RESUMEN

Paragonimiasis is a parasitic disease caused by Paragonimus trematodes, which are typical food-borne zoonoses prevalent in Asia, West Africa, and Latin America. A clinical infection by paragonimiasis usually begins with cough, haemoptysis, and dyspnoea, some of which overlap with symptoms of tuberculosis and other pulmonary disorders. Human paragonimiasis presents as a complex set of symptoms that may result in misdiagnosis and delayed treatment. The parotid is seldom affected. We describe the case of a 5-year-old boy with paragonimiasis who presented with a swelling of the parotid and no pulmonary signs or symptoms.

RESUMEN

Endemic to South India and Sri Lanka, Rhinosporidiosis is a chronic granulomatous infection caused by an agent of uncertain taxonomy: Rhinosporidium seeberi. Although it commonly manifests as a proliferative nasal lesion, many cases of Rhinosporidiosis have been reported where it has appeared as an extranasal lesion. The reported extranasal sites include the eye, ear, trachea, and parotid duct. However, the involvement of the parotid duct is quite rare, even among extranasal sites. The case presented is an adult female from the non-endemic zone of East India with a proliferative mass in the parotid duct. Although Rhinosporidiosis was not taken into consideration in the clinical differential diagnosis, eventual histopathological diagnosis confirmed Rhinosporidiosis. As this appears to be the second case of Rhinosporidiosis in the parotid duct in East India in 4 years, we encourage clinicians to be flexible in the differential diagnosis of proliferative growth in the parotid duct, even in those from non-endemic areas.

BACKGROUND: Most suppurative parotid abscesses are unilateral and result from bacterial infection via Stensen's duct. Formation of bilateral parotid abscesses rarely occur, and cases arising in the setting of parasite infection have not been described to date. METHODS AND RESULTS: A 41-year-old man presented with bilateral parotid abscesses and underwent incision and drainage of right-sided swelling because of poor response to the aspiration drainage and intravenous antibiotic therapy. A nematode larva, morphologically consistent with Strongyloides stercoralis (S. stercoralis), was found in the cytologic examination, and bilateral abscesses regressed significantly after administration of Ivermectin. CONCLUSION: We report the first case of S. stercoralis infection presenting as bilateral parotid abscesses. This case expands the spectrum of diseases associated with S. stercoralis and suggests that clinicians, especially in the endemic regions, should consider S. stercoralis infection in the differential diagnosis of refractory parotid abscess.

INTRODUCTION: The hydatid cyst is an anthropozoonosis due to the development of the Echinococcus granulosus tapeworm larva in humans. Its intraparotid localization is extremely rare. CLINICAL CASE: A 10-year-old-male patient was admitted for an isolated renitent swelling of the left parotid region having grown for several months. Cervical ultrasonography revealed a left parotid cystic mass, characterized by a double wall, and a small daughter cyst. The CT-scan typically showed the absence of enhancement after iodine contrast medium injection. The imaging-based diagnosis of a hydatid cyst was supported by positive hydatid serology, and confirmed by histological analysis of the cyst. No other hydatid location was found (chest and abdominal). DISCUSSION: In spite of its rarity, hydatid cyst can be a diagnosis in case of parotid cystic masses. Ultrasonography is the first examination to be performed; it must be completed by CT-scan or even better by MRI. Once the diagnosis of hydatid cyst is confirmed, a chest radiograph and an abdominal ultrasonography should be performed, to screen for other localizations. Surgical resection is the gold standard treatment.

RESUMEN

Rhinosporidiosis is a benign chronic granulomatous infection caused by Rhinosporidium seeberi. Rhinosporidiosis is endemic in south Asia, notably in southern India and Sri Lanka. Majority of the cases have been reported to occur in upper respiratory sites, notably anterior nares, nasal cavity, nasopharynx, larynx and soft palate. Only two rare cases of involvement of parotid duct, have been reported in literature. Hence, this case will probably be the third to be reported.

