RESUMEN

El priapismo es una erección dolorosa y persistente acompañada o no de estímulo sexual. Una causa poco frecuente de esta anormalidad es la leucemia mieloide crónica. Se han reportado pocos casos de priapismo como manifestación inicial de una leucemia de este tipo en pacientes adolescentes. A continuación, se informa el caso de un paciente de 16 años de edad que presentó priapismo como manifestación inicial de una leucemia mieloide crónica. Durante su evolución, no se realizó aspiración de los cuerpos cavernosos. Se inició tratamiento hematológico específico y, ante la persistencia del priapismo, fue necesario realizar un shunt de cuerpos cavernosos en dos ocasiones, tratamiento a pesar del cual existen altas probabilidades de secuelas.


Priapism is a painful and persistent erection, with or without sexual stimulation. A rare cause of such abnormality is chronic myeloid leukemia. Few cases of priapism as an initial manifestation of this type of leukemia have been reported in adolescent patients. Here we describe the case of a 16-year-old patient who presented with priapism as the initial manifestation of chronic myeloid leukemia. No cavernosal aspiration was performed. A specific hematological treatment was started and, given the persistence of priapism, the patient required 2 corpora cavernosa shunt procedures; despite this treatment, there is a high probability of sequelae.

RESUMEN

El priapismo es una erección peneana prolongada y dolorosa, que ocurre sin estímulo sexual previo. Existen dos tipos principales, el priapismo de alto flujo y el priapismo de bajo flujo. Aunque en la mayoría de las ocasiones la causa subyacente será desconocida, puede ser la primera manifestación de una enfermedad grave. En el paciente pediátrico con una erección prolongada se debe diferenciar entre la erección peneana recurrente y los distintos tipos de priapismo, puesto que cada entidad requiere un manejo concreto e implica un pronóstico diferente. Priapism is a prolonged and painful penile erection, which occurs without prior sexual stimulation. There are two main types, high-flow priapism and low-flow priapism. Although on most occasions the underlying cause will be unknown, it may be the first manifestation of serious disease. In the pediatric patient with prolonged erection we must differentiate between recurrent penile erection and the different types of priapism since each entity requires a specific management and implies a different prognosis. INTRODUCTION: Ischemic priapism is a medical emergency that, if not treated, could lead to permanent erectile dysfunction. The association between cocaine and priapism is well-known; however, data on patient characteristics, treatment, and outcomes is missing. This work aimed to answer the research question: What are the characteristics, management strategies, and erectile prognosis of patients consuming cocaine and presenting with priapism? METHODS: We conducted a systematic review according to PRISMA guidelines and described our case series. RESULTS: Eight studies were selected for qualitative synthesis, presenting information on ten patients. In our case series, we showed information regarding four patients. From the systematic review, the mean presentation time was 42.6 h, and the mean number of procedures to solve priapism was 2,4; in our case series was 42.75 h and 2, respectively. CONCLUSION: Cocaine-related priapism might present with a delayed diagnosis, need more procedures to be managed, and have a worse prognosis. More extensive and prospective studies are required.

RESUMEN

El priapismo se caracteriza por la presencia de una erección peneana prolongada y persistente con duración de más de 4 horas y no asociada con estimulación sexual. El priapismo neonatal es una rara entidad con pocos casos reportados hasta la fecha. Se describe un caso de un neonato varón con priapismo neonatal. Dentro de sus antecedentes, fue pequeño para la edad gestacional, por lo que fue hospitalizado. Al momento del examen, el escroto y el pene no presentaban signos de cianosis, ambos testículos estaban descendidos. En los exámenes de laboratorio se encontró policitemia. Fue evaluado por el servicio de urología, donde se hizo el diagnóstico final de priapismo neonatal secundario a policitemia. El tratamiento fue conservador (AU)


Priapism is characterized by the presence of a prolonged and persistent penile erection lasting more than 4 hours that is not associated with sexual stimulation. Neonatal priapism is a rare entity with few cases reported till the date. A case of a male neonate with neonatal priapism is described. Within his history, he was small for gestational age (SGA), thus he was hospitalized. At the time of examination, the scrotum and penis showed no signs of cyanosis, both testicles were descended. Laboratory tests revealed polycythemia. He was evaluated by the Urology service who made the final diagnosis of neonatal Priapism secondary to Polycythemia. The treatment was conservative. (AU)

