BACKGROUND: Oral cancer screening requires analysis capable of detecting changes preceding the clinical manifestation. Oral cytopathology studies as an oral cancer screening have shown promising results. This study aims to evaluate the use of cytopathology in a quali-quantitative analysis using Papanicoloau staining. METHODS: Four experimental groups were evaluated: control (CG), exposed to carcinogens (EG), potentially malignant disorder with and without epithelial dysplasia (D-OPMDG and ND-OPMDG) and oral squamous cell carcinoma (OSCCG). Oral smears were collected using a Cytobrush® and conventional exfoliative cytology. RESULTS: Oral Papanicolaou smears from 143 individuals were analysed in 8100 images. Qualitatively, non-lesional groups exhibited minimal suspected cases (20% in CG and 5% in EG), in the OPMD groups the ability to identify altered cells was low (40% in D-OPMDG and 0% in ND-OPMDG). Conversely, a notable 100% accuracy was achieved in the OSCCG. Quantitatively, a higher percentage of anucleated and a lower percentage of intermediate cells were observed in the OPMDG; a higher proportion of parabasal/suspicious cells was observed in OSCCG. The optimal threshold for improved accuracy in detecting suspected malignancies occurs when the count of parabasal/suspicious cells exceeds 8/100 cells examined. Cytomorphometric analysis revealed a higher nucleus/cytoplasm ratio (N/C) in OSCCG, with a best cutoff point indicating enhanced accuracy in discerning suspected malignancies when exceeding 17% of cells exhibiting this elevated ratio. CONCLUSION: It is possible to suggest updates in cytomorphometric and quantitative analysis in the modified Bethesda system for the oral cavity including objective criteria in the risk classification, therefore improving oral cancer screening.

Objective: To compare the adequacy, agreement, and acceptability of Papanicolaou testing (cytology) for cervical cancer screening using self-collected samples compared to physician-collected samples in Grenada in the Caribbean. Furthermore, the study identifies the human papillomavirus (HPV) genotypes present among asymptomatic women testing positive for HPV, the etiologic cause of cervical cancer. Methods: Participants were divided into two groups and two cervical samples were collected from the women in each group: a self-collected sample and a physician-collected sample. Cervical specimens were tested for cytology and HPV. HPV genotyping was performed on positive specimens. Results: Self-collected samples were adequate and in agreement with physician-collected samples, showing no difference between the two sampling methods. Oncogenic high-risk HPV genotypes were identified in cervical samples which were positive for atypical squamous cells and low-grade squamous intraepithelial lesions. The high-risk HPV genotypes found, notably HPV 45 and 53, differed from those most commonly reported. Although the commonly reported high-risk genotypes HPV 16 and 18 were found, so were 31, 33, 35, 52, 66, 68, and 82. Conclusions: Using self-collection facilitated the discovery of unexpected HPV genotypes among asymptomatic women in Grenada. These findings add new information to the literature regarding cervical cancer and neoplasia screening and HPV genotypes in the Caribbean. This genotype information may impact surveillance of women with low-grade lesions, HPV vaccine selection, and possibly further vaccine research. Research regarding HPV in Caribbean pathology samples of cervical neoplasia and cancer is needed.

BACKGROUND: Breast and cervical cancer are major public health issues globally. The reduction in incidence and mortality rates of these cancers is linked to effective prevention, early detection, and appropriate treatment measures. This study aims to analyze the temporal trends in the prevalence of mammography and Papanicolaou test coverage among women living in Brazilian state capitals between 2007 and 2023, and to compare the coverage of these tests before and during the Covid-19 pandemic. METHODS: A time series study was conducted using data from the Surveillance System for Risk and Protective Factors for Chronic Diseases by Telephone Survey from 2007 to 2023. The variables analyzed included mammography and Papanicolaou test coverage according to education level, age group, race/skin color, regions, and Brazilian capitals. The Prais-Winsten regression model was used to analyze the time series, and Student's t-test was employed to compare the prevalence rates between 2019 and 2023. RESULTS: Between 2007 and 2023, mammography coverage showed a stationary trend (71.1% in 2007 and 73.1% in 2023; p-value = 0.75) with a declining trend observed among women with 12 years or more of education (APC= -0.52% 95%CI -1.01%; -0.02%). Papanicolaou test coverage for all women aged between 25 and 64 exhibited a downward trend from 82% in 2007 to 76.8% in 2023 (APC= -0.45% 95%CI -0.76%; -0.13%). This decline was also noticed among those with 9 years or more of education; in the 25 to 44 age group; among women with white and mixed race; and in the Northeast, Central-West, Southeast, and South regions. When comparing coverage before and during Covid-19 pandemic, a reduction was noted for both tests. CONCLUSIONS: Over the years, there has been stability in mammography coverage and a decline in Papanicolaou test. The COVID-19 pandemic negatively impacted the number of these tests carried out among women, highlighting the importance of actions aimed at increasing coverage, especially among the most vulnerable groups.

