RESUMEN

A female patient in her early 50s presented with a complaint of multiple right thigh swellings for eight years which on evaluation were found to be guinea worms. The patient had no risk factors as she was living in a rural area of Maharashtra. We performed radiological investigations which only revealed calcifications in subcutaneous planes. On exploring the excised specimen post-surgery there was a worm-like appearance which on histopathological study confirmed guinea worm disease. We highlight this unusual presentation of guinea worm disease so that surgeons can treat the condition effectively.

Dracunculiasis (Guinea Worm Disease) is a chronic disease that is primarily found in the arid and poor areas of our planet where water supply systems consist of open wells. This parasitic disease is transmitted to humans not only through the consumption of water contaminated with crustaceans harbouring larvae of Dracunculus medinensis, but also through the ingestion of paratenic (frogs) or transport hosts (fish). The natural progression of the disease is caused by adult worms invading connective tissues, leading to blistering and ulceration of the extremities, approximately one year after infection. In 1986, the Guinea Worm Eradication Program (GWEP) was launched and since then, the incidence of the disease has been reduced by over 99%. Indeed, the most recent global report from 2022 shows only 13 cases of human dracunculiasis worldwide, the lowest annual incidence ever reported. The new found knowledge of potential animal reservoirs and the recent discovery of possible edible paratenic hosts could pose challenges to the future eradication of this debilitating disease. Therefore, attempts to eradicate this parasitosis should not be postponed. Intensive research is needed in this neglected area of medicine, now that the goal is within reach.

Nematode species of the genus Dracunculus (Spirurida: Dracunculoidea) infect tissues and body cavities of reptiles, domestic and wild carnivores, and humans. The definitive hosts acquire the infection by ingesting intermediate (i.e., cyclopoid copepod) or paratenic (i.e., amphibians and fishes) hosts. Here we report the jaguar (Panthera onca) as a potential new host for Dracunculus sp. The nematode was collected from an ulcerated cutaneous nodule on the left anterior limb of a female jaguar in the municipality of Miranda, Mato Grosso do Sul state, Brazil. Based on the morphology of first stage larvae collected from a small fragment of the uterus of the adult nematode, the species was identified as Dracunculus sp. Reichard, 1759. Additionally, the morphological identification was molecularly confirmed by sequencing the cox1 gene. This report advocates for further investigations into the transmission cycle of this parasite in the Brazilian Pantanal wetland, considering the role of wildlife hosts and the zoonotic potential of Dracunculus species in that area.

Guinea worm disease (dracunculiasis) is a debilitating waterborne disease. Once widespread, it is now on the brink of eradication. However, the Guinea Worm Eradication Programme (GWEP), like guinea worm itself, has been under-studied by historians. The GWEP demonstrates an unusual model of eradication, one focused on primary healthcare (PHC), community participation, health education and behavioural change (safe drinking). The PHC movement collided with a waterborne disease, which required rapid but straightforward treatment to prevent transmission, creating a historical space for the emergence of village-based volunteer health workers, as local actors realigned global health policy on a local level. These Village Volunteers placed eradication in the hands of residents of endemic areas, epitomising the participation-focused nature of the GWEP. This participatory mode of eradication highlights the agency of those in endemic areas, who, through volunteering, safe drinking and community self-help, have been the driving force behind dracunculiasis eradication. In the twenty-first century, guinea worm has become firstly a problem of human mobility, as global health has struggled to contain cases in refugees and nomads, and latterly a zoonotic disease, as guinea worm has shifted hosts to become primarily a parasite of dogs. This demonstrates both the potential of One Health approaches and the need for One Health to adopt from PHC and the GWEP a focus on the health of humans and animals in isolated and impoverished areas. Guinea worm demonstrates how the biological and the historical interact, with the GWEP and guinea worm shaping each other over the course of the eradication programme.

