RESUMEN

Objective: Evaluate the prevalence of macrosomic newborns (birth weight above 4000 grams) in a high-risk maternity from 2014 to 2019, as well as the maternal characteristics involved, risk factors, mode of delivery and associated outcomes, comparing newborns weighing 4000-4500 grams and those weighing above 4500 grams. Methods: This is an observational study, case-control type, carried out by searching for data in hospital's own system and clinical records. The criteria for inclusion in the study were all patients monitored at the service who had newborns with birth weight equal than or greater than 4000 grams in the period from January 2014 to December 2019, being subsequently divided into two subgroups (newborns with 4000 to 4500 grams and newborns above 4500 grams). After being collected, the variables were transcribed into a database, arranged in frequency tables. For treatment and statistical analysis of the data, Excel and R software were used. This tool was used to create graphs and tables that helped in the interpretation of the results. The statistical analysis of the variables collected included both simple descriptive analyzes as well as inferential statistics, with univariate, bivariate and multivariate analysis. Results: From 2014 to 2019, 3.3% of deliveries were macrosomic newborns. The average gestational age in the birth was 39.4 weeks. The most common mode of delivery (65%) was cesarean section. Diabetes mellitus was present in 30% of the deliveries studied and glycemic control was absent in most patients. Among the vaginal deliveries, only 6% were instrumented and there was shoulder dystocia in 21% of the cases. The majority (62%) of newborns had some complication, with jaundice (35%) being the most common. Conclusion: Birth weight above 4000 grams had a statistically significant impact on the occurrence of neonatal complications, such as hypoglycemia, respiratory distress and 5th minute APGAR less than 7, especially if birth weight was above 4500 grams. Gestational age was also shown to be statistically significant associated with neonatal complications, the lower, the greater the risk. Thus, macrosomia is strongly linked to complications, especially neonatal complications.

A one-year-old female miniature goat was presented to an emergency service after calving a dead goatling. Physical and ultrasonographic examination revealed the presence of a viable fetus; therefore, the goat was submitted to an emergency cesarean section. In the postoperative period, the animal had septic peritonitis caused by Enterococcus faecium and Enterococcus casseliflavus. Both bacterial strains showed contrasting antimicrobial resistance profiles. Laparohysterectomy and abdominal cavity lavage were performed, but, once the animal had adhesions and necrotic lesions in abdominal organs, euthanasia was executed. A post-mortem examination revealed fibrino-necrotic septic peritonitis secondary to uterine rupture. To the authors' knowledge, this is the first detailed report of polymicrobial septic peritonitis in a miniature goat and the first report of septic peritonitis caused by E. faecium and E. casseliflavus.

This study aimed to estimate (co)variance components and genetic parameters for calving ease (CE) and their genetic correlations with growth, reproductive, carcass, and feed efficiency traits in Nellore cattle. Phenotypes for CE are scored in two categories: normal calving and assisted calving. The traits considered were probability of precocious calving, age at first calving, stayability, adjusted scrotal circumference at 365 days of age, accumulated cow productivity, age at puberty of males, gestation length, birth weight, adjusted weights at 210 and 450 days of age, adult cow weight, frame score, hip height, rib eye area, subcutaneous backfat thickness, rump fat thickness, intramuscular fat percentage, residual feed intake and dry matter intake. The estimation of genetic parameters was performed using a two-trait threshold-linear animal model, except for CE, stayability, and probability of precocious calving, which were evaluated through a two-trait threshold animal model. The direct (0.27) and maternal (0.19) heritability estimates for CE in heifers primiparous Nellore indicated that selecting for this trait is feasible. The selection to improve the female sexual precocity should consider CE during the selection and mating decisions to reduce calving problems. Genetic correlation estimates between CE and BW suggest that selecting low birth weight to reduce calving problems is not an appropriate strategy to improve calving ease in heifers Nellore. Therefore, adopting a multi-trait selection model with CE and BW in the Nellore breed would reduce calving difficulties, particularly in sexually precocious heifers, without impairing the growth, reproductive, feed efficiency conversion, and carcass indicator traits.

