BACKGROUND: Undergraduate nursing students experience significant differences between practice with models, manikins, or simulation applications and real patients in a clinical setting. Students' experiences applying their theoretical knowledge to real patient-care practices are little understood. OBJECTIVE: To determine the experiences of nursing students in providing skin, chronic wound, and ostomy care to real patients for the first time in a clinical setting within the content of the Ostomy and Wound Care Nursing Track Program (OWCNTP) and to define factors affecting this program. METHODS: The research was conducted qualitatively using the individual critical incident technique, and 17 senior undergraduate nursing students enrolled in the Nursing OWCNTP were selected using a simple random sampling method. In the classroom setting, individual face-to-face interviews were conducted using the critical incident technique. Data were analyzed with inductive content analysis. RESULTS: The research found that students experience genuine caregiving in putting their experiences from the Track Program into practice with real patients in a clinical setting. Three main themes were identified: experiencing real patient care in a clinical setting, being a competent student, and being a novice student. CONCLUSIONS: The study found that nursing students enrolled in the OWCNTP could apply their theoretical knowledge to care for real patients in clinical settings. Therefore, it is recommended that these programs be integrated into nursing curricula.

BACKGROUND: Self-efficacy interventions, which include the acquisition of skills that enable patients to manage their health on a daily basis, play a key role in ostomy patients, which leads to significant changes in the quality of life of patients. In this context, nursing interventions to increase self-efficacy of ostomy patients are very important. In this context, nursing interventions are crucial to increase the self-efficacy of ostomy patients. OBJECTIVES: The aim of this systematic review is to describe nursing interventions for ostomy patients' self-efficacy (primary outcome) and the impact of these interventions on patient outcomes (complications, quality of life, satisfaction, psychological resilience, stoma adaptation) (secondary outcomes). METHOD: As a systematic review, this study included articles published in PUBMED, Web of Science, Science-Direct, TUBITAK-ULAKBIM, and TRDizin databases between January 2013 and January 2023 that included nursing interventions for self-efficacy ostomy patients. This systematic review was developed using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) checklist. The risk of bias was assessed using the RoB2 tool developed by Cochrane. RESULTS: A total of 1211 articles were retrieved from the databases using Turkish and English keywords. Fifteen studies met the study criteria. These studies found that various interventions, such as training, telephone follow-up, psychosocial support groups, or mobile applications provided to intervention groups, increased self-efficacy, decreased stoma-related complications, improved stoma adaptation, and improved quality of life by increasing patients' knowledge and awareness of stoma. CONCLUSION: Nursing interventions to improve the self-efficacy and adaptation of ostomy patients are critical. This improvement leads to a reduction in adverse patient outcomes and ostomy complications, shorter hospital stays, and increased patient and nurse satisfaction.

Resumo Objetivos Conhecer a percepção da equipe de enfermagem da unidade neonatal sobre os cuidados prestados ao recém-nascido hospitalizado com estomia intestinal e discutir os fatores que interferem na assistência de enfermagem. Método Estudo exploratório, descritivo, com abordagem qualitativa. Participaram oito enfermeiros e oito técnicos de enfermagem que trabalham em uma Unidade Neonatal do Rio de Janeiro. Os dados foram coletados entre abril e junho de 2022, através de entrevista semiestruturada e análise de conteúdo. Resultados Emergiram duas categorias: "percepções da equipe de enfermagem quanto ao cuidar de recém-nascidos hospitalizados com estomias intestinais e a educação em saúde da família"; e "aspectos facilitadores e dificultadores da assistência de enfermagem ao recém-nascido com estomia intestinal e a importância da educação permanente no cenário da Unidade Neonatal". Conclusão e implicações para prática O manejo de neonatos com estomias intestinais é atual e implica em cuidados de enfermagem com o estoma e pele do recém-nascido, estendendo-se para a prática da educação em saúde dos familiares. É desafiador o manejo de complicações, a indisponibilidade de materiais e o cuidado fragmentado. Tal achado pode subsidiar o desenvolvimento de intervenções de enfermagem sistematizada para os recém-nascidos e seus pais na unidade neonatal.


