Background: Mammary tumors, for which mastectomy is the main treatment, are the most common neoplasms in bitches. Mastectomy is painful and, in order to reduce the pain stimulus in the transoperative period, tumescent local anesthesia is associated with general inhalation anesthesia. However, despite the numerous benefits of tumescence, intraoperative hypothermia is the most common complication. In Medicine, especially in plastic and dermatological surgery, it is common to use a heated tumescence solution to prevent intraoperative hypothermia; however, in Veterinary medicine, no previous study has examined the advantages and disadvantages of using heated tumescence solution. Thus, this study aimed to investigate the transanesthetic cardiorespiratory effects of heated tumescence solution in bitches submitted to radical unilateral mastectomy. Materials, Methods & Results: Eight animals were treated with 0.1% lidocaine solution, warmed to 37-42°C, using a Kleins cannula for administration. Chlorpromazine (0.3 mg/kg) and meperidine (3 mg/kg) were used as pre-anesthetic medication intramuscularly, and induction was performed with intravenous propofol and maintenance with isoflurane. The data collection times were as follows: 15 min after starting isoflurane administration (M1), 5 min after tumescence (M2), after beginning of surgical incision (M3), during breast pullout (M4), after clamping of the superficial caudal epigastric vein, and artery (M5), after the beginning of the approximation of the subcutaneous tissue (M6), after the beginning of the intradermal suture (M7), and at the end of the surgical procedure (Mfinal). Background: Postoperative care after thoracolumbar hemilaminectomy in dogs generally includes rest, physical therapy,and analgesics such as opioids. Currently, there is no established standard for the management of postoperative pain inpatients undergoing hemilaminectomy. Ideally, an analgesic protocol should provide adequate pain relief with limited sedation, low adverse effects, and postoperative patient comfort. Therefore, this study aimed to evaluate postoperative analgesiausing transdermal fentanyl or intramuscular methadone in dogs undergoing thoracolumbar hemilaminectomy associatedwith intervertebral disc fenestration (HT) for the treatment of intervertebral disc extrusion (IVDE).Materials, Methods & Results: Eight dogs from the Department of Neurology and Neurosurgery of a Veterinary TeachingHospital, submitted to HT for the treatment of IVDE, were included. The dogs were randomly distributed into 2 groups ofequal numbers, namely the transdermal fentanyl (FT) group and the intramuscular methadone (IM) group. At the end ofthe surgical procedure, a fentanyl adhesive patch was applied to the animals in the FT group, which remained there for 72h. In the IM group, analgesia was induced by intramuscular administration of methadone at intervals of 6 h until 72 h aftersurgery. Background: Ovariohysterectomy (OHE) is the most commonly performed elective surgical procedure in companionanimals. OHE offers benefits of control of population and decreased risk of potentially life-threatening diseases such asmammarian tumours and pyometra. Traditional OHE intervention causes inflammation and pain due to trauma duringorgan manipulation. The purpose of this study was to compare the effect of intraperitoneal and incisional administrationof bupivacaine (BP) or levobupivacaine (LP) on postoperative analgesia in dogs undergoing the OHE procedure.Materials, Methods & Results: A total of 24 mix-breed bitches aged between 1 - 3 years and weighed 19 - 20 kg wereused in this study. The animals were divided into three groups as control (n = 8), BP (n = 8) and LP group (n = 8). Theanimals were kept under surveillance at the hospitalisation unit of the animal hospital for one day before the elective OHE.The dogs were fasted for 12 h before the surgery, with adlib water consumption. Atropine sulphate 0.045 mg/kg was administered subcutaneously approximately 30 min before general anaesthesia. Midazolam (0.3 mg/kg) was intravenouslyinjected into all dogs for pre-anaesthetic medication. After sedation, anaesthesia was induced with propofol (4 mg/kg, IV)and then the dogs were orotracheally intubated using cuffed endotracheal tubes. General anaesthesia was maintained byadministration of 2% isoflurane. The ventral abdomen was prepared aseptically for OHE following the general anaesthesia.All animals were operated on by the same surgeon. During surgery, sprayed bupivacaine 0.5% (4.4 mg/kg) with an equalvolume of saline in BP group, levobupivacaine 0.5% (4.4 mg/kg) with an equal volume of saline in LP were then appliedover the ovaries, uterine broad ligaments and cervix uteri. É importante que se obtenha informações sobre tutores de cadelas portadoras de tumor de mama, pois o tutor precisa cuidar do animal principalmente no pós-operatório já que o tratamento é mastectomia. Foram entrevistados 11 tutores de cadelas submetidas à mastectomia. A maioria dos entrevistados tem ensino médio e superior, morando em casa própria com renda de um a cinco salários mínimos. Apenas uma pessoa disse que não sabia que cadelas podem ter câncer de mama. Entretanto, a maioria não sabe como prevenir o câncer de mama em cadelas, demonstrando que são necessárias campanhas educacionais sobre a doença para tutores de cadelas. A maneira mais frequente de aquisição do animal foi a adoção, o que demonstra o crescimento da posse responsável entre os tutores brasileiros. Três tutores afirmaram que aplicaram anticoncepcional nas suas cadelas várias vezes. Este fato é bastante preocupante porque tais fármacos possuem diversos efeitos colaterais adversos, inclusive tumor de mama. Oito tutores informaram que nunca haviam consultado um médico veterinário antes sobre o tumor, embora o mesmo fosse evidente. A maioria dos tutores só procurou ajuda para a cadela porque a participação no projeto era gratuita. Dificuldades financeiras é um dos principais motivos para que os tutores retardem as consultas. Entretanto, quando questionados por qual motivo as cadelas não tinham sido tratadas antes, nenhum tutor citou dificuldades financeiras. Concluiu-se que alguns tumores de mama foram causados por aplicação de anticoncepcional e que o desconhecimento sobre a doença ainda é grande entre os tutores entrevistados.(AU)