Dracunculiasis (Guinea Worm Disease) is a terrible disease limited, even historically, to the arid and poor areas of our planet and which in the West has always been seen as an exotic disease and therefore has never taken root in the collective imagination. This parasitosis is transmitted to humans by drinking water contaminated with crustacean harboring larvae of Dracunculus medinensis, a nematode. The natural history of the disease is caused by adult worms invading connective tissues and causing blistering, ulceration and edema. Well known in Ancient Egypt where the disease was endemic in its southern area, was known in Europe mainly from the reports of medical writers starting from the Roman imperial period but without direct knowledge. In Middle age the descriptions of this disease that physicians and surgeons could read on medical books, at the end, were attributed to veterinary parasitic disease. In Modern age only during the colonialist era dracunculiasis was perceived as a problem, however sporadic. In 1986 Guinea Worm Eradication Program (GWEP) was launch without success. Thus, the disappearance of this parasitosis should still be postponed but not abandoned.

A nematode parasite, Dracunculus medinensis, causes dracunculiasis. Despite being non-fatal, this condition causes significant morbidity. Dracunculiasis is considered an eradicated disease in India since 1999. We report two cases that document the unusual linear morphea-like morphology of the calcified D. medinensis and the rare periorbital location of the worm. The cases presented here are rare and a diagnostic challenge, considering the eradicated status of dracunculiasis.

BACKGROUND: Dracunculiasis, also known as Guinea worm disease (GWD), is a neglected tropical disease (NTD) caused by a parasite (Dracunculus medinensis). In the past, dracunculiasis was known as "the disease of the empty granary" because of the difficulties patients had in going to work in fields or to school when affected by this disease. In tropical areas, the condition has been widespread in economically disadvantaged communities, and has been associated with reduced economic status and low levels of education. METHODS: we searched PubMed, Scopus, Google Scholar, EMBASE, Cochrane Library, and WHO websites for literature addressing dracunculiasis published in the last 50 years. RESULTS: by development and optimization of multi-layered control measures, transmission by the vector has been interrupted, but there are foci in several African countries with a high risk of compromising the results obtained in the control of this neglected disease. CONCLUSION: this review features state-of-the-art data on the infection prevalence, geographical distribution, diagnostics, parasite-host interactions, and the pathology of dracunculiasis. Also described are the current state and future perspectives for vector control and elimination strategies.

Exploitation of natural resources is a driver of human infectious disease emergence. The emergence of animal reservoirs of Guinea worm Dracunculus medinensis, particularly in domestic dogs Canis familiaris, has become the major impediment to global eradication of this human disease. 93% of all Guinea worms detected worldwide in 2020 were in dogs in Chad. Novel, non-classical pathways for transmission of Guinea worm in dogs, involving consumption of fish, have been hypothesized to support the maintenance of this animal reservoir. We quantified and analyzed variation in Guinea worm emergence in dogs in Chad, across three climatic seasons, in multiple villages and districts. We applied forensic stable isotope analyses to quantify dietary variation within and among dogs and GPS tracking to characterize their spatial ecology. At the end of the hot-dry season and beginning of the wet season, when fishing by people is most intensive, Guinea worm emergence rates in dogs were highest, dogs ate most fish, and fish consumption was most closely associated with disease. Consumption of fish by dogs enables a non-classical transmission pathway for Guinea worm in Chad. Seasonal fisheries and the facilitation of dogs eating fish are likely contributing to disease persistence and to this key impediment to human disease eradication. Interrelated natural resource use, climatic variation, companion animal ecology, and human health highlight the indispensability of One Health approaches to the challenges of eradicating Guinea worm and other zoonotic diseases.

Neglected tropical disease (NTD) is a set of 20 deadliest endemic diseases which shows its presence in most of the developing countries worldwide. Nearly 1 billion of the population are affected by it and suffered from poverty yearly. These diseases offer their own unique challenges and limitations towards effective prevention and treatment methods. Neglected tropical diseases are severe infections they may not kill the patient but debilitate the patient by causing severe skin deformities, disfigurement and horrible risks for several infections. Existing therapies for neglected diseases suffer from the loopholes like high degree of toxicity, side effects, low bioavailability, improper targeting and problematic application for affected populations. Progress in the field of nanotechnology in last decades suggested the intervention of nanocarriers to take over and drive the research and development to the next level by incorporating established drugs into the nanocarriers rather than discovering the newer drugs which is an expensive affair. These nanocarriers are believed to be a sure shot technique to fight infections at root level by virtue of its nanosize and ability to reach at cellular level. This article highlights the recent advances, rationale, targets and the challenges that are being faced to fight against NTDs and how the novel therapy tactics are able to contribute to its importance in prevention and treatment of NTDs.