Resumen Objetivos Conocer la percepción del equipo de enfermería de la unidad neonatal sobre el cuidado prestado al neonato hospitalizado con ostomías intestinales y discutir los factores que interfieren en el cuidado de enfermería. Método Estudio exploratorio, descriptivo, con abordaje cualitativo. Participaron ocho enfermeros y ocho técnicos de enfermería que actúan en una Unidad Neonatal de Rio de Janeiro. Datos colectados entre abril y junio de 2022, por entrevistas semiestructuradas y análisis de contenido. Resultados Emergieron dos categorías: "percepciones del equipo de enfermería sobre el cuidado al recién nacido hospitalizado con ostomías intestinales y la educación en salud de la familia"; y "aspectos que facilitan y dificultan el cuidado de enfermería al recién nacido con ostomía intestinal y la importancia de la educación continua en el ámbito de la Unidad Neonatal". Conclusión e implicaciones para la práctica El manejo de neonatos con ostomías intestinales es actual e implica cuidados de enfermería con el estoma y la piel del recién nacido, extendiéndose a la práctica de educación en salud para familiares. El manejo de complicaciones, la falta de materiales y la atención fragmentada son desafíos. Este hallazgo puede apoyar el desarrollo de intervenciones de enfermería sistematizadas para los recién nacidos y sus padres en la unidad neonatal.


Abstract Objectives To understand the perception of the nursing team of the neonatal unit about the care provided to hospitalized newborns with intestinal ostomy and to discuss the factors that interfere in nursing care. Method Exploratory and descriptive study, with a qualitative approach. Eight nurses and eight nursing technicians who work in a Neonatal Unit in Rio de Janeiro took part. Data were collected between April and June 2022, through semi-structured interviews and content analysis. Results Two categories emerged: "perceptions of the nursing team regarding the care of hospitalized newborns with intestinal ostomies and family health education"; and "facilitating and hindering aspects of nursing care for newborns with intestinal ostomy and the importance of continuing education in the setting of the Neonatal Unit". Conclusion and implications for practice The management of newborns with intestinal ostomies is current and involves nursing care with the stoma and skin of the newborn, extending to the practice of health education for family members. The management of complications, the unavailability of materials and fragmented care are challenging. This finding can support the development of systematized nursing interventions for newborns and their parents in the neonatal unit.

Objetivo:Relatar a experiência de uma equipe de enfermeiros estomaterapeutas na construção de um algoritmo para a indicação de equipamento coletor para estomias de eliminação. Método: Relato de experiência, do período de janeiro de 2018 a setembro de 2019, sobre o processo de construção de um algoritmo para indicação de equipamento coletor para estomias de eliminação. Resultados: A partir de determinadas características clínicas (parâmetros de avaliação) e da categorização dos equipamentos coletores (solução), foi desenvolvido um algoritmo para indicação de equipamento coletor para estomias de eliminação. Conclusão: Espera-se que esse instrumento possa auxiliar os enfermeiros na sua prática profissional quanto à escolha do equipamento coletor e na construção de protocolos clínicos.


Objective:To report the experience of a team of enterostomal therapists in the construction of an algorithm for the indication of collecting equipment for elimination stomas. Method: Experience report, from January 2018 to September 2019, on the process of building an algorithm to indicate collecting equipment for elimination stomas. Results: Based on certain clinical characteristics (assessment parameters) and the categorization of collecting equipment (solution), an algorithm was developed to indicate collecting equipment for elimination stomas. Conclusion: It is expected that this instrument can help nurses in their professional practice regarding the choice of collecting equipment and the construction of clinical protocols.


Objetivo:Relatar la experiencia de un equipo de enfermeros estomaterapeutas en la construcción de un algoritmo para la indicación de equipos recolectores para estomas de eliminación. Método: Informe de experiencia, de enero de 2018 a septiembre de 2019, sobre el proceso de construcción de un algoritmo para indicar equipos colectores para estomas de eliminación. Resultado: A partir de ciertas características clínicas (parámetros de evaluación) y la categorización de los equipos colectores (solución), se desarrolló un algoritmo para indicar equipos colectores para estomas de eliminación. Conclusión: Se espera que este instrumento pueda ayudar a los enfermeros en su práctica profesional en cuanto a la elección de equipos de recolección y la construcción de protocolos clínicos.