It is important to collect information about owners of dogs with mammary tumor, since the owner must take care of the animal after surgery, given that the treatment of choice is a mastectomy. This study interviewed eleven owners of female dogs submitted to mastectomy. Most respondents have high school and/or college education, live in their own homes with incomes between one to five minimum salaries. Only one person said they did not know that dogs could have mammary cancer. However, most do not know how to prevent mammary cancer in dogs, demonstrating the need for educational campaigns on the disease. The most frequent way of acquiring the animal was adoption, which demonstrates the growth of responsible ownership among Brazilian owners. Three owners stated that they used contraceptive drugs on their dogs several times. This fact is quite worrisome since such drugs have several adverse effects, including mammary tumor. Eight owners reported that they had never been to a veterinarian before about the tumor, although the tumor was evident. Most owners only sought help for the dog because participation in the project was free of charge. Financial difficulties are one of the main reasons for owners to delay consultations. However, when asked why the dogs had not been treated before, no owner mentioned financial difficulties. It can be concluded that some breast tumors were caused by the use of contraceptive drugs and that ignorance regarding the disease is still high among the interviewed tutorsd.(AU)


Es importante que se obtenga informaciones sobre los tutores de perras portadoras de tumor de mama, pues el tutor necesita cuidar del animal principalmente en el postoperatorio, ya que el tratamiento es la mastectomía. Se han entrevistado a once tutores de perras sometidas a la mastectomía. La mayoría de los encuestados tienen educación secundaria y universitaria, y viven en sus propios hogares con ingresos de uno a cinco sueldos mínimos. Solo una persona dijo que no sabía que las perras pueden tener cáncer de mama. Sin embargo, la mayoría no sabe cómo prevenir el cáncer de mama en perras, demostrando que son necesarias campañas educativas sobre la enfermedad para dueños de perras. La forma más frecuente de adquirir el animal es la adopción, lo que demuestra el crecimiento de la posesión responsable entre los tutores brasileños. Tres tutores declararon que aplicaron anticonceptivos a sus perras varias veces. Background: Mammary tumors are the most common neoplasms in female dogs. Surgical removal of the mammary glandchain is considered the standard treatment and is usually performed along with ovariohysterectomy (OH) to suppressovarian hormonal influence. Mastectomies cause moderate to severe pain and require preferential multimodal analgesicprotocols. The aim of this study was to compare the postoperative analgesic efficacy and the recovery times on femaledogs undergoing mastectomies and OH between those treated with epidural levobupivacaine alone and those treated withassociated doses of tramadol and anesthetized with propofol.Materials, Methods & Results: Eighteen female dogs were pretreated with acepromazine (0.03 mg/kg), using propofol(4 mg/kg) for induction and anesthesia maintenance. The dogs were randomly divided into three groups (n = 6) treatedwith epidural anesthesia with levobupivacaine alone at 1.5 mg/kg (GL) or associated with tramadol at doses of 2 mg/kg(GLT2) or 4 mg/kg (GLT4). After anesthesia, the mean propofol infusion rate for each group were calculed. During thepostoperative period, the degree of analgesia according to the University of Melbourne Pain Scale for 6 hours were determined, considering seven time points for evaluation (M30, M60, M90, M120, M180, M240, and M360). Supplementalanalgesia with morphine (0.5 mg/kg) to those dogs with scores ≥13 were provided. During the recovery period, the timeintervals