This retrospective observational study aimed to assess the effect of temperature-humidity index (THI) at calving and in the last trimester of pregnancy and calve-related factors affecting passive transfer of maternal immunoglobulin using Brix refractometry in Holstein calves. Blood samples from 4411 Holstein calves from a single large dairy farm in spring 2022 were used. A subset of data containing 6318 calvings was used to determine the effect of climatic conditions on the occurrence of agammaglobulinemia. Risk factors predictive of failure of passive transfer (FPT) were calculated using multiple logistic models. Females were 1.4 times more likely not to have FPT (56%; Brix% > 8 equivalent to ≥ 10 g/L IgG) than males (47.2%). Calves born as singles increased the likelihood of not presenting FPT (52.6%) than calves born as twins (42.9%). Calves from cows with no dystocic delivery had a lower risk for FPT (odds ratio = 2.3) than calves from cows with dystocia. Agammaglobulinemia was 1.5 and 1.8 times more likely to occur in calves with THI ≥ 80 and ≥ 82 in the last trimester of gestation and at calving, respectively, than in calves not experiencing heat stress. Agammaglobulinemia was twice as likely to occur in male than in female calves. Calves with birth weight ≥ 37 kg and gestation length ≥ 275 kg were less likely to present agammaglobulinemia than lighter calves and calves with shorter gestation periods. This study raises questions for management practices in Holstein calves undergoing in utero heat stress and around calving to avoid agammaglobulinemia.

This study evaluated the effect of a natural source of vitamin D3 [1,25-(OH)2D3] and capsaicin (CAP) in the dietary supplementation of sows in the final phase (85-114 days) of gestation (Gest) and lactation (Lact) on the reproductive performance of the sows and health of piglets through two experiments (Exp I and II). In Exp I, 120 sows were subjected to four treatments: T1-control (without [1,25-(OH)2D3] and supplemental CAP); T2-3.5 µg 1,25-(OH)2D3/Gest/day and 7.0 µg Vit 1,25-(OH)2D3/Lact/day; T3-7.0 µg CAP/Gest/day and 14.0 µg CAP/Lact/day; T4-1.75 µg Vit 1,25-(OH)2D3 + 3.5 µg CAP/Gest/day and 3.5 µg 1,25-(OH)2D3 + 7.0 µg CAP/Lact/day. In Exp II, 200 sows were randomly blocked, factorial 2 × 2 (without or with Vit 1,25-(OH)2D3 and without or with CAP): T1-control (without Vit 1,25-(OH)2D3 and CAP); T2-3.5 µg Vit 1,25-(OH)2D3/Gest/day and 7.0 µg Vit 1,25-(OH)2D3/Lact/day; T3-7.0 µg CAP/Gest/day and 14.0 µg CAP/Lact/day; T4-3.5 µg Vit 1,25-(OH)2D3 + 7 µg CAP/Gest/day; and 7.0 µg Vit 1,25-(OH)2D3 + 14.0 µg CAP/Lact/day. The duration of delivery (3:48 vs. 4:57 h) and the percentage of stillbirths (5.37% vs. 7.61%) were improved (p < 0.05) in the group that received Vit 1,25-(OH)2D3 (Exp II) compared to the control group. Moreover, the dystocia rate decreased (p < 0.05) in Exp II, which received Vit 1,25-(OH)2D3 (4.21 vs. 27.63%), and in Exp I, which received the combination of Vit 1,25-(OH)2D3 + CAP (12 vs. 40%) compared to the respective control groups. Colostrum production was greater (p < 0.05) in sows that received Vit 1,25-(OH)2D3 supplementation compared to the control group, consequently resulting in higher colostrum intake (p < 0.05) of the piglets (330 vs. 258 g/piglet). The additives reduced the incidence of diarrhea (p < 0.05) in piglets (Exp I and II). Thus, the use of additives improved the reproductive performance of sows and contributed to litter growth.

