AIM: To assess the gloss and gloss retention of two esthetic restorative materials after brushing with a charcoal-infused toothbrush and activated charcoal toothpaste compared to their standard counterparts in all possible combinations. MATERIALS AND METHODS: In this study,104 disc-shaped specimens were prepared, 52 from each of the two materials: Beautifil II LS composite resin (Giomer) and Fuji II LC resin-modified glass ionomer (RMGI). Specimens of each material were divided into 4 groups (n = 13) according to the tested brushing procedure: group A-standard toothbrush + standard non-whitening toothpaste (S-S), group B-standard toothbrush + activated charcoal toothpaste (S-CH), group C-charcoal-infused toothbrush + standard non-whitening toothpaste (CH-S), and group D-charcoal-infused toothbrush + activated-charcoal toothpaste (CH-CH). Gloss was recorded using a gloss meter initially and after each brushing protocol. The data were statistically analyzed with two-way analysis of variance (ANOVA) and Tukey HSD post hoc tests using SPSS® v.27 software at 5% significance level. RESULTS: Beautifil II LS showed the highest gloss values after brushing in group D (44 ± 3.9), which was not significantly different from its initial value (46.1 ± 1.8), followed by groups A (32.2 ± 4), B (23.2 ± 3.6), and C (22.7 ± 3.9), while Fuji II LC showed its highest gloss values after brushing in groups D (16.6 ± 3.7) and a (15.4 ± 8.7), followed by groups C (10.9 ± 6) and B (4.4 ± 2.5), all were significantly lower than their respective initial gloss values. CONCLUSION: The gloss of Giomer and RMGI reduced significantly following the brushing with the tested brushing procedures except for the Giomer group brushed with a combination of charcoal-infused toothbrush and activated-charcoal toothpaste. CLINICAL SIGNIFICANCE: The brushing using a combination of charcoal-infused toothbrush and activated-charcoal toothpaste is recommended for the gloss retention of Beautifil II LS restorations. Aim of study: The goal of this paper is to find an association between the staining capacity of dental restorations used in pediatric patients and food items and to develop an optimum model to predict the most informative factor that causes the highest amount of color change through machine learning algorithms. Background: Color changes in restorative materials occur as a result of intrinsic and extrinsic factors, such as the type of restorative material, food items used, polished status of the material, and time interval. Materials and methods: This was an "in vitro study" conducted at Aligarh Muslim University, Aligarh, Uttar Pradesh, India. The study included 200 specimens, that is, 40 in each group A (orange juice), group B (Amul Kool Café), group C (Pepsi), group D (Amul Kesar Milk), and group E (artificial saliva). The materials were glass ionomer cement (GIC), resin-modified glass ionomer cement (RMGIC), microhybrid composite resin, and nanohybrid composite resin. These were further divided into polished and unpolished groups. The optimum modeling of the prediction of color change in materials by different effective factors was done by machine learning decision tree. We applied two algorithms: Chi-square automatic interaction detector (CHAID) and classification and regression tree (CART). In prediction modeling in the decision tree by CHAID and CART, color change is taken as the dependent variable, and group (type of restorative material), food items, time interval, and polished status are taken as independent variables. Results: The various beverages caused significant color variation due to different pigmentation agents. The agent that caused the highest color change was Kool Café. The Kesar Milk had the lowest pigmentation capacity. The greatest color variation was found on Glasionomer FX-II submerged in Pepsi and the least on Ivoclar Te-Econom Plus in Kesar Milk. The mean absolute error for the training dataset in the CART model and CHAID model is 0.379 and 0.332, and for the testing data set, it is 0.398 and 0.333, respectively. Therefore, the prediction of color change by the CHAID model is optimum, and we found that the restorative materials have a maximum predictor importance of 0.86 (86%), time interval 0.07 (7%), food items 0.04 (4%), and polished status has the least importance, that is, 0.03 (3%). Conclusion: The staining capacity of restorative material highly depends on the material itself, the initial time interval, and least on the food items used. Clinical significance: The clinical performance of dental restorations could be affected by various beverages consumed by children. This study thus provides important clinical insights into esthetic dentistry by offering valuable information on long-term color stability and the effect of polishing on common esthetic restorative materials used in pediatric dentistry. Physical attractiveness is commonly evaluated based on facial appearance, with the nose being the most noticeable feature. Botulinum toxin injection is a less invasive method for enhancing nasal esthetics, which can address minor nasal imperfections through relaxing muscles. To fully understand the uses of botulinum toxin in the nose area, a thorough review of the available information is necessary. We aimed to conduct a systematic review of literature to evaluate the effects and clinical application of botulinum toxin for nasal esthetics. We searched PubMed, Scopus, Google Scholar, and the Cochrane Central Register of Controlled Trials (CENTRAL) databases up until May 2023 to identify studies investigating the use of botulinum toxin for cosmetic enhancement in the nose area. Data was extracted from the selected studies regarding the baseline characteristics, type of botulinum toxin used, dosage, injection site, and outcomes. Initially, 1139 articles were found and 23 articles were included. The studies showed the usage of botulinum toxin can correct drooping nasal tips, dilated nostrils, minor post-rhinoplasty deformities, and high columellar show. It can also augment nasal dorsum, improve cleft lip-related deformities, and address other disorders related to the function and appearance of the nose. Botulinum toxin injection seems to be a safe and effective treatment option for individuals seeking to correct minor nasal defects. Further large-scale studies are recommended.