between the end of anesthesia, and the following events: extubation (EX), the first head movement (MC), and theestablishment of sternal (PE) and quadrupedal (PQ) positions were measured. Data were subjected to the Friedman testfor analysis of non-parametric variables within the same group and to the Mann–Whitney test for independent variables,comparing the mean scores between groups (statistical significance was set at P < 0.05). The aim of this study was to describe and analyse the adaptability and functionality of tie-in configuration in tibial osteosynthesis in dogs. Twenty dogs with tibial fracture were included in this study. An orifice was made on the proximal tibial fragment, on the medial side, close to the tibial crest. The drill piece was angled at 45º and projected into the same orifice on the distal sense of the bone. Others orifices made with the aid of a low rotation drill and drill piece with diameter smaller than the chosen implant. After 10 days post-operative, the animals were evaluated. X-ray analysis was performed at the time of clinical examination; immediate post-operative period; and at 30, 60, 90, and 120 days post-surgery. A questionnaire was given to the owners to provide details on the post-operative period. There were no trans-operative complications or suture dehiscence up to the day of suture removal. Partial development of bone callus was observed in 20 dogs within a mean period of 76 days. Three animals showed bone consolidation within 35 days, nine by 60 days, three by 90 days, and 5 by 120 days post-operative. Dynamization was carried out in 9 animals. The surgical access to the tibial medullary canal through the orifice at the proximal medial face, by the tibial tuberosity, enables the insertion of IMP without risks to articular and peri-articular lesions in the knee in dogs.(AU)

The aim of this study was to describe and analyse the adaptability and functionality of tie-in configuration in tibial osteosynthesis in dogs. Twenty dogs with tibial fracture were included in this study. An orifice was made on the proximal tibial fragment, on the medial side, close to the tibial crest. The drill piece was angled at 45º and projected into the same orifice on the distal sense of the bone. Others orifices made with the aid of a low rotation drill and drill piece with diameter smaller than the chosen implant. After 10 days post-operative, the animals were evaluated. O objetivo deste trabalho foi relatar o procedimento de cesariana em um caso de parto distócico em caprino. Uma cabra mestiça Anglo-Nubiana de quatro anos de idade, pesando 40kg foi atendida no Hospital Veterinário Antônio Francisco Fonzar da Faculdade de Ciências Agrárias de Andradina, apresentando dificuldade ao parto. Ao exame físico, o animal apresentava-se ofegante, inquieto, com apetite presente e bom estado corporal. Foi realizado exame vaginal constatando prolapso vaginal, retal, dilatação cervical insuficiente, porém, sem observação de feto e membranas fetais. No exame clínico constatou-se que a temperatura retal e frequência cardíaca e respiratória estavam dentro dos limites considerados como normais para a espécie. Realizou-se uma ultrassonografia uterina observando gestação gemelar, viabilidade fetal e apresentação fetal anormal devido a posição transversal de um dos fetos. O plano terapêutico escolhido foi a realização de cesariana, o tratamento pós-operatório consistiu na administração de Flunixin meglumina (2mg/kg de peso corporal, durante 3 dias), Oxitetraciclina(1mL/10kg de peso vivo,em uma única aplicação) e Penicilina Benzatina (20.000 UI/kg a cada 48 horas, totalizando 5 aplicações). Para o curativo local utilizou-se pomada a base de gentamicina, sulfadiazina de prata, vitamina A e unguento. Após duas semanas o animal já apresentava plena recuperação, conclui-se que a conduta e os procedimentos cirúrgico foram bem-sucedidos.