Emergências obstétricas são comuns na rotina clínica de cães e gatos. Dentre as emergências obstétricas uma das mais importantes é a distocia. De modo geral, a distocia ocorre quando há falha no momento do parto ou na expulsão do feto, sendo que a causa mais comum de distocia nas duas espécies é a inércia uterina primária. Outra emergência obstétrica de ocorrência frequente em cadelas e gatas é a piometra. Esta doença é uma infecção que ocorre no útero, causada por alterações hormonais na fase do diestro dos animais. As duas enfermidades (distocia e piometra) podem ter o envolvimento de anticoncepcionais, tanto nas cadelas como nas gatas. Em relação ao tratamento, na maioria das vezes, as distocias em cadelas e gatas são abordadas por meio da realização de cesarianas. Em piometra, a terapêutica mais efetiva é a realização de ovariohisterectomia. Neste estudo, 43 cadelas e 20 gatas com emergências obstétricas foram monitoradas durante 90 dias. Entre as cadelas, 29 (67,44%; 29/43) apresentaram piometra e 13 (30,23%; 13/43) distocia. Além disso, uma cadela apresentou prolapso vaginal. Entre as gatas, 13 apresentaram distocia (65%; 13/20) e sete (35%; 7/20) piometra. Algumas dessas emergências obstétricas foram relacionadas ao uso de anticoncepcional. Assim, os clínicos veterinários devem desencorajar esta prática e incentivar a castração das fêmeas.


Obstetric emergencies are common in the clinical routine of dogs and cats. One of the most important obstetric emergencies is dystocia. In general, dystocia occurs when there is a failure at the parturition time or expulsion of the fetus, and the most common cause of dystocia in both species is primary uterine inertia. Another frequently occurring obstetric emergency in bitches and cats is piometra. This disease is an infection that occurs in the uterus caused by hormonal changes in the diestrus phase of animals. In both diseases (dystocia and pyometra), contraceptives can be involved in female dogs and cats. Regarding the treatment, most of the time, dystocias in bitches and cats are addressed through cesarean sections. In pyometra, the most effective therapy is to perform ovariohysterectomy. In the present study, 43 female dogs and 20 female cats with emergency obstetric were monitored during 90 days. Among bitches, 29 (67.44%; 29/43) had pyometra, and 13 (30.23%; 13/43) showed dystocia. In addition, one female dog presented vaginal prolapse. Among the cats, 13 had dystocia (65%; 13/20) and seven (35%; 7/20) pyometra. Some of these obstetric emergencies were related to contraceptive use. Thus, veterinary clinicians must discourage this practice and encourage the neutering of females.

Atualmente muitos répteis se tornaram animais de companhia e são mantidos como pet's exóticos. A espécie Trachemys scripta elegans, Wied (1839) é um animal exótico da América do Norte, sua identificação é realizada pelas marcas avermelhadas encontradas lateralmente a sua cabeça. Na rotina clínica as principais enfermidades que acometem os quelônios são as de origem reprodutiva, como a estase folicular e distocia. O objetivo deste trabalho foi relatar um caso recorrente de distocia em um tigre d'água fêmea, para isso, a anamnese, o histórico da paciente, e seus sinais clínicos, em conjunto com os exames complementares de imagem foram essenciais para se obter diagnóstico definitivo. O tratamento foi realizado com a indução medicamentosa utilizando borogluconato de cálcio, seguida da aplicação de ocitocina, esta trouxe resultados positivos para a eliminação dos ovos. Porém devido ao histórico do paciente, optou-se pela intervenção cirúrgica de ovariossalpingectomia, sendo está a maneira permanente de resolução da patologia. O protocolo terapêutico escolhido proporcionou um resultado satisfatório e bem estar ao animal.(AU)


Currently, many reptiles have become companion animals and are kept as exotic pets. The species Trachemys scripta elegans, Wied (1839) is an exotic animal from North America, and its identification is based on the reddish markings found laterally on its head. In routine clinical practice, the main diseases that affect chelonians are those of reproductive origin, such as follicular stasis and dystocia. The aim of this study was to report a recurrent case of dystocia in a female red-eared slider turtle. For this purpose, the patient's anamnesis, history, and clinical signs, along with complementary imaging exams, were essential to obtain a definitive diagnosis. The treatment involved medical induction using calcium borogluconate, followed by the administration of oxytocin, which yielded positive results in egg elimination. However, due to the patient's history, surgical intervention in the form of ovariosalpingectomy was chosen as the permanent solution to the pathology. The chosen therapeutic protocol provided a satisfactory outcome and improved the animal's well-being.(AU)