Introducción: La prueba de Papanicolaou estudia las células exfoliadas de la unión escamo-columnar del cuello uterino para la detección de células anómalas y también es útil para el diagnóstico de infecciones cervicovaginales. Objetivo: Caracterizar a las pacientes con infecciones cervicovaginales asintomáticas, diagnosticadas por prueba de Papanicolaou. Métodos: Se realizó una investigación observacional, descriptiva y transversal, en 656 pacientes con infección cervicovaginal asintomática, diagnosticadas por prueba de Papanicolaou. Se estudiaron: gérmenes, edad, inicio de las relaciones sexuales, número de compañeros sexuales, anomalías de células epiteliales cervicales. La información se obtuvo mediante interrogatorio, examen clínico y prueba de Papanicolaou. Se utilizaron técnicas de estadística descriptiva para el cálculo de las frecuencias absolutas, porcentajes, media y desviación estándar. Resultado: Se halló un predominio de la vaginosis bacteriana (46 %), seguido de la infección por Candida spp (38,2 %). La edad media de las pacientes fue de 36,4 años. Se observó que en 244 mujeres la edad de inicio de las relaciones sexuales fue de 16 - 20 años (37,2 %). Tuvieron un compañero sexual activo 531 casos (80,9 %). El 86 % no presentó lesiones cervicales, pero el 72,7 % de las infecciones por VPH sí tuvieron. Conclusiones: La generalidad de las mujeres tiene vaginosis bacteriana, son adultas, con inicio precoz de las relaciones sexuales y un compañero sexual activo. Gran parte no presentan lesiones cervicales, la mayoría, con infección por VPH, poseen anomalías de las células epiteliales cervicales.
Introduction: The Papanicolaou test studies the exfoliated cells of the squamo-columnar junction of the cervix for the detection of abnormal cells and is also useful for the diagnosis of cervicovaginal infections. Objective: To characterize patients with asymptomatic cervicovaginal infections diagnosed by Papanicolaou test. Methods: An observational, descriptive and cross-sectional investigation was carried out in 656 patients with asymptomatic cervicovaginal infection, diagnosed by Papanicolaou test. The following were studied: germs, age, initiation of sexual relations, number of sexual partners, anomalies of cervical epithelial cells. The information was obtained through interrogation, clinical examination and Papanicolaou test. Descriptive statistical techniques were used to calculate absolute frequencies, percentages, mean and standard deviation. Result: A predominance of bacterial vaginosis was found (46%), followed by Candida spp infection (38.2%). The average age of the patients was 36.4 years. It was observed that in 244 women the age at which sexual relations began was 16 - 20 years (37.2%). 531 cases (80.9%) had an active sexual partner. 86% did not present cervical lesions, but 72.7% of HPV infections did. Conclusions: The majority of women have bacterial vaginosis, they are adults, with early onset of sexual relations and an active sexual partner. Most of them do not have cervical lesions, the majority, with HPV infection, have anomalies of the cervical epithelial cells.

Resumen OBJETIVO: Determinar las barreras personales, culturales e institucionales asociadas con la falta de interés de las pacientes en conocer (recoger) el reporte de la prueba de Papanicolaou. MATERIALES Y MÉTODOS: Estudio observacional, analítico, de casos y controles efectuado en el Centro de Salud Moyopampa, distrito de Lurigancho-Chosica, en Lima, Perú, durante diciembre del 2022. La muestra fue de casos (pacientes que no recogieron el reporte de la última prueba de Papanicolaou practicada en los dos últimos años) y controles (pacientes que sí acudieron a recibir su reporte). Parámetros de estudio: barreras personales, culturales e institucionales registradas en un cuestionario previamente validado. La asociación entre variables se estimó con χ² de Pearson y prueba exacta de Fisher, razón de momios con intervalo de confianza del 95% y p < 0.05. RESULTADOS: Se estudiaron 138 casos y 138 controles. En relación con las barreras personales, la edad de 25 a 35 años se asoció, significativamente, con el desinterés para recoger los reportes (RM = 0.021; IC95%: 0.35-0.92; p = 0.021). El carecer de estudios o solo haber cursado el nivel primario se asociaron con el desinterés para recoger los reportes de la prueba de Papanicolaou (RM = 2.83; IC95%: 1.30-6.15; p = 0.007); no haber oído hablar del virus del papiloma humano (VPH) (RM = 2.56; IC95%: 1.17-5.60; p = 0.016) y tener dificultad por el horario de trabajo (RM = 2.01; IC95%: 1.23-3.27; p = 0.005). CONCLUSIONES: Las barreras personales, como un escaso grado de instrucción, la falta de conocimiento del virus del papiloma humano y las dificultades relacionadas con el horario de trabajo, aumentaron la probabilidad de no acudir a recibir el informe de la prueba.
Abstract OBJECTIVE: To identify the personal, cultural and institutional barriers associated with patients' lack of interest in knowing (obtaining) the report of the Papanicolaou test. MATERIALS AND METHODS: Observational, analytical, case-control study carried out in the Moyopampa Health Center, district of Lurigancho-Chosica, in Lima-Peru, during December 2022. The sample consisted of cases (patients who did not obtain the report of the last Papanicolaou test performed in the last two years) and controls (patients who came to obtain their report). Study parameters: personal, cultural and institutional barriers recorded in a previously validated questionnaire. The association between variables was estimated with Pearson's χ² and Fisher's exact test, odds ratio with 95% confidence interval and p < 0.05. RESULTS: 138 cases and 138 controls were examined. Regarding personal barriers, age 25-35 years was significantly associated with disinterest in reporting (MR = 0.021; 95%CI: 0.35-0.92; p = 0.021). RESUMEN Objetivo. Comparar la idoneidad, concordancia y aceptabilidad de la prueba de Papanicolaou (citología) para el tamizaje del cáncer cervicouterino mediante la comparación de muestras obtenidas con automuestreo y muestras tomadas por personal médico en Granada, en el Caribe. Asimismo, en el estudio se identifican los genotipos del virus del papiloma humano (VPH) existentes en las mujeres asintomáticas con un resultado positivo en las pruebas del VPH, la causa etiológica del cáncer cervicouterino. Métodos. Las participantes se dividieron en dos grupos y se tomaron dos muestras cervicouterinas de las mujeres de cada grupo: una muestra tomada por la propia paciente y una muestra tomada por personal médico. Se realizó un examen citológico y una prueba de detección del VPH en las muestras. En las muestras positivas, se determinó el genotipo del VPH. Resultados. Las muestras tomadas por las propias pacientes fueron adecuadas y concordaron con las obtenidas por el personal médico, sin que se observaran diferencias entre ambos métodos de muestreo. Se identificaron genotipos de VPH de alto riesgo oncogénico en muestras cervicouterinas positivas para células escamosas atípicas y lesiones intraepiteliales escamosas de grado bajo. Los genotipos de VPH de alto riesgo encontrados, en especial VPH 45 y 53, diferían de los notificados con mayor frecuencia. Aunque se encontraron los genotipos de alto riesgo habituales 16 y 18 del VPH, también se encontraron los genotipos 31, 33, 35, 52, 66, 68 y 82. Conclusiones. El uso del automuestreo facilitó la detección de genotipos inesperados del VPH en mujeres asintomáticas de Granada. Estos resultados agregan nueva información a la bibliografía sobre el tamizaje de las neoplasias y el cáncer cervicouterino, así como sobre los genotipos del VPH, en el Caribe. Esta información sobre el genotipo puede repercutir en la vigilancia de las mujeres con lesiones de bajo grado, en la elección de la vacuna contra el VPH y, posiblemente, en las ulteriores investigaciones sobre vacunas. Es necesario investigar la presencia del VPH en muestras anatomopatológicas de neoplasias y cánceres cervicouterinos en el Caribe.
RESUMO Objetivo. Comparar a adequação, o nível de concordância e a aceitabilidade do exame de Papanicolau (citologia) para o rastreamento do câncer do colo do útero usando amostras autocoletadas em comparação com amostras coletadas por médicos em Granada, no Caribe. Além disso, o estudo identifica os genótipos de papilomavírus humano (HPV) presentes entre as mulheres assintomáticas com resultado positivo para HPV, a causa etiológica do câncer do colo do útero. Métodos. As participantes foram divididas em dois grupos, e duas amostras cervicais foram coletadas das mulheres de cada grupo: uma amostra autocoletada e uma amostra coletada por um médico. As amostras cervicais foram submetidas a exames citológicos e de HPV. A genotipagem do HPV foi realizada nas amostras positivas. Resultados. As amostras autocoletadas eram adequadas e compatíveis com as amostras coletadas por médicos, não havendo diferença entre os dois métodos de amostragem. Foram identificados genótipos de HPV de alto risco oncogênico em amostras cervicais positivas para células escamosas atípicas e lesões intraepiteliais escamosas de baixo grau. Os genótipos de HPV de alto risco encontrados, principalmente HPV 45 e 53, não correspondiam aos genótipos registrados com mais frequência na literatura. Embora os genótipos de alto risco HPV 16 e 18, que são frequentemente registrados, tenham sido observados, também foram detectados os genótipos 31, 33, 35, 52, 66, 68 e 82. Conclusões. O uso da autocoleta facilitou a detecção de genótipos inesperados de HPV entre mulheres assintomáticas em Granada. Esses achados adicionaram novas informações à literatura sobre o rastreamento de neoplasias e câncer do colo do útero e sobre os genótipos de HPV no Caribe. Essas informações genotípicas podem afetar a vigilância de mulheres com lesões de baixo grau, a seleção da vacina contra o HPV e, possivelmente, futuras pesquisas sobre vacinas. É necessário pesquisar o HPV em amostras patológicas de neoplasias cervicais e câncer do colo do útero no Caribe.

