RESUMEN

STUDY DESIGN: A systematic review of the literature about differential diagnosis between spine infection and bone tumors of the spine. BACKGROUND AND PURPOSE: The differential diagnosis between spine infection and bone tumors of the spine can be misled by the prevalence of one of the conditions over the other in different areas of the world. A review of the existing literature on suggestive or even pathognomonic imaging aspects of both can be very useful for correctly orientating the diagnosis and deciding the most appropriate area for biopsy. The purpose of our study is to identify which imaging technique is the most reliable to suggest the diagnosis between spine infection and spine bone tumor. METHODS: A primary search on Medline through PubMed distribution was made. We identified five main groups: tuberculous, atypical spinal tuberculosis, pyogenic spondylitis, and neoplastic (primitive and metastatic). For each group, we evaluated the commonest localization, characteristics at CT, CT perfusion, MRI, MRI with Gadolinium, MRI diffusion (DWI) and, in the end, the main features for each group. RESULTS: A total of 602 studies were identified through the database search and a screening by titles and abstracts was performed. After applying inclusion and exclusion criteria, 34 articles were excluded and a total of 22 full-text articles were assessed for eligibility. For each article, the role of CT-scan, CT-perfusion, MRI, MRI with Gadolinium and MRI diffusion (DWI) in distinguishing the most reliable features to suggest the diagnosis of spine infection versus bone tumor/metastasis was collected. CONCLUSION: Definitive differential diagnosis between infection and tumor requires biopsy and culture. The sensitivity and specificity of percutaneous biopsy are 72% and 94%, respectively. Imaging studies can be added to address the diagnosis, but a multidisciplinary discussion with radiologists and nuclear medicine specialists is mandatory.

The aim of this clinical study is to present an integrated digital project through the description of a clinical case, made entirely in digitized form, taking advantage of the opportunity offered by instrumental diagnostic software. A case report participant is a 65-year-old female patient presents with loss of diffuse bone support, caused by periodontal disease. After a sign of an informed consent and an explication of a plan of treatment, technical intraoral and extraoral pictures and intraoral digital impressions were taken. The digital images improved from the 2D Smile Lynx Software and the scanner stereolithographic (STL) file was matched into the CAD Lynx to obtain a virtual previsualization of teeth and smile design, and to mill the provisional and the definitive crowns. The digital prosthetic design allows the evaluation of the dental parameters in relation to the parameters of the patient's face for the new prosthetic project and the radiological examination using CBCT guides the insertion of the fixtures for the rehabilitation phase. The surgical and prosthetic design are subsequently integrated. The evaluation of the bone bases is carried out with a radiological diagnostic software for CT (Real Guide 5.0-3Diemme, Cantù-Italy) which can virtually design the implant insertion. The functional examination of the patient is carried out through an occlusion-postural examination that uses digital electromyographic assessments. The integrated digital protocol proposal inserts in the rehabilitation path the digital recording of the free mandibular movement, as well as the scan of the patient's face, data that will be integrated into the CAD software for the design of temporary and definitive prosthetic artifacts, made using the CAM method. This study showed guided implant placement and the application of fixed implant-supported prosthetic restorations carried out with a fully digital workflow, dependent on the functional digital evaluation of the patient's occlusion. The proposed protocol described the correct use of digitalization of clinical, surgical, and prosthetic procedures, and the matching of the data into a computerized environment, to improve team communication and to take advantage of the combination of collected data to not lose information using classic manual steps.

BACKGROUND AND OBJECTIVE: PPFE is characterized by fibrosis in the pleura and subpleural lung parenchyma in the upper lobes, while other types of ILD, mainly UIP, can be observed in about half of the patients in their lower lobes. The aim of this study was to evaluate the clinical significance of the radiologically defined PPFE in patients with IPF. METHODS: Clinical data and chest CT images were retrospectively analysed in 445 patients with IPF (biopsy-proven cases, n = 165). The radiological criteria of PPFE were defined as follows: (i) bilateral subpleural dense fibrosis with or without pleural thickening in the upper lobes, (ii) evidence of disease progression and (iii) no clinical evidence of identifiable aetiologies. RESULTS: The median follow-up period was 43.0 months. The mean age of the patients was 66.4 years and 76.4% were male. PPFE was identified in 28 patients (6.3%). The PPFE group showed lower BMI and lung function (FVC and TLC) at baseline, more frequent pneumothorax and pneumomediastinum, higher decline rates in lung function and poorer prognosis during follow-up than the no-PPFE group. PPFE was an independent risk factor (HR = 2.953, 95% CI: 1.350-6.460, P = 0.007) for pneumothorax or pneumomediastinum, but not for mortality in patients with IPF. CONCLUSION: Among patients with IPF, the PPFE group, when compared to the no-PPFE group, showed lower BMI and lung function and showed more frequent complications and poorer survival during follow-up.