Introdução:O câncer é causado por mutações celulares anormais. O câncer de cólon e reto ocupa a segunda posição no ranking de incidências no Brasil. Objetivo: Descrever o perfil sociodemográfico e clínico de pessoas com estomia por causa oncológica no estado do Ceará. Método: Estudo epidemiológico, observacional, longitudinal com abordagem retrospectiva, realizado no Ceará, em prontuários de pacientes acompanhados pelo Serviço de Atenção ao Paciente com Estomia. Resultado: Foram analisados 655 prontuários, dos quais a maioria dos pacientes eram do sexo feminino (53%), idosos (61,41%), aposentados e pensionistas (46,9%), casados (42%), com filhos (67,9%), cuja escolaridade predominante foi o fundamental (34,2%); 72,8% não possuíam comorbidades, não utilizaram quimioterapia (54,5%), colostomia (64,7%), terminal (65,3%), definitiva (46%), em quadrante inferior esquerdo (52,5%), vermelha (64,4%), ovalado (47,2%), baixo perfil (44,6%) e pastoso (33,9%), utilizavam bolsa de uma peça (60,6%), com uma média de 10 bolsas mensalmente (95%), realizando troca de 3 a 5 dias (43,1%), sem a necessidade de uso de adjuvantes (71%). Conclusão: Um perfil dos pacientes deve ser traçado, a fim de melhorar a assistência e planejamento das ações a esse público.


Introduction:Cancer is caused by abnormal cell mutations. Colon and rectum cancer occupies the second position in the ranking of incidences in Brazil. Objective: To describe the sociodemographic and clinical profile of people with an ostomy due to cancer in the state of Ceará. Method: Epidemiological, observational, longitudinal study with a retrospective approach, carried out in Ceará, in medical records of patients monitored by the Service of Attention to the Patient with Stoma. Result: A total of 655 medical records were analyzed, in which most of the patients were female (53%), elderly (61.41%), retired and pensioner (46.9%), married (42%), with children (67.9%), whose predominant schooling was elementary school (34.2%); 72.8% had no comorbidities, did not use chemotherapy (54.5%), colostomy (64.7%), terminal (65.3%), definitive (46%), in the lower left quadrant (52.5%) , red (64.4%), oval (47.2%), low profile (44.6%) and pasty (33.9%), used a one-piece bag (60.6%), with an average of 10 bags monthly (95%), changing every 3 to 5 days (43.1%), without the need to use adjuvants (71%). Conclusion: A profile of patients should be drawn in order to improve assistance and planning of actions for this public.

Objetivo: Describir el perfil sociodemográfico y clínico de las personas con estoma por cáncer en el Estado de Ceará. Método: Estudio epidemiológico, observacional, longitudinal con abordaje retrospectivo, realizado en Ceará, en prontuarios de pacientes acompañados por el Servicio de Atención al Paciente con Estoma. Resultado: Se analizaron 655 prontuarios, la mayoría del sexo femenino (53%), adulto mayor (61,41%), jubilado y pensionado (46,9%), casado (42%), con hijos (67,9%), cuya escolaridad predominante fue la primaria. Conclusión: Debe elaborarse un perfil de pacientes para mejorar la atención y planificación de acciones para este público.

Objetivo: Demostrar la importancia del rol de enfermería en los pacientes con ostomía. Metodología: Estudio cualitativo de un caso a través de una entrevista y artículos relacionados. Se ilustra el caso de una paciente ostomizada. Resultados: La ostomía supone un proceso de adaptación y aceptación, la cual afecta el autoestima y la percepción sobre sí mismo, siendo esto más frecuente en mujeres. Conclusiones: El paciente ostomizado sufre un fuerte impacto de su imagen corporal, lo que conlleva trastornos a nivel físico, psicológico y social. Es por esto que la importancia del profesional de enfermería quien brindará cuidados de manera integral a cada persona en el proceso de adaptación[AU]