Abstract Objective To compare cytological and histological results from women > 64 years old who followed the Brazilian national cervical cancer screening guidelines with those who did not. Methods The present observational retrospective study analyzed 207 abnormal cervical smear results from women > 64 years old in a mid-sized city in Brazil over 14 years. All results were reported according to the Bethesda System. The women were divided into those who followed the screening guidelines and those who did not. Results Atypical squamous cells of undetermined significance and low-grade squamous intraepithelial lesion cytology results were found in 128 (62.2%) cases. Of these, 112 (87.5%) had repeated cytology with positive results. The other 79 (38.1%) with abnormal results should have been referred to colposcopy and biopsy. Out of 41 (51.9%) biopsied women, 23 (29.1%) had a confirmed diagnosis of neoplasia or precursor lesion. In contrast, among the 78 (37.7%) biopsied patients, 40 (51.3%) followed the guideline recommendations, with 9 (22.5%) positive biopsies. Of the 38 (48.7%) women who did not follow the guidelines, there were 24 (63.1%) positive results. Women who did not follow the guidelines demonstrated higher chances of cancer and precursor lesions (odds ratio [OR]: 5.904; 95% confidence interval [CI]: 2.188-15.932; p = 0.0002). Conclusion Women > 64 years old who did not follow the national screening protocol showed significant differences in the frequency of abnormal results and severity of diagnosis compared with those who followed the protocol.
Resumo Objetivo Comparar os resultados citológicos e histológicos de mulheres > 64 anos que seguiram as diretrizes nacionais brasileiras de rastreamento do câncer do colo do útero com aquelas que não as seguiram. Método O presente estudo observacional retrospectivo analisou 207 resultados anormais de esfregaço cervical de mulheres > 64 anos de idade em uma cidade de médio porte no Brasil durante 14 anos. Todos os resultados foram relatados de acordo com o Sistema Bethesda. As mulheres foram divididas entre as que seguiram as diretrizes de rastreamento e as que não o fizeram. Resultados Resultados citológicos com células escamosas atípicas de significado indeterminado e lesão intraepitelial escamosa de baixo grau foram encontrados em 128 (62,2%) casos. Destes, 112 (87,5%) repetiram a citologia com resultados positivos. Os outros 79 (38,1%) com resultados anormais deveriam ter sido encaminhados para colposcopia e biópsia. Das 41 (51,9%) mulheres biopsiadas, 23 (29,1%) tiveram diagnóstico confirmado de neoplasia ou lesão precursora. Em contrapartida, entre as 78 (37,7%) pacientes biopsiadas, 40 (51,3%) seguiram as recomendações da diretriz, com 9 (22,5%) biópsias positivas. Entre as 38 (48,7%) mulheres que não seguiram as orientações, houve 24 (63,1%) resultados positivos. As mulheres que não seguiram as diretrizes demonstraram maiores chances de câncer e lesões precursoras (odds ratio [OR]: 5,904; intervalo de confiança [IC] de 95%: 2,188-15,932; p = 0,0002). Conclusão Mulheres > 64 anos que não seguiram a diretriz nacional de rastreamento apresentaram diferenças significativas na frequência de resultados anormais e gravidade do diagnóstico em comparação com aquelas que seguiram a diretriz.