Orthodontic relapse and the demand for improved esthetics often necessitate retreatment in adult patients. This case report highlights the successful management of an adult female patient with bimaxillary protrusion, previously treated with a non-extraction approach. Treatment included lingual passive self-ligating appliances, premolar extractions, and mini-screws to reinforce anchorage. The patient's facial profile and dental esthetics were significantly improved after 20 months of treatment, achieving a stable occlusion and reduced bimaxillary protrusion. This case demonstrates that lingual appliances can be an effective and esthetic treatment option for complex orthodontic retreatment in adult patients. Careful biomechanical planning and attention to specific challenges, including torque control and bowing effect, are essential for successful outcomes.

The aim of the present study was to evaluate the photothermal effects of a subdermal high-power diode laser at a wavelength (λ) of 1470 nm in the skin of rats. Twenty male Wistar rats were used, divided into 2 groups: placebo laser (PL) and active laser (AL). A high-power diode laser equipment was applied to 5 subdermal vectors on the animal's back region. The results demonstrated that active laser animals showed a better arrangement of collagen fiber bands, an increase in the thickness of the dermis and the number of vessels. Furthermore, animals treated with active laser showed an increased immunoexpression of TGF-ß and VEGF compared to the placebo. The present work demonstrated that the subdermal high-power diode laser increases the vascularization and the expression of factors that enhance skin regeneration and may be promising resource in the esthetic and dermatology clinical treatment of skin rejuvenation.

This study aimed to identify and analyze articles on enamel microabrasion for the treatment of white spot lesions. A search was conducted on the Web of Science. The following parameters were recorded and analyzed: number of citations, year, journal, impact factor, study design, theme, country and continent, institution, authors, and keywords. Data was analyzed using VOSviewer software. The initial search resulted in 1,126 documents, of which 94 articles were included. The highest number of citations an article received was 65. The oldest article was published in 1975, and the most recent in 2023. The most frequent study design was case report (n = 42). Regarding the themes, it was observed that the main objective of the studies was to evaluate the clinical performance of enamel microabrasion (n = 75), primarily using Opalustre (Ultradent Products Inc., South Jordan, UT, USA) (n = 37) for treating white stains caused by dental fluorosis (n = 41). Most articles originated from Latin America (n = 31), mainly from Brazil (n = 26). The most frequent author was Sundfeld RH (n = 10). This study reveals research trends in the field of enamel microabrasion. The publications were mainly case reports/series using Opalustre for the removal of fluorosis stains.