Objective: To demonstrate the importance of the nursing role in ostomy patients. Methodology: Qualitative case study through an interview and related articles. Objetivo:Caracterizar o perfil clínico epidemiológico de crianças com estomia atendidas no ambulatório de um hospital público de ensino de referência na área infantil no Brasil. Método: Estudo descritivo, documental, retrospectivo e de natureza quantitativa, realizado por meio da extração de dados sociodemográficos e clínicos de prontuários eletrônicos de crianças com estomia, acompanhadas de 2014 a 2018. Os dados passaram por estatística descritiva e, na análise, foram organizados em uma planilha no programa Microsoft Excel versão 2010, distribuídas em três tabelas, seus valores representados em frequência absoluta e relativa. Resultados: Das 85 crianças com estomia, houve predominância do sexo masculino, idade entre 1 a 4 anos. Anomalia anorretal foi considerada o principal diagnóstico. Constatou-se elevado número de colostomias confeccionadas. Dermatite de contato foi a complicação periestomal mais frequente. Verificou-se a dificuldade na rotina de comparecimento das famílias ao ambulatório e constatou-se a mãe como cuidadora principal. Conclusão: O estudo concluiu relevantes aspectos relacionados ao planejamento do cuidado pautado na demanda de cada criança e na educação em saúde para a prevenção de complicações, ações indispensáveis na oferta do cuidado de enfermagem seguro e de qualidade, como também favorecer operacionalização das políticas públicas relacionadas à saúde da criança com estomias.


Objective:To characterize the epidemiological clinical profile of children with ostomies treated in the outpatient clinic of a reference public teaching hospital for children in Brazil. Method: Descriptive, documentary, retrospective and quantitative study, carried out by extracting sociodemographic and clinical data from electronic medical records of children with ostomies, monitored from 2014 to 2018. The data underwent descriptive statistics, and in the analysis, the data were organized in a spreadsheet in Microsoft Excel version 2010, distributed in three tables, their values represented in absolute and relative frequency. Results: Of the 85 children with ostomy, there was a predominance of males, aged between 1 and 4 years. Anorectal anomaly was considered the main diagnosis. There was a high number of colostomies made. Contact dermatitis was the most frequent peristomal complication. There were difficulties in the routine of families attending the outpatient clinic, and the mother as the main caregiver. Conclusion: The study found relevant aspects related to care planning based on the demand of each child and health education for the prevention of complications, essential actions in the provision of safe and quality nursing care, as well as favoring the operationalization of public policies related to the health of children with ostomies.

RESUMO Objetivo: relatar a experiência sobre a elaboração de um instrumento implementado na consulta de Enfermagem a pacientes com estomia. Material e método: relato de experiência vivenciado por enfermeiros, acadêmicos de Enfermagem e professores, em um centro de referência em estomaterapia do Sul do Brasil, realizado por meio de encontros presenciais com busca de instrumentos padronizados em outros serviços, aprofundamento teórico sobre possíveis diagnósticos, intervenções e resultados de Enfermagem à pessoa com estomia, socialização com a equipe de Enfermagem sobre o instrumento e a relevância dele na prática clínica. Resultados e discussão: a utilização de um instrumento demanda a articulação teórica versus a prática, bem como a compreensão das particularidades advindas do paciente com estomia. A aplicabilidade do instrumento reforçou a importância do conhecimento, habilidade e atitude em coletar dados, realizar anamnese e exame físico, elencar possíveis diagnósticos de Enfermagem associando resultados e prevendo intervenções de Enfermagem junto ao paciente. Conclusões: o instrumento implementado mostrou-se apropriado para a consulta de Enfermagem, pois resultou em melhoria na abordagem ao estomizado, além de permitir uma coleta de dados completa, direcionada e organizada, que colabora para um melhor conhecimento do paciente, criação de vínculo e identificação dos problemas, possibilitando a definição do diagnóstico de Enfermagem, o planejamento, a implementação e a avaliação dos cuidados.


RESUMEN Objetivo: relatar la experiencia sobre la elaboración de un instrumento implementado en la consulta de Enfermería a pacientes con ostomía. Material y método: relato de experiencia vivido por enfermeros, académicos de Enfermería y profesores, en un centro de referencia en estomaterapia del Sur de Brasil, realizado por medio de encuentros presenciales con búsqueda de instrumentos estandarizados en otros servicios, profundización teórica sobre posibles diagnósticos, intervenciones y resultados de Enfermería a la persona con ostomía, socialización con el equipo de Enfermería sobre el instrumento y la relevancia de él en la práctica clínica. Resultados y discusión: la utilización de un instrumento demanda la articulación teórica versus la práctica, así como la comprensión de las particularidades provenientes del paciente con ostomía. La aplicabilidad del instrumento reforzó la importancia del conocimiento, la habilidad y actitud en recolectar datos, realizar anamnesis y examen físico, enumerar posibles diagnósticos de Enfermería asociando resultados y previendo intervenciones de Enfermería junto al paciente. Conclusiones: el instrumento implementado se mostró apropiado para la consulta de Enfermería, pues resultó en mejoría en el abordaje al ostomizado, además de permitir una recolección de datos completa, dirigida y organizada, que colabora para un mejor conocimiento del paciente, creación de vínculo e identificación de los problemas, posibilitando la definición del diagnóstico de Enfermería, la planificación, la implementación y la evaluación de los cuidados.