Variation in the spatial ecology of animals influences the transmission of infections and so understanding host behavior can improve the control of diseases. Despite the global distribution of free-ranging domestic dogs Canis familiaris and their role as reservoirs for zoonotic diseases, little is known about the dynamics of their space use. We deployed GPS loggers on owned but free-ranging dogs from six villages in rural Chad, and tracked the movements of 174 individuals in the dry season and 151 in the wet season. We calculated 95% and core home ranges using auto-correlated kernel density estimates (AKDE95 and AKDEcore ), determined the degree to which their movements were predictable, and identified correlates of movement patterns. The median AKDE95 range in the dry season was 0.54 km2 and in the wet season was 0.31 km2 , while the median AKDEcore range in the dry season was 0.08 km2 and in the wet season was 0.04 km2 . Seasonal variation was, in part, related to owner activities; dogs from hunting households had ranges that were five times larger in the dry season. At least 70% of individuals were more predictably "at home" (<50 m from the household) throughout the day in the dry season, 80% of dogs demonstrated periodicity in activity levels (speed), and just over half the dogs exhibited periodicity in location (repeated space use). In the wet season, dogs mostly exhibited 24-h cycles in activity and location, with peaks at midday. In the dry season, dogs exhibited both 12- and 24-h cycles, with either a single peak at midday, or one peak between 06:00 and 12:00 and a second between 18:00 and 22:00. Strategies to control canine-mediated zoonoses can be improved by tailoring operations to the local spatial ecology of free-ranging dogs. Interventions using a door-to-door strategy in rural Chad would best conduct operations during the dry season, when access to dogs around their household more reliably exceeds 70% throughout the day. Given the importance of use in hunting for explaining variation in dog space-use, targeting approaches to disease control at the household level on the basis of owner activities offers potential to improve access to dogs.

Guinea-worm disease (GWD) was thought to be almost eliminated in Chad when it reemerged in 2010. The disease now shows a peculiar pattern of spreading along Chari River and its tributaries, rather than clustering around a particular drinking water source. We create a mathematical model of GWD that includes the population dynamics of the parasite as well as the dynamics of its hosts (copepods, fish, humans, and domestic dogs). We calibrate our model based on data from the literature and validate it on the recent GWD annual incidence data from Chad. The effective reproduction number predicted by our model agrees well with the empirical value of roughly 1.25 derived directly from the data. Our model thus supports the hypothesis that the parasite now uses fish as intermediate transport hosts. We predict that GWD transmission can be most easily interrupted by avoiding eating uncooked fish and by burying the fish entrails to prevent transmission through dogs. Increasing the mortality of copepods and even partially containing infected dogs to limit their access to water sources is another important factor for GWD eradication.

STUDY DESIGN: Case report. BACKGROUND: It is a case of dracunculiasis of the spine mimicking lumbar intervertebral disc herniation. CASE PRESENTATION: A 57 year-old Caucasian male was admitted to the hospital because of the left L5 radiculopathy lasting for 2 months. The pain in the left lower limb was associated with muscle weakness on dorsal flexion of the foot, paresthesia of the dorsal aspect of the foot and tingling in the big toe. Neurological examination revealed: muscle weakness on dorsal flexion of the foot, impaired light touch and pin prick test on the dorsal aspect of the foot and positive Lasègue's sign. Magnetic resonance imaging (MRI) examination revealed L4-L5 intervertebral disc herniation with sequester compressing the left L5 nerve root. The open L4-L5 left side discectomy was performed. During the sequester evacuation 3 pieces of nematodes were removed and preserved in 10% of formaldehyde solution. After the surgery the patient was pain free with normal neurological examination. The diagnosis of dracunculiasis was based on the morphology of the nematode and on exclusion of the other parasites. DM infestation could not be confirmed with molecular testing that was impaired by the formaldehyde. CONCLUSIONS: Parasite infestation should be considered even in cases with obvious MRI of lumbar intervertebral disc herniation. If a nematode was found accidentally during any surgery it should be preserved in a 0.9% saline, not in formaldehyde, not to disturb the molecular tests.