BACKGROUND: Cosmetic tourism is an increasingly common phenomenon, both in Ireland and internationally. Complications associated with procedures performed abroad may manifest after the patient has traveled back to their home country with patients often presenting to local health services. Such complications can be infectious in nature requiring either antimicrobial therapy and/or source control in the form of wound debridement or radiologically guided interventional procedures. METHODS: Patients who had presented to a regional plastic surgery unit between September 2021 and December 2022 with complications related to aesthetic procedures performed outside Ireland were identified in this retrospective-prospective design. Medical records were reviewed to ascertain what procedures were performed, where the procedure had taken place, the nature of complications, and microbial culture and sensitivity results. RESULTS: Thirty patients were identified during the study timeframe, predominantly female (n = 28) with a mean age of 40.27 years (SD 10.6). Abdominoplasty was the most common procedure (n = 17), and Turkey was the most frequently cited destination (70%). Wound dehiscence accounted for the majority of complications (n = 18). Wound cultures were obtained in 80% (n = 24) of patients and a causative organism was isolated in 85% (n = 34) of cultures. Eighteen species of bacteria were identified and antimicrobial sensitivities were obtained for 16 of these. Antimicrobial resistance to commonly prescribed empiric antibiotics was noted in several isolates. CONCLUSION: Post-operative infections related to cosmetic tourism are a growing concern in plastic surgery. The bacterial etiology is varied, and antimicrobial resistance poses significant challenges, highlighting the need for early intervention and wound cultures to guide effective management.

Early childhood caries (ECC) continues to be a major global dental health concern for young children. This case report examines a young patient's transformation from decay to delight via comprehensive esthetic treatment. ECC in a four-year-old child resulted in significant dental caries and poor esthetics. In addition to space maintenance and orthodontic examination, essential treatments included dental extractions, stainless steel crowns, and composite restorations. The result showed that the child's smile and self-esteem had been successfully restored, as well as their oral health and function. This case emphasizes the value of early intervention, multidisciplinary teamwork, and individualized treatment plans in the management of early childhood caries and the restoration of pediatric patients' oral health.

Objective: To validate and ascertain the reliability of the Orofacial Esthetic Scale specifically within the context of prosthodontic patients, aiming to establish it as a credible and standardized tool for evaluating orofacial esthetics in this specialized cohort. Methods: The systematic analysis adhered to the guidelines outlined by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). A systematic search using four databases PubMed /Medline, Web of Science, Scopus, Science Direct along with manual and grey literature (ProQuest and Google scholar) till 30 October 2023 was performed. The risk of biasness was assessed using Joanna Briggs Institute (JBI) critical appraisal checklist followed by validity analysis. For meta-analysis, psychometric values (Cronbach alpha and Intra class correlation coefficient) were used through MedCalc software. Results: The overall ICC value was 0.95 (95% CI: 0.94 to 0.96) and the overall Cronbach alpha value was 0.94 (95% CI: 0.92 to 0.96). There was evidence of high heterogeneity and no publication bias among the studies included. Conclusion: This study underscores the pivotal role of orofacial esthetics in prosthodontic care, introducing the Orofacial Esthetic Scale as a validated tool to quantitatively assess subjective perceptions. This will highlight its potential for tailored treatment planning and improved patient outcomes.

Background: In Modern dentistry, the implant is the most popular and desirable management of tooth loss. Traditionally two stage (submerged) or one-stage (non-submerged) system has been added by many investigators. In the present study we evaluated the crestal bone loss during osseointegration phase among the three groups (i.e. submerged implants, non-submerged implants with anatomical healing abutment and non- submerged implants with esthetic healing abutment). Material and Methods: 10 subjects with 30 implants, were enrolled in the study. Subjects were randomized in three groups i.e., group 1 submerged (n=10), group 2 non-submerged with anatomical healing abutment (n=10), group 3 non submerged with esthetic healing abutments (n=10). Intraoral periapical radiograph (IOPA), IMAGE J software and CBCT were used to evaluate the crestal bone loss around each implant at baseline, 1 and 3 months after implant placement. Results: Crestal bone loss at the end of the 3months (osseointegration phase) was lowest in the submerged group (0.18+-0.06mm) followed by non-submerged esthetic group (0.21+-0.03mm) but it was statistically insignificant. Maximum amount of bone loss was observed in non-submerged anatomical abutment group (0.34+-0.03mm) which was highly significant. Conclusion: It can be concluded that submerged implants technique is a better option in comparison to non-submerged implant technique in terms of radiographical performance during initial phases of osseointegration.