Most preauricular masses are parotid neoplasms; however, some infectious and inflammatory causes may exhibit similar presentation. Toxoplasmosis is a worldwide parasitary disease. The clinical presentation of toxoplasmosis is mostly asymptomatic and may include subfebrile fever, fatigue, and lymphadenopathy. Only 13 clinical reports of intraglandular toxoplasmic lymphadenitis have been previously reported in the English literature. A careful history and examination with appropriate investigations, including immunoglobulin G avidity assay, will frequently provide the diagnosis of intraparotid toxoplasmosis. Surgery might therefore have been delayed or avoided depending on a satisfactory clinical improvement in consequence of proper medical treatment.

RESUMEN

OBJECTIVE: A case of solitary extracranial cysticercosis of the parotid gland is reported. A discussion concerning the clinical features, imaging modalities, diagnostic methods and management of cysticercosis, along with a review of relevant literature is also presented. CASE REPORT: A young female patient presented with features resembling acute parotitis. Solitary cysticercosis of the parotid gland was diagnosed, based on fine needle aspiration cytology and radiological investigations. Medical therapy was effective, consequently surgery was deferred. CONCLUSION: This is the first report of solitary extracranial cysticercosis of the parotid gland. A high index of suspicion for such parasitic infestations is essential even in non-endemic areas due to the ease of worldwide travel.

RESUMEN

Although rhinsporidiosis caused by Rhinosporidium seeberi is known to mankind since hundred years, many aspects of this enigmatic disease have remained mysterious till date. Parotid duct as a site of involvement has rarely been reported. Our case interestingly presented with a cystic mass of left parotid duct accompanied by an ulcer and mucopurulent discharge was finally confirmed to be a case of rhinosporidiosis by histopathological examination.

RESUMEN

Toxoplasmosis is an ubiquitous protozoan infection, caused by the coccidian Toxoplasma gondii. It usually presents clinically with lymphadenopathy, mainly in the head and neck region. However, in the English literature, very scarce cases of intraparotid Toxoplasma exist, none having been diagnosed preoperatively. In this article, we present 2 cases of intraglandular parotid toxoplasmosis, one diagnosed postoperatively and the other preoperatively. We also review the relevant literature concerning Toxoplasma infection of the parotid gland.

RESUMEN

A 40-year-old female suffering from hydatid disease located in the parotid gland is presented. Although Greece remains an endemic area for echinococcosis, this presentation of the disease is rare. Total excision of the cyst with partial parotidectomy was performed. The patient refused to receive general anesthesia and the operation was carried out under local anesthesia. Perioperative adjuvant medical therapy with albendazole was administered. In a two-year follow-up no recurrence has occurred.

RESUMEN

Primary hydatid cyst of the parotid gland is still an exceptional localization. The parotid gland is an uncommon site, even in our country, in which echinococcal disease is endemic. We report six cases of primary hydatid cyst of the parotid gland who presented with an isolated mass. The diagnosis was based on ultrasonography, which showed a parotid gland cystic mass. The echinococcal immunologic test (enzyme-linked immunosorbent assay) was positive in two cases. The surgical treatment consisted of total resection of the cyst, without rupture of the cystic wall and preserving the gland, in four cases and resection of the prominent dome in two cases. The diagnosis was confirmed on macroscopic examination of the resected pieces. In all cases, the postoperative course was uneventful. There was no recurrence at the 17-month follow-up.

RESUMEN

Hydatid disease is a zoonotic infection caused by Echinococcus species. Cystic form of this infection mostly involves liver and lung. Hydatid disease of the parotid gland is a very rare entity that may be easily overlooked in daily practice. On the other hand, rupture of the cyst during surgery may give rise to serious complications. We present a case of hydatid cyst of the parotid and discuss the differential diagnosis of this rare condition.

RESUMEN

Toxoplasmosis is a protozoan infection manifested by systemic findings as well as generalized or localized lymphadenopathy. Although lympadenitis is the most common clinical form, involvement of the intraglandular nodes within the parotid gland found is a rare occurrence during the course of the disease. This unusual form may mimic a parotid mass, and can cause difficulties in differential diagnosis. In this article, a case of intraglandular parotid lymphadenitis which developed due to toxoplasmosis is presented, and the relevant literature is reviewed.

Toxoplasmosis is a common infection often presenting as lymphadenopathy in the cervicofacial region. Despite this it is only rarely considered in differential diagnosis. Two cases are presented, the first showing the usual presentation, the second showing an uncommon presentation of toxoplasmosis.
