RESUMEN

Introduction: Soft tissue sarcomas (STS) are low-incidence tumors whose clinical and histopathological factors are associated with adverse oncological outcomes. This study evaluated prognostic factors (PF) associated with tumor recurrence and overall survival (OS) in patients diagnosed with STS of the extremities, treated at the Instituto Nacional de Cancerología (INC), Bogotá, Colombia. Materials and Methods: An analytical observational study of a historical cohort was carried out, including patients diagnosed with STS and managed surgically in the Functional Unit for Breast and Soft Tissue Tumors of the INC from January 2008 to December 2018. Results: A total of 227 patients were included; 74.5% had tumors greater than 5 cm. Most patients (29.1%) were in stage IIIB at diagnosis. Age was associated with higher mortality (HR = 1.01; CI95%: 1-1.02; p = 0.048). Tumor persistence at admission to the INC (HR = 2.34; CI95%: 1.25-4.35; p = 0.007) and histologic grade III (HR = 5.36; CI95%: 2.29-12.56; p = <0.001) showed statistical significance in the multivariate analysis for recurrence of any type, as did the PFs associated with a higher risk of local recurrence (HR = 2.85; CI95%: 1.23-6.57; p = 0.014 and HR = 6.09; CI95%: 2.03-18.2; p = 0.001), respectively. Tumor size (HR = 1.03; CI95%: 1-1.06; p = 0.015) and histologic grade III (HR = 4.53; CI95%: 1.42-14.49; p = 0.011) were associated with a higher risk of distant recurrence. Conclusions: This cohort showed that in addition to histologic grade and tumor size, tumor persistence at the time of admission has an impact on disease recurrence, so STS should be managed by a multidisciplinary team with experience in this pathology in high-volume reference centers.

RESUMEN

Hydrogels are three-dimensional structures with specific features that render them useful for biomedical applications, such as tissue engineering scaffolds, drug delivery systems, and wound dressings. In recent years, there has been a significant increase in the search for improved mechanical properties of hydrogels derived from natural products to extend their applications in various fields, and there are different methods to obtain strengthened hydrogels. Cationic guar gum has physicochemical properties that allow it to interact with other polymers and generate hydrogels. This study aimed to develop an ultra-stretchable and self-healing hydrogel, evaluating the influence of adding PolyOX [poly(ethylene oxide)] on the mechanical properties and the interaction with cationic guar gum for potential tissue engineering applications. We found that variations in PolyOX concentrations and pH changes influenced the mechanical properties of cationic guar gum hydrogels. After optimization experiments, we obtained a novel hydrogel, which was semi-crystalline, highly stretchable, and with an extensibility area of approximately 400 cm2, representing a 33-fold increase compared to the hydrogel before being extended. Moreover, the hydrogel presented a recovery of 96.8% after the self-healing process and a viscosity of 153,347 ± 4,662 cP. Therefore, this novel hydrogel exhibited optimal mechanical and chemical properties and could be suitable for a broad range of applications in different fields, such as tissue engineering, drug delivery, or food storage.

RESUMEN

This work consists of analyzing the impact of geometrical features (thickness and curvature) on the estimation of circumferential residual stresses in arteries. For this purpose, a specific sample of lamb abdominal artery is chosen for analysis and, through computational tools based on Python libraries, the stress-free geometry is captured after the ring opening test. Numerical simulations are then used to reconstruct the sample in order to estimate the circumferential residual stresses. Then, four stress-free geometry models are analyzed: an ideal geometry, i.e., constant curvature and thickness; a constant curvature and variable thickness geometry; a variable curvature and constant thickness geometry; and a variable curvature and thickness geometry. The numerical results show that models perform well from a geometric point of view, where the most different feature was the closed outer perimeter that differs about 14% from the closed real sample. As far as residual stress is concerned, differences up to 198% were found in more realistic models taking a constant curvature and thickness model as reference. Thus, the analysis of a realistic geometry with highly variable curvature and thickness can introduce, compared to an idealized geometry, significant differences in the estimation of residual stresses. This could indicate that the characterization of arterial residual stresses is not sufficient when considering only the opening angle and, therefore, it is also necessary to incorporate more geometrical variables.

