RESUMEN

Dementia represents a potentially overwhelming health burden, both for the UK and worldwide. Addressing this fast-growing issue is a key priority for the government, health service and the public. Advances in care including the development of efficacious disease-modifying, and eventually curative, treatments can only be achieved through effective dementia research. Specifically, research directly involving participants with dementia is essential to further understanding. However, working with cognitively impaired participants with and without capacity to consent to research presents unique ethical and legal challenges. For clinicians and scientists on the frontline of dementia research, scenarios frequently arise that pose such challenges. A lack of guidance for a consistent approach in navigating these scenarios limits researchers' ability to proceed with confidence. This represents a threat to the rights and wishes of research participants as well as the field at large, as it may lead to studies being unnecessarily terminated or, worse, poor practice. In this article, we take a multiprofessional approach, informed by carer input, to these issues. We review the relevant ethical and legal literature relating to the conduct of non-interventional research studies in patients with dementia. This includes a thorough recap of the Mental Capacity Act (2005), which provides a legal framework in England and Wales for conducting research with participants who lack capacity to consent. We also discuss the important, but sometimes incomplete, role of research ethics committees in guiding researchers. We then present and discuss a series of case vignettes designed to highlight areas of incomplete coverage by existing governance. These vignettes describe theoretical scenarios informed by our own real-word experiences of encountering ethical issues when conducting dementia research. They include scenarios in which participants demonstrate varying degrees of understanding of the research they are involved in and ability to communicate their wishes and feelings. Building on these vignettes, we then provide a checklist for researchers to work through when presented with similar scenarios. This checklist covers the key ethical, legal and practical considerations that we have argued for. Taken together, this article can act as a guide, previously lacking in the literature, for colleagues in the field to enable much needed ethical, legal and effective research.

For this CHIMIA special issue on the United Nations Sustainable Development Goals (SDGs) and the coincidental 10th anniversary of the association of Women in Natural Sciences (WiNS) at ETH Zurich, there is no better opportunity to share what we have achieved in the last decade. WiNS was originally founded by female PhD students and PostDocs at the Department of Chemistry and Applied Biosciences (D-CHAB) and has expanded to include three additional departments at ETH Zurich. Join us in celebrating our anniversary, reflecting on what we and other associations with the same mission have done to promote gender equality and reduced inequalities for all. We invite you to reflect on what yet has to be done to enact a systemic change towards achieving equal opportunities for all, rather than simply accommodating the female perspective to Switzerland's predominantly masculine working culture.

BACKGROUND: Increasing work-related stress in academia can have an impact on physical and mental health. The aim of this study was to analyse the coping strategies of staff employed at the University of Udine and to verify whether sociodemographic data, professional position, and the presence of anxiety or depression symptoms are related to the use of different coping strategies. METHODS: We conducted a cross-sectional study between June and December 2020 using the Brief COPE questionnaire. We correlated coping strategies with professional position, sociodemographic data, and the presence of anxiety or depressive symptoms measured with the Patient Health Questionnaire-9 and the General Anxiety Disorder-7. RESULTS: A total of 366 people participated in the study, including 109 junior academics, 146 senior academics, and 111 administrative staff (response rate 23.6%). The three most frequently used coping strategies in terms of approach coping style were planning (6.77 ± 1.41), active coping (6.58 ± 1.45) and acceptance (6.23 ± 1.44). Women were more likely than men to report using approach and avoidant coping strategies (p < 0.001). Positive reframing and religion were most commonly used by administrative staff (p < 0.05), in contrast to junior academics, who were more likely to use substances and self-blame (p < 0.05). Anxiety was found to correlate with self-blame (OR 1.94) as a coping strategy, while depression was associated with venting (OR 2.83), self-blame (OR 3.27), and humor (OR 3.02). CONCLUSION: Identifying profiles of coping strategies can help higher education institutions to implement support strategies for the academic community, ultimately promoting healthier lives and more effective teaching and research. Our study has shown that women and junior academics among staff at the Udine University would benefit from a tailored health promotion intervention that encourages the use of approach coping styles to reduce their risk of developing anxiety and depressive symptoms.

Job insecurity is now one of the major stressors affecting well-being at work. In academia, researchers appear to be in the most precarious position. To explore the relationship between job insecurity and well-being at work, we analyzed a sample of research fellows belonging to STEM disciplines in Italy. Using a latent profile approach, we identified three "hidden" subgroups: "Safe & Sound"; "Safe not so Sound" and "Neither Safe or Sound." Compared to previous studies, our results show that even within a population of STEM researchers that tends to have good levels of employability and mobility, there are subgroups of people characterized by greater job insecurity and low work commitment, who suffer from emotional exhaustion and cynicism at work level, i.e., more exposed to the risk of burnout.