A four-year-old Anglo-Nubian goat, weighing 40kg, was attended at the Veterinary Hospital Antônio Francisco Fonzar of the Faculty of Agrarian Sciences of Andradina, presenting difficulty in parturition. On physical examination, the animal was restless, with a present appetite and good body condition. The vaginal examination revealed vaginal prolapse, rectal, insufficient cervical dilatation, but without fetal observation and fetal membranes. In the clinical examination it was verified that the rectal temperature and heart and respiratory rate were within the limits considered normal for the species. A uterine ultrasound was performed observing twin pregnancy, fetal viability and abnormal fetal presentation due to the transverse position of one fetus. The therapeutic plan chosen for cesarean section, the postoperative treatment consisted of administration of Flunixin meglumine (2 mg/kg body weight for 3 days), Oxytetracycline 20% (20 mg/kg body weight in a single application) and Benzathine Penicillin (20,000 IU/kg every 48 hours, totaling 5 applications). For the local dressing was used ointment based on gentamicin, silver sulfadiazine, vitamin A and ointment. After two weeks the animal already had full recovery, it is concluded that the surgical procedures and conduct were successful.

OBJECTIVE: Cataracts are the most common ocular disorder in dogs. Phacoemulsification is the preferred treatment method among ophthalmologists, but the cellularity of the endothelium must be considered for its success, as endothelial lesions may produce permanent corneal decompensation. The objective of this study was to evaluate the effects of intracameral ascorbic acid, a known antioxidant, on the corneal endothelium of dogs undergoing phacoemulsification. ANIMAL STUDIED: In all, 40 eyes from 20 dogs, males and females from 7 to 12 years of age, were assessed for mature cataracts. PROCEDURES: Two groups were formed (n = 20): Group 1 (G1) received a balanced salt solution (BSS), whereas Group 2 (G2) received sterile ascorbic acid diluted in a BSS, at a final concentration of 0.001 m ascorbic acid. The corneal endothelium was assessed via non-contact specular microscopy at multiple time points before and after phacoemulsification. Cell density (cells/mm2 ) and area (mm2 ), corneal thickness (mm), hexagonality, and the coefficient of variation of cell size were all assessed. P values equal to or less than 0.05 were considered significant. RESULTS: With respect to the density of endothelial cells, both groups showed losses, but they were less severe in G2. There were no differences in corneal thickness. Hexagonality decreased significantly in the postoperative period in G1. Also in G1, the coefficient of variation of cell size increased significantly. CONCLUSION: According to the results obtained, ascorbic acid minimizes cellular losses in the corneal endothelium.