RESUMEN

We describe a case of extensive human cutaneous protothecosis in the northeastern of Brazil. The diagnosis was made through histopathological examination of skin samples and culturing on Sabouraud dextrose agar, with identification through mass spectrometry. Treatment with itraconazole failed and was switched to liposomal amphotericin followed by voriconazole with clinical cure. Prothotecosis should be a diagnostic hypothesis when there are skin lesions after trauma, specially in situations of atypical evolution.

RESUMEN

Objective: To evaluate the aesthetic preferences of individuals by observing photographs of different facial profiles with different sagittal and vertical growth patterns. Materials and Methods: Cross-sectional study that evaluated 300 students from a private university. Edited photographs of a man and a woman were used. The maxilla being moved in a vertical direction while the mandible was moved in a sagittal direction, various combinations of the photographic profile of the two individuals were created. These movements created 9 photographic profiles for the woman and the man; for intra-observer reliability, two photo profiles of both sexes were duplicated. Chi square tests and binary logistic regression were used. P<0.05. Results: In general, the evaluated individuals mostly preferred the type 6 profile (65.7%), followed by the type 5 profile (21.7%) (slightly convex). It was found that for every year of age that an individual increases, there is a 31% greater chance of choosing one of these profiles. Conclusions: Both sexes prefer a straight or slightly convex profile, and as age increases, individuals prefer a straight profile.

RESUMEN

Biophotonics is defined as the combination of biology and photonics (the physical science of the light). It is a general term for all techniques that deal with the interaction between biological tissues/cells and photons (light). Biophotonics offers a great variety of techniques that can facilitate the early detection of diseases and promote innovative theragnostic approaches. As the COVID-19 infection can be transmitted due to the face-to-face communication, droplets and aerosol inhalation and the exposure to saliva, blood, and other body fluids, as well as the handling of sharp instruments, dental practices are at increased risk of infection. In this paper, a literature review was performed to explore the application of Biophotonics approaches in Dentistry focusing on the COVID-19 pandemic and how they can contribute to avoid or minimize the risks of infection in a dental setting. For this, search-related papers were retrieved from PubMED, Scielo, Google Schoolar, and American Dental Association and Centers for Disease Control and Prevention databases. The body of evidence currently available showed that Biophotonics approaches can reduce microorganism load, decontaminate surfaces, air, tissues, and minimize the generation of aerosol and virus spreading by minimally invasive, time-saving, and alternative techniques in general. However, each clinical situation must be individually evaluated regarding the benefits and drawbacks of these approaches, but always pursuing less-invasive and less aerosol-generating procedures, especially during the COVID-19 pandemic.

RESUMEN

Articular degeneration can be characterized by fibrillation and eburnation of the articular layers of the metacarpophalangeal (MP) joint. Structural changes within the articular joint predispose the development of osteophytes, enthesophytes and, in many cases is associated with changes the collateral ligaments of the MP joint. The aim of the present study was to investigate the relationship between degenerative processes of the articular cartilage and the enthesopathies of collateral ligaments (superficial or deep portion) of the equine MP joint, using radiographic, ultrasonographic and anatomopathological exam, as well as establishing their sensitivity. Thirty equine forelimbs were selected from animals that died due to various clinical conditions with an average age of 5.7 years which came from a private clinic or sent to the Laboratory of Veterinary Pathology of UFSM. The specimens were placed in a hydraulic press and submitted to radiographic evaluation of the MP joint (lateromedial, flexed lateromedial, dorsopalmar, dorsolateral-palmaromedial oblique 450 and dorsomedial-palmarolateral oblique 450 projections). Transversal and longitudinal ultrasound images of the dorsal and collateral aspect of the MP joint were obtained using a Sonosite Edge machine with a 5 to 10 MHz linear transducer. Subsequently, the specimens were sent for gross and histopathological examination. A highly positive correlation (P value <.001) was observed between ultrasonographic and macroscopic techniques, showing a relationship between the cartilaginous degenerations of the MP joint and the medial and lateral LC enthesopathies.