ABSTRACT Objective: to report the experience on the development of an instrument implemented in the Nursing consultation with ostomate patients. Material and method: an experience report by nurses, nursing students, and professors, at a stomal therapy reference center in the South of Brazil, carried out through face-to-face meetings, with the search for instruments standardized in other services, theoretical deepening on possible diagnoses, interventions, and outcomes of Nursing to the person with ostomy; socialization with the Nursing team on the instrument and its relevance in clinical practice. Results and discussion: the use of an instrument demands the articulation of theory versus practice, as well as the understanding of particularities arising from the ostomy patient. The applicability of the instrument reinforced the importance of knowledge, skill, and attitude in collecting data, performing anamnesis, and physical examination, listing possible nursing diagnoses, associating results, and predicting nursing interventions with the patient. Conclusions: the implemented instrument proved to be appropriate for the nursing consultation as it resulted in an improved approach to the ostomy, in addition to allowing for a complete, targeted, and organized data collection, which allows for a better familiarity with the patient, the creation of a bond, and the identification of problems, thus facilitating the definition of nursing diagnoses, planning, implementation, and evaluation of care.

OBJECTIVE: To critically appraise peer-reviewed evidence concerning the value, or implied sense of worth or benefit, of nurses specialized in wound, ostomy, and continence (WOC) care. DATA SOURCES: The Preferred Reporting Items for Systematic Reviews and Meta-analyses was used to systematically review current literature in a single database from 2009 to the date of search (July 2019). STUDY SELECTION: The initial search retrieved 2,340 elements; 10 studies were retained following removal of duplicate records, title and abstract reviews, and application of the inclusion/exclusion criteria. DATA EXTRACTION: Literature was graded and critiqued with regard to design and research quality and then synthesized using a narrative approach. DATA SYNTHESIS: Nine values that WOC nurses demonstrate were identified: improved quality of life for patients, teaching and mentoring, cost reduction, improved efficiency, improved wound outcomes, improved incontinence outcomes, advanced treatments, research, and leadership. CONCLUSIONS: Although current studies suggest that there is value in the WOC nurse role, in all areas of the trispecialty, there is a need for high-quality literature with higher-level designs focused on bias reduction.

Leaks and peristomal skin complications are highly prevalent among people with a stoma, reported by over 80% of ostomates within 2 years of surgery. This suggests that there is room for improvement in ostomy appliances, particularly in their hydrocolloid-based adhesive flanges. Hydrocolloid has an absorptive method of moisture management that, over time, risks maceration and skin stripping, potentially leading to moisture-associated skin damage (MASD) and medical adhesive-related skin injury (MARSI). The newly developed Genii ostomy appliances (Trio Healthcare) use novel Sil2 Breathable Silicone Technology to provide secure, effective adhesion and manage moisture levels by replicating natural transepidermal water loss (TEWL). This has the potential to increase appliance wear time, reduce incidence of MASD and permit atraumatic removal without adhesive remover, reducing the risks of MARSI, as well as time burdens on the user and economic burdens on the healthcare system. Meanwhile, the silicone flanges and water-resistant sports fabric pouches are lightweight, flexible and unobtrusive, and they are the first appliances to be available in colours to match different skin tones, all of which provides security, comfort, confidence and discretion. This article explores the features of Sil2 and Genii ostomy appliances, with reference to preliminary data from a user evaluation.

OBJECTIVE: To report the experience of adapting the stomatherapy service during the COVID-19 pandemic. METHOD: Experience report related to adaptations in the work routine in times of COVID-19 pandemic, from March to May 2020, in a specialized stomatherapy center in a city in the South of Brazil. RESULTS: The work routines were adapted to suit the protection measures for workers and users who used stomatherapy services. Some assistance processes were implemented to make users' access to care more flexible, and to modify routines to increase the safety of health professionals and users. CONCLUSION: The need to adapt the physical area, rethink the dynamics of care, use personal protective equipment, and guidance for servers and patients were of fundamental importance to continue attending the population safely in times of pandemic.

