RESUMEN

Eccrine syringofibroadenoma is a rare benign neoplasia derived from acrosyryngium cells of the eccrine sudoriferous glands. It affects the extremities of elderly individuals as solitary tumors, or may also present as multiple lesions. There are controversies about the pathogenesis and differentiation of the tumor. Eccrine syringofibroadenoma has been associated with subjacent conditions, such as for example, hypohidrotic ectodermic dysplasias. The authors describe a case report of a patient with Clouston Syndrome, who presented papules and nodules in extremities, clinically and histologically compatible with eccrine syringofibroadenoma. There are only three cases described in the literature, associated with Clouston Syndrome, and this is the fourth case.

BACKGROUND: Ninety-five percent of women have mammary tissue at the axilla. However, the fibroadenoma seldom occur in this area and there are only isolated case reports of this tumor. This paper presents a series of three cases of this rare entity. CASES REPORT: Three women came to our hospital by axillary lumps. The patients had a median age of 36 +/- 9-years-old with an average time of evolution of 34 +/- 53 months. One patient had the history of a breast phyllodes tumor previously excised, and two were nulliparous. In two cases, neither ultrasound nor mammogram gave useful information for its diagnosis. Mammary glands of all cases were normal. The tumors were firm and movable, similar to a lymphadenopathy, and one was evident. The mean tumor size was 28 +/- 18 mm; all were oval and with white appearance. Histological diagnosis for all cases was fibroadenoma. Our findings match with literature reports. CONCLUSIONS: The axillary fibroadenomas may occur in women aged 30 to 50-years-old, mimicking a lymphadenopathy. Finally, the ultrasound and the mammography are not useful tools for its diagnosis.

RESUMEN

PURPOSE: To present a rare case of multiple fibroadenomas in breast and vulva and myomas in the uterus. In addition, to show the usefulness of receptor detection as a basis for the indication of antiestrogens. CASE: A 30-year-old patient presenting with a history of mammary fibroadenomas since the age of 16 years and uterine myomas since the age of 25 years. From age 26 to 29 years, she was operated on 3 times for fibroadenomas of the vulva. On the third time, she presented with 8 vulvar fibroadenomas. On this last occasion, a low expression of estrogen receptors was identified, together with a high expression of progesterone receptors (ER/PR pharmDx; Dako). Tamoxifen was indicated at 10 mg/d during the luteal phase of the menstrual cycle for 12 months. After 18 months from initiation of the treatment, the patient was found free of tumors in the vulva, with mammary tumor regression and a nonhomogeneous diffuse uterine myometrium. CONCLUSIONS: The presence of solid benign-type tumors in the vulva associated with mammary fibroadenomas and uterine myomas leads one to suspect the fibroadenomas. The histopathological diagnosis and the identification of the hormonal receptors are important for control purposes and therapeutic indications.

OBJECTIVES: Fibroadenoma is the most common benign mammary condition among women aged 35 or younger. Expression of Ki-67 antigen has been used to compare proliferative activity of mammary fibroadenoma epithelium in the follicular and luteal phases of the menstrual cycle. MATERIALS AND METHODS: Ninety eumenorrheic women were selected for tumour excision; they were assigned to either of the two groups, according to their phase of menstrual cycle. At the end of the study, 75 patients with 87 masses were evaluated by epithelial cell Ki-67 expression, blind (no information given concerning group to which any lesion belonged). RESULTS: Both groups were found to be homogeneous relative to age, menarche, body mass index, previous gestation, parity, breastfeeding, number of fibroadenomas, family history of breast cancer and tabagism. Median tumour size was 2.0 cm and no relationship between proliferative activity and nodule diameter was observed. No typical pattern was observed in the expression of Ki-67 in distinct nodules of the same patient. Average values for expression of Ki-67 (per 1000 epithelial cells) in follicular and luteal phases were 27.88 and 37.88, respectively (P = 0.116). CONCLUSION: Our findings revealed that proliferative activities in the mammary fibroadenoma epithelium did not present a statistically significant difference in the follicular and luteal phases. The present study contributes to clarifying that fibroadenoma is a neoplasm and does not undergo any change in the proliferative activity during the menstrual cycle.

RESUMEN

Los tumores benignos de la mama representan el 20 por ciento de las afecciones de la glándula y pueden desarrollarse sea a partir del tejido mamario específico, sea del estroma. De ellos el 70 por ciento corresponde a los fibroadenomas, el 10 por ciento a tumores papilares y el 9 por ciento a lipomas. Se analizan las características clínicas, semiológicas e histológicas de los principales tumores mamarios benignos, destacando entre ellos los fibroadenomas y los tumores papilares únicos y múltiples. Se mencionan también algunas lesiones poco freceuentes como la papilomatosis juvenil, el adenoma del pezón y el hamartoma. Se discute el valor de diversos porcedimientos actualmente disponibles en el estudio de los tumores benignos de la mama. Entre ellos destacan la mamografía, la ecotomografía, la biopsia con aguja fina y la citología por punción
