), weight, scoliosis, average time of analysis and diagnostic, and also the operation cost evaluation of equipment. Subjects (9,91 ± 0,79 years) showed total body mass of 37,83 ± 10,45kg, stature 1,42 ± 0,11m and BDI 18,53 ± 4,15 kg/m². The average time of analysis was 3,25 ± 0,29min. Among subjects 75% had deviation of one fringe in toracolumbar region that shows a scoliosis less than 10º. MT represented a fast method and of low operation cost that could be an important instrument in the screening of scoliosis. We concluded that MS confirmed to be practical and of easy handling where 94,6% subjects showed scoliosis. Our data suggests that the MSP insertion in the students' health care program of scoliosis identification is possible...(AU)


A Estereofotografia de Moiré (EFM) é um método que através da assimetria das sombras formadas no dorso detecta escoliose. Poucos os estudos tem discutido o tempo de análise, interpretação e custo operacional desta técnica. Os objetivos deste trabalho foram analisar os aspectos operacionais da (EFM) em escolares e; verificar desvios posturais pela EFM nesse mesmo grupo. Neste estudo foram analisadas 58 meninos e através da EFM, foram avaliadas a presença de sombras (franjas) no dorso, onde cada assimetria corresponde cerca de 10º no ângulo de Cobb. As marcações do EFM foram utilizadas para determinar os possíveis desvios. Os dados interesse receberam tratamento estatístico descritivo nas variáveis massa corporal total, índice de massa corporal (IMC), estatura, escoliose, tempo médio de análise e diagnóstico e, uma avaliação do custo operacional do equipamento. Os sujeitos (9,91 ± 0,79 anos) apresentaram uma massa corporal total de 37,83 ± 10,45kg, estatura 1,42 ± 0,11m e o IMC de 18,53 ± 4,15 kg/m². O tempo médio para análise foi de 3,25 ± 0,29min. Na amostra, 75% tiveram um desvio de até uma franja na região toracolombar o que indica uma escoliose menor que 10º. A EFM constituiu um método rápido e de baixo custo operacional podendo ser um instrumento importante no rastreamento de escoliose. Concluímos que a EFM de confirmou ser prático e de fácil manuseio que 94,6% dos sujeitos apresentaram escoliose.