PURPOSE: This article is an executive summary of Italian guidelines for nursing management of enteral and urinary ostomies in adults. METHODS: Scoping review and generation of evidence and consensus-based clinical guidelines. SEARCH STRATEGY: The Multidisciplinary Italian Study group for STOmas (MISSTO) was founded in 2018. This group created guidelines for management of enteral and urinary ostomies in adults based on a scoping review of the literature. The research included previous guidelines, systematic reviews, meta-analyses, randomized clinical trials, cohort studies, and case reports. Five main topics were identified: "stoma preparation," "stoma creation," "stoma complications," "stoma care," and "stoma reversal" (for enteral stomas)." All the studies were evaluated according to the GRADE system and AGREE II tool. Recommendations were elaborated in the form of statements, with an established grade of recommendation for each statement. For low levels of scientific evidence statements, a consensus conference composed of expert members of the major Italian scientific societies in the field of stoma management and care discussed, corrected, validated, or eliminated the statements. A final version of the guidelines with definitive recommendations was elaborated and prepared for publication. FINDINGS/CONCLUSIONS: This document represents the first Italian guidelines on enteral and urinary stoma management to assist nurses caring for persons with an enteral or urinary ostomy.

BACKGROUND: Italy instituted a lockdown from March 10 to May 3, 2020, due to the coronavirus disease-19 pandemic. All nonessential businesses were closed during this time, and health care services were reorganized. On March 11, the Stoma Care Center started providing telehealth services for patients with a stoma. PURPOSE: This retrospective observational study describes the experience of the Stoma Care Center of the University Hospital Federico II, Naples, Italy, before and during the lockdown. METHODS: Consultation records from January 1 through April 29 were retrieved, patient demographics and reasons for consultation abstracted, and pre-lockdown (January 1 to February 29) and lockdown (March 1 to April 29) information was compared. Patients who used telehealth services were also asked to rate their satisfaction with these services on a scale of 0 (extremely dissatisfied) to 4 (extremely satisfied). The authors analyzed all consultations provided from January to April 2020 and evaluated the use of telemedicine services for patients with an ostomy. Consultations were divided into 2 groups. Group A included consultations provided from January 1 to February 29. Group B included consultations provided from March 1 to April 29, which included the lockdown period. Group B included both in-person and telemedicine consultations. RESULTS: During the pre-lockdown period, 240 in-person consultations were provided. During the lockdown period, 181 in-person and 99 telemedicine consultations were provided. The number of in-person consultations for mechanical bowel preparation and transanal irrigation system training was lower (12.5% vs 6.6% [P = .046] and 3.3% vs 0% [P = .03]), whereas the number of consults for stoma care follow-up and stoma complications was higher (202 [84.1%] vs 266 [95%]). Of the 65 patients who completed the questionnaire, 82% indicated being extremely satisfied. CONCLUSIONS: The reorganization of stoma care services, including the availability of telemedicine, did not result in a decrease in the number of consultations provided. The results suggest that stoma care services using telemedicine may provide valid support for patients with an ostomy in the future.

RESUMO Objetivo analisar as facilidades e dificuldades percebidas por egressos de uma pós-graduação em Estomaterapia para atuação no mundo do trabalho. Métodos pesquisa qualitativa, desenvolvida com 22 egressos de um Curso de Pós-graduação em Enfermagem em Estomaterapia, de uma universidade pública da Região Sudeste do Brasil. A coleta de dados ocorreu por meio de entrevista semiestruturada, entre janeiro e abril de 2018. As entrevistas foram tratadas com base na análise temática de conteúdo. Resultados apreenderam-se como facilidades: reconhecimento do especialista, autonomia profissional, conhecimentos adquiridos por meio da especialização, disponibilidade de tecnologias de cuidados. Como fatores dificultadores, citaram-se: carência de recursos humanos e materiais, baixa remuneração do especialista, política institucional desfavorável, não valorização do estomaterapeuta. Conclusões e implicações para a prática as facilidades identificadas se relacionaram com o reconhecimento do especialista; a autonomia; a ascensão e o crescimento profissional; os conhecimentos adquiridos por meio da especialização; e a disponibilidade de tecnologias do cuidado para realização da prática profissional. As dificuldades analisadas no estudo estiveram vinculadas predominantemente à configuração do mundo do trabalho, cujos fundamentos se pautam no ideário neoliberal do Estado mínimo e no enxugamento da máquina pública.