Actualmente muchos reptiles se han convertido en animales de compañía y se mantienen como mascotas exóticas. La especie Trachemys scripta elegans, Wied (1839) es un animal exótico de América del Norte, su identificación se realiza por las marcas rojizas que se encuentran lateralmente a su cabeza. En la rutina clínica, las principales enfermedades que afectan a los quelonios son las de origen reproductivo, como la estasis folicular y la distocia. El objetivo de este trabajo fue reportar un caso recurrente de distocia en una hembra de tigre de agua, para ello la anamnesis, la historia de la paciente y sus signos clínicos, junto con los exámenes imagenológicos complementarios fueron fundamentales para obtener un diagnóstico definitivo. El tratamiento se realizó con inducción farmacológica con borogluconato de calcio, seguido de la aplicación de oxitocina, que arrojó resultados positivos con la eliminación de huevos. Sin embargo, debido a los antecedentes de la paciente, se optó por la intervención quirúrgica de ovarialpingectomía, que es la forma definitiva de resolución de la patología. El protocolo terapéutico elegido proporcionó un resultado satisfactorio y bienestar al animal.(AU)

Introducción. La distocia de hombros es una complicación del parto vaginal que se produce por dificultad en el parto de los hombros fetales. Puede desencadenarse en forma impredecible e imprevista, por lo que debería ser considerada como riesgo potencial de todo nacimiento. La mayoría de las recomendaciones sobre las maniobras de resolución de distocia de hombros lo hacen desde la posición de litotomía y sin considerar los movimientos intrínsecos de la pelvis durante el parto. Objetivos. Analizar las maniobras de resolución de distocia de hombros a partir del conocimiento de la biomecánica de la pelvis y su relación con los hombros fetales, teniendo en cuenta las diferentes posiciones de parto. Métodos. Revisión bibliográfica no sistematizada. Resultados. Ante la distocia del hombro anterior, si la gestante se encuentra en litotomía podría recomendarse la maniobra de McRoberts con presión suprapúbica seguida de la extracción del brazo posterior. Si la gestante se encuentra en posición vertical, se sugiere pasar a posición de cuatro apoyos y una variante original resultado del análisis de los movimientos de la pelvis llamada 'cuatro apoyos en asimetría'. Esta puede ser realizada desde cualquier posición, no es invasiva y requiere un mínimo de entrenamiento. Conclusiones. La resolución de distocia de hombros no posee un único algoritmo; dependerá del tipo de distocia, la posición de la gestante, el contexto y la mayor o menor habilidad de una maniobra sobre otra. La postura de Gaskin y cuatro apoyos en asimetría debería ser tenida en cuenta antes de realizar maniobras internas para la resolución de la distocia de hombros.


Introduction: Shoulder dystocia is a complication of vaginal delivery caused by a difficulty in delivering the fetal shoulders. It can be triggered in an unpredictable and unplanned manner, so it should be considered as a potential risk for every vaginal birth. Most of the recommendations on shoulder dystocia resolution maneuvers are made from the lithotomy position and without considering the intrinsic movements of the pelvis during labor. Objectives : To analyze the maneuvers for resolving shoulder dystocia based on knowledge of the biomechanics of the pelvis and its relationship with the fetal shoulders, considering the different birthing positions. Methods: Non-systematized bibliographic review. Results : In the case of anterior shoulder dystocia, the McRoberts maneuver with suprapubic pressure followed by extraction of the posterior arm could be recommended for a birthing woman in lithotomy position. If the birthing woman is in an upright position, it is suggested to move to the four-support position and an original variant resulting from the analysis of the biomechanics of the pelvis called 'four-lying in asymmetry'. These maneuvers are non-invasive techniques, require minimal training and resources, and can be performed from any childbirth position. Conclusions : The resolution of shoulder dystocia does not have a single algorithm; it will depend on the type of dystocia, the position of the birthing woman, the context, and the greater or lesser ability of one maneuver over another. Gaskin maneuver and four supports in asymmetry should be considered before performing internal maneuvers for the resolution of shoulder dystocia.