Investigar a percepção do homem transexual sobre acesso e utilização da atenção ginecológica em serviços da Atenção Primária à Saúde no SUS. Estudo qualitativo e descritivo. A população foi de cinco participantes; todos homens trans, residentes em municípios do estado de Minas Gerais, Brasil. A coleta de dados foi realizada por meio de entrevistas virtuais, entre o final de 2021 e início de 2022. Para análise dos dados obtidos, a técnica de Bardin foi utilizada. O desrespeito e o despreparo dos profissionais de saúde no atendimento foram indicados em todas as entrevistas, dificultando o acesso à saúde da população LGBTQIAP+. Baixa procura da população trans pelo serviço de saúde por receio e medo de (re)viver experiências traumatizantes indica que o enfermeiro de serviços de Atenção Primária à Saúde necessita desenvolver conhecimento e habilidades para acolher a população transexual, a fim de garantir humanização do atendimento.
To investigate the perception of transgender men of access and use of gynecological care in Primary Health Care services in SUS. A qualitative, descriptive study. A population of five participants, all transgender men, living in cities of the state of Minas Gerais, Brazil. Data collection occurred via online interviews between late 2021 and early 2022. To analyze data, the Bardin technique was used. The lack of respect and preparation of health professionals in service were mentioned in all interviews. This hindered access of the LGBTQIAP+ population. Low demand from the transgender population for health services due to dread and fear of (re)experiencing traumatizing situations suggests that the Primary Health Care services professional must build knowledge and skills to welcome the transgender population in order to guarantee humanized services.

Objectives: The Papanicolaou's (Pap's) stain is used for cervical cancer screening. It employs toxic-carcinogenic expensive reagents, which may not be easily accessible to many communities worldwide. The objective of this study was to validate the grape-based alcohol-extracted dye (Vinatela) on normal cervical samples for the Pap test. Material and Methods: Samples of the two grape species were collected from two vineyards through the Agroindustrial Research Institute of Universidad Privada San Juan Bautista. The dye extraction from the grape species and the dye performance to stain cells were conducted in three phases: (a) direct staining with pre-fermentation wine products, (b) direct fragmentation of grapes and direct staining with shells of the grapes, and (c) alcoholic extraction of the dye. Vinatela obtained from two species (Vitis vinifera "Tempranillo" and "Malbec") and posterior staining of cervical samples. We conducted a double-blind validation on 30 cervical samples. Results: The basophilic components of the cervical cells were stained. Alcoholic extraction staining protocol had a low yield. The nuclear and cytoplasmic borders, the nuclear details, and the polymorphonuclear nuclei were stained with Vinatela and could be differentiated during nuclear coloration. The initial staining protocol was 10- 20 min × (mean ~12 min) staining time. We noted a slightly better staining with V. v. Tempranillo as compared to V. v. Malbec (P = 0.045). Conclusion: Cervical cells stained with Vinatela stain from two grape species cultivated in the Southern of Peru, showed basophilic nuclear details.

Introduction: Preventive screening for cervical cancer is the best available strategy to reduce the incidence and mortality from this neoplasm. However, the low proportion of women who undergo routine screening is a pending concern for healthcare systems worldwide. Objective: To estimate the prevalence and factors associated with preventive cervical cancer screening in a Peruvian region. Methods: Cross-sectional, multicenter study. It enrolled 1146 women users of healthcare centers in a Peruvian sanitary region. The dependent variable was the performance of cervical cancer preventive screening with Papanicolaou or visual inspection with acetic acid in the last two years. The independent variables were sociodemographic and socio-sanitary factors, knowledge about cervical cancer and human papillomavirus, attitudes and information towards screening tests. To evaluate the association between the variables, crude and adjusted prevalence ratios were calculated with generalized linear models of Poisson. Results: The overall prevalence of preventive screening was 50.5%. This was associated with being tested, having a higher education level and urban area of residence, using contraceptive methods, having health insurance, being recommended by healthcare personnel to be screened, and being concerned about developing cervical cancer. OBJECTIVE: To compare the frequency of abnormal cervical cytological results with and without representation of the transformation zone (TZ) in a medium-sized city in Brazil according to the Bethesda categorization. METHODS: We performed an analytical observational study evaluating cytological results over 15 years from conventional smears collected within the Public Brazilian Health System in Araçatuba, Brazil. Tests were stratified by age and cytological result according to Brazilian guidelines and the Bethesda System. RESULTS: There were 261,881 examinations, 159,251 with and 102,630 without TZ representation. The global frequency of abnormal results was higher in smears with TZ representation (crude odds ratio [OR] 4.03; 95% confidence interval [CI] [3.71-4.38]). Considering only women between 25 and 64 years (the target of the national guidelines), there were higher results for high-grade squamous intraepithelial lesions (OR 11.54; 95% CI 6.74-19.78), low-grade squamous intraepithelial lesions (OR 2.82; 95% CI 2.17-3.67) atypical squamous cell in which high-grade intraepithelial lesion cannot be excluded (OR 6.90; 95% CI 4.91-9.66) and atypical squamous cell of undetermined significance (OR 3.53; 95% CI 3.11-4.01) among cases with TZ representation. CONCLUSION: Samples with TZ representation were associated with higher rates of abnormal results. Health professionals should be regularly trained to understand the weakness of cytological examinations when the TZ is not represented, especially in developing countries.