RESUMEN Objetivo analizar las instalaciones y dificultades percibidas por los egresados de una Postgrado en Estomatoterapia para trabajar en el mundo laboral. Métodos investigación cualitativa, desarrollada con 22 graduados de un curso de Postgrado en Enfermería en Estomatoterapia, de una universidad pública en la Región Sudeste de Brasil. La recopilación de datos se realizó a través de entrevistas semiestructuradas, entre enero y abril de 2018. Las entrevistas se trataron en función del análisis de contenido temático. Resultados fue aprehendido como instalaciones: reconocimiento del especialista, autonomía profesional, conocimiento adquirido a través de la especialización, disponibilidad de tecnologías de atención. Como factores que obstaculizan, se citaron los siguientes: falta de recursos humanos y materiales, baja remuneración del especialista, política institucional desfavorable, falta de valoración del terapeuta del estoma. Conclusión e implicación para la práctica las dificultades analizadas en el estudio estuvieron principalmente relacionadas con la configuración del mundo del trabajo, cuyos fundamentos se basan en las ideas neoliberales del estado mínimo y la eliminación de la maquinaria pública.


ABSTRACT Objective to analyze the practicalities and difficulties perceived by graduates of a post-graduate program in Stomatherapy to perform in the world of work. Methods a qualitative research study, developed with 22 graduates from a Postgraduate course in Nursing in Stomatherapy, from a public university in the Southeast of Brazil. Data collection took place through semi-structured interviews, between January and April 2018. The interviews were treated based on the Thematic Content Analysis. Results the following were learned as practicalities: recognition of the specialist, professional autonomy, knowledge acquired through specialization, availability of care technologies. As hindering factors, the following were cited: lack of human and material resources, low remuneration of the specialist, unfavorable institutional policy, lack of appreciation for the stomatherapist. Conclusion and implication for the practice the difficulties analyzed in the study were predominantly linked to the configuration of the world of work, whose foundations are based on the neoliberal ideas of the minimum state and the wiping out of the public machinery.

Subjects undergoing ostomy are increasing and share a reduced quality of life. The patient flow (PF) is the pathway of a patient from hospital admission to discharge and should provide care appropriateness to the patient himself. In the recent literature no paper exists regarding the PF of the patient undergoing (intestinal or urinary) ostomy, which is the objective of the present article. This paper stems from the work done during the Educational Camp entitled "The Patient Flow in Stoma Care," which took place on three separate days (27th May, 18th September, and 21st November 2019) at B. Braun Milano S.p.A. and regarded 33 stomatherapy nurses from all over Italy supervised by the authors. The participants, divided into heterogeneous groups, developed the PF by means of three specific work methodologies: mental maps, timeline and appreciative inquiry. The elaborated PF was inspired to the International Charter of the Ostomates' Rights. The efficacious and empathic communication and the role of the patient and/or the caregiver as the main characters are transversal to every step and must be always pursued. The PF is developed in eight macro-areas: diagnosis; pre-admission; admission and preoperative phase; surgical operation; awakening; postoperative phase; discharge; follow-up. In agreement with the recent literature, this systematic approach will give benefits to the patients in terms of outcome and perception of taking charge before, during and after the ostomy. At the same time the performances, the therapeutic appropriateness, the optimization of technology and healthcare resources and the staff satisfaction will equally be guaranteed.

OBJETIVO: evaluar la eficacia del uso de un aerosol protector barrera para la prevención de la maceración e irritación de la piel periestomal, en el paciente quirúrgico, antes de la colocación de los discos, y valorar la eficacia de la aplicación de dicho aerosol previo a la retirada de los adhesivos. MÉTODO: se llevó a cabo un ensayo clínico aleatorizado con dos grupos de comparación: grupo control (cura habitual) y grupo experimental (cura habitual + aplicación de aerosol como método barrera antes de la colocación del disco y spray quita-adhesivo para su retirada). Se incluyeron a los pacientes ostomizados en el Hospital Universitario Infanta Cristina (Parla, Madrid) en 2019, atendidos en la Consulta de Cirugía General. Se aleatorizaron a dos grupos, 15 por grupo. Se recogieron variables descriptivas el día de la captación y de resultado a los 10 días del seguimiento. Se llevaron a cabo análisis univariante y bivariante. La relación entre las distintas variables se evaluó mediante pruebas estadísticas. RESULTADOS: se aleatorizaron 30 pacientes, 15 en grupo control y 15 en grupo experimental. Se observaron diferencias clínicas en los grupos de comparación al inicio del estudio. Al comparar las variables de resultado a los 10 días de la cirugía en los dos grupos del estudio, se observaron diferencias estadísticamente significativas en la presencia de complicaciones en la piel periestomal (p< 0,001) con un 80% (n= 12) en el grupo control frente al 6,7% (n= 1) en el grupo experimental, y en el tipo de complicaciones entre ambos grupos (0,002), destacando las lesiones de tipo L2 Erosivas. CONCLUSIONES: la utilización tanto de un aerosol protector barrera sobre la piel del paciente antes de la colocación del disco de ostomías como la aplicación para su retirada de un spray quita-adhesivo, parece reducir el riesgo de complicaciones en la piel periestomal en el paciente quirúrgico