INTRODUCTION: Maternal mortality is a health problem in developing countries and is the result of several factors such as sociodemographic and economic inequalities and difficulties in accessing the health services. In addition, training strategies in obstetric emergencies targeting the non-medical personnel such as traditional midwives are scarce. The focus of this study is to develop learning and communication bridges on the management of obstetric emergencies and on policies of patients' referral to the biomedical health system in rural areas. METHODOLOGY: A Participant Action Research (PAR) study with a mixed methods approach was set up to elaborate culturally adapted flowcharts. The project lasted approximately 3,5 years, from September 2016 to January 2021. RESULTS: The study was conducted with 94 traditional midwives from southern Ecuador and is divided into 4 phases, namely: 1) Exploration: focus groups and interviews were conducted to document the management of obstetric emergencies through the presentation of "clinical case" scenarios in three important topics, namely: pre-eclampsia, shoulder dystocia and postpartum hemorrhage, 2) Planning: a number of reflective sessions were conducted between the researchers and the healers/midwives to elaborate flowcharts. 3) Action: the training was conducted in rooms dedicated to proficiency in the aforementioned topics and using the flowcharts, 4) Evaluation: 90% of the participants reported having used the flowcharts during the first year after the training. The most frequently used flowchart was that of pre-eclampsia for the recognition of warning signs during pregnancy control. CONCLUSION: This study documents common practices of pregnancy and delivery management by traditional midwives. Furthermore, cultural flowcharts were developed for and together with midwives to improve the clinical response to obstetric emergencies. The preliminary evaluation was favorable; the most frequently used flowchart concerned preeclampsia. In this process, establishing a partnership was crucial for successful intercultural collaboration.

The aim of this work was to identify causes of abortion through laboratory investigations in sheep flocks in Uruguay. One hundred cases of abortion, comprising 58 fetuses, 36 fetuses with their placentas, and 6 placentas were investigated in 2015-2021. Cases were subjected to gross and microscopic pathologic examinations, and microbiological and serological testing for the identification of causes of abortion, including protozoal, bacterial, and viral pathogens. An etiologic diagnosis was determined in 46 (46%) cases, including 33 (33%) cases caused by infectious pathogens, as determined by the detection of a pathogen along with the identification of fetoplacental lesions attributable to the detected pathogen. Twenty-seven cases (27%) were caused by Toxoplasma gondii, 5 (5%) by Campylobacter fetus subspecies fetus, and 1 (1%) by an unidentified species of Campylobacter. Fourteen cases (14%) had inflammatory and/or necrotizing fetoplacental lesions compatible with an infectious etiology. Although the cause for these lesions was not clearly identified, T. gondii was detected in 4 of these cases, opportunistic bacteria (Bacillus licheniformis, Streptococcus sp.) were isolated in 2 cases, and bovine viral diarrhea virus 1 subtype i (BVDV-1i) was detected in another. Campylobacter jejuni was identified in 1 (1%) severely autolyzed, mummified fetus. BVDV-2b was identified incidentally in one fetus with an etiologic diagnosis of toxoplasmosis. Microscopic agglutination test revealed antibodies against ≥1 Leptospira serovars in 15/63 (23.8%) fetuses; however, Leptospira was not identified by a combination of qPCR, culture, fluorescent antibody testing nor immunohistochemistry. Neospora caninum, Chlamydia abortus, Chlamydia pecorum, Coxiella burnetii and border disease virus were not detected in any of the analyzed cases. Death was attributed to dystocia in 13 (13%) fetuses delivered by 8 sheep, mostly from one highly prolific flock. Congenital malformations including inferior prognathism, a focal hepatic cyst, and enterohepatic agenesis were identified in one fetus each, the latter being the only one considered incompatible with postnatal life. Toxoplasmosis, campylobacteriosis and dystocia were the main identified causes of fetal losses. Despite the relatively low overall success rate in establishing an etiologic diagnosis, a systematic laboratory workup in cases of abortion is of value to identify their causes and enables zoonotic pathogens surveillance.