The case report of a 32 years patient with history of paranoid schizophrenia is described. She was assisted in the Orthopedics and Traumatology Service of Dr. Juan Bruno Zayas Alfonso Teaching General Hospital in Santiago de Cuba, due to an increase of volume in the internal face of the left thigh that difficulted her from walking. A tumor of approximately 20 cm was observed with the physical exam, that was not painful to the palpation, but soft and doughy, with imprecise limits and adhered to deep planes. According to the imaging studies it was an adipose tissue, reason why there was no correlation with the elements obtained with the physical exam. Taking into account the above-mentioned the case was discussed with the imaging specialists and the decision was removing the tumor. A sample for biopsy was taken whose result informed the presence of a lipoma. The woman had a favorable clinical course and she returned to her usual activities 2 months later.

The objective of the study was to determine the effect of platelet-rich fibrin (PRF) on the healing of soft tissues of socket after atraumatic exodontia. The present is a single-blind, cross-sectional randomized controlled trial. It was carried out in the Dental Service of the Santa Isabel District Hospital of Porvenir - Peru, during the years 2016 and 2017. The sample consisted of 51 patients whose alveoli post exodontia were randomly divided into 2 groups, each patient signed an informed consent. Group A (control) was left with a blood clot for normal healing and group B (experi- mental) was given PRF (as a plug and membrane), obtained according to the Choukroun protocol. The effect of PRF on the healing of the soft tissues of the alveoli after atraumatic exodontia was used the Landry index. The measurements were made at 7 and 14 days after surgery. For the comparison of each one of the variables of the study, the Mc Nemar test and the Fisher exact test were used. The statistical significance was 5 %. When comparing both groups in the healing of the soft tissues of alveoli after atraumatic exodontia, a statistically significant difference was found at 7 and 14 days after surgery (p 0.05). The PRF has a positive effect on the healing of the soft tissues of the alveoli after atraumatic exodontia independently of sex and age.

RESUMEN

OBJECTIVES: This study set out to determine the average temperature of skin and soft tissue tumors in cats using infrared thermography and to investigate correlations between thermographic findings and tumor type. Correlations between thermographic findings, histologic subtype and tumor grade were also investigated in cases of feline injection site sarcoma (FISS). METHODS: Thermographic images of normal skin and skin overlying neoplastic lesions were prospectively obtained. Following thermographic assessment, tumors were resected and submitted to histopathologic and immunohistochemical analysis. Mean temperatures detected in tumoral areas were compared between different tumor types and between FISSs of different histologic subtypes and grades. RESULTS: Thermograms obtained from 11 healthy cats and 31 cats presenting with skin and soft tissue tumors (eight benign and 23 malignant tumors, including 21 FISSs) were evaluated in this study. Thermal behavior varied widely in normal skin, as well as in skin overlying neoplastic lesions. Mean temperatures were significantly higher in malignant compared with benign tumors (35.4 ± 1.8ºC and 34.5 ± 1.7ºC respectively; P = 0.01), with a temperature above 34.7ºC being associated with malignancy (sensitivity 76%, specificity 80%; P = 0.01). Temperatures detected in FISS did not differ significantly according to histologic subtype (P = 0.91) or tumor grade (P = 0.46), or between primary and recurring tumors (P = 0.25). CONCLUSIONS AND RELEVANCE: Infrared thermography proved to be a sensitive and effective method for detection of temperature differences between malignant and benign skin and soft tissue tumors in cats. Thermographic assessment may contribute to diagnosis and prognostic estimation in feline oncologic patients.

RESUMEN

OBJECTIVES: To present the ballistic gelatin as a new material capable of simulating the soft tissues in cone-beam CT (CBCT) images. METHODS: CBCT images of three piglet heads were acquired with their soft tissues intact (standard group). Subsequently, the piglet heads were fixed in a container using metallic pins and moulded with acrylic resin; the soft tissues were then removed and replaced by ballistic gelatin, with the same thickness of the original soft tissues. The images were evaluated by two oral radiologists, to check the adaptation on bone surfaces, thickness and density, penetration into large bone cavities and cancellous bone, and the presence of air bubbles using a 5-score scale. Additionally, an objective analysis was carried out by one oral radiologist. For each CBCT scan, three axial reconstructions were selected to represent the mandibular, occlusal, and maxillary levels. The mean and standard deviation (SD) of the grey values were calculated in four regions of interest determined on soft tissue areas and compared by two-way ANOVA. RESULTS: The ballistic gelatin showed subjective scores ranging from good to excellent for all parameters evaluated. There was no significant difference in the mean and SD values of the grey values between ballistic gelatin and the gold standard groups for all levels (p > 0.05). Higher SD values were observed in the occlusal level for both groups (p < 0.05). CONCLUSIONS: Ballistic gelatin has visual and objective similarity with the gold standard. Thus, the ballistic gelatin is a promising material capable of simulating soft tissues in CBCT images.