BACKGROUND: The evolution of artificial intelligence (AI) has significantly impacted various sectors, with health care witnessing some of its most groundbreaking contributions. Contemporary models, such as ChatGPT-4 and Microsoft Bing, have showcased capabilities beyond just generating text, aiding in complex tasks like literature searches and refining web-based queries. OBJECTIVE: This study explores a compelling query: can AI author an academic paper independently? Our assessment focuses on four core dimensions: relevance (to ensure that AI's response directly addresses the prompt), accuracy (to ascertain that AI's information is both factually correct and current), clarity (to examine AI's ability to present coherent and logical ideas), and tone and style (to evaluate whether AI can align with the formality expected in academic writings). Additionally, we will consider the ethical implications and practicality of integrating AI into academic writing. METHODS: To assess the capabilities of ChatGPT-4 and Microsoft Bing in the context of academic paper assistance in general practice, we used a systematic approach. ChatGPT-4, an advanced AI language model by Open AI, excels in generating human-like text and adapting responses based on user interactions, though it has a knowledge cut-off in September 2021. Microsoft Bing's AI chatbot facilitates user navigation on the Bing search engine, offering tailored search. RESULTS: In terms of relevance, ChatGPT-4 delved deeply into AI's health care role, citing academic sources and discussing diverse applications and concerns, while Microsoft Bing provided a concise, less detailed overview. In terms of accuracy, ChatGPT-4 correctly cited 72% (23/32) of its peer-reviewed articles but included some nonexistent references. Microsoft Bing's accuracy stood at 46% (6/13), supplemented by relevant non-peer-reviewed articles. In terms of clarity, both models conveyed clear, coherent text. ChatGPT-4 was particularly adept at detailing technical concepts, while Microsoft Bing was more general. In terms of tone, both models maintained an academic tone, but ChatGPT-4 exhibited superior depth and breadth in content delivery. CONCLUSIONS: Comparing ChatGPT-4 and Microsoft Bing for academic assistance revealed strengths and limitations. ChatGPT-4 excels in depth and relevance but falters in citation accuracy. Microsoft Bing is concise but lacks robust detail. Though both models have potential, neither can independently handle comprehensive academic tasks. As AI evolves, combining ChatGPT-4's depth with Microsoft Bing's up-to-date referencing could optimize academic support. Researchers should critically assess AI outputs to maintain academic credibility.

BACKGROUND: It is important to have a diverse workforce in clinical and research environments. A cross-sectional study was conducted to examine the possible presence of gender, perceived skin colour, education, academic productivity, and career progression inequalities among neurosurgeons in the United Kingdom. METHODS: A list of all consultant neurosurgeons working in the United Kingdom (N=384) was obtained from the Specialist Info website. Data about their education, research activity, academic productivity, impact and progression were obtained from publicly available online sources. RESULTS: Over 90% of consultants and all full professors were men. There was a small number of black or brown consultant neurosurgeons and no black full professors. Very few black or brown consultants obtained their medical degree from a top university or had a PhD. There were no gender disparities in currently being affiliated with a university (or top university), obtaining a PhD or Master's degree, ranking of the university from which they obtained their medical degree, Master's degree or PhD, or number of pages and open access availability of their PhD thesis. Neurosurgeons who obtained their medical degree from the UK and those with a PhD had more publications, citations and a higher h-index. Male (vs female) academic neurosurgeons had more publications, non-self-citations, and a higher h-index, and white (vs brown) female neurosurgeons had more citations per paper. CONCLUSION: This study identified important gender, skin colour, education, academic productivity, and career progression inequalities in the 2023 UK consultant neurosurgery workforce.

A recent resolution with approximately 100 signatories entitled "Sunsetting All Diversity, Equity, and Inclusion (DEI) Programs" administered by the American Academy of Dermatology (AAD) sparked controversial debate within the field. Despite the AAD voting against the proposal to eliminate DEI initiatives, many underrepresented medical groups (URMs) wondered how to move forward and create safe spaces for everyone. We discuss the relevance of DEI programs in today's society and the ethical challenges that may arise. We conclude with actionable recommendations on how organizations can improve their current DEI strategies to ensure they are more inclusive and not perceived as discriminatory.

BACKGROUND: Despite the significant role of scientific knowledge pertaining to public health, the discipline of public health remained outside the centre stage within the pandemic discourse. Against this background, we investigated the role of German public health academics during the pandemic in our study, focusing on their orientations and associated values. METHODS: We interviewed 21 public health scholars from Germany and collected 36 documents published by public health scientific societies. We analyzed data by grounded theory and situational mapping. RESULTS: We identified five types of self-images identified among healthcare academics: the scientific study supplier, the expert facing political issues, the restrained scholar, the public informer and the changemaker. The typology yields insights into the multiple dimensions of public health and its role in times of crisis. CONCLUSIONS: The findings provide implications to inter- and transdisciplinary interaction and to managing the expectations of public health professionals in relation to crisis management.