Emergências reprodutivas, como as piometras e distocias, são frequentes na rotina clínica de cadelas e gatas. A distocia ocorre quando há falha no momento do parto ou na expulsão do feto e sua causa mais comum, nas duas espécies, é a inércia uterina primária. A piometra é uma infecção que ocorre no útero, causada por alterações hormonais na fase do diestro dos animais. As duas enfermidades (distocia e piometra) podem ter o envolvimento de anticoncepcionais, tanto nas cadelas como nas gatas. Na maioria das vezes, as distocias, em cadelas e gatas, é abordada por meio da realização de cesarianas, enquanto para a piometra, a terapêutica mais efetiva é a realização de ovariohisterectomia. Desta forma, tendo em vista a deficiência na quantificação de emergências reprodutivas obstétricas em cadelas e gatas diagnosticadas em serviços veterinários do Brasil, este trabalho teve como objetivo relatar os achados obtidos através do monitoramento, durante 90 dias, de 43 cadelas e 20 gatas com emergências reprodutivas. Dentre as cadelas, 29 (67,44%; 29/43) apresentaram piometra, 13 (30,23%; 13/43) distocia e 1 prolápso vaginal. Dentre as gatas, 13 (65%; 13/20) apresentaram distocia e sete (35%; 7/20) piometra. Algumas das emergências reprodutivas foram relacionadas ao uso de anticoncepcional. Assim, os clínicos veterinários devem desencorajar esta prática e incentivar a castração das fêmeas.


Reproductive emergencies, such as pyometras and dystocias, are frequent in the clinical routine of female dogs and cats. Dystocia occurs when there is a failure during delivery or expulsion of the fetus, and its most common cause, in both species, is primary uterine inertia. Pyometra is an infection that occurs in the uterus caused by hormonal changes in the diestrous phase of animals. In both diseases (dystocia and pyometra), contraceptives can be involved in both female dogs and cats. Most of the time, dystocias, in female dogs and cats, is addressed through cesarean sections, while in pyometra, the most effective therapy is to perform ovariohysterectomy. Thus, given the deficiency in the quantification of obstetric reproductive emergencies in female dogs and cats diagnosed in veterinary services in Brazil, this study aimed to report the findings obtained by monitoring 43 female dogs and 20 female cats with reproductive emergencies for 90 days. Among bitches, 29 (67.44%; 29/43) presented pyometra, 13 (30.23%; 13/43) showed dystocia, and 1 had vaginal prolapse. Among the cats, 13 presented dystocia (65%; 13/20), and 7 (35%; 7/20) showed pyometra. Some reproductive emergencies were related to contraceptive use. Thus, veterinary clinicians must discourage this practice and encourage the castration of females

A anasarca fetal é caracterizada pelo edema generalizado do tecido subcutâneo, pela ampliação excessiva do feto, e, consequentemente, pela distocia obstrutiva durante o parto. Sua etiologia não foi elucidada, porém, acredita-se na relação da anasarca fecal com genes autossômicos recessivos, consanguinidade, malformações congênitas, dentre outros fatores. Além disso, o diagnóstico é feito por meio de ultrassonografia, pois não são observados sinais clínicos durante a gestação. Assim sendo, o objetivo deste estudo foi descrever o caso de um cão recém-nascido da raça Husky Siberiano diagnosticado com anasarca fetal. O laudo, o qual foi obtido com a realização de uma ultrassonografia, estimou a presença de seis fetos com movimentação presente e normal. Entretanto, em um dos fetos os achados ultrassonográficos foram edema e cistos repletos de líquido no tecido subcutâneo, efusão pleural e peritoneal, os quais são compatíveis com anasarca fetal. Assim, foi realizado o procedimento de cesariana programada. Devido ao edema, o filhote nasceu pesando um quilo, enquanto os outros filhotes pesavam em média 350 gramas. Apesar de nascer com batimentos cardíacos presentes, o animal veio a óbito instantaneamente após o parto. A necropsia confirmou os achados ultrassonográficos e revelou a presença de hipoplasia pulmonar. O caso relatado apresenta como causa a consanguinidade, destacando a importância de proporcionar estratégias de reprodução que visem evitar a endogamia.