Objectives Three infusion rates of remifentanil were used in isoflurane-anesthetized cats undergoing ovariohysterectomy. The aim of this study was to identify a dosage regimen that would provide optimal anesthetic and surgical conditions, as well as to compare cardiovascular response to surgical stimulation, postoperative analgesia, anesthetic duration and quality of recovery among the tested remifentanil infusion rates. Methods Twenty-seven client-owned, mixed-breed adult healthy female cats were randomized to receive remifentanil 0.1 µg/kg/min (REMI01), remifentanil 0.2 µg/kg/min (REMI02) or remifentanil 0.4 µg/kg/min (REMI04). After premedication with acepromazine and induction of anesthesia with propofol, cats were mechanically ventilated and anesthesia was maintained at approximately 1.0 minimum alveolar concentration (MAC) of isoflurane (1.63% end-tidal isoflurane [ETISO]). Remifentanil infusion rate was increased or decreased by 20% if blood pressure had increased or decreased by 20% from previous values. Pulse rate (PR), systolic arterial pressure (SAP), esophageal temperature, pulse oximetry, end-tidal partial pressure of CO2 and ETISO were recorded at different time points during surgery. Meloxicam was administered before the end of surgery. Data within each treatment group were analyzed using a mixed-model ANOVA and Friedman's test followed by the Wilcoxon signed rank test. Bonferroni or Dunnett's post-hoc tests were used. The Kruskal-Wallis test followed by Dunn's post-hoc test were used to compare data between groups; significance was set at P <0.05. Results Time to sternal recumbency and time to standing were significantly longer in REMI04 than in the other groups. SAP was higher when compared with baseline in REMI01 and REMI02 groups than in REMI04. No significant difference in PR among groups was observed. One cat in REMI01 and another in REMI02 required postoperative rescue analgesia. Conclusion and relevance The dosage regimen of 0.4 µg/kg/min seemed to be the most appropriate to be used in cats undergoing ovariohysterectomy and anesthetized with 1.0 MAC of isoflurane.

O dermóide, presença de tecido cutâneo, constituído ou não de derme, epiderme, folículos pilosos e pelos ectópico, origina-se durante o período embrionário do animal e em ruminantes. Como tratamento de eleição foi utilizada a exérese cirúrgica, entretanto foi necessário um tratamento pós-cirúrgico com terapias à base de antibióticos e anti-inflamatórios. Utilizado como forma de substituição de colírios à base de antibióticos e anti-inflamatórios, o soro autólogo (SA) tem ganhado cada vez mais espaço no que diz respeito à tratamentos alternativos. Foi encaminhado para o Hospital Veterinário da Universidade Federal Rural do Semiárido (HOVET/UFERSA), uma bezerra com aproximadamente 10 meses de idade, apresentando uma protrusão de tecido ectópico na região periocular. O procedimento para exérese do dermóide foi realizado no centro cirúrgico do HOVET e como tratamento pós-cirúrgico, a fim de se elucidar a atividade do soro autólogo no controle bacteriano, foi preconizado o uso de 1 gota do soro autólogo, quatro vezes ao dia, durante 30 dias. O tratamento com soro autólogo na concentração de 100% se mostrou como um eficiente agente bactericida no tratamento pós-cirúrgico.

A 10 months of age calf, resenting a protuberance of ectopic tissue in the periocular region, was addimited in the Veterinary Hospital of the Federal Rural Semi-Arid Federal University (HOVET /UFERSA). The procedure for the removal of the dermoid was performed at the surgical center of HOVET and as a post surgical treatment to elucidate the autologous serum activity in the bacterial control, the use of 1 drop of autologous serum was recommended four times a day during 30 days. Treatment with 100% autologous serum was shown to be an efficient bactericidal agent in post-surgical treatment.

This study examined the efficacy of an intravitreal dexamethasone-loaded device for the control of postoperative ocular inflammation in dogs following phacoemulsification. Twenty dogs with bilateral mature senile cataracts were prepared for surgery using routine protocols. A biodegradable poly (lactic-co-glycolic acid) copolymer device was inserted through the pars plana into the vitreous chamber immediately before phacoemulsification (device group [DG], n=20). Following surgery, a conventional group (CG) received local and systemic steroids, mydriatics, and antibiotic therapy. The same treatment protocol was adopted in DG, except for steroids. All eyes were examined before surgery and at various times after phacoemulsification. Ultrasonography showed gradual device shrinkage, with only remnants remaining at postoperative day (POD) 60. Signs of uveitis were observed in 35% of the DG on POD 7, but by POD 14, 50% of eyes showed signs of uveitis and these eyes required local steroid therapy. The intraocular pressure (IOP) was higher in the DG than in the CG immediately after surgery. IOP did not differ on POD 7 and POD 14, but was lower during the late postoperative period (POD 30 to 90). Flare values were greater in the DG than in the CG immediately following surgery, but showed no subsequent differences. In summary, the intravitreal dexamethasone device did not adequately control intraocular inflammation in dogs undergoing phacoemulsification.(AU)