This Commentary recounts an academic journey from dentistry to neuroimmunology, highlighting pivotal moments such as a PhD fraught with challenges and an unexpected postdoctoral experience in France. My decision to settle in Belgium for a postdoc and subsequent transition to an assistant professorship at KU Leuven reflects resilience, adaptability and a commitment to both scientific exploration and family life. Balancing career uncertainties, motherhood and academic achievements, it encapsulates a trajectory shaped by a passion for neuroimmunology.

Positive research cultures provide the environment for scientists to explore ideas, grow as individuals, develop team science and create a positive impact on those around them. While positive research cultures need to grow from the kindness and integrity of team members, organization policy can either help or hinder this organic positive behavior. A focus on policies to enhance positive research culture can benefit even high-functioning organizations, by expanding and extending the benefits. Here we focus on key actionable areas to create and reinforce a positive research culture in your organization. We discuss the role of aligning staff recognition to the organization's missions, the influence of the organization unit and career structure on the research culture, the pyramid of building respectful interactions, the value of openness and transparency and the overarching goal of equality, diversity and inclusivity within the organization.

Background: When properly utilized, artificial intelligence generated content (AIGC) may improve virtually every aspect of research, from data gathering to synthesis. Nevertheless, when used inappropriately, the use of AIGC may lead to the dissemination of inaccurate information and introduce potential ethical concerns.Research Design: Cross-sectional. Study Sample: 65 top surgical journals. Data Collection: Each journals submission guidelines and portal was queried for guidelines regarding AIGC use.Results: We found that, in July 2023, 60% of the top 65 surgical journals had introduced guidelines for use, with more surgical journals (68%) introducing guidelines than surgical subspecialty journals (52.5%), including otolaryngology (40%). Furthermore, of the 39 with guidelines, only 69.2% gave specific use guidelines. No included journal, at the time of analysis, explicitly disallowed AIGC use.Conclusions: Altogether, this data suggests that while many journals have quickly reacted to AIGC usage, the quality of such guidelines is still variable. This should be pre-emptively addressed within academia.

Background: The crisis in research culture is well documented, covering issues such as a tendency for quantity over quality, unhealthy competitive environments, and assessment based on publications, journal prestige and funding. In response, research institutions need to assess their own practices to promote and advocate for change in the current research ecosystem. The purpose of the scoping review was to explore ' What does the evidence say about the 'problem' with 'poor' research culture, what are the benefits of 'good' research culture, and what does 'good' look like?' Aims: To examine the peer-reviewed and grey literature to explore the interplay between research culture, open research, career paths, recognition and rewards, and equality, diversity, and inclusion, as part of a larger programme of activity for a research institution. Methods: A scoping review was undertaken. Six databases were searched along with grey literature. Eligible literature had relevance to academic research institutions, addressed research culture, and were published between January 2017 to May 2022. Evidence was mapped and themed to specific categories. The search strategy, screening and analysis took place between April-May 2022. Results: 1666 titles and abstracts, and 924 full text articles were assessed for eligibility. Of these, 253 articles met the eligibility criteria for inclusion. A purposive sampling of relevant websites was drawn from to complement the review, resulting in 102 records included in the review. Key areas for consideration were identified across the four themes of job security, wellbeing and equality of opportunity, teamwork and interdisciplinary, and research quality and accountability. Conclusions: There are opportunities for research institutions to improve their own practice, however institutional solutions cannot act in isolation. Research institutions and research funders need to work together to build a more sustainable and inclusive research culture that is diverse in nature and supports individuals' well-being, career progression and performance.

Bioethicists, through their writings, have been known to represent the conscience of the times. Speaking up against injustices, they have acted as moral compasses in the past. The events of October 7, 2023 and the resulting armed onslaught of Israeli forces on Gaza has created a huge humanitarian crisis. However, response of the global bioethics community appears muffled. In order to gain an objective insight, we conducted a scoping review of articles published on the current conflict in the top ten bioethics journals, as classified by Google metrics. Broadening this search, we included relevant grey literature and selected medical and global health journals in our review. All types of articles published from the initiation of the conflict to the end of March 2024 were included. Findings from our exercise highlight the paucity of articles published on this crisis. This apparent indifference towards the Gaza crisis can be explained either by reluctance by bioethicists to write on this issue or perhaps due to editorial restraints. We argue that bioethicists, instead of focusing on esoteric issues, have a greater moral responsibility to speak out against injustices in Gaza. Their silence amounts to complicity and erodes the very foundations of the discipline of bioethics.