RESUMEN

La pandemia causada por el SARS -COV 2 ha sido uno de los hitos más importantes de la última década, provocando desestabilización de los sistemas económicos, salud y educación. La infección por SARS COV2 se característica principalmente por la afección del sistema respiratorio, sin embargo, logra afectar muchos órganos extrapulmonares, aunque la fisiopatología no está totalmente dilucidada se evidencia una gran incidencia de eventos tromboembólicos en pacientes complicados. La infección por SARS-Cov2 tienen un amplio abanico de signos y síntomas, durante la pandemia debemos sospechar de esta viremia en pacientes con priapismo refractario presentamos el caso por la inédita presentación y por el curso atípico de la enfermedad.

Introduction and Objective: The penile metastasis is a rare clinical entity. The objective is to present the first documented case report of penile metastasis from right colon. Clinical Case: A case of a 78-year-old man who was diagnosed with penile metastasis from right colon. The patient came to our consultation complaining of colic pain in the kidney and swelling of the penile which finally result in a malignant priapism. The diagnosis was histopathologic and was treated with chemotherapy and died few months later. Conclusion: Metastatic lesions in the penile are extremely rare; only 300 cases have been reported in the literature. It is a sign of bad prognosis. The mechanism of metastatic spread to the penis is not well established. Even there are several treatment options, is usually paliative (AU)


Introducción y Objetivo: La metástasis penena esuna entidad muy infrecuente. El objetivo es la presentacióndel primer caso documentado en la literatura de metástasispenana con origen en el colon derecho.Caso Clínico: Presentamos a un varón diagnosticadode priapismo producido por una metástasis peneana cuyotumor primitivo tiene lugar en el colon derecho. El pacienteacudió por dolor en fosa renal y éstasis venoso en el peneque finalmente le provocó un priapismo por afectación tumoral. El diagnóstico fue anatomopatológico, siendo el paciente tratado con quimioterapia.Conclusión: La presencia de metástasis en el penees una presentación clínica infrecuente y un signo de malpronóstico. El mecanismo fisiopatológico no está determinado. El tratamiento, aunque variado, es generalmente paliativo (AU)

Objective: To describe two cases of man with the diagnosis of ischemic priapism after the intake of tamsulosin and to revise the scientific literature. Methods: We present two cases of men that developed an ischemic priapism after the intake of tamsulosin prescribed for STUI and were treated in our hospital. We described the two cases, from the diagnosis until the surgery that was performed. Also, we review the scientific literature about this topic. Results: In one hand, a 67 years old man with the previous diagnosis of diabetes mellitus, hypertension and dyslipidemia that take a one single dosis of tamsulosin and developed a priapism of 9 hours of duration. He was diagnosticated of low-flow priapism that was reverted after the use of intracavernosal phenylephrine. On the other hand, a 61 years old man without any medical condition. He developed a priapism after the intake of also one single dosis of tamsulosin and came to the hospital after 48 hours of the beginning of the erection. In this case, the use of intracavernosal phenylephrine wasn´t effective so we decided to performed a distal shunt between cavernosal and spongy body according to the techniques of Winter, Ebbehoj and Al-Ghorab. All of them without results. At the end, we tried a proximal shunt according Quackles technique, also ineffective. The patient declined another surgery for implantation of a pennis prothesis and went home after four days of hospitalization with the disappearance of the pain. Conclusions: The tamsulosin is a drug well known by urologist that have a safety profile probed with the years. Nevertheless, it's association with a disease like the priapism forced us to explain to our patients this rare adverse effect (AU)