O estudo examinou a eficácia de um dispositivo intravítreo de liberação de dexametasona para o controle da inflamação ocular em cães, após facoemulsificação. Um dispositivo de copolímero poli (ácido lático-co-glicólico) foi implantado via pars plana na câmera vítrea, imediatamente antes da facoemulsificação (grupo dispositivo [GD], n=20). Após a cirurgia, o grupo controle (GC) recebeu terapia esteroide, midriático e antibiótico. O mesmo protocolo de tratamento foi adotado no GD, exceto pelos esteroides. Todos os olhos foram examinados antes do procedimento e em diferentes tempos após a facoemulsificação. A ultrassonografia mostrou que o dispositivo diminuiu em tamanho, sendo observado, apenas, remanescentes aos 60 dias de pós-operatório (DPO). Sinais de uveíte foram observados em 35% do GD no DPO 7, entretanto, no DPO 14, 50% dos olhos tiveram sinais de uveíte e requereram terapia esteroide local. A pressão intraocular (PIO) foi maior no GD, comparativamente ao GC, imediatamente após a cirurgia. A PIO não diferiu no DPO 7 e no DPO 14, entretanto foi menor nos momentos pós-operatórios mais tardios (DPO 30 a 90). Valores de flare foram maiores no GD que no GC, imediatamente após a cirurgia, mas não mostraram diferenças nos momentos subsequentes. Em suma, o dispositivo intravítreo de dexametasona não controlou adequadamente a inflamação intraocular em cães submetidos à facoemulsificação.(AU)

RESUMEN

A infecção de sítio cirúrgico (ISC) tem sido apontada como a terceira causa mais comum de infecção nosocomial. Este estudo objetivou determinar o perfil epidemiológico da ISC e sua associação aos fatores de risco descritos. Trata-se de um estudo transversal, realizado no Hospital São João Batista de Viçosa-MG e na Clínica Cirúrgica de Cães e Gatos do Hospital Veterinário da Universidade Federal de Viçosa-MG, no período de setembro de 2012 a fevereiro de 2013. As taxas globais de ISC foram de 0,7% no hospital humano e 3,46% no veterinário. No hospital veterinário, a taxa de ISC não mostrou relação com o potencial de contaminação, apresentando a maior taxa nos procedimentos classificados como limpos. Quanto ao tipo de cirurgia, as ortopédicas são as mais comuns em ambos os hospitais e também as que apresentam maior taxa de ISC. Cirurgias com duração maior que 120 minutos corresponderam a 15,25% do total de procedimentos no hospital humano e são ainda menos comuns no veterinário, com 1,26%. A taxa de ISC não parece estar relacionada à duração da cirurgia nesta estratificação. As bactérias isoladas das feridas cirúrgicas foram multirresistentes e os dados levantados indicam que não houve critério quanto ao emprego da antibioticoprofilaxia, principalmente nas cirurgias limpas. Este cenário mostra que é de extrema relevância a atuação de uma comissão de controle de infecção hospitalar, a fim de garantir obtenção de dados fidedignos, para que se possa avaliar a qualidade do serviço prestado e assim promover a redução dos riscos de complicações pós- operatórias...


Surgical site infection (SSI) has been indicated as the third cause of nosocomial infection. The present study aimed to determine the epidemiological profile of SSI and its association with the risk factors. It is a transversal study done at the São João Batista Hospital of Viçosa-MG and at the Surgery Service of the Small Animals Veterinary Hospital of the Universidade Federal de Viçosa-MG, from September 2012 to February 2013. Global SSI rates were 0.7% at the human and 3.46% at the veterinary hospitals. At the veterinary hospital, SSI rates were not related to contamination potential, with clean procedures presenting the greater rates. As for the type of surgery, orthopedic ones are the most common in both hospital and also the ones presenting the greater SSI rates. Surgeries during more than 120 minutes were 15.25% of the total of procedures at the human hospital and are even less common in the veterinary, with 1.26%. Rate of SSI does not seem to be related to surgery duration in this classification. Bacteria isolated from surgical wounds were multi-resistant and the obtained data indicated that no criteria of antibiotic prophylaxis existed, mainly for clean surgeries.