Fetal anasarca is characterized by generalized swelling of the subcutaneous tissue, excessive enlargement of the fetus, and, consequently, obstructive dystocia during delivery. Its etiology has not been elucidated; however, it is believed that the fetal anasarca is related to autosomal recessive genes, consanguinity, congenital malformations, among other factors. In addition, the diagnosis is made through ultrasound, as clinical signs are not observed during pregnancy. Thus, this study aimed to describe the case of a newborn Siberian Husky dog diagnosed with fetal anasarca. The report, which was obtained with ultrasonography, estimated the presence of six fetuses with present and normal movement. However, in one of the fetuses, the sonographic findings were edema and cysts filled with liquid in the subcutaneous tissue, pleural and peritoneal effusion, which are consistent with fetal anasarca. Therefore, a scheduled cesarean section was performed. Due to the edema, the puppy was born weighing one kilo, while the other puppies weighed an average of 350 grams. Despite being born with a heartbeat, the animal died instantly after delivery. The necropsy confirmed the sonographic findings and revealed the presence of pulmonary hypoplasia. The reported case presents consanguinity as the cause, highlighting the importance of providing reproduction strategies to avoid inbreeding.

O Schistosomus reflexus (SR) é uma anomalia congênita fatal de baixa incidência observada principalmente em ruminantes. O objetivo deste estudo foi relatar a ocorrência de SRem uma vaca com um quadro de distocia, multípara e de partos anteriores normais, atendida em uma propriedade localizada em Irati, região Centro-Sul do Paraná, Brasil. Durante a anamnese, foi relatado que o trabalho de parto ocorreu normalmente, com dilatação cervical e rompimento da bolsa alantoidiana e amniótica. Contudo, não ouve exteriorização do feto. Devido à inviabilidade da tração forçada observada no exame físico, optou-se pela cesariana. O feto extraído era disforme, apresentando dorsoflexão da coluna vertebral, órgãos abdominais expostos, anquilose de membros e face deformada, as quais são alterações anatômicas compatíveis com as características de Schistosomus reflexusdescritas na literatura. O caso apresentado contribui de forma relevante para o aprimoramento da compreensão e estudos sobre esse distúrbio, uma vez que sua ocorrência é rara.


Schistosomus reflexus (SR) is a fatal congenital anomaly of low incidence mainly observed in ruminants. This study aimed to report the occurrence of SR in a cow with dystocia, multiparous, and normal previous deliveries, treated in a property located in Irati, south-central region of Paraná, Brazil. During anamnesis, it was reported that labor occurred normally, with cervical dilation and disruption of the allantoic and amniotic stalk. However, there was no externalization of the fetus. Due to the infeasibility of the forced traction observed in the physical examination, a cesarean was performed. The extracted fetus was misshaped, presenting dorsiflexion of the spine, exposed abdominal organs, ankylosis of limbs, and deformed face, which are anatomical alterations compatible with the characteristics of Schistosomus reflexus described in the literature. The case reported contributes significantly to the improvement of understanding and studies on this disorder, since its occurrence is rare.