INTRODUCTION: Preventive screening for cervical cancer is the best available strategy to reduce the incidence and mortality from this neoplasm. However, the low proportion of women who undergo routine screening is a pending concern for healthcare systems worldwide. OBJECTIVE: To estimate the prevalence and factors associated with preventive cervical cancer screening in a Peruvian region. METHODS: Cross-sectional, multicenter study. It enrolled 1146 women users of healthcare centers in a Peruvian sanitary region. The dependent variable was the performance of cervical cancer preventive screening with Papanicolaou or visual inspection with acetic acid in the last two years. The independent variables were sociodemographic and socio-sanitary factors, knowledge about cervical cancer and human papillomavirus, attitudes and information towards screening tests. To evaluate the association between the variables, crude and adjusted prevalence ratios were calculated with generalized linear models of Poisson. RESULTS: The overall prevalence of preventive screening was 50.5%. This was associated with being tested, having a higher education level and urban area of residence, using contraceptive methods, having health insurance, being recommended by healthcare personnel to be screened, and being concerned about developing cervical cancer. It was also associated with responding that this cancer is preventable, being aware of cervical cancer or human papillomavirus, and believing it can cause cervical cancer. In contrast, considering preventive screening risky was associated with not having the test. CONCLUSIONS: The proportion of women with preventive cervical cancer screening is low. Objetivo : Determinar la relación entre la ausencia de células endocervicales y resultados citológicos Papanicolaou falsos negativos. Material y métodos : Estudio observacional, de enfoque cuantitativo, transversal, relacional y retrospectivo realizado en el Servicio de Anatomía Patológica del Hospital General de Huacho. Se realizó la búsqueda de resultados de biopsias de cuello uterino positivos para lesiones intraepiteliales escamosas y sus respectivos resultados citológicos Papanicolaou previos durante los años 2016 al 2018. Se seleccionó los resultados negativos con ausencia de células endocervicales. Resultados : De los 158 resultados citológicos, 23 mostraron ausencia de células endocervicales y de éstos, sólo dos (8,7%) fueron reportados como negativos. En total 11 (7%) citologías fueron negativas. Conclusiones : No se encontró relación entre la ausencia de células endocervicales y resultados citológicos falsos negativos; por tanto, no influyó de manera significativa en la detección de lesiones escamosas premalignas de cuello uterino.
SUMMARY Objective : To determine the relationship between absence of endocervical cells and false negative Pap smears in patients with squamous premalignant cervix lesions. Methods : A cross sectional study was carried out at Servicio de Anatomía Patológica del Hospital General de Huacho. A search for positive cervix biopsies for squamous intraepithelial lesions and their Pap smears from 2016 to 2018 was done. Negative Pap smears with absence of endocervical cells were investigated. Results : 23 out of 158 cytology results showed absence of endocervical cells, only two (8.7%) were reported negative. Eleven cytologies were negative (7%). Conclusions : No relationship was found between absence of endocervical cells and false negative results at Pap smears. Therefore, no influence in the detection of squamous pre-malignant cells was found.

BACKGROUND: The objective was to determine the prevalence of abnormal cytologic smears in a large population of a Brazilian city. METHODS: Retrospective study of cervical cytology results performed at a private laboratory in São Paulo - Brazil. A total of 1,030,482 cytology tests were performed between January 2010 and December 2015. RESULTS: Among the satisfactory cytologies, we observed abnormal results in 8.9% (91,371). Analyzing the proportion of exams with altered results over the years, there was no change in these rates. We observed that the proportion of abnormal exams decreased with increasing age. Atypical squamous cells of undetermined significance, possibly non-neoplastic (ASC-US) cytology was the most common abnormality found in the general population with 73.19%, followed by low-grade intraepithelial lesion (LSIL) with 20.5%; these 2 cytological abnormalities add up to 93.69 % of all abnormal results. There was an increase in ASC-US with advancing age, a decrease in prevalence of LSIL, especially after the age of 30 and high-grade intraepithelial lesion (HSIL) shows two peaks of elevation, respectively in the age groups of 30-39 years and over 70 years. CONCLUSION: Women under the age of 30 showed a higher prevalence of cytological abnormalities, however most of these changes are low-grade lesions. In older women, cytological abnormalities are largely high-grade lesions, requiring greater care to prevent progression to cancer.

Introdução: O câncer do colo uterino é um sério problema de saúde pública, mas pode ser prevenido por vacinação, rastreamento e tratamento das lesões precursoras. Objetivo: Verificar o conhecimento e o comportamento das mulheres de Governador Valadares, no que diz respeito ao câncer do colo do útero e às ações relacionadas com a sua prevenção. Material e Métodos: Foi realizado um estudo descritivo, transversal de setembro de 2019 a fevereiro de 2020, quando 202 mulheres maiores de 18 anos, foram convidadas, de forma aleatória, em locais públicos, a responder um questionário estruturado. Resultados: A idade média das mulheres que participaram do estudo foi de 31,2 anos, sendo a maioria delas solteiras e com ensino médio completo. Algumas delas (7,9%) não souberam responder o que é o câncer do colo uterino, que ele pode ser evitado (10,9%) e nem de sua relação com a infecção pelo papilomavírus humano (38,1%). Além disso, 6,4% delas nunca ouviram falar do exame preventivo e 25,7% nunca o realizaram, sendo descuido (17,3%), vergonha (12,9%) e falta de tempo (7,9%) os principais motivos. A maioria (48,4%) das mulheres que já realizaram o exame o fizeram a menos de um ano e 49,5% disseram sentir-se mais confortáveis quando o profissional coletor é mulher. Para 49% um sistema de auto-coleta aumentaria a possibilidade de realizar o exame regularmente. Aproximadamente um quarto das participantes desconhecem a existência de uma vacina contra o Papilomavírus Humano e mais da metade delas não sabem que ela protege apenas contra os quatro tipos mais frequentes. Conclusão: Um número significativo de mulheres apresentou conhecimento insatisfatório sobre o câncer do colo uterino e sua prevenção, o que impacta na adesão às práticas de rastreamento e justifica o investimento em ações de educação em saúde.
Introduction: Cervical cancer is a serious public health problem, but it can be prevented by vaccination, screening and treatment of precursor lesions. Objective: To verify the knowledge and behavior of women in Governador Valadares, with regard to cervical cancer and actions related to its prevention. Material and Methods: A descriptive, cross-sectional study was carried out from September 2019 to February 2020, when 202 women over 18 years old were invited, randomly, in public places, to answer a structured questionnaire. Results: The average age of the women who paticipated in the study was 31.2 years, most of whom was single and had completed high school. Some of them (7,9%) were unable to answer what cervical cancer is, that it can be avoided (10,9%) or its relationship with Human Papillomavirus infection (38,1%). In addition, 6,4% of them had never heard of the preventive exam and 25,7% had never taken it, with carelessness (17,3%), shame (12,9%) and lack of time (7,9%) main reasons. The majority (48,4%) of the women who had already taken the test had done so less than a year ago and 49,5% said they felt more comfortable when the collector was a woman. For 49% a self collection system would increase the possibility of performing the exam regularty. Approximately a quarter of the participants are unware of the existence of a vaccine against Human Papillomavirus and more than half of them are unaware that it only protects agains the four most common types. Conclusion: A significant number of women had unsatisfactory knowledge about cervical cancer and its prevention, which impacts on their adherence to screening practices and justifies the investiment in health education actions.