OBJECTIVE: This article introduces the smile design and space (SDS) concept for managing the vertical dimension of occlusion (VDO) in full-mouth reconstructions and its influence on the selection of esthetic restorative materials. Limitations of traditional VDO adjustment methods are being addressed through a facially driven treatment approach that enhances esthetics improves function and minimizes invasiveness. CLINICAL CONSIDERATIONS: The SDS concept involves detailed evaluations of the incisal edges of the maxillary central incisors in relation to the upper lip at rest and at full smile. This includes analyzing variations in incisal exposure due to differing lip mobility, which can significantly impact gingival esthetics, particularly in cases with gingival exposure during smiling. The concept employs a strategic formula to determine the necessary alterations in VDO based on specific dental relationships and wear patterns. CONCLUSIONS: The SDS concept provides guidelines for full-mouth reconstructions, emphasizing minimal invasiveness and the integration of esthetics with functional dynamics to enhance the predictability of clinical outcomes and reduce complications associated with traditional restoration techniques. CLINICAL SIGNIFICANCE: The SDS concept enhances the precision of full-mouth reconstructions through individualized adjustment of the VDO, tailored to the esthetic and functional requirements of each patient.

BACKGROUND: The choice of the correct color is of paramount importance in esthetic dentistry; however, there is still no consensus on the best technique to determine it. The aim of the present study is to compare the accuracy of a recently introduced colorimeter in shade matching with human vision. In addition, possible variables affecting color-matching by human eye have been analysed. METHODS: 18 disc-shaped composite samples with identical size and shape were produced from a composite flow system (Enamel plus HriHF, Micerium): Nine were considered control samples (UD 0-UD 6), and nine were test samples with identical flow composite shade to the control ones. Parallelly, 70 individuals (dental students and dental field professionals) were individually instructed to sit in a dark room illuminated with D55 light and to perform visual shade matching between control and test discs. An error matrix containing ΔE94 between control and test discs was generated, containing four match-clusters depending on perceptibility and acceptability thresholds. The frequency and severity of errors were examined. RESULTS: The colorimeter achieved a 100% perfect matching, while individuals only achieved a 78%. A higher occurrence of mismatches was noted for intermediate composite shades without a statistically significant difference. No statistically significant differences were reported for age, sex, and experience. A statistically significant difference was present among the Optishade match and the visual determination. CONCLUSIONS: The instrumental shade-matching evaluation proved to be significantly more reliable than the human visual system. Further research is needed to determine whether the same outcomes are achieved in a clinical setting directly on patients.

This study aimed to examine the impact of substrates, ceramic shades, and brands on the color and masking ability of highly translucent monolithic zirconia (HTMZ) using CIELab and CIEΔE2000 metrics. A total of 156 1-mm thick HTMZ disks in shades A1, A2, and A3 were produced using Dental Direkt and Kerox zirconia brands. Four 3-mm thick substrates (nickel-chromium alloy, non-precious gold alloy (NPG), zirconia shade A2, and resin composite shade A2) were prepared. HTMZ disks were overlaid on these substrates, and color measurements were taken with a spectrophotometer. Color differences (ΔE) were analyzed using CIELab and CIEΔE2000 formulas. The influence of brand, shade, substrate, and their interactions on ΔE values was assessed with a General Linear Model (GLM) and LSD pairwise comparison test. Spearman's correlation test examined the relationship between CIELab and CIEΔE2000 values. Results indicated that ΔEab was significantly influenced by substrate type and shade, while ΔE2000 was also affected by the ceramic brand. Mean color differences across ceramic-substrate groups were within clinically acceptable and perceptible ranges (clinically perceptible: ∆Eab ≥ 1.3 and ∆E2000 ≥ 0.8; clinically acceptable: 0.8 < ∆E2000 ≤ 1.8 and 1.3 < ∆Eab ≤ 2.7), except for NPG, which had ΔE values exceeding the perceptible range (ΔE2000: 1.1 ± 0.11 to 1.8 ± 0.31; ΔEab: 1.61 ± 0.15 to 2.16 ± 0.36). A significant correlation (r = 0.974, P < 0.001) was found between ΔEab and ΔE2000. Various ceramic brands and shades led to notable ΔE variations, yet average color differences within all ceramic-substrate groups remained clinically acceptable. Both ΔEab and ΔE2000 were reliable methods with a strong correlation for measuring color differences.