Gap years taken between undergraduate completion and entrance into medical school have become increasingly popular. We examine the role of gap years among college graduates interested in medicine and how they might contribute to academic research productivity within clinical environments. Recently, academic faculty have struggled to balance increasing clinical responsibilities with their scholarly endeavors. Academic medical departments may have incentives to hire pre-medical students to help ease the research burden on faculty. Properly motivated pre-medical students may view research positions in academic medical departments as ideal opportunities to learn in areas that will broaden their scientific knowledge and help prepare them for medical school, while greatly enhancing their medical school applications through distinguishing themselves as co-authors published in medical journals. Our experience, with two co-authors working as research associates while preparing for their medical school applications and careers, suggests that pre-medical students can strengthen their medical school applications during their gap year(s) while proving instrumental in enhancing research output thus alleviating the workload of clinical faculty.

Introduction While prior research showed gender gaps in industry payments for medical professionals in the United States, there are limited data in Japan. So, this study seeks to investigate the potential gender gap in the receipt of pharmaceutical companies (PFCs) across all medical fields in Japan. Based on the results of previous studies, we developed a hypothesis that male doctors get more PFC than female doctors. Materials and methods Data from 92 pharmaceutical companies in Japan, covering 2016 to 2019, were analyzed. The analysis was conducted on professors and associate professors at all national and public medical universities in Japan, with gender as a factor variable and payments as an outcome variable, and variables that may have influenced the factor or outcome variables in previous studies, such as specialization, university type, region, rank and years since graduation, as control variables. Payments were converted to US dollars using the December 31, 2021, rate of 115 yen to the dollar for comparison purposes. Results Out of 1,825 subjects, 1,755 were males and 70 females. Males consistently received higher median payments from pharmaceutical companies (PFCs) than females across categories. In particular, among physicians specializing in internal medicine, the median PFC for men was $25 compared to $8 for women. For physicians affiliated with former imperial universities (the seven former imperial universities founded before World War II), the median PFC for men was $32 compared to $5 for women. Multivariate analysis confirmed significantly higher total benefits for males from 2016 to 2019, with the gender gap widening during this period (incidence rate ratio (IRR) for 2016: 0.51, IRR for 2019: 0.44). Conclusions Japanese male professors and associate professors received significantly higher PFCs than their female counterparts, and this gender gap expanded from 2016 to 2019, highlighting persistent gender inequality in the medical field in Japan, similar to trends observed in the United States.

Scholars of color remain underrepresented in US institutions in academia. In this paper, we will examine one factor that contributes to their continued marginalization in psychology and management: the scientific method's commitment to traditional notions of objectivity. We argue that objectivity-defined as practices and policies rooted in the heightened value placed on a research process that is ostensibly free from bias-is central to the prominence of primarily White scholarship in psychology and management research and remains central to knowledge production. To investigate this, we employ a mixed-methods approach, integrating qualitative and quantitative data to codify how scholars of color experience objectivity interrogations, or written and verbal questioning in academic contexts that implicates their scientific rigor. We also identify how scholars of color engage in objectivity armoring, or self-presentational strategies (toning down and stepping up) to contend with these interrogations. Finally, we reveal these toning down processes in language use within publications on racial scholarship. Overall, these studies reveal the unique challenges scholars of color face to legitimize and validate their work on race and racism within predominantly White institutions and disciplines.

What factors are associated with career outcomes among biomedical PhDs? Research to date has focused on drivers of interest in (and intention to pursue) various careers, especially during graduate school, but fewer studies have investigated participants' ultimate career outcomes. Even less is known about what factors matter for groups historically underrepresented in the US science, technology, engineering, and mathematics (STEM) workforce, such as women, some racial and ethnic groups, and persons with disabilities ( National Center for Science and Engineering Statistics (NCSES), 2021a). This study reports a new analysis of data from 781 PhD neuroscientists that were obtained from a retrospective survey ( Ullrich et al., 2021) to investigate the factors that influence the career sector in which neuroscience PhDs are employed and whether there were group differences according to social identity. We find evidence of academia as a "default path" for incoming PhD students, but interest in different careers increases over time. Those who remained in academia had greater acceptance of the structural aspects of academic careers, such as promotion and tenure processes, and greater faculty support during postdoctoral training. Conversely, prioritizing monetary compensation and/or varied work were associated with not being in academia, while a strong interest in research was positively associated with being in nonacademic research. Somewhat surprisingly, there were few interactions with gender, and no interactions with underrepresentation status, although perhaps this was due to lower statistical power for these analyses. Our findings also underscore the role of advisors, networking, and personal relationships in securing employment in STEM.