Introdução: O câncer do colo do útero é um dos mais frequentes na população feminina. Objetivo: Realizar um levantamento dos exames colpocitológicos realizados no município de Governador Valadares (GV), a fim de avaliar a frequência de amostras satisfatórias e insatisfatórias para avaliação oncótica, o percentual de esfregaços que apresentaram representatividade dos elementos da zona de transformação e comparar com os dados nacionais e do estado de Minas Gerais (MG). Material e Métodos: Foi realizado um estudo observacional, retrospectivo, a partir de dados obtidos do Sistema de Informação do Câncer (SISCAN). Foi verificado o quantitativo de exames colpocitológicos realizados, a frequência de amostras satisfatórias e insatisfatórias para avaliação oncótica, o percentual de esfregaços que apresentavam representatividade da junção escamo-colunar, o quantitativo de amostras rejeitadas, além do tempo gasto para a realização dos exames em cada esfera, no período de janeiro de 2016 a dezembro de 2021.Resultados: A taxa de amostras insatisfatórias para avaliação oncótica foi de 1,2%, 1,0% e 0,7% (Brasil, MG e GV, respectivamente), sendo a presença de artefatos de dessecamento o principal motivo, principalmente na faixa etária dos 50 aos 54 anos, em todas as esferas. Essa taxa foi significativamente mais alta em GV que aquelas observadas em MG e no Brasil. A partir dos 45 anos em GV e dos 55 anos no Brasil e MG, a maioria das amostras não apresentou elementos da zona de transformação no esfregaço. Foram rejeitadas 0,36%, 0,23% e 0,013% das amostras no Brasil, MG e GV, respectivamente, com destaque para aquelas com ausência ou erro na identificação da lâmina. A liberação dos resultados ocorreu de forma significativamente mais rápida em GV. Conclusão: Tais achados justificam a necessidade de treinamento dos profissionais envolvidos com a coleta, a fim de possibilitar a obtenção de amostras de melhor qualidade e resultados mais confiáveis.
Introduction: Cervical cancer is one of the most frequent in female population. Objective: To carry out a survey of colpocitological exams performed in the city of Governador Valadares (GV), in order to evaluate the frequency of samples satisfactory and unsatisfactory for oncotic evaluation, the percentage of smears that were representative of the elements of the squamo-columnar junction and compare with national and Minas Gerais (MG) data. Material and Methods: A observational, retrospective study was carried out, based on data obtained from the Cancer Information System (SISCAN). The number of performed colpocitological exams was verified, the frequency of satisfactory samples and unsatisfactory for oncotic evaluation, the percentage of smears that presented representativeness of the elements of transformation zone, the quantitative of rejected samples, in addition to the time spent carrying out the tests in each sphere, from January 2016 to December 2021. Results: The rate of unsatisfactory samples for oncotic evaluation was 1.2%, 1.0% and 0.7% (Brazil, MG and GV, respectively), with the presence of desiccation artifacts being the main reason, mainly in the age group of 50 to 54 years, in all spheres. This rate was significantly higher in GV than those observed in MG and Brazil. After 45 years in GV and 55 years in Brazil and MG, most samples did not show elements of the transformation zone in the smear. 0.36%, 0.23% and 0.013% of the samples were rejected in Brazil, MG and GV, respectively, highlighting those with absence or error in the identification of the slide. The release of the results occurred significantly faster on GV. Conclusion: Such findings justify the need for training of professionals involved with the collection, in order to enable to obtaining samples better quality and more reliable results.

INTRODUCTION: The early identification of precursor lesions followed by appropriate treatment prevents development of cervical cancer and its consequences. OBJECTIVE: The present study evaluated the influence of the COVID-19 pandemic on cervical cancer screening by comparing the quantity of tests to detect cervical cellular changes performed in São Paulo state in 2019, prior to the detection of SARS-CoV-2 in Brazil, to the first (2020) and second (2021) years following its appearance. MATERIALS AND METHODS: Data from Fundação Oncocentro de São Paulo (FOSP), the agency that analyses approximately 220,000 Papanicolaou (Pap) tests annually, were reviewed. RESULTS: A median of 1,835 Pap tests were performed in 55 municipalities in 2019. This was reduced to 815 tests in 2020, a 56% decrease (p = 0.0026). In 2021, the median number was 1,745, a 53% increase over 2020 levels (p = 0.0233). The 26 municipalities with >1,000 tests in 2020 had a median reduction from 4,433 in 2019 to 2,580 in 2020 (p = 0. 0046). The 29 municipalities with <1,000 tests had a median reduction from 951 in 2019 to 554 in 2020 (p < 0.0001). There was a 44% reduction in the number of follow-up cytological evaluations from 2019 to 2020, followed by a 30% increase in the following year. However, the percentage of women with a normal finding or with any abnormality remained unchanged. The findings from a histological evaluation of women in São Paulo city indicated that the percent of cases positive for CIN-1 (p < 0.0410) and CIN-3 (p < 0.0012) increased in 2020 and 2021 as compared to 2019 levels. CONCLUSION: A reduction in testing for cervical cancer in the first year of the COVID-19 pandemic, accompanied by an elevated incidence of precancerous lesions in each of the first 2 years following its initiation, may portend a subsequent increased occurrence of cervical cancer in Brazil.

