RESUMEN

The Barratt Impulsiveness Scale was designed to assess the impulsivity construct in three dimensions: attentional, motor and unplanned. The scale has been applied to patients with different diagnoses in which the impulsivity symptoms are characteristic. In Argentina there aren't studies that evaluate the psychometrics properties of the scale in clinical population. The objective of the research was to evaluate the psychometrics properties in a sample of the city of Buenos Aires. 150 adult subjects were selected: 67 didn't have relevant psychiatric diagnoses, 56 met criteria for bipolar disorder and 27 had Attention Deficit Disorder. The internal consistency, the construct validity, discriminative validity and the factorial structure were assessed. The Cronbach alpha was 0.84 for the total scale. Also the instrument has demonstrated acceptable indicators of construct and discriminative validity. Significant differences were found when the results of the present study and the original factorial structure were compared. The results support the usefulness of the scale in the psychiatric, scientific and clinical context to evaluate the impulsivity construct. The total score of the scale obtained the strongest indicators of reliability and validity.

OBJECTIVE: The aim of the work described here was to measure the role of psychopathological features, specifically impulsivity and depression, in suicidality in patients with temporal lobe epilepsy (TLE). METHODS: Neuropsychiatric evaluation of 66 outpatients with TLE was performed with the following instruments: a structured clinical interview (Mini International Neuropsychiatric Interview Plus), the Barratt Impulsiveness Scale, the Hamilton Anxiety Scale, the Beck Depression Inventory, and the Brief Psychiatric Rating Scale. RESULTS: A current Axis I psychiatric diagnosis, mainly mood and anxiety disorders, was assigned to 37 subjects (56.1%) Presence of suicide risk was identified in 19 patients (28.8%), and 14 (21.2%) had attempted suicide. Frequency of seizures (P=0.012), current major depression (P=0.001), and motor impulsivity (P=0.005) were associated with suicide risk on univariate analysis. Logistic regression stressed the main relevance of major depression (OR=12.82, 95% CI=2.58-63.76, P=0.002) and motor impulsivity (OR=1.21, 95% CI=1.06-1.38, P=0.005) to suicide risk. CONCLUSION: Depression has a major influence on suicidality in epilepsy. Motor impulsivity is also relevant and may be an important component of depression in TLE associated with suicide risk.

The Barratt Impulsiveness Scale version 11 is one of the scales mostly used to measure impulsivity and it does not have a validated version for Brazilian Portuguese. The objective of this study is to adapt and conduct the construct validation of the Barratt Impulsiveness Scale version 11 for adolescents. METHOD: The scale was translated and adapted into Portuguese and then back-translated into English. The psychometric proprieties, factor analysis and construct validity were evaluated in two samples: 18 bilingual undergraduate medical students and 464 male adolescents between 15 and 20 years old from a well-delimited geographical area in the city of Canoas, southern Brazil. RESULTS: The adolescent sample had a mean age of 17.3 +/- 1.7 years. Intra-class correlation coefficient achieved a value of 0.90, and internal consistency had a of 0.62. Factor analysis did not identify the 3 factors of the original scale. Impulsivity scores from the Barratt Impulsiveness Scale version 11 had a correlation with scores for attention deficit/hyperactive disorder and oppositional defiant disorder and with number of symptoms of conduct disorder, suggesting an appropriate construct validity of the scale. CONCLUSION: Even considering some limitations in the Portuguese version, Barratt Impulsiveness Scale version 11 can be used in male adolescents and should be tested in other populations.