Reproductive diseases have been well documented in domestic livestock such as sheep, goat, cattle and pigs. However, there is very little information on these diseases in the agouti (Dasyprocta leporina). The agouti is used for its meat in South America and the Caribbean. More recently, intensive farming of this animal is being practiced in the Neotropics. There is dearth of information on dystocia and vaginal prolapses in the agouti. This document reports on three cases of reproductive diseases in captive reared agoutis in Trinidad and Tobago. The first case was a female agouti weighing approximately 3 kg that was in the last stage of pregnancy, which was found dead in its cage. The vulva of the mother had the protruding hind-limbs of the fetus. Necroscopic evaluation of carcass revealed little fat tissue and the mother had two fetuses in the right horn of the uterus. Each fetus weighed approximately 200 g. The fetuses were well formed with fur, teeth and eyes. The placenta was attached to each fetus. The pathological findings suggested that dystocia resulted from secondary uterine inertia which was the cause of death of the adult female agouti. The second case was that of an adult female agouti weighing 2.5 kg. This female had given birth to an offspring three weeks prior and was observed to have had a vaginal prolapse. Surgery was performed and the prolapsed vagina was placed back into the pelvic cavity. Further to this intervention, the vagina prolapsed twice. Subsequent to the re-insertion of the vaginal tissue the agouti was euthanized. The third case was also that of a dystocia. However, the fetuses weighed 235 g and 165 g respectively and were in normalpresentation, posture and positioning. The fetus however was unable to pass via the vagina and was trapped in the pelvic cavity. This caused secondary uterine inertia which was the cause of death. As doenças reprodutivas têm sido bem documentadas em rebanhos domésticos, como ovinos, caprinos, bovinos e suínos. Porém, há muito pouca informação sobre essas doenças na cutia (Dasyprocta leporina). A cutia é usada como carne na América do Sul e no Caribe. Mais recentemente, a criação intensiva desse animal está sendo praticada na região neotropical. Há escassez de informações sobre distocia e prolapsos vaginais na cutia. Este documento relata três casos de doenças reprodutivas em cutias criadas em cativeiro em Trinidad e Tobago. O primeiro caso foi de uma cutia de aproximadamente 3 kg que estava na última fase de gestação, encontrada morta em sua gaiola. A vulva da mãe tinha as patas traseiras salientes do feto. A avaliação necroscópica da carcaça revelou pouco tecido adiposo e a mãe tinha dois fetos no corno direito do útero. Cada feto pesava aproximadamente 200 g. Os fetos eram bem formados com pelos, dentes e olhos. A placenta foi presa a cada feto. Os achados patológicos sugeriram que a distocia resultou de inércia uterina secundária, que foi a causa da morte da cutia adulta. O segundo caso foi ode uma cutia adulta pesando 2,5 kg. Essa fêmea deu à luz uma cria três semanas antes e foi observado que tinha prolapso vaginal. A cirurgia foi realizada e a vagina prolapsada foi colocada de volta na cavidade pélvica. RESUMEN Fundamento: La fascitis necrotizante es una infección que se desarrolla de manera rápida, afecta la piel, tejido celular subcutáneo, fascia superficial y en ocasiones la profunda, en heridas quirúrgicas al producir necrosis hística y severa toxicidad sistémica. Es una afectación sistémica acompañante, que a su vez a ella se asocian factores predisponentes endógenos como: diabetes mellitus, obesidad, alcoholismo, infección por VIH, y exógenos: cirugía ginecobstétrica, inyecciones, traumatismos; por lo que estos pacientes resultan ser enfermos críticos en las unidades de cuidados intensivos, con un curso clínico no uniforme, al llevar a una evolución fulminante cuando corresponde con la variante hiperaguda o fulminante. Objetivo: Describir la presencia de fascitis necrotizante hiperaguda causada por Enterococcus gallinarum, en una puérpera. Reporte de caso: Puérpera que se le practicó parto distócico por cesárea electiva a las 38.3 semanas, a las 24 h presentó manifestaciones generales que se acompañaron de lesiones en la piel; se le realizó cirugía ginecobstétrica, su evolución fue tórpida y falleció por fallo multiorgánico a los 3 días debido a las consecuencias de fascitis necrotizante hiperaguda o fulminante, causada esta por un Enterococcus gallinarum. Conclusiones: La fascitis necrotizante es una enfermedad poco frecuente y mortal si no se trata a tiempo; su diagnóstico resulta difícil en su fase inicial, debido a que casi siempre es clínico. El tratamiento quirúrgico debe ser urgente, combinarse este con antibióticoterapia sistémica, debido al germen causal y la toxicidad que producen.


ABSTRACT Background: Necrotizing fasciitis is an infection that develops rapidly, affecting the skin, subcutaneous cellular tissue, superficial fascia and sometimes deep fascia, in surgical wounds by producing hystenotic necrosis and severe systemic toxicity. It is a supplementary systemic affectation, which in turn is associated with endogenous predisposing factors such as: diabetes mellitus, obesity, alcoholism, HIV infection, and exogenous: gynecobstetric surgery, injections, traumatisms; so these patients are critical patients in intensive care units, with a non-uniform clinical progression, leading to a fulminant evolution when parallel to the hyperacute or fulminant variant. Objective: To describe the presence of hyperacute necrotizing fasciitis caused by Enterococcus gallinarum in a puerperal. Case report: A puerperal who suffered from dystocia delivery by elective cesarean section at 38.3 weeks, at 24 h presented general manifestations accompanied by skin lesions; she underwent gynecobstetric surgery, her evolution was torpid and she died of multiorgan failure after 3 days due to the consequences of hyperacute or fulminant necrotizing fasciitis, caused by Enterococcus gallinarum. Conclusions: Necrotizing fasciitis is a rare and fatal disease if not treated in time; its diagnosis is difficult in its early phase because it is often clinical. Surgical treatment should be urgent and combined with systemic antibiotic therapy due to the causative germ and the toxicity they produce.