Cervical cancer is still one of the leading causes of cancer and mortality in women, especially in low- and middle-income countries. Normally, the prevention of its occurrence is done through efficient screening and treatment programs for high-grade epithelial lesions, which are pre-malignant lesions. Cheaper diagnostic techniques ensure greater access to women, which can prevent a large number of cancer cases worldwide. Objective: The aim of the study was to evaluate the accuracy of visual inspection either with acetic acid or with Lugol's iodine, cervical cytology and colposcopy in the diagnosis of cervical intraepithelial neoplasia 2 and 3. Methods: This is a study of diagnostic accuracy. We evaluated 115 women with high-grade squamous intraepithelial lesion confirmed by biopsy, 54 with cervical intraepithelial neoplasia 2 and 61 with cervical intraepithelial neoplasia 3, from January 2016 to December 2018 at the Lower Genital Tract Pathology and Colposcopy Service of the Hospital de Clínicas de Porto Alegre, Porto Alegre, Brazil. A comparative analysis of the visual inspection, Pap smear and colposcopy diagnostic methods was performed. Results: The average age was 33.1 years (standard deviation=9.83) for cervical intraepithelial neoplasia 2 cases and 35.2 years (standard deviation=7.97) for cervical intraepithelial neoplasia 3. In the cervical intraepithelial neoplasia 2 group, visual inspection tests were positive for high-grade squamous intraepithelial lesion in 98.1% of the cases with acetic acid and 94.4% with Lugol's iodine. Colposcopy identified a probable high-grade squamous intraepithelial lesion in 94.4% of the cases, while cytology only in 42.6%. In the cervical intraepithelial neoplasia 3 group, the visual inspection tests were positive for high-grade squamous intraepithelial lesion in 91.8% of the cases with acetic acid and 95.1% with Lugol's iodine. Colposcopy identified a probable high-grade squamous intraepithelial lesion in 93.5% of the cases, while cytology in 65.6%. Conclusion: Visual inspection with acetic acid and Lugol's iodine, and colposcopy test were more accurate for the diagnosis of cervical intraepithelial neoplasia 2 and 3 than through cytopathology. Introdução: O câncer do colo de útero ainda é uma das principais causas de câncer e mortalidade em mulheres, especialmente em países de baixa e média renda. Normalmente, a prevenção de sua ocorrência é feita por meio de programas eficientes de triagem e tratamento de lesões epiteliais de alto grau, que são as lesões pré-malignas. Técnicas diagnósticas mais baratas garantem maior acesso às mulheres, podendo evitar um grande número de casos de câncer no mundo inteiro. Objetivo: O objetivo deste estudo foi avaliar a acurácia da inspeção visual (com ácido acético e com solução de lugol), da citologia cervical e da colposcopia no diagnóstico de neoplasias intraepiteliais cervicais 2/3. Métodos: Trata-se de um estudo de acurácia diagnóstica. Foram avaliadas 115 mulheres com lesão intraepitelial escamosa de alto grau confirmada por biópsia, 54 com neoplasias intraepiteliais cervicais 2 e 61 com neoplasias intraepiteliais cervicais 3, no período de janeiro de 2016 a dezembro de 2018 no Serviço de Patologia e Colposcopia do Trato Genital Inferior do Hospital de Clínicas de Porto Alegre, em Porto Alegre, Brasil. Foi realizada análise comparativa dos métodos de diagnóstico Inspeção visual com ácido acético, Inspeção visual com Solução de Lugol, colpocitologia oncótica e colposcopia. Resultados: A média de idade foi de 33,11 anos (DP 9,83) para os casos de neoplasias intraepiteliais cervicais 2 e de 35,28 anos (DP 7,97) para neoplasias intraepiteliais cervicais 3. No grupo de neoplasias intraepiteliais cervicais 2, os testes de inspeção visual foram positivos para tratamento de lesões epiteliais de alto grau em 98,1% dos casos com Inspeção visual com ácido acético e em 94,4% daqueles com Inspeção visual com Solução de Lugol. A colposcopia identificou provável tratamento de lesões epiteliais de alto grau em 94,4% dos casos, enquanto a citologia apenas 42,6%. No grupo neoplasias intraepiteliais cervicais 3, os testes de inspeção visual foram positivos para tratamento de lesões epiteliais de alto grau em 91,8% dos casos com Inspeção visual com ácido acético e em 95,1% daqueles com Inspeção visual com Solução de Lugol. A colposcopia identificou provável tratamento de lesões epiteliais de alto grau em 93,5% dos casos, enquanto a citologia em 65,6%. Conclusão: A inspeção visual (com ácido acético e com Solução de Lugol) e a colposcopia foram mais precisas para o diagnóstico de neoplasias intraepiteliais cervicais 2/3 do que a citopatologia. (AU)

