RESUMEN

The article is dedicated to therapeutic efficacy of a combination of the antihypoxant bemithyl and the antidepressant pyrazidol in patients with asthenoneurotic and asthenovegetative syndromes after moderate isolated brain injury (BI). The combined therapy was shown to reduce long-term sequences of BI such as asthenic symptoms, stabilize both lipid peroxidation processes and serum antioxidative systems.

RESUMEN

Effects were studied of graded veloergometer exercise on the level of arterial pressure and blood content of prostaglandins in patients with hypertensive disease (n = 14) and in persons with hypertonic type neurocirculatory dystonia (n = 22), who ranged from 17 to 19 years old. The reaction was different in the two groups: an increased diastolic pressure in hypertonic patients and decreased one in those with neurocirculatory dystonia, which event was accompanied by a striking (more than 2-fold) augmentation of the blood content of prostaglandins E. The data secured can be used in differential diagnosis of the above conditions.

AIM: To study manifestations of vegetative dystonia in children of probands from families with hereditary IHD load regarding the main risk factors. MATERIALS AND METHODS: 44 individuals aged 21 +/- 0.7 years were selected out of 111 children from 103 examined families the fathers of which had transmural or macrofocal myocardial infarction at the age under 50. 22 of 44 examinees had syndrome of vegetovascular dystonia (VVD). RESULTS: Children of probands with family history of cardiovascular disease and having VVD had levels of cholesterol, triglycerides and apoB higher, but HDLP cholesterol lower than those free of VVD. In daughters of the probands insulin and hydrocortisone levels were high whereas sons with VVD had only insulin levels higher. CONCLUSION: Children of probands of both sexes had high coefficients of hormonal and metabolic adaptation dependent on the severity of VVD syndrome.

Levels of phosphatidylinosites (PI) in blood lymphocytes were measured in 59 children and adolescents (mean age 11 +/- 3.8 years) with neurocirculatory dystonia (NCD). Sex-specific minimal PI differences were determined. Contrary to childhood, in adolescence PI levels are elevated both in healthy and NCD subjects (p < 0.01). PI in controls and NCD patients differed more in arrhythmogenic, cardialgic and hypotonic NCD, less--in hypertensive variant. PI values closely correlated both with clinical course of NCD and type of neurodynamic regulation of autonomic nervous system.

RESUMEN

Profiles of external respiration and circulation, self-assessment of health, mental performance, sensory functions, and peripheral blood were studied in patients with neurocirculatory dystonia in the course of normobaric hypoxic training. The hypoxic training proved to successfully normalize arterial pressure, eliminate cardiac arrhythmias, and benefit general condition. Substantial strengthening of hypoxic resistance was stated along with an oxygen debt reduction during hypoxia, improvement of sensory coordination and mental performance, and activation of erythropoiesis.

RESUMEN

Adaptation to periodic hypoxia (decompression 490 mm Hg) in a multiplace medical pressure chamber of patients with hypertension, cardial neurocirculatory asthenia, coronary heart disease produced a persistent hypotensive effect with improvement of central and peripheral hemodynamics, oxygen homeostasis, electrolyte balance of blood. In patients with neurocirculatory asthenia associated with ventricular and supraventricular extrasystole positive hemodynamic changes accompanied a persistent antiarrhythmic effect. In addition to the latter, CHD patients experienced improvement in myocardial contractility. Clinical effect of baroadaptation to hypoxia manifesting in symptom relief permitted the physicians to diminish the intensity of chemotherapy and to abolish it in patients with neurocirculatory asthenia.

RESUMEN

Radioimmunoassay revealed in participants of Chernobyl disaster sequelae preclinical changes of some hormonal functions: moderate increase of the basal concentration of blood insulin, somatotropin, C-peptide against the background of persistent hypercortisolemia. Increase of the level of "hyperglycemic" hormones (cortisol, somatotropin, C-peptide) is explained by adaptative and compensatory reactions of the body in response to lesioning effects of ionizing radiation and other negative factors of the Chernobyl accident. Relative hyperinsulinemia is, apparently, compensating the total hyperglycemic effect of these hormones leading to stabilization of the blood sugar level.