Objetivo: Describir la fisiopatología del priapismoasociado a tamsulosina a través de dos casos clínicos tratados en nuestro centro.Método: Se presentan dos casos de varones que desarrollan un priapismo isquémico tras la toma de tamsulosina yque fueron tratados en nuestro hospital. Describimos amboscasos, desde el diagnóstico hasta el tratamiento. Además,revisamos la literatura científica sobre dicho tema.Resultado: Introducimos el caso de un hombre de 67años con comorbilidad cardiovascular que desarrolla un priapismo isquémico de 9 horas de duración que revirtió confenilefrina intracavernosa. Por otro lado, se presenta el casode un varón de 61 años sin patología de base con un priapismo de 48 horas que no mejoró tras tratamiento conservador ni tras cirugía de derivación cavernoso-esponjosa.Conclusiones: La tamsulosina es un fármaco seguroque en, raras ocasiones, puede asociarse a un priapismo isquémico. Priapism is a prolonged unintended erectile state unrelated to sexual stimulation or sexual desire. There is a very rare relationship between the use of alpha blockers and the development of priapism. Here, we describe 2 cases of alpha blocker induced priapism and a literature review. One of these cases is related to the use of silodosin and the other is related to the use of tamsulosin. So far, 18 alpha blocker induced priapism cases have been reported. We are presenting the first case of silodosin induced priapism and the eighth case of priapism secondary to tamsulosin. Despite silodosin having a much greater affinity for the α1-a receptor than the α1-b receptor, as represented in this case it can cause this rare side effect. Before starting alpha blocker treatment, side effects such as priapism, which may be very rare but may cause serious problems, should be kept in mind. (AU)


El priapismo es un estado eréctil prolongado no intencionado y no relacionado con la estimulación o el deseo sexual. Existe una relación muy infrecuente entre el uso de alfabloqueantes y el desarrollo de priapismo. Describimos aquí dos casos de priapismo inducido por alfabloqueantes y una revisión de la literatura. Uno de estos casos guarda relación con el uso de silodosina, y el otro con el uso de tamsulosina. Hasta el momento se han reportado 18 casos de priapismo inducido por alfabloquantes. Presentamos aquí el primer caso de priapismo inducido por silodosina y el octavo caso de priapismo secundario a tamsulosina. A pesar de que silodosina tiene mucha mayor afinidad por el receptor α1-a que el receptor α1-b, según lo representado en este caso, puede causar este efecto secundario raro. Antes de iniciarse tratamiento con alfabloquantes deben tenerse en cuenta los efectos secundarios, tales como priapismo, que pueden ser muy raros pero pueden causar problemas graves. (AU)

La trombosis del cuerpo cavernoso es una afección rara. La etiología y la fisiopatología de esta entidad todavía son poco entendidas y no existe un algoritmo de diagnóstico y tratamiento claro. Introducción: El priapismo consiste en una erección mantenida durante más de cuatro horas. Es una patología infrecuente en la población pediátrica, estimada en 0,3 a 1,5 por cada 100.000 niños al año. La secuencia diagnóstica incluye anamnesis, exploración física y ecografía doppler peneana (EcoDP). No siempre es necesaria la punción de cuerpos cavernosos para establecer el diagnóstico diferencial entre priapismo de alto y bajo flujo. El tratamiento de elección en la edad pediátrica no está bien definido.Material y métodosEstudio multicéntrico, retrospectivo, descriptivo de pacientes menores de 14 años con priapismo de alto flujo, entre los años 2010 y 2020. Revisión de la literatura.ResultadosUn total de siete pacientes fueron diagnosticados de priapismo de alto flujo. Ninguno requirió punción de cuerpos cavernosos. Se realizó un manejo conservador en todos ellos, dos pacientes necesitaron embolización arterial superselectiva por persistencia de la clínica.ConclusionesEl priapismo de alto flujo es una entidad muy infrecuente en la edad pediátrica por lo que es importante conocer el diagnóstico y manejo adecuados. Actualmente, la ecografía doppler suele ser suficiente para el diagnóstico, obviando el uso de la gasometría. El manejo inicial en niños es conservador, reservando la embolización para los casos refractarios. (AU)