OBJECTIVE: The objective of this study was to identify the existing challenges in the last mile of the global Guinea Worm Eradication Program. METHODS: Systematic Review of articles published from 1 January 2000 until 31 December 2019. Papers listed in Cochrane Library, Google Scholar, ProQuest PubMed and Web of Science databases were searched and reviewed. RESULTS: Twenty-five articles met inclusion criteria of the study and were selected for analysis. Hence, relevant data were extracted, grouped and descriptively analysed. Results revealed 10 main challenges complicating the last mile of global guinea worm eradication: unusual mode of transmission; rising animal guinea worm infection; suboptimal surveillance; insecurity; inaccessibility; inadequate safe water points; migration; poor case containment measures, ecological changes; and new geographic foci of the disease. CONCLUSION: This systematic review shows that most of the current challenges in guinea worm eradication have been present since the start of the campaign. However, the recent change in epidemiological patterns and nature of dracunculiasis in the last remaining endemic countries illustrates a new twist. Considering the complex nature of the current challenges, there seems to be a need for a more coordinated and multidisciplinary approach of dracunculiasis prevention and control measures. These new strategies would help to make history by eradicating dracunculiasis as the first ever parasitic disease.


OBJECTIF: L'objectif de cette étude était d'identifier les défis existants sur le dernier kilomètre du programme mondial d'éradication de la dracunculose. MÉTHODE: Revue systématique des articles publiés du 1er janvier 2000 au 31 décembre 2019. Les articles répertoriés dans les bases de données Cochrane Library, Google Scholar, ProQuest PubMed et Web of Science ont été recherchés et examinés. RÉSULTATS: Vingt-cinq articles répondaient aux critères d'inclusion de l'étude et ont été sélectionnés pour l'analyse. Par conséquent, les données pertinentes ont été extraites, regroupées et analysées de manière descriptive. Les résultats ont révélé 10 principaux défis compliquant le dernier kilomètre de l'éradication mondiale du ver de Guinée: mode de transmission inhabituel, infection animale croissante du ver de Guinée, surveillance sous-optimale, insécurité, inaccessibilité; points d'eau salubres inadéquats, migration, mauvaises mesures de confinement des cas, changements écologiques et de nouveaux foyers géographiques de la maladie. CONCLUSION: Cette revue systématique montre que la plupart des défis actuels de l'éradication du ver de Guinée ont été présents depuis le début de la campagne. Cependant, le changement récent des profils épidémiologiques et de la nature de la dracunculose dans les derniers pays d'endémie restants illustre une nouvelle tournure. Compte tenu de la nature complexe des défis actuels, il semble nécessaire d'adopter une approche plus coordonnée et multidisciplinaire des mesures de prévention et de lutte contre la dracunculose. Ces nouvelles stratégies contribueraient à faire l'histoire en permettant l'éradication de la dracunculose en tant que toute première maladie parasitaire.

A fragment of a Dracunculus-like worm was extracted from the hind limb of a 2-year-old dog from Toledo, Spain. Cytochrome oxidase I and rRNA sequences confirmed an autochthonous mammalian Dracunculus worm infection in Europe. Sequence analyses suggest close relation to a parasite obtained from a North American opossum.