OBJECTIVES: to assess the efficacy of using a protective barrier spray for preventing peristomal skin maceration and irritation in surgical patients before disc placement, and to assess the efficacy of the application of said spray before removing the adhesive discs. METHOD: a randomized clinical trial was conducted with two comparison arms: the control group (standard cure) and the experimental arm (standard cure + application of spray as barrier method before disc placement and adhesive remover spray for removing them). The study included ostomized patients from the Hospital Universitario Infanta Cristina (Parla, Madrid) in 2019, seen at the General Surgery Unit. Patients were randomly assigned to two 15-patient arms. Descriptive variables were collected on recruitment day, and for outcomes at 10 days follow-up. Univariate and bivariate analyses were conducted. The relationship between different variables was evaluated through statistical tests. RESULTS: thirty (30) patients were randomized, 15 patients to the control arm and 15 patients to the experimental arm. Clinical differences were observed in the comparison arms at study initiation. When comparing the outcome variables at 10 days after surgery in the two study arms, statistically significant differences were observed regarding the presence of complications in the peristomal skin (p< 0.001) with 80% (n= 12) in the control arm vs. 6.7% (n= 1) in the experimental arm, and in terms of type of complications between both arms (0,002), particularly L2-type erosive lesions. CONCLUSIONS: the use of a protective barrier spray on the skin of the patient before placing the ostomy disc as well as the application of an adhesive remover spray for removal seems to reduce the risk of complications in the peristomal skin of surgical patients

PURPOSE: This study aimed to describe the frequency of colostomy and ileostomy complications and types of nursing interventions as well as to examine patient and ostomy variables associated with early and late complications. METHODS: The records of 572 patients who received ostomy care from a wound ostomy care (WOC) nurse between 2013 and 2017 were abstracted. Patient demographic and ostomy variables, early period (< 30 days after surgery) and late period (> 30 days after surgery) complications, as well as documented nursing interventions were retrieved. Percentages and rates, chi-square statistics, and logistic regression were used to analyze the data. RESULTS: The mean patient age was 59.1 years (standard deviation [SD], 13.86), and the majority of patients were male (302 patients; 52.8%), married (454; 79.4%), and had a temporary (438; 76.6%) end colostomy (253; 44.2%). One (1) or more complications developed in 323 patients (56.5%) in the early period and in 207 patients (36.2%) in the late period. The most common complications in the early period were peristomal irritant contact dermatitis (PICD) (181; 31.6%) and mucocutaneous separation (135; 23.6%). PICD was also the most common complication in the late period (149; 26%). The risk of PICD was significantly higher in patients with a body mass index > 24.9 kg/m2 (odds ratio [OR] = 1.547), who had an ileostomy (OR = 1.654), or a temporary ostomy (OR = 1.728). Variables associated with an increased risk of mucocutaneous separation included obstacles to ostomy care (OR = 2.222), having an end ostomy (OR = 2.171), and ostomy height < 10 mm (OR = 1.964). Complications were treated by the WOC nurse in 67.5% of patients, and the most common intervention was application of skin barrier powder and wipe layers. CONCLUSIONS: The rate of ostomy complications, especially PICD and mucocutaneous separation, in this study was high. Results confirm that patient and ostomy characteristics might significantly affect the risk of complications. The findings support the importance of outpatient follow-up by a WOC nurse. Explorative or randomized controlled studies are needed to identify optimal nursing strategies to decrease complication rates.