Antecedentes: El priapismo isquémico es una emergencia urológica que requiere atención médica temprana para evitar daños irreversibles en la función eréctil. La aspiración cavernosa es el primer paso en el tratamiento. Actualmente no hay modelos de entrenamiento satisfactorios para desarrollar habilidades en un ambiente controlado. Por lo tanto, hemos desarrollado un novedoso modelo de entrenamiento para enseñar a los alumnos los pasos de la aspiración del pene de una manera segura y representativa.Materiales y métodosHemos desarrollado un modelo de priapismo usando un antiguo simulador para el entrenamiento del cateterismo. La validez del modelo fue evaluada por participantes y urólogos experimentados en un Boot Camp de urología. Todos habían tratado al menos cinco casos reales de priapismo. Las respuestas fueron reportadas usando una Escala Likert de cinco puntos. Los datos se analizaron utilizando IBM SPSS Statistics V25. La correlación intraclase se calculó mediante un «Modelo aleatorio unidireccional».ResultadosOnce urólogos y siete aprendices participaron en la evaluación. La apariencia del modelo fue la característica mejor evaluada del modelo de priapismo. La retroalimentación táctil de la inserción de la aguja para la aspiración también se consideró realista, con un 72,6% informándolo como «bueno» o «muy bueno» y un 85,7% opinó que el modelo era realista para la inserción de la aguja. La correlación intraclase entre los expertos fue de 0,552. La mayoría de los aprendices (83,3%) informó una simulación realista. Todos los evaluadores estuvieron de acuerdo o muy de acuerdo en que el modelo proporcionaba una buena experiencia de simulación que sería útil para la enseñanza. (AU)


Background: Ischaemic priapism is a urological emergency with early treatment required to prevent irreversible loss of erectile function. Corporal aspiration is the first step in management. Currently, there are no satisfactory training models to develop skills in a controlled environment. We have therefore developed a novel training model to teach trainees the steps of penile aspiration in a safe and representative way.Materials and methodsWe have developed a priapism model using an old catheterisation teaching model. Face validity of the model was assessed by participants and experienced urologists teaching on a urology boot camp. All had managed at least 5 cases of actual priapism. Responses were reported using a 5-point Likert Scale. Data were analysed using IBM SPSS Statistics V25. The intra-class correlation was calculated using a «One-way Random model».ResultsEleven urologists and seven trainees participated in the evaluation. The model appearance was reported as the best simulation trait of the priapism model. Tactile feedback from needle insertion for aspiration was also felt to be realistic with 72.6% reporting it as «Good» or «very good» and 85.7% reported the model to be realistic for needle insertion. Intra-class correlation amongst experts was 0.552. Majority of trainees (83.3%) reported a realistic simulation. All evaluators agreed or strongly agreed that the model provided a good simulated experience that would be useful in training. (AU)

This condition has been reported in severe COVID-19 cases, probably caused by coagulopathy secondary to the infection. However, it has not been reported in patients with mild COVID-19. Case presentation: A 65-year-old patient on hemodialysis and with mild COVID-19 presented with a 30-hour painful penile erection. Physical examination revealed an erection, but no other significant findings were observed. Based on the clinical history, physical examination and laboratory test results, the patient was diagnosed with ischemic priapism. Corpora cavernosa drainage was performed and two injections of epinephrine (each with a dose of 2mL of epinephrine solution 1/100 000) were administered, achieving complete resolution of the symptoms. Conclusions: At the time of writing this case report, there is no information available on cases of priapism in patients with mild manifestations of COVID-19, nor is it clear how the risk of thrombosis should be assessed in this group of patients. From a pathophysiological point of view, factors related to dialysis and kidney disease could have predisposed this patient to priapism, although the role of SARS-CoV-2 infection is uncertain. Therefore, further studies are required to confirm or rule out the association between COVID-19 and priapism in dialysis patients.