ABSTRACT Objective: To estimate prevalence, assess trends and identify factors associated with non-performance of Pap smears among postpartum women residing in Rio Grande, Southern Brazil. Methods: Between 01/01 and 12/31 of 2007, 2010, 2013, 2016 and 2019, previously trained interviewers applied a single standardized questionnaire at the hospital to all postpartum women residing in this municipality. It was investigated from the planning of pregnancy to the immediate postpartum period. The outcome consisted of not performing a Pap smear in the last three years. The chi-square test was used to compare proportions and assess trends, and Poisson regression with robust variance adjustment in the multivariate analysis. The measure of effect was the prevalence ratio (PR). Results: Although 80% of the 12,415 study participants had performed at least six prenatal consultations, 43.0% (95%CI 42.1-43.9%) had not been screened in the period. This proportion ranged from 64.0% (62.1-65.8%) to 27.9% (26.1-29.6%). The adjusted analysis showed a higher PR for not performing Pap smears among younger puerperal women, living without a partner, with black skin color, lower schooling, and family income, who did not have paid work during pregnancy or planned pregnancy, who attended fewer prenatal consultations. smoked during pregnancy and were not being treated for any illness. Conclusion: Despite the improvement in coverage, the observed rate of non-performance of Pap smears is still high. Women most likely to have cervical cancer were those who had the highest PR for not having this test.
RESUMO Objetivo: Estimar a prevalência, avaliar a tendência e identificar fatores associados à não realização de citopatológico de colo uterino (CP) entre puérperas em Rio Grande (RS). Métodos: Entre 1o de janeiro e 31 de dezembro de 2007, 2010, 2013, 2016 e 2019, entrevistadores previamente treinados aplicaram, ainda no hospital, questionário único e padronizado a todas as puérperas residentes neste município que tiveram filho nos hospitais locais. Investigou-se desde o planejamento da gravidez até o pós-parto imediato. O desfecho foi constituído pela não realização de CP nos últimos três anos. Utilizou-se teste χ² para comparar proporções e avaliar tendência e regressão de Poisson com ajuste da variância robusta na análise multivariável. A medida de efeito utilizada foi a razão de prevalências (RP). Resultados: Apesar de 80% das 12.415 participantes do estudo terem realizado 6+ consultas de pré-natal, 43,0% (intervalo de confiança de 95% — IC95% 42,1-43,9%) não realizaram CP no período. Essa proporção variou de 64,0% (62,1-65,8%) a 27,9% (26,1-29,6%). Após a análise ajustada, puérperas de menor idade, cor da pele preta, sem companheiro, de menor escolaridade e renda familiar, que não exerciam trabalho remunerado, não planejaram a gravidez, realizaram menor número de consultas de pré-natal, fumaram na gravidez e não fizeram tratamento para alguma doença mostraram RP significativamente maior à não realização de CP em relação às demais. Conclusão: Apesar de melhora na cobertura, a taxa observada de não realização de CP ainda é elevada. Mulheres mais propensas a ter câncer de colo uterino foram as que apresentaram as maiores RP à não realização desse exame.

Resumo Objetivo Investigar a presença de infecção cervical pelo papilomavírus humano (HPV, human papillomavirus) em mulheres idosas e fatores relacionados. Método Trata-se de um estudo transversal, retrospectivo e descritivo, com abordagem quantitativa. A amostra foi constituída por 106 mulheres com idade igual ou superior a 60 anos atendidas em serviços de saúde pública de uma cidade do Sul do Brasil, as quais realizaram coleta de material cervical para análise citológica e detecção molecular do DNA do HPV, bem como levantamento de dados clínicos e sociodemográficos por meio de um questionário padronizado e requisição do exame citopatológico. Resultados A idade das pacientes variou entre 60 e 82 anos, com média de 64,9 ± 5,1. O HPV foi detectado em 14 (13,2%) idosas avaliadas no estudo e 8 tipos virais foram identificados, a maioria (n=7; 87,5%) de alto risco oncogênico. Observou-se, por análise de qui-quadrado, que casos HPV positivos possuem associação com maior número de parceiros sexuais (p= 0,018). Na citologia, a maioria das mulheres (n=102; 96,2%) apresentou resultado negativo para lesão intraepitelial ou malignidade e duas (1,8%) apresentaram citologia alterada, mas destas, nenhuma apresentou infecção por HPV no teste molecular. Das 10 mulheres avaliadas em duas visitas, sete (70%) não apresentaram infecção pelo HPV em ambas as avaliações, duas (20%) eliminaram a infecção pelo HPV e uma (10%) apresentou conversão para positividade. Nenhuma delas apresentou infecção persistente. Conclusão Mulheres idosas estão suscetíveis à infecção por HPV e às lesões causadas por esse vírus, por isso devem manter o rastreamento citológico.
Abstract Objective To investigate the presence of cervical infection by human papillomavirus (HPV) and associated factors in older women Method A cross-sectional, retrospective descriptive study with a quantitative approach was conducted. The sample comprised 106 women aged 60 years or over, seen at public health services of a city in southern Brazil, who underwent cervical cell collection for cytological analysis and molecular detection of HPV DNA. Clinical and sociodemographic data were collected using a standardized questionnaire and from Pap test result Results Patient age was 60-82 years, with a mean of 64.9 ± 5.1 years. HPV was detected in 14 (13.2%) of the study participants and 8 viral types were identified, the majority (n=7; 87.5%) of high oncogenic risk. Chi-square analysis revealed that positive HPV cases were associated with a higher number of sexual partners (p= 0.018). On cytology, most of the women (n=102; 96.2%) had a negative result for intraepithelial lesion or malignancy, and two (1.8%) had abnormal cytology, but neither were positive for HPV infection on molecular testing. Of the 10 women evaluated at two visits, seven (70%) tested negative for HPV infection on both evaluations, two (20%) eliminated the HPV infection, and one (10%) showed conversion to positive infection status. None of the cases had persistent infection. Conclusion Older women are susceptible to HPV infection and to the lesions caused by the virus. This group should therefore continue regular cytological screening.