Dracunculiasis is the first parasitic disease set for eradication. However, recent events related to the Dracunculus medinensis epidemiology in certain African countries are apparently posing new challenges to its eradication. Two novel facts have emerged: the existence of animal reservoirs (mainly dogs but also cats and baboons), and possibly a new food-borne route of transmission by the ingestion of paratenic (frogs) or transport (fish) hosts. Therefore, instead of being exclusively a water-borne anthroponosis, dracunculiasis would also be a food-borne zoonosis. The existence of a large number of infected dogs, mainly in Chad, and the low number of infected humans, have given rise to this potential food-borne transmission. This novel route would concern not only reservoirs, but also humans. However, only animals seem to be affected. Dracunculus medinensis is on the verge of eradication due to the control measures which, classically, have been exclusively aimed at the water-borne route. Therefore, food-borne transmission is probably of secondary importance, at least in humans. In Chad, reservoirs would become infected through the water-borne route, mainly in the dry season when rivers recede, and smaller accessible ponds, with a lower water level containing the infected copepods, appear, whilst humans drink filtered water and, thus, avoid infection. The total absence of control measures aimed at dogs (or at other potential reservoirs) up until the last years, added to the stimulating reward in cash given to those who find parasitized dogs, have presumably given rise to the current dracunculiasis scenario in Chad.

La inclusión curricular de la historia de la medicina se hace cada vez más importante, y dentro de la misma el estudio de la simbología en medicina es un tema muy interesante y a veces polémico. Los símbolos asociados a la medicina usualmente presentan serpientes asociadas a los mismos, siendo los más reconocidos el bastón de Asclepio/Esculapio y el caduceo de Hermes/Mercurio. Las serpientes se encuentran asociadas a consideraciones positivas y negativas en las diferentes culturas del mundo. En el Antiguo Egipto eran símbolos de sabiduría y poder. En la Biblia tenían un rol dual y una participación en al menos un evento relacionado a curaciones masivas. La mitología griega nos provee de elementos que conectan a serpientes enrolladas en torno a una rama o vara directamente con la medicina. El tratamiento de la dracunculiasis nos otorga otra vía de conexión entre esta imagen y la medicina. El bastón de Asclepio/Esculapio se reconoce como símbolo de la medicina y las ciencias de la salud en general, mientras que el caduceo se asocia a otras profesiones no médicas.


The curricular inclusion of the history of medicine is becoming more and more important, and within it the study of symbology in medicine is a very interesting and sometimes controversial subject. The symbols associated with medicine usually present snakes associated with them, the most recognized being the rod of Asclepius/ Aesculapius and the Hermes/Mercury caduceus. Snakes are associated with positive and negative considerations in the different cultures of the world. In Ancient Egypt they were symbols of wisdom and power. In the Bible they had a dual role and a participation in at least one event related to massive healings. Greek mythology provides us with elements that connect snakes coiled around a branch or rod directly to medicine. The treatment of dracunculiasis gives us another way of connection between this image and medicine. The rod of Asclepius/ Aesculapius is recognized as a symbol of medicine and health sciences in general, while the caduceus is associated with other non-medical professions

Background: The objective of this study was to document the worldwide decline of dracunculiasis (Guinea worm disease, GWD) burden, expressed as disability-adjusted life years (DALYs), from 1990 to 2016, as estimated in the Global Burden of Disease study 2016 (GBD 2016). While the annual number of cases of GWD have been consistently reported by WHO since the 1990s, the burden of disability due to GWD has not previously been quantified in GBD. Methods: The incidence of GWD was modeled for each endemic country using annual national case reports. A literature search was conducted to characterize the presentation of GWD, translate the clinical symptoms into health sequelae, and then assign an average duration to the infection. Prevalence measures by sequelae were multiplied by disability weights to estimate DALYs. Results: The total DALYs attributed to GWD across all endemic countries (n=21) in 1990 was 50,725 (95% UI: 35,265-69,197) and decreased to 0.9 (95% UI: 0.5-1.4) in 2016. A cumulative total of 12,900 DALYs were attributable to GWD from 1990 to 2016. Conclusions: Using 1990 estimates of burden propagated forward, this analysis suggests that between 990,000 to 1.9 million DALYs have been averted as a result of the eradication program over the past 27 years.