Measurements were made of the content of gonadotropic pituitary hormones (prolactin/PRL), luteinizing hormone (LH), follicle-stimulating hormone (FSH) and estradiol in 151 children aged 11 to 15 years. Of these, there were 58 healthy children (24 boys and 34 girls), 51 children (25 boys and 26 girls) with vegetovascular dystonia (VVD) of the vagotonic type and 42 children (22 boys and 20 girls) with VVD of the sympathotonic type. It has been established that in VVD children, alterations in the hypothalamus-pituitary-gonadal system agreed with the general biological reactions of the body, while deviations in the LH and FSH levels as well as in estradiol got stabilized by 13 to 15 years. A positive correlation has been revealed between the high blood PRL level and the high tone of the sympathetic nervous system. In view of this fact the blood PRL content can serve an early diagnostic test for recognizing hypersympathicotonia in VVD children.

Chronic fatigue syndrome (CFS), including myalgic encephalomyelitis (ME) and postviral syndrome (PVS), is a term used today to describe a condition of incapacity for making and sustaining effort, associated with a wide range of symptoms. None of the reviews of CFS has provided a proper consideration of the effort syndrome caused by chronic habitual hyperventilation. In 100 consecutive patients, whose CFS had been attributed to ME or PVS, the time course of their illness and the respiratory psychophysiological studies were characteristic of chronic habitual hyperventilation in 93. It is suggested that the labels 'CFS', 'ME' or 'PVS' should be withheld until chronic habitual hyperventilation - for which conventional rehabilitation is available - has been definitively excluded.

Hyperventilation causes hypocapnia and respiratory alkalosis and thereby predisposes to coronary vasoconstriction and cardiac arrhythmia. Diagnostic methods for use between episodes have not been established. In this study of 100 patients and 25 control subjects the resting end-tidal PCO2 (Pet CO2) levels and the results of a forced hyperventilation test did not show a significant difference between the groups. However the patients hyperventilated more profoundly in response to emotional stimulation, and were less aware of inappropriate breathing and hypocapnia. It is suggested that these differences should be accommodated in cardiac rehabilitation.

To evaluate the cardiovascular and plasma catecholamine responses to dynamic exercise in patients with cardiac neurosis (CN), treadmill testing was performed. Thirty-four patients with CN were chosen for this study based on exercise tolerance and the results were compared with those in 31 patients with organic heart disease and 12 normal subjects. Patients with CN showed an augmentation of cardiovascular and plasma catecholamine responses. The augmentation of the norepinephrine response in patients with CN was not as remarkable as that in patients with organic heart disease. On the other hand, the augmentation of the epinephrine response was greater in patients with CN than in those with organic heart disease. Administration of metoprolol (40 mg/day) for two weeks improved exercise tolerance in patients with CN. We suggest that anxiety augments both sympatho-neural and sympatho-adrenal activity and that it is the symptoms induced by the augmented cardiovascular response which reduce exercise tolerance in patients with CN.

Clinical studies showed that short-term "physiological" stress (bicycle ergometry) did not cause increase of the blood plasma enkephalin content. Moderate-term (60-minute) stress, according to O. Desiderato, causes a rise of the blood enkephalin level in rats. It is supposed that such a reaction of the opioid system is characteristic of borderline conditions between normal and pathological states in stress. Severe "pathological" stress (myocardial infarction, experimental coronary occlusion, 6-hour stress after O. Desiderato) in rats led to a decrease of the concentration of enkephalins in the blood and disorders of their secretion and storage in the adrenals. It is suggested that changes of the activity of the opioid system are a predictor of the transition of a "physiological" stress to a "pathological" stress.