Resumen Introducción. El priapismo es una condición infrecuente en pacientes en diálisis que se puede presentar por factores inherentes a esta terapia y a la enfermedad renal. Por otra parte, es una entidad que se ha reportado en pacientes con síntomas severos de COVID-19 y se ha asociado a coagulopatía secundaria a la infección, pero de la que no hay registro en casos leves de esta enfermedad. Presentación de caso. Paciente masculino de 65 años en hemodiálisis y con infección por SARS-CoV-2 con síntomas leves, quien consultó al servicio de urgencias de un hospital de cuarto nivel por erección peneana dolorosa de 30 horas de evolución. Al examen físico se evidenció la erección, sin otros hallazgos relevantes. Con base en la historia clínica, el examen físico y los resultados de laboratorio, se diagnosticó priapismo isquémico y se realizó drenaje de senos cavernosos y aplicación de dos inyecciones de epinefrina (cada una con una dosis de 2mL de solución de epinefrina 1/100 000), lográndose resolución completa de la condición. Conclusiones. Hasta el momento de la elaboración del presente reporte de caso no hay información disponible de casos de priapismo en pacientes con manifestaciones leves de COVID-19, y tampoco hay claridad de cómo se debe evaluar el riesgo de trombosis en este grupo de pacientes. Desde el punto de vista fisiopatológico, factores relacionados con la diálisis y la enfermedad renal pudieron predisponer a este paciente al priapismo, aunque no es claro el rol de la infección por SARS-CoV-2. Por tanto, se requieren estudios que permitan confirmar o descartar la asociación entre COVID-19 y priapismo en pacientes en diálisis.

BACKGROUND: Ischaemic priapism is a urological emergency with early treatment required to prevent irreversible loss of erectile function. Corporal aspiration is the first step in management. Currently, there are no satisfactory training models to develop skills in a controlled environment. We have therefore developed a novel training model to teach trainees the steps of penile aspiration in a safe and representative way. MATERIALS AND METHODS: We have developed a priapism model using an old catheterisation teaching model. Face validity of the model was assessed by participants and experienced urologists teaching on a urology boot camp. All had managed at least 5 cases of actual priapism. Responses were reported using a 5-point Likert Scale. Data were analysed using IBM SPSS Statistics V25. The intra-class correlation was calculated using a «One-way Random model¼. RESULTS: Eleven urologists and seven trainees participated in the evaluation. The model appearance was reported as the best simulation trait of the priapism model. Tactile feedback from needle insertion for aspiration was also felt to be realistic with 72.6% reporting it as «Good¼ or «very good¼ and 85.7% reported the model to be realistic for needle insertion. Intra-class correlation amongst experts was 0.552. Majority of trainees (83.3%) reported a realistic simulation. All evaluators agreed or strongly agreed that the model provided a good simulated experience that would be useful in training. CONCLUSION: Our model provides a realistic simulation of corporal aspiration. It can be used repeatedly. Overall, the proposed model appears to be a promising tool for training junior doctors in the initial management of ischaemic priapism.

RESUMEN

Partial thrombosis of corpus cavernosum is an uncommon condition that needs a high level of suspicious to be diagnosed. Imaging techniques may be very useful, and MR is the state-of-the-art technique. We present a case of a young male who presented to the emergency department with perineal and painful mass. MR imaging clearly depicted a thrombosed corpus cavernosum and a bilateral membrane that is possibly the underlying predisposing condition. With only conservative treatment the patient had an excellent outcome.

ABSTRACT Objective To evaluate epidemiological aspects of priapism in patients with sickle cell disease, and these aspects impact on adult sexual function. Methods This was a cross-sectional study including individuals with sickle cell disease who were evaluated at a reference center for sickle cell. Participants completed a structured questionnaire about their sociodemographic characteristics and priapism events. Sexual function was assessed using validated two instruments, the Erection Hardness Score and one about the sex life satisfaction. Results Sixty-four individuals with median aged of 12 (7 to 28) years were interviewed. The prevalence of priapism was 35.9% (23/64). The earliest priapism episode occurred at 2 years of age and the latest at 42 years. The statistical projection was that 71.1% of individuals of the study would have at least one episode of priapism throughout life. Patients with episodes of priapism (10/23) had significantly worse erectile function Erection Hardness Score of 2 [1-3]; p=0.01 and were less satisfied with sexual life 3 [3-5]; p=0.02. Conclusion Priapism is usually present in childhood, and severe episodes are associated with cavernous damage, impairment in the quality of the erection, and lower sexual satisfaction.