A escala de impulsividade de Barratt é uma das mais utilizadas para medir impulsividade mas não há uma versão validada em português. O objetivo do estudo é adaptar para o português e realizar a validação de construto da escala de impulsividade de Barratt para adolescentes. MÉTODO: A escala foi traduzida e adaptada para o português e retro-traduzida para o inglês. Foram avaliadas propriedades psicométricas, análise fatorial e validade de constructo em duas amostras: 18 estudantes de medicina bilíngües e 464 adolescentes masculinos, entre 15 e 20 anos, de uma região geograficamente delimitada de Canoas, sul do Brasil. RESULTADOS: A média de idade dos adolescentes foi de 17,3 ± 1,7 anos. O coeficiente de correlação intraclasse foi de 0,90 e a consistência interna teve alfa de 0,62 para os 30 itens. A análise fatorial não identificou os três fatores da escala original. Os escores de impulsividade da escala de impulsividade de Barratt versão 11 foram correlacionados com os escores para os transtornos de déficit de atenção e hiperatividade, oposicional desafiante, e para o número de sintomas de transtorno de conduta, conferindo validade de constructo à escala. CONCLUSÃO: Mesmo considerando as limitações da versão brasileira, a escala de impulsividade de Barratt versão 11 pode ser utilizada em adolescentes masculinos e deve ser testada em outras populações.

Prefrontal cortex (PFC) dysfunction can lead to impairment in planning and behavioral inhibition, as well as personality changes. As ascending monoaminergic brainstem systems modulate PFC functioning, it is possible that lesions in the brainstem lead to symptoms similar to prefrontal dysfunction. A 29-year-old man developed several cognitive and behavioral symptoms after neurosurgery for resection of a pilocytic astrocytoma in the pontine-mesencephalic area. A careful analysis of symptoms indicated PFC dysfunction that could be attributed to lesions in the ascending monoaminergic brainstem systems. Interestingly, the cognitive symptoms improved after treatment with methylphenidate, which is a drug that modules catecholaminergic neurotransmission, thereby supporting this hypothesis. This is a unique case of PFC dysfunction that may be related to post-operative lesion of the catecholaminergic nuclei in the brainstem.

OBJECTIVE: The goal of this study was to examine normalization of functioning among youths with persistent attention-deficit hyperactivity/disorder (ADHD) symptoms. RESEARCH DESIGN: Subjects were 85 referred boys with persistent ADHD as defined by the Diagnostic and Statistical Manual of Mental Disorders, Revised Third Edition (DSM-III-R) who were followed up prospectively into mid adolescence and 68 boys without ADHD. These subjects were assessed at baseline and follow-up visits by using measures from 3 domains of functioning: school, social, and emotional. For each of these domains, we defined boys with ADHD as having normalized functioning if they attained scores above the 5th percentile of scores in the non-ADHD group. RESULTS: Twenty percent of boys with ADHD were functioning poorly in all 3 domains, 20% were functioning well in all 3 domains, and 60% had intermediate outcomes. Increased exposure to maternal psychopathology, larger family size, DSM-III-R psychiatric comorbidity, and symptoms of impulsivity were negatively associated with normalization of functioning among children with persistent ADHD. CONCLUSION: Our results show that children with ADHD have a variable emotional, educational, and social adjustment despite syndromatic persistence. This suggests that normalization of functioning and syndromatic persistence of ADHD may be partially independent.

RESUMEN

En este trabajo se presenta una escala diseñada para evaluar la dimensión impulsividad en la prueba de Rorschach, que predicen dificultad o pérdida en el control de los impulsos y los pondera en 5 categorías: ausente, leve, moderada, severa y muy severa. se aplicó la EIR a 20 protocolos Rorschach, correspondientes a 10 pacientes impulsivos, de acuerdo a criterios clínicos. Se encontró concordancia entre la impulsividad evaluada clínicamente en 19 de los protocolos estudiados. Se concluye que la EIR puede ser un instrumento útil para evaluar la impulsividad

The relationship between children's reflection-impulsivity styles and their human figure drawings was examined for a sample of Mexican children of middle (n = 97) and low (n = 110) socioeconomic status. Drawings were scored for Developmental, Emotional, and Impulsivity indicators. As in previous research with American preschoolers, the drawings of 23 Mexican preschoolers did not correlate with the styles; however, significant relationships between the styles and Impulsivity scores of older children of low status (n = 87) were also found, suggesting that, when a higher-level of impulsivity is present in a population, a relationship between styles and drawings may be observed. The findings for low-status preschoolers, even their higher Impulsivity scores, support the notion that Kagan's measure of impulsivity is not appropriate for use with younger children.