Background The standard management for distal fibula fractures is open reduction plus internal fixation with plate and screws (blocked or not), however, there are complications associated with this type of intervention. In elderly patients with poor soft tissue quality or in those with high-energy trauma as the etiology of ankle fractures, the rate of previously mentioned complications increases, so that the management of distal fibula fractures with percutaneous screws represents a alternative with lower risk of complications and similar outcomes to conventional management. Methods A series of cases of patients with fibula fracture managed percutaneously was carried out. A sample of 17 patients was obtained for one year, who were followed up on consolidation rate, functionality, and postoperative complications. Results 10 (62.5%) were men, with a mean age of 59 years (range between 24 and 90). 100% were secondary to high-energy trauma, 10 (62.5%) were left, 14 (87.5%) were classified as Weber B and 7 cases (56.25%) presented concomitantly with open fractures. Discussion In order to avoid complications, this approach should be considered in patients with comorbidities, the elderly, or those with severely injured or poor-quality soft tissues.

Embryonic rhabdomyosarcoma, a botryoid variant, is a malignant neoplasm of soft tissues that derives from embryonic mesenchymal muscle cells. Some genetic risk factors are known, but the disease usually presents itself sporadically. It's rarely manifested in adolescents, just as it is rare to be primary in the cervix. It occurs with the presence of polyps and even masses that protrude in the vagina with cases of abnormal vaginal bleeding. The diagnosis is made essentially by history and anatomopathological examination. The longer the time to confirm the diagnosis, the worse the prognosis. There are several treatment modalities involving a multidisciplinary team that must be individualized and basically include chemotherapy, radiotherapy and surgery. The results are generally less favorable in adolescents, when compared with those of children with the same neoplasia.(AU)

RESUMEN

ABSTRACT Myoepithelial carcinoma is a rare neoplasm of very heterogeneous manifestation that represents a major challenge to the adoption of categorical prognostic criteria since, despite being classified as a low-grade cancer, it behaves very aggressively. While several studies have indicated complete surgical resection as the best treatment for myoepithelial carcinoma, adjuvant therapies have proven effective in preventing relapse. This study describes the case of a patient who developed myoepithelial carcinoma in the right thigh, affecting deep tissues, which were conducted as indicated in the current literature, however, presenting an unfavorable outcome. The available information comes from few studies; therefore, we emphasize the importance of gathering data on the subject, which are still scarce.

Abstract Fournier's Gangrene is a type II necrotizing fascitis that leads to thrombosis of small subcutaneous vessels and spreads through the perianal and genital regions and the skin of the perineal. Most cases have a perianal or colorectal focus and in a smaller proportion it originates from the urogenital tract. The mortality rate varies between 7.8 and 50%1-3, only timely diagnosis decreases the morbidity and mortality of this condition. Treatment includes surgical debridement of all necrotic tissue and the use of broad-spectrum antibiotics.

ABSTRACT Introduction: Soft tissue calcifications are the accumulation of calcium salts in tissues other than bones. They are a set of radio-opacities easily identifiable on a radiographic image, generally the panoramic sort, this being one of the most commonly used techniques in dental practice. The presence of these calcifications in terms of their location, shape, size and number, will suggest the type of soft tissue calcified. Objective: Identify the types of soft tissue calcifications, their description on a dental diagnosis image and their probable association with an underlying systemic disease. Methods: A narrative literature review was conducted of papers about soft tissue calcifications published in the databases PubMed and Google Scholar from January 2014 to May 2019. The search terms used included "calcification", "panoramic radiograph" and "soft tissues", among others. Data analysis and integration: The review is a detailed summary of several types of calcifications, their clinical implication and imaging features. Conclusions: Soft tissue calcifications are conditions identified as findings of dental image techniques which often go unnoticed. The revision stresses that it is the responsibility of oral and maxillofacial radiologists to identify and report these calcifications. However, general dental practitioners should also familiarize themselves with this sort of condition and likewise inform the patients(AU)