In the modern era, patients are increasingly concerned about dental esthetics. Edentulism can significantly impact the appearance, occlusion, and self-esteem of the patient. Treatment options like removable dentures, fixed crown and bridge prostheses, and resin-retained bridges are available to replace missing teeth. Implant therapy is popular due to its high success rates and long-term durability. However, its efficacy can be compromised by errors in treatment planning, surgery, tissue care, and infections. Thus, meticulous planning and execution are crucial. Clinicians must have the expertise to manage difficulties during surgery and maintain stable soft tissue. The stability of tissues around osseointegrated implants affects long-term clinical stability and esthetics. Early implant placement is recommended for missing teeth in the esthetic zone, aiming to place implants in anatomically and functionally correct positions for durable and esthetic results. Patients with high cosmetic demands, thin gingival biotypes, and high smile lines pose challenges. Assessing Pink and White esthetics guides treatment planning. Advantages of early implant placement include simplified procedures and reduced post-surgical complications. Soft tissue molding is achieved using customized healing abutments and temporizing with fiber-reinforced resin-bonded prostheses. Customized healing abutments preserve socket width, prevent soft tissue collapse, and promote natural contouring, eliminating the need for secondary surgeries and aiding spontaneous healing. This case report outlines a comprehensive approach to achieving predictable esthetics through early implant placement and soft tissue modeling in a patient with a traumatically avulsed maxillary central incisor.

BACKGROUND: Esthetic orthodontic wires are preferred for their ease to fit in with natural tooth color, but their susceptibility to staining in the oral environment poses a concern. Various Coatings such as Teflon and Epoxy aim to enhance appearance and biocompatibility but may still result in discoloration. Understanding the color stability of these wires under different staining conditions is crucial for a better and enhanced treatment plan. OBJECTIVE: This study intended to assess the color stability of esthetic orthodontic wires under various staining solutions that are often used in daily life. METHOD: Color changes of Teflon and Epoxy-coated esthetic orthodontic wires were meticulously measured at baseline, 7, 14, and 21-day intervals utilizing the precise CIE Lab* color measurement system. A total of thirty-two samples of wires from each brand were prepared (n= 8/group) and immersed in staining solutions (coffee, tea, cola, and saffron). The color change within and between the groups was statistically evaluated (p< 0.05). RESULTS: Significant variations in color stability were observed across different staining solutions. Saffron emerged as the most potent agent, inducing the most pronounced color changes, whereas cola demonstrated the least impact. Furthermore, Epoxy-coated wires consistently exhibited superior color stability compared to their Teflon-coated counterparts across all staining solutions and time intervals. CONCLUSION: This study underlines the significance for orthodontists to consider staining agents' possible effects on orthodontic wires into account when selecting the orthodontic wires. The findings suggest that Epoxy-coated wires hold promise in mitigating discoloration issues during orthodontic therapy.

BACKGROUND: Taxanes are effective chemotherapy drugs for breast cancer care, but adverse effects pose a significant challenge in cancer treatment. Taxane-induced fluid retention and lower-extremity edema are two of the important dose-limiting toxicity and result in decreased quality of life (QoL). However, there is no standard of care to alleviate the symptoms. We conducted a clinical study to assess the efficacy of short-term aroma lymphatic tressage therapy (ALTT) in reducing taxane-induced edema in breast cancer patients. METHODS: In this phase 2 clinical trial, patients with edema of CTCAE grade 2 or higher were enrolled and conducted 8 sessions of ALTT. The primary endpoint was to determine the proportion of patients with a reduction in lower extremity circumference of 3% or more before and 6 weeks after starting ALTT. The change in QoL was assessed as the secondary endpoint using QoL questionnaires. RESULTS: A total of 37 breast cancer patients completed the protocol and were analyzed. The median sum of the 3-point circumference (thigh, calf, and ankle) was 230.8 cm (IQR 218-243) in the baseline and 220.2 cm (IQR 212-236) at the end of the study. The average decrease of circumference was 3.8%. About, 23 patients (62%) experienced a circumference decrease of 3% or more. An improvement in every scale of FACT-TAXANE and EORTC-QLQ-C30 was observed when comparing questionnaire results before and at the end of the intervention (P < 0.0001). CONCLUSION: Eight sessions of ALTT over 4 weeks were effective in reducing lower-extremity edema and resulted in improvement of QoL in patients with breast cancer.

