RESUMEN

Genital tract bleeding in prepubertal girls is a rare clinical condition, which can occur for multiple reasons. It frequently generates anxiety in the family and in health care professionals. A thorough anamnesis and careful genital inspection can give important diagnostic hints; however, there are cases in which the cause remains doubtful and a complete gynecological evaluation (including cultures and vaginoscopy) is necessary. Therefore, the attending physician should always consider less frequent diagnoses in order to perform the necessary studies in a sequential and rational manner. We present the case of a preschool girl with vaginal bleeding due to pinworm endometritis, which, to our knowledge, has never been reported before as a cause of genital bleeding in prepubertal girls.

RESUMEN

A case of enterobiasis in pregnancy that presented as copious nocturnal vaginal discharge is reported. Enterobius vermicularis is the most common parasite infecting humans. Transmission can be fecal-oral or via fomites, and recently arrived immigrants from developing countries and individuals who live in households with young children are particularly at risk. Pinworms are most frequently found in the gastrointestinal tract but can also enter the vagina and bladder. Patients typically present with nocturnal anal itching, and diagnosis can be made by clinical history. Treatment includes an antihelminthic agent for the patient and members of the household as well as home hygiene measures to prevent transmission.

Enterobius vermicularis (pinworm) is one of the most prevalent intestinal parasites in the world. The urinary tract is rarely affected and few cases have been reported. We report a case of bladder infestation by mature female worms of E. vermicularis in a woman presenting with irritative voiding symptoms.

RESUMEN

The anthelmintic activity of the extracts obtained from Luxemburgia octandra was evaluated naturally infected mice with Aspiculuris tetraptera and Vampirolepis nana. The leaves extracts were obtained through maceration and given to the animals by gavage in doses 8 and 20 mg/kg during three days. The ethanolic and ethyl acetate extracts presented significant increase of the V. nana elimination, but did not present the nematicide effect against A. tetraptera.

During 1995, we examined 2,666 Graham's tests (five perianal swabs with adhesive cellulose tape) prescribed to ambulatory patients attended in 14 public outpatient clinics and four hospitals from the north section of Santiago. The infection rates found by age groups were the following: 6,1 percent in infants (< 2 years old), 20,9 percent in pre-schoolchildren (2 < 6 years old), 35,2 percent in elementary schoolchildren (6 < 15 years old) and 15,2 percent in adults (> 15 years old). Similar percentages of E. vermicularis infections were observed in females and males patients. Frequency of infection in each month of the year varied between 17,4 and 26,8 percent, with no seasonal variation. This study and previous surveys are stressing that E. vermicularis is the most frequent helminthic agent found in Chile, specially in elementary school children.

Pinworm infection was prospectively studied during one year in 469 children attending three day care centers. Each child was examined at six months intervals using up to three perianal swabs with adhesive tape. Those found infected were treated with mebendazole. At the beginning of the study we found a prevalence of 28% that dropped to 13% and 12% in the following study periods. The reinfection rate was twice the incidence rate in both study periods. We also found a small percentage (10%) of the children reinfected in most or all study periods. There was a high correlation between reinfection and perianal itching. Our results add further knowledge to the epidemiology of intestinal parasites in day care centers.