RESUMO Objetivo Avaliar aspectos epidemiológicos do priapismo em pacientes com doença falciforme e o impacto desses aspectos na função sexual de adultos. Métodos Trata-se de estudo transversal, que incluiu indivíduos com doença falciforme acompanhados em um centro de referência. Os participantes responderam a um questionário estruturado acerca das características sociodemográficas e eventos de priapismo. A função sexual foi avaliada por meio de dois instrumentos validados, a Escala de Rigidez de Ereção e um sobre satisfação com a vida sexual. Resultados Foram entrevistados 64 indivíduos com média de idade de 12 (7-28) anos. A prevalência de priapismo foi de 35,9% (23/64). O episódio mais precoce ocorreu aos 2 anos de idade e o mais tardio, aos 42 anos. A projeção estatística foi de que 71,1% desses sujeitos teriam pelo menos um episódio de priapismo ao longo da vida. Pacientes adultos com episódios de priapismo (10/23) apresentaram função erétil significativamente pior Escala de Rigidez de Ereção de 2 [1-3]; p=0,01 e estavam menos satisfeitos com a vida sexual 3 [3-5]; p=0,02. Conclusão O priapismo manifesta-se desde a infância, e episódios graves estão associados a dano cavernoso, prejuízo na qualidade da ereção e menor satisfação sexual.

ABSTRACT BACKGROUND AND OBJECTIVES: Priapism is one of the complications of sickle cell disease characterized by a persistent and painful erection, which can lead to erectile dysfunction and sexual impotence. The objective of this study was to understand how men with sickle cell disease and priapism access emergency care. METHODS: A qualitative study conducted in a reference healthcare unit to people with sickle cell disease in the second largest city in Bahia. Seven adult men with sickle cell disease who had experienced priapism participated in the study. The data were collected by semi-structured interview and thematic story designs and submitted to content analysis. RESULTS: Priapism is seen as a lack of genital health. Participants use strategies to manage it at home to avoid embarrassment, which ends up in cocooning. Access to emergency services is motivated by persistent and relentless pain; and limited by the fear of priapism being mistaken for sexual deviance, lack of knowledge about the complication as a urologic emergency and financial shortfall, which confers a worse prognosis about erectile function. Men are embarrassed and discriminated by healthcare and support professionals, which discourages them from accessing these services in the future. CONCLUSION: This study emphasizes the importance of early diagnosis of sickle cell disease, the orientation of family members and the need for healthcare professionals to educate young boys and men with sickle cell disease and their caregivers about priapism in advance to allow adequate self-care and prevent complications.


RESUMO JUSTIFICATIVA E OBJETIVOS: O priapismo é uma das complicações da doença falciforme caracterizada por ereção persistente e dolorosa, podendo levar à disfunção erétil e impotência sexual. O objetivo deste estudo foi compreender como os homens com doença falciforme e priapismo acessam os cuidados nos serviços de emergência. MÉTODOS: Estudo qualitativo realizado em unidade de saúde referência para pessoas com doença falciforme no segundo maior município baiano. Participaram do estudo 7 homens adultos com doença falciforme que já vivenciaram priapismo. Utilizou-se entrevista semiestruturada e desenhos-história com o tema, analisados por análise de conteúdo. RESULTADOS: O priapismo é visto como uma falta de saúde genital. Os participantes usam estratégias para seu manuseio em domicílio para evitar constrangimentos, o que acaba isolando-os socialmente. O acesso aos serviços de emergência é motivado pela dor persistente e irredutível; e limitado pelo temor do priapismo ser confundido como resultado de desvio sexual, desconhecimento da complicação como emergência urológica e carência financeira, o que confere pior prognóstico sobre a função erétil. Os homens sofrem constrangimento e discriminação pelos profissionais de saúde e de apoio das unidades, o que os desmotiva a acessar esses serviços no futuro. CONCLUSÃO: Este estudo ressalta a importância do diagnóstico precoce da doença falciforme, da orientação de familiares e da necessidade de os profissionais de saúde educarem os meninos/homens jovens com doença falciforme e seus cuidadores sobre o priapismo de forma prévia, para permitir o adequado autocuidado futuro e prevenção de complicações.