OBJECTIVE: The principal aim of this report is to address the challenge of integrating prosthetic crowns with natural dentition in the esthetic zone. It highlights the utilization of a prosthetically driven treatment plan, designed to ensure predictable esthetic outcomes. CLINICAL CONSIDERATIONS: This report details a technique that utilizes three digital guides, all derived from a 3-dimensional digital smile design. The integration of a restoration guide, an implant drilling guide, and an alveoloplasty guide is described. These tools collectively facilitate the precise execution of both surgical and prosthetic procedures, enhancing treatment accuracy and esthetic integration. CONCLUSIONS: This technique considers the esthetic prosthetic crowns, implant positions, and alveoloplasty collectively. It enhances the predictability of esthetic outcomes in oral implantology and potentially provides an integrated prosthetically driven workflow in cosmetic dental treatments. CLINICAL SIGNIFICANCE: The application of multiple digital guides derived from the same prosthetically driven treatment significantly enhances the predictability of esthetic outcomes in oral implantology.

RESUMEN

Aim: This study was designed to compare the clinical efficacy of two esthetic restorative materials, nanoceramic (Magma NT®) and giomer (Beautifil II®), as full coronal restoration in primary maxillary incisors. Materials and methods: A total of 15 patients aged 3-5 years presenting with mutilated primary maxillary incisors due to caries or trauma were selected for the study using randomized simple sampling. A total of 40 maxillary incisors were randomly divided into two equal groups, with 20 teeth in each group. Teeth in group I (GP I) were restored with nanoceramic (Magma NT®) and group II (GP II) with giomer (Beautifil II®). The full coronal restorations were done using strip crowns (3M ESPE). The restorations were evaluated for gross fracture, marginal integrity, and secondary caries according to modified Ryge's criteria [United States Public Health Service (USPHS)] at baseline (immediate postoperative), 3, 6, and 9 months. Parental satisfaction with each type of restoration was also evaluated using the Likert 5-point scale. Results: The data obtained was statistically analyzed using the Chi-squared test, and the level of significance, that is, the p-value, was determined. The Chi-squared test showed no significant changes to all modified USPHS criteria for each material at baseline and 3-month evaluation period. The changes recorded were after a 3-month follow-up between the two materials; nanoceramic (Magma NT®) restoration demonstrated marginally better than giomer (Beautifil II®) in terms of gross fracture and marginal integrity; however, there was no statically significant difference between them (p > 0.05), while giomer (Beautifil II®) was better than nanoceramic in terms of secondary caries (p < 0.05). Parental satisfaction for both entities was comparable in terms of color and durability; however, they were cost-ineffective. Conclusion: Nanoceramic restoration demonstrated better results in terms of gross fracture and marginal integrity, while giomer was better in terms of secondary caries. Clinical significance: Nanoceramics and giomers can serve as an alternative to conventional restorative materials in primary anterior teeth because of their improved qualities. Background: Smile esthetics can be subjective. This study aims to assess the effect of culture/geographic location on judging smile esthetics by comparing the perception of smile characteristics between American laypersons (Western culture) and Iranians (Eastern). Materials and Methods: One hundred residents from each country were recruited for this analytical cross-sectional study. A frontal picture of a posed smile with standard norms which are mentioned in the Contemporary Orthodontics textbook by William R. Proffit was selected. Adobe Photoshop 7 was used to alter midline diastema, gingival display (GD), maxillary midline-to-face discrepancy, buccal corridor (BC), and smile arc. Images were given to groups to determine the most attractive and acceptable smile. Data were analyzed by Chi-square, Mann-Whitney, and Spearman correlation coefficient in SPSS-22 (α =0.05). Results: Americans were less critical in judging diastema (P < 0.001), GD (P = 0.013), and BC (P = 0.004) for smile attractiveness than Iranians. No difference was between the two groups in choosing the acceptability threshold and determining the most attractive smile except for BC (P = 0.002). Conclusion: Overall, Americans were more tolerant of variations in smile characteristics than Iranians. Both groups agreed on most smiles. Therefore, it seems that it is reasonable to use similar standards for smile characteristics while considering the small differences.
